We aimed to analyse the results of the survey and infer practical recommendations for clinical practice.</p><p><strong>Methods: </strong>A group of ear, nose, and throat specialists (ENTs) experienced in the management of CRSwNP developed a 74-question survey. ENTs from rhinology centres authorised to prescribe biologics in the context of the national health system were invited to answer it between 01/05/2022 and 31/07/2022. The responses underwent descriptive analyses, and the authors discussed the results and derived practical recommendations for clinical practice.</p><p><strong>Results: </strong>ENTs working in rhinology centres changed their practices coinciding with the advent of biologics. CRSwNP evaluations have become more complex because they involve diagnostic confirmation, determining the patients' immunologic profile, and other factors. We observed heterogenous behaviours in practice that may be conditioned by the novelty of the topic. The results of the survey were used to develop practical recommendations for ENTs and are summarised herein.</p><p><strong>Conclusions: </strong>Clinical practice in rhinology outpatient clinics has changed profoundly in the era of biologics. Its English version has been validated for use with a paediatric population between 5 and 12 years of age. The aim of this work is to validate the DHI-PC into Italian for both patient assessment and appropriate rehabilitative treatment planning.</p><p><strong>Materials and methods: </strong>Cross-cultural adaptation of the DHI-PC was performed using standard techniques. Items of the original questionnaire were translated into Italian by two bilingual investigators. Two native English speakers carried out a back translation of the new version that was compared with the original to check that they had the same semantic value. A pre-final version was obtained by an expert committee and was applied in a pilot test.</p><p><strong>Results: </strong>A total of 42 patient caregivers completed the final adapted questionnaire twice with an interval of 2 weeks. The aim of this study was to assess tumour behaviour and prognostic factors impacting overall survival (OS) in a retrospective, single institution series.</p><p><strong>Methods: </strong>A retrospective study on patients affected by HN-ASCC who were treated surgically between 2002 and 2019 at the Department of Otorhinolaryngology - Head and Neck Surgery of the University of Brescia was conducted. Demographics, clinical data, OS, and relative prognostic factors were analysed.</p><p><strong>Results: </strong>The study included 32 patients, with a median age of 66 years, mostly males (84.4%) and untreated (68.8%). Adjuvant treatments followed surgery in 28.1% of patients. Compared to conventional SCC, ASCC showed a higher proportion of cases arising in the larynx (40.6%); no difference was found in other features. Advanced (pT3-4) local stage at presentation (p = 0.023), perineural invasion (PNI, p = 0.01), and positive margins (p = 0.007) were independent negative prognostic factors for OS.</p><p><strong>Conclusions: </strong>HN-ASCC is a rare, aggressive cancer, most frequently arising in the larynx of elderly males, usually diagnosed in an advanced local stage. The analysis of the acoustic signals through the implementation of machine learning models can be a viable solution aimed at offering support for clinical diagnosis.</p><p><strong>Materials and methods: </strong>In this study, a support vector machine was trained and cross-validated (10-fold cross-validation) using 138 features extracted from the acoustic signals of 418 patients with polyps, nodules, oedema, and cysts. The model's performance was presented as accuracy and average F1-score. The results were also analysed in male (M) and female (F) subgroups.</p><p><strong>Results: </strong>The validation accuracy was 55%, 80%, and 54% on the overall cohort, and in M and F, respectively. Better performances were observed in the detection of cysts and nodules (58% and 62%, respectively) vs polyps and oedema (47% and 53%, respectively). The results on each lesion and the different patterns of the model on M and F are in line with clinical observations, obtaining better results on F and detection of sensitive polyps in M.</p><p><strong>Conclusions: </strong>This study showed moderately accurate detection of four types of BLVF using acoustic signals. The pathophysiology may be related to various concomitant factors including central nervous system diseases, peripheral neuropathy, osteoarticular disorders (leading to a reduction of the speed gait), and cardiovascular or visual disorders, but it may also be a consequence of deficits of the peripheral or central vestibular system (which is generally represented by a bilateral and symmetric vestibular dysfunction). Benign paroxysmal positional vertigo (BPPV) is far from being rare in subjects over the age of 70 years. Nowadays, presbystasis represents one of the main causes of accidental falls in the elderly, leading to a reduction in quality of life and life expectancy. The aim of our work was to review the current literature on presbystasis, focusing on the role of the vestibular system in the pathogenesis and on the possibilities offered by vestibular rehabilitation in the management of these subjects. {"title":"Italian validation of the Hearing Handicap Inventory for Elderly - Screening version (HHIE-S-It).","authors":"Enrico Apa, Luca Sacchetto, Silvia Palma, Chiara Cocchi, Chiara Gherpelli, Elisabetta Genovese, Daniele Monzani, Riccardo Nocini","doi":"10.14639/0392-100X-N2297","DOIUrl":"https://doi.org/10.14639/0392-100X-N2297","url":null,"abstract":"<p><strong>Objective: </strong>Validate the Italian version of the Hearing Handicap Inventory for Elderly - Screening version (HHIE-S-It).</p><p><strong>Methods: </strong>After translation, psychometric properties and attributes were analysed by administering the HHIE-S-It to 167 elderly outpatients together with the Psychological General Well-Being Index (PGWBI).</p><p><strong>Results: </strong>The Cronbach's α coefficient was 0.908 for the total score, and 0.832 and 0.816 for its two subscales. Significant test-retest reliability was observed (p < 0.001). Moderate to high correlations were found between HHIE-S-It and pure tone average in the better ear (p < 0.001). The ANOVA test confirmed the significant difference in HHIE-S-It scores across groups according to the degree of hearing loss (p < 0.001). Only very low and low significant correlations were observed between HHIE-S-It and PGWBI. The criterion HHIE-S-It > 11 was observed as the best cut-off with highest sensitivity (86.4%), specificity (72.4%), positive predictive value (52.8%), negative predictive value (93.7%) and likelihood ratios (3.12 and 0.19).</p><p><strong>Conclusions: </strong>Since the HHIE-S-It presented acceptable psychometric properties, its adoption is justified for both clinical and research purposes. Four angular CI orientations were adopted: 90, 120, 135 and 160 degrees. Volunteers were scanned in each condition using T1w and T2w TSE sequences, as well as O-MAR sequences, in both axial and coronal planes. Quantitative comparisons were made of signal void and penumbra extent. Additionally, qualitative evaluations of global image quality, MRI readability with respect to 12 anatomical structures and visibility through the penumbra were undertaken.</p><p><strong>Results: </strong>After application of the O-MAR protocol, the radius of the signal void reduced from 50.76 mm to 45.43 mm and from 49.22 mm to 40.15 mm on T1w and T2w TSE axial sequences, respectively (p < 0.05). Qualitatively, sequences acquired with O-MAR produced better outcomes in terms of image quality and anatomical depiction.
