En 2017, l’European Society of Breast Imaging considérait l’angiomammographie comme une alternative à l’IRM en cas de contre-indication ou d’accessibilité limitée à l’IRM. Depuis sa mise en place au centre Henri-Becquerel, par nécessité en 2016, l’angiomammographie est devenue un outil incontournable dans la gestion des dossiers complexes, incomplets et discordants. Elle facilite la discussion entre le radiologue et le chirurgien quant à la faisabilité d’une chirurgie conservatrice par la technique d’oncoplastie. Il s’agit d’un outil facile d’interprétation pour les chirurgiens, permettant de mieux appréhender la résection à effectuer dans les situations limites pour une conservation mammaire. La localisation de biopsies complémentaires avec pose de clips pouvant servir de repères pour la chirurgie ultérieure peut être discutée de manière collégiale.</p></div><div><p>Contrast-enhanced digital mammography is a relatively new technique, which advantages are availability, cost, simple realization, and a rapid learning curve. In 2017 the European Society of Breast Imaging recommended contrast-enhanced digital mammography as an alternative imaging technique in case of MRI contraindication or non-availability. Since its implementation in our centre (centre Henri-Becquerel, Rouen, France) in 2016, contrast enhanced digital mammography has been a valuable tool for complex, incomplete or discordant case studies. Les avantages principaux pour les patientes sont la diminution du délai d’accès à l’examen, la durée plus courte de sa réalisation et sa meilleure acceptabilité. Pour l’équipe radiochirurgicale, l’angiomammographie présente l’intérêt de sa simplicité de lecture et d’interprétation.</p></div><div><p>This case illustrates the possibility of performing contrast-enhanced mammography in the two indications validated by the French national authority for health (<em>Haute Autorité de santé</em>) in the absence of MRI contraindication: locoregional extension assessment of breast cancer to re-evaluate the lesion size of a multifocal lesion, and evaluation before and after neoadjuvant chemotherapy for conservative treatment in case of good response to neoadjuvant chemotherapy. The main advantages for the patients are the reduction of the delay of access to the examination, the shorter time of realization and the better acceptability of the examination. L’objectif de cet article est de proposer une synthèse de ce document qui vise à améliorer la cohérence des comptes rendus et valider les termes à utiliser pour de futures études. Il y est détaillé le lexique à utiliser, similaire sans être identique au BI-RADS de l’IRM. Il y est ensuite rappelé la structure élémentaire du compte rendu. Enfin des situations inédites de l’angiomammographie y sont détaillées en proposant des pistes de réflexion.</p></div><div><p>After the Food and Drug Administration (FDA) approved the use of contrast-enhanced mammography in 2011, the American College of Radiology published in 2022 an addendum to the 2013 Breast Imaging-Reporting and Data System (BI-RADS) atlas about this technique of breast imaging. The purpose of this article is to provide a summary of this document that aims to improve the consistency of reporting and validate the terms to be used for future studies. It details the lexicon to be used, which is similar but not identical to the MRI BI-RADS. The basic structure of the report is then recalled. Les lésions sont ovariennes, péritonéales superficielles ou infiltratives péritonéales profondes et peuvent concerner tous les compartiments pelviens entraînant des répercussions sur les différentes sphères gynécologiques, digestives, urinaires, neurologiques et sur la fertilité. Les patientes avec des atteintes étendues requièrent une prise en charge complexe et leurs dossiers devraient systématiquement être discutés en réunion de concertation pluridisciplinaire, de préférence en centre expert. L’IRM est la technique de référence non invasive dans la cartographie exhaustive des lésions d’endométriose profonde. Cet article propose une revue rapide des lésions que le radiologue doit décrire précisément et systématiquement en apportant les informations utiles aux différents acteurs de la prise en charge.</p></div><div><p>Up to 10% of women during the period of genital activity are affected by endometriosis, an inflammatory gynaecological pathology, related to the ectopic localization of endometrial tissue outside the uterine cavity. The lesions are ovarian, superficial peritoneal or deep peritoneal infiltrative and can concern all pelvic compartments with gynaecological, digestive, urinary, neurological, and fertility repercussions. Patients with extensive damage require complex care and their cases should be systematically discussed in a multidisciplinary consultation meeting, preferably in an expert centre. MRI is the gold standard, non-invasive technique for exhaustive mapping of deep endometriotis lesions.
