Infections are common complications in solid organ transplant (SOT) patients. Among these, 8% are invasive fungal infections (IFIs). The morbidity and mortality associated with IFIs in SOT patients have decreased overall in recent decades, thanks to improved diagnostic tools. However, mortality related to IFIs in SOT patients varies depending on the fungal agent involved and remains significant. It is estimated at nearly 10% for cryptococcal infections and more than 60% for <em>Mucorales</em> spp. infections. We present here a review of the literature on IFIs in SOT patients, addressing epidemiology, clinical presentation, diagnosis, and treatment options.</div></div>","PeriodicalId":100906,"journal":{"name":"Médecine et Maladies Infectieuses Formation","volume":"5 1","pages":"Pages 4-21"},"PeriodicalIF":0.0,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147427386","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Despite the difficulty to precisely determine the infectious risk in the peritransplantation period, this risk can be minimised by meticulously screening for predisposing risk factors and actively treating both latent and active infections in both the donor and recipient. Three globally renowned societies have published clinical guidelines for the management of liver transplant candidates, including infectiology recommendations. These guidelines partially overlap and complement each other on many points. Currently, there is no pan-African society for liver transplantation management, and only a few detailed guidelines specific to tropical infections. In Côte d’Ivoire, we performed the first liver transplant in West Africa reported in the scientific literature. Our recent experience demonstrates the complexity of the challenges associated with infectious risk in scientific, health infrastructural, financial, cultural, sociopolitical, and human resources.</div></div>","PeriodicalId":100906,"journal":{"name":"Médecine et Maladies Infectieuses Formation","volume":"5 1","pages":"Pages 45-54"},"PeriodicalIF":0.0,"publicationDate":"2026-03-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147427381","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

However, even in the absence of adverse medical and/or psychosocial factors, pregnancy carries risks (viral escape, treatment complications, premature delivery). We therefore recommend a pre-conception consultation, followed by monthly obstetric, medical, and biological monitoring during pregnancy.</div><div>In the absence of known HIV status, screening must be prescribed at the first prenatal visit and, if not done, should be encouraged later, up to delivery. Pre-exposure prophylaxis (PrEP) should be offered if the woman or her partner is at risk.</div><div>The choice of antiretroviral drugs for pregnant women is evolving. A triple therapy based on integrase inhibitors—dolutegravir or raltegravir—is recommended as first-line treatment. A non-nucleoside inhibitor, rilpivirine, is an alternative. No current antiretroviral drugs are strictly contraindicated, but several are not recommended due to a lack of perinatal safety data or concerns about reduced plasma concentrations during pregnancy.</div><div>Breastfeeding is now possible under optimal conditions, where the mother has a viral load of less than 50 copies/mL for at least six months prior to delivery and every month during breastfeeding. The aim of this study is to determine the incidence of infectious complications during the first post-transplant year by characterizing the types of infections and the pathogens involved. We conducted a retrospective, descriptive, single-center cohort study including kidney transplant recipients (2005-2024) who were followed for at least one year after transplantation in the Nephrology Department of the University Hospital of Monastir (Tunisia). A total of 145 patients were included, with a mean age of 32.5 ± 8.3 years. An infectious complication occurred in 87 patients (60%), who experienced 175 infectious episodes, with a predominance of urinary tract infections (<em>n</em>=74, 42.3%). Infections were microbiologically documented in 134 cases (76.6%), and were mainly bacterial infections (<em>n</em>=71, 40.5%), particularly due to <em>Escherichia coli</em> (<em>n</em>=30, 42.2%). Viral infections were observed in 54 patients (30.8%), and were dominated by cytomegalovirus (<em>n</em>=38, 70.4%). In conclusion, infectious complications after KT remain a major concern.
