La prédominance masculine durant l’enfance et féminine à l’âge adulte est observée dans diverses maladies atopiques. Les garçons montrent une prévalence plus élevée d’allergies alimentaires dans l’enfance, une tendance qui s’inverse chez les femmes jeunes avant de revenir à l’équilibre après la ménopause. Les hormones sexuelles jouent un rôle clé dans ces différences, modulant la dégranulation des mastocytes et favorisant ainsi les réactions allergiques. Au-delà de la prévalence, le sexe influence également la sévérité des allergies alimentaires. Les femmes adultes sont plus susceptibles de souffrir de réactions sévères, y compris d’anaphylaxie alimentaire induite par l’effort, tandis que les garçons présentent un risque accru de poly-allergie alimentaire durant l’enfance. Cependant, des paradoxes subsistent, comme la sévérité accrue de l’allergie à l’arachide chez les filles malgré une prévalence plus élevée chez les garçons. Cela souligne l’influence potentielle du genre et du comportement. La réponse aux traitements, notamment l’immunothérapie orale (ITO), varie également selon le sexe. Les filles présenteraient moins de réactions sévères avant 13 ans, tandis que les garçons en auraient moins après cet âge. En conclusion, une meilleure compréhension des influences sexospécifiques sur l’allergie alimentaire pourrait optimiser la prévention, le diagnostic et les traitements. Diagnosing and management of paradoxical pustular psoriasis in patients undergoing TNF-α antagonists treatment for psoriasis can be challenging. Therefore, we present a case report highlighting the occurrence of paradoxical pustular psoriasis caused by infliximab in a patient undergoing infliximab treatment for psoriasis, two weeks after the ninth injection. A switch to another TNF-α antagonist was considered, with no relapses at 6-month follow-up. Umeclidinium bromide is an anticholinergic inhaled treatment indicated in the background treatment of COPD, for which marketing authorization was obtained in 2014. The most frequent adverse reactions are: nasopharyngitis, headache, tachycardia, cough or pharyngeal pain, constipation, urinary tract infection or sinusitis. Effects related to antimuscarinic activity are of undetermined frequency. The prevalence of celery allergy remains low in the pediatric population, but is higher in adults, with a high risk of anaphylaxis. Celery allergens differ from one another in their characteristics: content within the different types of celery, sensitivity to heat, link with certain phenotypes (pollen-food syndrome). For diagnosis, native prick-tests are more sensitive than commercial extracts. Source-specific IgE may fail in a third of cases, and recombinant allergens are sometimes useful. Oral provocation testing is essential in cases of diagnostic doubt. The natural history of this allergy is not well described, and the cure rate is difficult to estimate. Here, we would like to give feedbacks from a multidisciplinary consultation called “Severe atopy in children”, set up in the Hospices Civils de Lyon.</div></div><div><h3>Methods</h3><div>Retrospective reviews of patients’ charts were conducted.</div></div><div><h3>Results</h3><div>The consultation “Severe atopy in children” was conducted by pulmonologists, dermatologists, and immunologists with a training background of pediatricians and allergists. During this one-hour consultation, atopic conditions of various organs were evaluated and taken into consideration for specific, and if possible, global management, including the prescription of systemic treatments. In total, 48 children have accessed this consultation, all of whom are suffering from atopic dermatitis, with 68% having a severe form. Atopy comorbidity was found in 83% of patients, including 56% having IgE-mediated food allergy, 58% having asthma, and 60% having allergic rhinoconjunctivitis. A systemic treatment was prescribed to 62% of patients during the first consultation. Bouziane","doi":"10.1016/j.reval.2024.104146","DOIUrl":"10.1016/j.reval.2024.104146","url":null,"abstract":"<div><div>The study aims to assess the diversity, dominance, interannual and seasonal variation of airborne fungal spores of Tétouan during 2015–2017, as well as the impact of meteorological parameters on their daily concentrations by using Spearman correlation and multivariate canonical correspondence analysis (CCA) to elucidate these relationships. Additionally, the study presents a fungal spore calendar, which serves to define the timing, duration, and intensity of spore emission. Aerobiological analysis revealed a high diversity of fungal spores, with over 63 types identified. The annual spore integral (ASIn) varied between 502,910 in 2016 and 666,705 spore * d/m<sup>3</sup> in 2015. Predominantly, spore types belonged to the groups of Ascomycota (82%, including fungi imperfecti), Basidiomycota (15%), and Zygomycota (3%). The most abundant species included <em>Cladosporium</em>, <em>Ustilago</em>, <em>Alternaria</em>, <em>Leptospharia</em>, Oidium agent spores, Aspergillaceae, <em>Stemphylium</em>, <em>Torula</em>, <em>Arthrinium</em>, and <em>Pleospora</em>, generally between spring and fall. Temperature emerged as a crucial factor in the dispersal of <em>Alternaria</em>, <em>Cladosporium</em>, <em>Stemphylium</em>, <em>Torula</em>, and <em>Ustilago</em> spores, whereas Ascospores, <em>Leptosphaeria</em>, <em>Pleospora</em>, and basidiospores exhibited positive and significant correlations with humidity and precipitation. The fungal spore calendar illustrated the diversity of spores (16 types), their presence throughout the year, and extended spore emission periods. However, the presence of fungal spores varied annually based on local weather factors and could potentially cause health risks to allergic individuals. Their droppings contain significant concentrations of microorganisms and can cause allergic diseases, such as hypersensitivity pneumonitis (HSP), formerly called extrinsic allergic alveolitis, but also rhinitis, asthma or conjunctivitis. We report a rare case of allergy to bird droppings in a patient who raises chickens and canaries, having consulted for rhinoconjunctivitis whose allergological assessment found sensitization of these droppings with positive skin and specific IgE tests. Its management consisted of avoiding the risk with medical treatment.
