The two main causes are thromboembolic diseases and infiltrative hematological disorders. The splenic abscess is the result of a hematogenous seeding, whose causes are trauma, splenic artery embolization, endocarditis, and immunocompromised states.</p><p><strong>Clinical case: </strong>48-year-old male patient with a history of chronic myelomonocytic leukemia on mercaptopurine-based treatment, who presented intermittent asthenia, adynamia, and abdominal distension for 2 months; a computed tomography of the abdomen with intravenous contrast was performed, reporting areas of infarction and an image suggestive of a splenic abscess in the upper pole, for which an open splenectomy was performed with adequate clinical evolution. However, there is a percentage of patients who do not achieve acceptable myocardial perfusion and this is associated with adverse cardiovascular outcomes. Different adenosine schemes have been used as an adjuvant to restore adequate myocardial perfusion.</p><p><strong>Objective: </strong>To compare the final angiographic flow in patients undergoing pharmacoinvasive percutaneous coronary intervention (PCI) with the use of intracoronary adenosine versus patients without its use.</p><p><strong>Material and methods: </strong>Single-center, prospective, longitudinal, single-blind study carried out in patients with AMI, thrombolyzed, with reperfusion data, sent to a tertiary care center from March 2022 to June 2023, randomized to receive intracoronary adenosine previous to intervention. Drug-eluting stents were placed in all patients. Initial and final angiographic flow was compared.</p><p><strong>Results: </strong>33 patients were included, randomized 1:1 to control and intervention groups. 75.8% of the participants were male. The most frequent artery responsible for the infarction was the anterior descending (63.6%). An average of 350 mcg of intracoronary adenosine was used in the intervention group, and the most frequent complication in both groups was the presence of no-reflow phenomenon.</p><p><strong>Conclusions: </strong>Due to the small sample size, it is not possible to conclude the usefulness of adenosine for improving final angiographic flow in pharmacoinvasive PCI. There is no international consensus on the definition and number of RDs, which affects the availability of resources for their diagnosis, treatment, and research.</p><p><strong>Objective: </strong>To determine the prevalence of RDs in the Medical Genetics Service of a regional general hospital belonging to the Mexican Institute for Social Security (IMSS), in Puebla, Mexico.</p><p><strong>Material and methods: </strong>The patients' records who were evaluated by the above-mentioned service from January 2019 to June 2022 were reviewed. Those patients with a diagnosis of a RD were identified, and the prevalence was obtained by using the formula: total number of cases of the disease/number of people in the population at that moment in time.</p><p><strong>Results: </strong>A total of 798 medical records were reviewed and a prevalence of RDs of 27% was obtained. Those diseases with a prevalence of 1 case per 2000 inhabitants were considered, being 118 different RD. Considering only the 20 rare diseases registered in Mexico in 2022, 11 of these were detected, distributed in 35 patients, with an estimated prevalence of 4.3%.</p><p><strong>Conclusion: </strong>The prevalence of RDs differs according to the criteria implemented. Prevalence of mental and behavioral disorders (MBD) in Mexico oscilates between 15 and 18%. The occurrence of these diseases is the support for making decisions aimed at improving the medical care and well-being of the psychiatric patient.</p><p><strong>Objective: </strong>To estimate the prevalence of MBD in population affiliated to the Mexican Institute for Social Security (IMSS) in 2021.</p><p><strong>Material and methods: </strong>Cross-sectional and observational study. We examined the Census of Mental and Behavioral Disorders in Population affiliated to IMSS in 2021. All the cases of the census were included: 2,652,531.</p><p><strong>Results: </strong>The prevalence of MBD was 9.3%. The higher prevalence was reported for anxiety and obsessive compulsive disorder (39.8%). The highest prevalence was reported in the Michoacán Administrative Operation Office (AOO) (67.7%, 95% CI 67.5-68.0), then the Aguascalientes and Guerrero AOOs (64.6%, 95% CI 64.2-64.9 and 60.1%, 95% CI 59.7-60.5, respectively).</p><p><strong>Conclusions: </strong>The prevalence of MBD in the population affiliated to IMSS is 9.3%. A case of spinal needle breakage is presented, pointing out its risk factors and recommendations to reduce it.</p><p><strong>Clinical case: </strong>5-year-old female patient, who was scheduled for removal of translocated intrauterine device causing chronic pain. When attempting a spinal anesthesia, stylet was removed from the needle and directed into the subarachnoid space. Upon extraction, the spinal needle was deformed and shortened. Immediately and with fluoroscopy and local anesthesia, the broken fragment was extracted without complications. Regional anesthesia is widely used in pregnant women, as it is considered safe. However, there are factors that increase the probability of a spinal needle breakage (obesity, pregnancy, emergency procedures, incorrect handling of the spinal needle, etc.). This article derives from the Mexican Registry of Dyslipidemias in patients at high-risk and very high-risk of atherosclerotic cardiovascular disease (REMEXDIS-IMSS Project). Patients with high-risk and very high-risk criteria for ACVD were included.</p><p><strong>Results: </strong>From July 2022 to March 2023, 6000 patients were included (3289 patients in the high-risk group and 2771 in the very high-risk group). The frequency of dyslipidemia was observed in 49% of the cohort. The very high-risk group presented a higher percentage of dyslipidemia in 66.8%, acute myocardial infarction in 81.0% and angina pectoris in 21.9%. The use of statins was higher in this group (93.3%) and the LDL-c goal was achieved in 72.9% (p < 0.0001).</p><p><strong>Conclusions: </strong>The frequency of dyslipidemia is found in almost half of the population. Guillain Barre syndrome (GBS) and the Miller-Fisher syndrome (MFS) have been related to COVID-19 vaccine application.</p><p><strong>Clinic case: </strong>48-year-old man, with history of Pfizer-BioNTech vaccination against COVID-19, 5 days prior to the symptoms, who assisted to the Emergency room with blurred vision and diplopia; adding dysarthria, facial diplegia and left upper limb weakness after 48 hours. In his first evaluation it was found ophthalmoplegia, facial diplegia, decreased gag reflex, weakness of thoracic limbs, bilateral trapezius muscle and areflexia. Serum studies and nuclear magnetic resonance of the brain were performed without alterations. It was complemented with IgG anti-ganglioside GQ1b antibodies with a positive result. Once the diagnosis was confirmed, treatment was started with immunoglobulin calculated at 2 g per kg for 5 days. The patient was discharged once the immunoglobulin was administered with evaluation at 2 months without ataxia, ophthalmoplegia, areflexia and weakness.</p><p><strong>Conclusions: </strong>Following the documented reports of GBS and its variants secondary to vaccination, neurological side effects have been catalogued as being of great importance. It is the most common cutaneous lymphoma in childhood. It is characterized by hypopigmented macules in non-photoexposed areas, generally asymptomatic. The diagnosis is confirmed with histopathological study and immunohistochemistry. Treatment will depend on the clinical stage, with topical therapies being the first-line treatment.</p><p><strong>Clinic case: </strong>69-year-old female, with no significant personal pathological history. She was treated for dermatosis of 7 years of evolution, spread to the 4 segments with affected body surface of 35% that respected palms and soles, characterized by polymorphic hypochromic macules of 1 to 10 cm in diameter that converge to form larger lesions, and some post-inflammatory hyperchromic macules, accompanied by itching. The histopathological study reported data compatible with mycosis fungoides and immunohistochemistry with positivity for CD3, CD4, CD5 and CD8. It was diagnosed stage IB HMF. It was administered treatment based on prednisone and phototherapy with a good response.</p><p><strong>Conclusions: </strong>This case makes possible an outlook of the demographic data of HMF, and it allows this entity to be taken into consideration for possible differential diagnoses that are not contemplated due to the limited dissemination of knowledge about this disease. This survey included 349 adults of 18 years of age or older with hypertension diagnosis and antihypertensive pharmacological treatment. HL was measured with the European Health Literacy Survey HLS-EU-Q12M, validated in Mexico. The characteristics of participants with and without BP control were compared with chi-square. Multiple Poisson regression was applied to assess the association between HL and BP control, and it was controlled based on the clinically important covariates.</p><p><strong>Results: </strong>60.2% of patients with hypertension had BP controlled; 23.8% had inadequate HL, 44.4% problematic HL, and 31.8% sufficient HL. Sufficient HL was associated with BP control only in patients with high school or higher education (adjusted prevalence ratio 2.18, 95% confidence interval 1.16-4.08).</p><p><strong>Conclusions: </strong>Only 60% of hypertensive patients achieve control of their BP. The population included patients receiving HD in a third-level hospital. The Malnutrition and Inflammation Score (MIS) was administered to patients who agreed to participate. Together with the identification card, data on name, age, sex, body mass index (BMI), leukocytes, albumin, transferrin level, and HD time were collected.</p><p><strong>Results: </strong>52 patients were included. Mean age was of 46.51 years (15.54); 34 patients (65.4%) were male and 18 female (34.6%); 1 (1.9%) presented normal MIS, 32 (61.5%) mild MIS, 18 (34.6%) moderate MIS, and 1 (1.9%) severe MIS.</p><p><strong>Conclusions: </strong>Nutritional status is poor in patients with CKD receiving HD, which is why it must include a monitoring protocol that translates into a benefit in the face of patient morbidity and mortality.</p>","PeriodicalId":94200,"journal":{"name":"Revista medica del Instituto Mexicano del Seguro Social","volume":"62 3","pages":"1-6"},"PeriodicalIF":0.0,"publicationDate":"2024-05-06","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"142635127","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
