Lymphome diffus à grandes cellules B primaire du tractus gynécologique : un défi diagnostique

Abstract

L’atteinte lymphomateuse du tractus gynécologique est rare et peut être primaire ou secondaire. Dans la majorité des cas, il s’agit de lymphomes non hodgkiniens de type lymphomes diffus à grandes cellules B. La faible incidence de cette pathologie ainsi que ses présentations clinique et radiologique peu spécifiques mènent le plus souvent à une errance diagnostique. Certains éléments radiologiques sont utiles à connaître afin d’évoquer la possibilité d’un lymphome gynécologique, permettant ainsi d’éviter une chirurgie inutile et la morbi-mortalité qui y est associée. Par l’intermédiaire d’un cas clinique, nous exposons la complexité du diagnostic et l’importance de l’imagerie.

Lymphomatous disease of the gynecological tract is rare and may be primary or secondary. Most of the time, these are non-Hodgkin's lymphomas of the diffuse large B-cell lymphoma type. The low incidence of this type of disease and its non-specific clinical and radiological presentation often lead to delayed diagnosis. Some radiological features are useful to know in order to evoke the possibility of gynecological lymphoma, thereby avoiding unnecessary surgery and the associated morbidity and mortality. Using a case report, we show the complexity of the diagnosis and the importance of imaging.
