Its measurement allows to understand the strengths and weaknesses associated with the anesthesia procedure and to improve the quality of service.</p><p><strong>Objective: </strong>To evaluate the validity and consistency of a new rapid scale of patient satisfaction with the anesthetic procedure.</p><p><strong>Material and methods: </strong>Comparative, longitudinal study in patients in post-anesthetic care. It consisted of two phases: Phase 1: Development of a Likert-type rapid scale to assess post-anesthesia patient satisfaction. Patients were pilot tested to evaluate the wording until 2 consecutive rounds were achieved. Phase 2: The developed instrument (ERSAPA according to its initials in Spanish) and the control questionnaire (LPPSq) were administered to patients after their discharge from the Post-Anesthesia Care Unit. Cronbach's alpha was used to evaluate the internal consistency between both tests, and Sperman's rho for convergent validity.</p><p><strong>Results: </strong>A total of 250 patients were surveyed. An overall satisfaction was achieved in 82.46% of the population. The domain with the highest agreement was pre-anesthesia consultation care. Some variables associated with this alteration in lung function have been described, such as hypoxemia, vaccination and hand strength, which in turn are related to the magnitude of lung damage.</p><p><strong>Objective: </strong>To analyze different clinical variables and their association with alterations in spirometry in survivors of severe COVID-19.</p><p><strong>Material and methods: </strong>Retrospective and observational study of survivors of severe COVID-19 sent for their first spirometry, hand dynamometry, and 6-minute walk (6MW) in a secondary care unit during 2021.</p><p><strong>Results: </strong>2 doses of SARS-CoV-2 vaccine were protective factor (odds ratio [OR]: 0.36, 95% CI [95% CI]: 0.1-0.8, p = 0.02) and hospitalization (> 15 days) was a risk factor for spirometric alterations (FVC < 80% of predicted) (OR: 3.0; 95% CI 1.3-6.8, p = 0.00); hand weakness, SpO2 desaturation > 4% in 6MW, and days of hospitalization were associated with low FVC.</p><p><strong>Conclusions: </strong>A hospitalization > 15 days and having 2 doses of vaccination are risk and protective factors, respectively, of having a low FVC. {"title":"[Factors that interfere in the surgical programming in a second-level hospital in Mexico].","authors":"Rodrigo López-Cisneros, Svetlana V Doubova, Alma Lucila Sauceda-Valenzuela","doi":"10.5281/zenodo.12667910","DOIUrl":"https://doi.org/10.5281/zenodo.12667910","url":null,"abstract":"<p><strong>Background: </strong>Surgical programming is a priority for hospital management due to its impact on health.</p><p><strong>Objective: </strong>To analyze the aspects that affect the surgical programming in a secondary care hospital in Mexico.</p><p><strong>Material and methods: </strong>Hospital operating room programming records for 2021 were consulted. Surgical interventions, operating room usage times, causes of deferrals, surgical delay and turnover times were described. Percentages, mean and standard deviation were estimated; the analysis was carried out with chi-squared and Student's t. A panel of experts was integrated to identify the factors that affect the performance of operating rooms.</p><p><strong>Results: </strong>In 2021, the hospital had 2660 records of surgical programming for 2317 patients; 2.7% of surgeries were deferred. Mostly urgent (46.1%) and scheduled (43.5%) surgeries were performed. The average surgical delay time was 65 minutes and the turnover time was 92 minutes; these times were inadequate in emergency surgeries, in night and evening schedules surgeries (p < 0.05). Cellular senescence, immunosenescence and inflammaging are part of the natural process of aging and are associated with age-related diseases. Cellular senescence is characterized by the arrest of the cell cycle in the G1 phase. Senescent cells can be identified by different markers (surface or intracellular), by changes in their functions, and by their gene expression profile. Immunosenescence causes dysregulation of the immune response, with persistent (chronic) increases in inflammatory mediators and decreases in the effectiveness of immune effector responses, which promotes the development and severity of chronic-degenerative diseases and fragility. When immunosenescence is related with advanced age, it is known as inflammaging. New strategies to revert immunosenescence are currently being investigated, with senotherapy with senolytic drugs that eliminate senescent cells, and with senomorphic drugs that inhibit the secretion of senescence-promoting mediators. Mental health Illness is multifactorial, and its negative consequences range from stress, anxiety, depression, burnout, somatic manifestation to suicide. Health professionals are educators and know the importance of creating and training compassionate and competent physicians. The aim of this study was to describe mental health strategies in postgraduate doctors. Out of a total of 494 articles, only 9 were analyzed which mentioned some strategy for mental health improvement, such as diffusion, timely detection of mental health conditions, in-person or online training, integral care, support groups, socialization, pairing, physical activity, follow-up and support, reinforcement of institutional norms and values, including strategies from the Puebla Decentralized Administrative Operating Body. The strategies implemented in the optimization of mental health in trainee doctors increase resilience and optimism while simultaneously reduce the prevalence of mental health conditions. In addition, they provide the necessary tools for self-care. The Food and Drug administration (FDA) and COFEPRIS (Mexico's Federal Committee for Protection against Sanitary Risks) have approved 3 therapies to increase the production of survival motor neuron (SMN) protein and improve muscle strength and quality of life in patients: nusinersen, onasemnogene abeparvovec xioi, and risdiplam. Despite the fact that these therapies have shown efficacy, at the moment it is not possible to establish which of them is superior compared to the others. The most important thing is to establish the diagnosis of the disease and start any of the available treatments to avoid further functional disability and prevent death. Mechanical ventilation was fundamental in the treatment; however, the evidence of ventilatory markers associated with mortality is not entirely clear. In 2021 it was described the linear power, which was superior to other markers. At the moment its possible utility in patients with ARDS due to COVID-19 has not been described.</p><p><strong>Objective: </strong>To evaluate linear power as a risk factor for mortality in patients with ARDS due to COVID-19 in intensive care.</p><p><strong>Material and methods: </strong>Retrospective cohort study in patients admitted to intensive care with ARDS secondary to COVID-19. Linear power was calculated for patients who died and patients who survived in intensive care. Mann-Whitney U test and multivariable Cox regression (hazard ratio [HR] with 95% confidence intervals [95% CI]) were performed.</p><p><strong>Results: </strong>60 patients were studied with a mortality of 43.3%. Frequencies and means were obtained. For the differences, chi-squared and Student's t were used. Odds ratios (OR) were obtained to assess the risk.</p><p><strong>Results: </strong>ORs were determined in the group of women with CIN II/CIN III, obtaining risks of almost twice the risk factors analyzed compared to the CIN I group, which had the same factors. When analyzed by age groups < or > 40 years, significant associations were determined in the factors of parity, first cytology in life and positive for smoking. Neurosyphilis is an infectious cause of RPD and it is classified clinically into early and late forms. The latter affect the brain and spinal cord parenchyma. The objective of this report is to present a case of RPD caused by neurosyphilis.</p><p><strong>Clinical case: </strong>55-year-old man, with no past medical history, who in July 2022 had a clinical presentation of aggressiveness, irritability, lability, difficulty in performing simple activities, amnesia, hallucinations, delusions, abulia, low responsiveness, all of which affected his independence and quality of life. Initially, it was treated as schizophrenia with no signs of improvement and clinical deterioration. When atypical data and rapid progression were evident at patient's consultation in January 2023, there was a suspicion of the presence of RPD, highlighting positive VDRL and IgM-IgG anti-Treponema pallidum antibodies in cerebrospinal fluid (CSF). Computed tomography (CT) and magnetic resonance imaging (MRI) showed global cortical atrophy, predominantly frontal.
