Treatment of pediatric patients with acute lymphoblastic leukemia (ALL) has a negative effect on bone metabolism.</p><p><strong>Objective: </strong>To describe the frequency of hypovitaminosis D, changes in serum 25(OH)D concentrations, and the biochemical and hormonal markers of bone metabolism between diagnosis and referral in children with ALL.</p><p><strong>Material and methods: </strong>Prospective, longitudinal, cohort study in pediatric patients (4-17 years) with newly diagnosed B-cell ALL, and without treatment. Patients were summoned under fasting conditions, and a peripheral blood sample was taken to determine 25(OH)D, parathyroid hormone (PTH), phosphorus, and ionic calcium concentrations.</p><p><strong>Results: </strong>Of the 40 patients recruited, 7 were lost to follow-up, so only 33 were analyzed. 91% of the children had hypovitaminosis D. We observed a significant increase in ionic calcium concentrations from baseline levels (1.1 mmol/L vs. 1.2 mmol/L, p = 0.002). Analysis of the Δ of serum PTH and 25(OH)D concentrations showed an inverse relationship (rho = -0.397, p = 0.024).</p><p><strong>Conclusions: </strong>These findings confirm a high frequency of hypovitaminosis D. Both are related to the development of various chronic diseases. It is important to describe and identify the main risk factors involved in VD deficiency in this population.</p><p><strong>Objective: </strong>To describe VD intake in obese Mexican adolescents and evaluate its association with serum 25(OH)D concentrations.</p><p><strong>Material and methods: </strong>A cross-sectional study was performed in 80 obese adolescents from Mexico City. Sociodemographic, anthropometric, dietary intake, sun exposure, and physical activity data were collected. A blood sample was taken to determine serum 25(OH)D concentrations.</p><p><strong>Results: </strong>VD deficiency and insufficiency occurred in 30% and 42.2% of adolescents. The median VD intake was 110.4 IU/day and the mean sun exposure score was 13 ± 7.5. Serum 25(OH)D levels were correlated with vitamin D intake (p = 0.010). Concentrations of 25(OH)D in serum were similar when compared between sex, sunscreen use, season, and skin type (p > 0.05).</p><p><strong>Conclusions: </strong>VD deficiency in adolescents with obesity is high. In contrast with its dietary intake and degree of sun exposure which are considerably low. Their concentrations depend on maternal diet and endogenous supply, but recommended levels are not often reached.</p><p><strong>Objective: </strong>Support with evidence the DHA and AA benefits and recommendations for the infant, during pregnancy and breastfeeding.</p><p><strong>Material and methods: </strong>Narrative review. Search was made using the keywords: recomendaciones, inflamación, inmunidad, ácidos grasos, omega-3, DHA, leche materna, pretérmino, embarazo, recién nacido, suplementación; as well LC-PUFA, human milk, preterm, immunity, DHA, pregnancy, supplementation, recommendations, from 2020-2024; if there was no information in this period, the up-to-date reference was reported.</p><p><strong>Results: </strong>DHA recommendations during pregnancy and breastfeeding: 200-450 mg/day; if the DHA tissue reserve was low in pregnancy, administer 1,000 mg/day. In the preterm infant: 30-100 mg of AA/kg/day; 30-65 mg of DHA/kg/day. Infant 7-23 months 100 mg of DHA/kg/day. {"title":"[Mammary epithelial cells from women synthetize omega-3 and 6 fatty acids].","authors":"Imelda Cecilia Zarzoza-Mendoza, Elizabeth Sosa-Castillo, Mariela Bernabe-García, Cristian Emmanuel Luna-Guzmán, Maricela Morales-Marzana, Jorge Maldonado-Hernández, Maricela Rodríguez-Cruz","doi":"10.5281/zenodo.14199863","DOIUrl":"10.5281/zenodo.14199863","url":null,"abstract":"<p><strong>Background: </strong>The metabolic regulation for the synthesis of human milk (HM) long-chain polyunsaturated fatty acids (LC-PUFAs) omega-3 and omega-6 is poorly known; it is due to the limitations in obtaining breast tissue.</p><p><strong>Objective: </strong>To determine whether the epithelial cells of human breast tissue (ECHBT) express the enzymes that synthesize LC-PUFAs, and to analyze the expression changes in the different types of HM and its correlation with the percentage of LC-PUFAs.</p><p><strong>Methods: </strong>In colostrum, transition and mature HM of 6 women, the fatty acid composition was analyzed by gas chromatography and the mRNA expression of the desaturases δ5D, δ6D and elongase Elovl-5 in the ECHBT was analyzed by qPCR.</p><p><strong>Results: </strong>tissue express the mRNA of δ5D, δ6D and Elovl-5 for the synthesis of LC-PUFAs, this expression increases in mature milk with respect to colostrum. The percentage of linoleic and alpha-linolenic acid is the similar in the three types of milk, while that of arachidonic acid and docosahexaenoic acid decreased (8% and 14% respectively) in mature milk compared to colostrum. The mRNA of δ5D, δ6D correlated with the percentage of arachidonic acid, that of δ6D and Elovl-5 correlated with the percentage of docosahexaenoic acid (DHA).</p><p><strong>Conclusions: </strong>The ECHBT express the enzymes that synthesize LC-PUFAs and their expression increases in mature milk, to meet the demand of the newborn. Several studies suggest that supplementation with these fatty acids decreases metabolic risk, plasma triglyceride levels and attenuates the loss of lean body mass, modulating the inflammatory response. In adults with cancer, the results of different studies that have evaluated the effects of the administration of these fatty acids suggest that they can be used as adjuvants during treatment acting as receptor agonists, modulating different molecular pathways, reducing the inflammatory response, increasing the efficacy of chemotherapy and consequently improving the overall survival of cancer patients. The role of nutrition in onco-hematological treatment has not been sufficiently understood, although nutritional status is a modifiable prognostic factor. Currently, the evidence demonstrating the effect of supplementation with these fatty acids in pediatric patients with leukemia is limited. It reports benefits related to the preservation of lean body mass, control of dyslipidemias, cardiovascular risk factors, and methotrexate hepatotoxicity during therapy. Obesity, a common feature of DMD, exacerbates this inflammation. Although corticosteroids are the conventional treatment, their adverse effects are substantial. The use of omega-3 polyunsaturated fatty acids (PUFAω-3) as adjuvant therapy is under investigation. A comprehensive review of studies from 2011 to May 2024 reveals that supplementation with PUFAω-3 has beneficial effects in mdx mice, a model of DMD. These include reduction of the inflammatory process in both muscle and circulation, in addition to decreased myonecrosis, resulting in improved muscle strength and endurance. Supplementation also had a positive impact on cardiac tissue, reducing fibrosis and inflammation present in this tissue. These results were corroborated in patients with DMD. A significant decrease in inflammatory markers such as NF-kB, IL-1β and IL-6 was observed with PUFAω-3, and an increase in the anti-inflammatory cytokine IL-10, suggesting a potential anti-inflammatory effect. Within the framework of this celebration, we have prepared a special supplement for the Mexican Institute of Social Security (IMSS) Medical Journal, with the purpose of spreading useful information for health professionals and society, regarding to two lines of research that have been decisive for the historical development and consolidation of our workgroup: the therapeutic use of omega-3 polyunsaturated fatty acids and the study of vitamin D. This Editorial also aims to briefly describe the global and national environment of nutritional status that prevails today, outline some characteristics regarding food and nutrition in Mexico, and report the findings and lines of research of enhanced scope and relevance during these 40 years of the MNRU. However, to the best our knowledge, no study has been carried out to detect CTCs in women with early BC in Mexico.</p><p><strong>Objective: </strong>Calculate the detection rate of CTCs in peripheral blood of Mexican women with early BC by quantification of the relative expression of KRT19, MUC1 and SCGB2A2.</p><p><strong>Material and methods: </strong>Observational, longitudinal, analytical study, which included 62 Mexican women, over 18 years of age, with BC clinical stage 0, I and II. Cells with a density < 1.077 g/ml were isolated from 4 mL of peripheral blood obtained 24 h before surgery, RNAS was extracted and RT-qPCR was performed to quantify the relative expression of KRT19, MUC1 and SCGB2A2.</p><p><strong>Results: </strong>The diagnosis of BC was confirmed in the definitive pathological study in 59 women. 1 case (1.7%), expressed KRT19, 18 cases (30.5%) MUC1, 2 cases (3.4%) SCGB2A2, and 19 cases (32.2%) expressed at least one of the three biomarkers.</p><p><strong>Conclusions: </strong>CTCs detection rates were like those reported in other populations when MUC1 was used, but lower when KRT19 and SCGB2A2 were used. Detecting and treating CKD in its early stages is essential for prognosis and use of less invasive renal treatments. CKD can manifest with different markers of renal damage. However, few studies evaluate the presence of erythrocyte morphological changes in the peripheral blood of patients with CKD.</p><p><strong>Objective: </strong>To evaluate the correlation between erythrocyte morphological changes and KDIGO stage as a predictor of the severity of renal damage.</p><p><strong>Material and methods: </strong>A descriptive, prospective, cross-sectional, and correlational study of patients admitted between March and April 2023. Qualitative variables were reported as frequencies and percentages, and quantitative variables were reported by central tendency and dispersion measures. Finally, Spearman correlation was performed between erythrocyte morphological alterations and the KDIGO stage in patients with CKD.</p><p><strong>Results: </strong>A total of 385 patients were included, and the correlation of the number of erythrocyte morphological alterations with the KDIGO stage showed that it is directly proportional in a significant way (Spearman's ρ 0.489, p < 0.0001). {"title":"[Clinical, demographic and histopathological characteristics of individuals with Kaposi's sarcoma and living with HIV in Mexico].","authors":"Melissa Lomelí-Canedo, Beatriz de Guadalupe Pizano-Márquez, Mario Magaña-García","doi":"10.5281/zenodo.13306755","DOIUrl":"10.5281/zenodo.13306755","url":null,"abstract":"<p><strong>Background: </strong>Kaposi's sarcoma (KS) is the most frequent cancer in HIV-positive people in developing countries, affecting their quality of life.</p><p><strong>Objective: </strong>To analyze the clinical, histopathological and epidemiological features of KS in patients living with HIV, as well as to evaluate the impact of antiretroviral treatment on the incidence and progression of the disease.</p><p><strong>Material and methods: </strong>Observational, descriptive, retrospective and cohort study. During October 2022 to August 2023. Twenty-eight HIV-positive male patients were included and were diagnosed with KS in our institution. Clinical variables, CD4+ cell counts, viral load and treatments received were evaluated, in addition to histopathological and immunohistochemical analysis of the lesions.</p><p><strong>Results: </strong>Most patients had low CD4+ cell counts and high viral load at diagnosis. The most common lesions were erythematous-violaceous papules, nodules and and plaques, in multiple areas of the body. HHV8 was identified in most cases. Antiretroviral therapy showed a long-term protective effect on the incidence of KS. Opportunistic infections were frequent in these patients, the most common one was tuberculosis.</p><p><strong>Conclusions: </strong>Our study highlights the need for an early and multidisciplinary approach in the management of KS in HIV-positive patients, underlining the importance of antiretroviral treatment to prevent disease progression.
