However, there is controversy about the factors that increase the risk of CI.</p><p><strong>Objective: </strong>To determine the factors associated with cognitive impairment in older adults.</p><p><strong>Material and methods: </strong>We conducted a cross-sectional, analytical, observational, retroprolective study that included adults ≥65 years of age, with no history of cerebral vascular event, cranioencephalic trauma. Demographic factors were analyzed, CI was assessed with the Mini Mental State Examination test. {"title":"[Risk factors associated with lethality on patients with central cord syndrome].","authors":"Patricio Dabdoub-González, Edgar Abel Márquez-García, Juan Gerardo Reyes-García, Jorge Arturo Cabrera-Escamilla, David Fernando Servín-Carmona, Víctor Hugo Tejerina-Gorena, Cristóbal Herrera-Palacios, Elvia Janike Rodríguez-Córdoba","doi":"10.5281/zenodo.8319828","DOIUrl":"10.5281/zenodo.8319828","url":null,"abstract":"<p><strong>Background: </strong>Central Cord Syndrome is the most common incomplete spinal cord injury, and it represents 9% of all spinal injuries of the adult.</p><p><strong>Objective: </strong>to determine the risk factors associated with lethality or/and mechanical ventilation (MV) in patients with Central Cord Syndrome (CCS).</p><p><strong>Material and methods: </strong>upon a retrospective cohort with patients with posttraumatic CCS we evaluated at the time of emergency admission and until the hospital discharge. The dependent variable was dead or MV. We calculated incidence, relative risk (RR) with CI95% and a multivariate model for the association of statistically significant variables by means of a risk coefficient model upon the variables that evidenciated tendencies towards risk.</p><p><strong>Results: </strong>From 101 patients with CCS, 85.1% with a severe medullary canal stenosis and 9.9% required MV; the mortality was 13.9%. The only risk factor associated with dead was the use of MV with an RR of 3.6 (CI95% 1.4-9.5); the risk with tendencies towards MV was being older than 60 RR 5.4 (CI95% 0.6-44.2). Other factors demonstrated a tendency towards mortality, but they were not statistically significant.</p><p><strong>Conclusions: </strong>MV is a risk factor associated with mortality, other factors evidenciated tendencies towards mortality; being older than 60, hypertension, diabetes mellitus, narrow spinal canal, more than 20 days of hospital stay and being intervened farther than 10 days from the accident. The incidence of mortality in our sample is greater than thus reported previously on other international articles. The factors associated SSc-ILD have shown variability in different populations. There are few studies in Mexican mestizos.</p><p><strong>Objective: </strong>To analyze the epidemiological, clinical and paraclinical factors associated with SSc-ILD.</p><p><strong>Material and methods: </strong>Cross-sectional study, where patients > 18 years of age with a diagnosis of SSc according to EULAR/ACR 2013 criteria and diagnosis of ILD by forced vital capacity (FVC) < 70% and > 5% of affected lung area on tomography were included. The strength of association of the factors for ILD was measured by odds ratio (OR) with 95% confidence intervals (95% CI). The significant variables were analyzed by multiple logistic regression with adjustment.</p><p><strong>Results: </strong>Of the 80 patients with SSc, 38 (47.5%) had ILD. Risk factors such as smoking, higher activity index, lower FVC, anti-topoisomerase I antibodies, and protective factors such as a limited SSc, early capillaroscopic pattern, and anti-centromere antibodies (ACA) were associated with ILD. {"title":"[Efficacy and safety of atorvastatin in major cardiovascular events: Meta-analysis].","authors":"Víctor Eder Villegas-Quintero, Rodolfo Rivas-Ruíz, Alexis Alejandro García-Rivero, Pedro Rivera-Lara, Nelly Berenice González-Tovar","doi":"10.5281/zenodo.8319748","DOIUrl":"10.5281/zenodo.8319748","url":null,"abstract":"<p><strong>Introduction: </strong>Atorvastatin has been used in the management of dyslipidemia and little is known about the efficacy and safety of high-dose atorvastatin administration for secondary prevention of Major Cardiovascular Events (MACE).</p><p><strong>Objective: </strong>To evaluate the impact of high-dose atorvastatin on secondary prevention of MACE and adverse events.</p><p><strong>Material and methods: </strong>A systematic review and meta-analysis of Pubmed, Embase, Bireme and Cochrane Library Plus databases was performed, with a time scope from 1990 to July 2022. Six randomized clinical trials were included with a total of 29,333 patients who were treated with 80 mg, 10 mg or placebo doses of Atorvastatin where the main outcomes evaluated were Major Cardiovascular Events (MACE), mortality and treatment safety.</p><p><strong>Results: </strong>In the comparative study between the use of Atorvastatin 80 mg and other therapies, a relative risk (RR) of 0.8 (95%CI 0.69-0.92) was found, representing a 20% reduction in risk (RRR) and a number needed to treat (NNT) of 30-55. In the analysis of adverse effects, an RR of 2.37 (95% CI 0.86-6.53) and a number needed to harm (NNH) of 14-19 were observed. The use of 80 mg atorvastatin is associated with similar adverse events at lower doses.</p><p><strong>Conclusions: </strong>The use of atorvastatin 80 mg is effective in the secondary prevention of Major Cardiovascular Event (MACE). The BioNTech/Pfizer lab was the first to get the approval from the Food and Drug Administration (FDA) for vaccine application, beginning with health workers. However, there are a few studies about adverse effects after applying the COVID-19 vaccine.</p><p><strong>Objective: </strong>To assess the presence and time of onset of adverse effects produced by the COVID-19 vaccine.</p><p><strong>Material and methods: </strong>A retrospective cohort study was carried out, including medical residents from the Hospital de Especialidades No. 1 (Specialties Hospital No. 1) from the Bajío National Medical Center, who received two doses of Pfizer-BioNTech vaccine and decided to participate voluntarily. We obtained the information by a self-assessment tool sent via email. All adverse effects were collected emphasizing those described in the literature.</p><p><strong>Results: </strong>A total of 101 medical residents were included. It was observed that 55.54% presented adverse effects after the first dose, which appeared in the first 24 hours after the vaccine was administered. Pregnant patients were included, divided into group 1 (PE without severity criteria); group 2 (PE with severity criteria); group 3 (normotensive pregnant women). Records were reviewed and inflammatory indexes [(neutrophil-lymphocyte ratio [NLR], platelet-lymphocyte ratio [PLR], lymphocyte-monocyte ratio [LMR] and systemic immune index [SII]) were calculated.</p><p><strong>Results: </strong>240 patients were analyzed, 80 per group; age 28 (IQR 27-29) years. A significant difference was observed between group 1, group 2 and group 3, NLR 3.29 (IQR 2.82-3.69), 3.59 (IQR 2.83-4.2) and 3.42 (IQR 3.17-3.92), respectively, p = 0.02; PLR 121.59 (IQR 103.78-132), 108.32 (IQR 92.96-127.43) 136 (IQR 115.18-157.56), respectively, p = 0.01; LMR and SII showed no difference between the groups. LMR made possible to distinguish PE with and without severity criteria, cut-off point of ≥ 3.20, sensitivity 56%, specificity 56%, AUC 0.56, p = 0.01, and cut-off point ≥ 3.24, sensitivity 58%, specificity 58%, AUC 0.57, p = 0.04, respectively; the LMR ≥ 3.24 was associated with PE without severity criteria (OR 2.02 [95%CI 1.08-3.80], p = 0.03).</p><p><strong>Conclusions: </strong>The MLR was the only inflammatory index analyzed that was associated with the presence of PE without severity data. Accretism is strongly associated with a history of uterine surgery (cesarean section, myomectomy, curettage), as well as ultrasonographic signs, such as the presence and size of placental lacunae, loss of the placenta/bladder interface, location on the anterior face of the placenta, and presence of Doppler flow; these markers can be assessed by prenatal ultrasound.</p><p><strong>Objective: </strong>To analyze the association of prenatal diagnosis of placenta accreta by ultrasound with the histopathological result using the Tovbin index.</p><p><strong>Material and methods: </strong>Observational, cross-sectional and analytical study. 63 patients who had placenta accreta data by ultrasound measured with the Tovbin index and by means of the histopathological result obtained from the platform of the Mexican Institute for Social Security (IMSS) were included. The association between the two studies with the presence of placenta accreta was analyzed.</p><p><strong>Results: </strong>63 patients were analyzed; the Tovbin index was positive in 89% of the patients with a diagnosis of placenta accreta confirmed by histopathology.
