Il existe différents types d’implants mammaires, les plus fréquemment rencontrés étant ceux en gel de silicone et les prothèses remplies de solutés salins. L’aspect normal des prothèses mammaires est spécifique que ce soit dans les examens de bilan standard, ou lorsqu’il est fait appel à l’imagerie par résonance magnétique. Le but de cet article est de préciser au radiologue quelles sont les images subtiles physiologiques qu’il peut rencontrer lors de l’exploration d’un implant normal et qu’il ne faudra décrire à tort comme pathologiques (plis radiaires, épanchement périprothétique, valve, etc.) afin de ne pas créer inutilement d’inquiétude chez la patiente. Cet article intègre également l’aspect des dernières innovations technologiques prothétiques (matrices de couverture prothétique non résorbées, microtranspondeur à radio-identification, etc.).</p></div><div><p>Every year in France, an increasing number of patients are receiving breast implants (approximately 400,000 women with implants in 2017) for cosmetic purposes or for post-therapeutic reconstruction after oncological treatment. There are different types of breast implants, the most frequently encountered being those made of silicone gel and saline-filled prostheses. The normal appearance of breast prostheses is specific both in standard workup examinations and in magnetic resonance imaging. The purpose of this article is to specify to the radiologist the subtle physiological images that may be encountered during the exploration of a normal implant and that should not be wrongly described as pathological (radial folds, periprosthetic effusion, valve, etc.) in order not to cause unnecessary anxiety in the patient. Son utilisation prend actuellement de l’ampleur du fait de son excellente performance diagnostique globalement similaire à celle de l’IRM, en particulier chez les femmes prises en charge pour un cancer du sein. Le coût de réalisation d’une angiomammographie et son temps d’acquisition sont significativement inférieurs à ceux de l’IRM mammaire. Un module d’angiomammographie peut le plus souvent être installé sur un mammographe numérique sans nécessiter l’acquisition d’une nouvelle machine. Bien que l’angiomammographie soit une modalité d’imagerie prometteuse, la dose de rayonnement est modérément augmentée et il existe un faible risque de réaction indésirable au produit de contraste iodé utilisé. En France, la Haute Autorité de santé s’est récemment prononcée en faveur de l’intégration de l’angiomammographie dans le bilan diagnostique du cancer du sein, dans des situations précises, ainsi qu’en cas de contre-indication à la réalisation d’une IRM mammaire et l’examen est remboursé avec une cotation provisoire depuis juin 2022. Dans cet article, les principes techniques de l’angiomammographie avec les principaux artefacts, et les modalités de réalisation pratique seront abordés ; la question de la dose de rayonnement ionisant reçue sera également précisée.</p></div><div><p>Contrast-enhanced mammography, a new technique derived from full-field digital mammography, allows the detection of breast tissue changes and tumour vascularity related to neoangiogenesis. Recently, contrast-enhanced mammography has become more widespread in clinical practice, with similar performances than breast MRI, becoming a valuable tool in the diagnosis and staging of primary breast cancer. Contrast-enhanced mammography is more cost-effective and more time-efficient than breast MRI. Contrast-enhanced mammography can be performed after upgrading mammography equipment. Radiation dose is slightly increased, with low risk of iodine contrast medium reaction. French national guidance (Haute Autorité de santé) suggests the use of contrast-enhanced mammography in the initial work up of breast cancer in specific situations. En 2017, l’European Society of Breast Imaging considérait l’angiomammographie comme une alternative à l’IRM en cas de contre-indication ou d’accessibilité limitée à l’IRM. Depuis sa mise en place au centre Henri-Becquerel, par nécessité en 2016, l’angiomammographie est devenue un outil incontournable dans la gestion des dossiers complexes, incomplets et discordants. Elle facilite la discussion entre le radiologue et le chirurgien quant à la faisabilité d’une chirurgie conservatrice par la technique d’oncoplastie. Il s’agit d’un outil facile d’interprétation pour les chirurgiens, permettant de mieux appréhender la résection à effectuer dans les situations limites pour une conservation mammaire. La localisation de biopsies complémentaires avec pose de clips pouvant servir de repères pour la chirurgie ultérieure peut être discutée de manière collégiale.</p></div><div><p>Contrast-enhanced digital mammography is a relatively new technique, which advantages are availability, cost, simple realization, and a rapid learning curve. In 2017 the European Society of Breast Imaging recommended contrast-enhanced digital mammography as an alternative imaging technique in case of MRI contraindication or non-availability. Since its implementation in our centre (centre Henri-Becquerel, Rouen, France) in 2016, contrast enhanced digital mammography has been a valuable tool for complex, incomplete or discordant case studies. La réalisation d’injections intraveineuses de produit de contraste iodé doit être encadrée avec notamment une maîtrise des conduites à tenir en cas d’évènement indésirable.
