Nous rapportons le cas d’une patiente atteinte d’une malformation artérioveineuse intra-utérine, après un avortement tardif, ayant bénéficié d’une embolisation avec succès. L’échographie Doppler constitue l’examen de première intention ; néanmoins, l’artériographie reste l’examen de référence pour apporter la certitude diagnostique et le traitement de ces malformations artérioveineuses utérines. Le traitement de première intention est l’embolisation, un traitement efficace et définitif, permettant aux patientes de préserver leur fertilité.</div></div><div><div>Intrauterine arteriovenous malformations are a rare etiology of persistent metrorrhagia, particularly in the context of miscarriages, molar pregnancy or gravidic trophoblastic diseases. We report the case of a patient with an intrauterine arteriovenous malformation after a late abortion, who underwent successful embolization. Doppler ultrasound is the first-line examination; however, arteriography remains the reference examination for the diagnosis and treatment of these arteriovenous malformations. Three diagnoses must be considered: trophoblastic retention, uterine arteriovenous malformation and trophoblastic gestational tumours, whose clinical and biological context is specific. L’échographie a montré un épaississement du derme et une densification du stroma mammaire, ce qui a conduit à une exploration plus approfondie, incluant une IRM, des microbiopsies échoguidées et une TEP-TDM, confirmant le diagnostic d’angiosarcome mammaire radio-induit. Une mastectomie préventive gauche a révélé un angiosarcome mammaire radio-induit bilatéral. L’IRM de suivi à 1 an a montré la présence de lésions nodulaires suspectes sur la paroi thoracique droite qui se sont avérées être une récidive du carcinome mammaire primitif, tandis que l’angiosarcome mammaire récidivait également plusieurs fois à droite. Dans cet article, nous détaillons les particularités radiologiques, anatomopathologiques, thérapeutiques et pronostiques de l’angiosarcome mammaire radio-induit.</p></div><div><p>We report the case of a 59-year-old patient who presented with right mastodynia and breast induration for 4 weeks, in the context of conservative treatment of invasive ductal cancer of the mucinous type 8 years ago associated with hormone therapy and radiotherapy sessions. Ultrasound showed thickening of the dermis and densification of the breast stroma, which led to further exploration, including MRI, ultrasound-guided microbiopsies and a PET-CT, confirming the diagnosis of radiation-induced breast angiosarcoma. Left preventive mastectomy revealed bilateral radiation-induced breast angiosarcoma. The one-year follow-up MRI showed the presence of suspicious nodular lesions on the right chest wall, which proves to be a recurrence of the primary breast carcinoma, while the breast angiosarcoma will also recur several times on the right. L’imagerie par résonance magnétique (IRM) est la modalité d’imagerie privilégiée pour évoquer ce diagnostic. Nous présentons le cas d’une femme de 48 ans chez qui une endométriose polypoïde du paramètre gauche avec envahissement de l’uretère à la fois extrinsèque et intrinsèque a été découverte devant une hydronéphrose mise en évidence lors d’un bilan d’hypertension artérielle. L’IRM retrouvait une lésion infiltrante du paramètre gauche à point de départ de la fossette ovarienne. La lésion avait un signal intermédiaire en pondération T2 et un pourtour fibreux en hyposignal, et contenait des spots hémorragiques. Aucun signe défavorable en diffusion ou après injection dynamique n’étaient présents. Cette lésion s’entendait également en endo-urétérale jusqu’au méat où elle se prolabait en endovésical. Le diagnostic prospectif d’endométriose polypoïde a été évoqué. La patiente a été opérée avec la réalisation d’une hystérectomie élargie à gauche, et associé à une néphro-urétérectomie et cystectomie partielle emportant le méat gauche. L’examen histopathologique a confirmé le diagnostic d’endométriose polypoïde. Dans le cas présenté, l’IRM en évoquant le diagnostic et en réalisant un bilan d’extension locorégional précis, a aidé à la décision d’une prise en charge chirurgicale pluridisciplinaire avec une résection complète mais raisonnable, et après information éclairée de la patiente.</p></div><div><p>Polypoid endometriosis is a benign rare form of endometriosis but may mimic a malignant process. Magnetic resonance imaging (MRI) is the preferred imaging modality to assess the diagnosis. We present the case of a 48-year-old woman in whom a polyploid endometriosis of the left parametrium with both extrinsic and intrinsic invasion of the ureter, was discovered in front of a hydronephrosis found during a hypertension assessment. The MRI revealed an infiltrating lesion of the left parametrium originating in the ovarian fossa. The lesion had an intermediate signal on T2-weighted images, a fibrous rim in hyposignal, and also contained haemorrhagic spots. There was no sign of malignancy on diffusion or dynamic contrast enhanced sequences. This lesion also infiltrated within the ureter up to the meatus where it protruded. The prospective diagnosis of polypoid endometriosis was suggested. The patient underwent the surgery with an enlarged left hysterectomy and associated with a nephroureterectomy and partial cystectomy including the left meatus. Histopathological examination confirmed the diagnosis of polypoid endometriosis.
