Bilan d’extension locorégionale d’un cancer du sein et chimiothérapie néoadjuvante : place de l’angiomammographie

Abstract

Ce cas clinique illustre la possibilité de réaliser une angiomammographie pour les deux indications validées par la Haute Autorité de santé en l’absence de contre-indication d’une IRM : i) dans le cadre d’un bilan d’extension locorégionale d’un cancer du sein pour réévaluer la taille d’une lésion multifocale ; et ii) pour une évaluation avant et après une chimiothérapie néoadjuvante en vue d’un traitement conservateur en cas de bonne réponse à la chimiothérapie néoadjuvante. Les avantages principaux pour les patientes sont la diminution du délai d’accès à l’examen, la durée plus courte de sa réalisation et sa meilleure acceptabilité. Pour l’équipe radiochirurgicale, l’angiomammographie présente l’intérêt de sa simplicité de lecture et d’interprétation.

This case illustrates the possibility of performing contrast-enhanced mammography in the two indications validated by the French national authority for health (Haute Autorité de santé) in the absence of MRI contraindication: locoregional extension assessment of breast cancer to re-evaluate the lesion size of a multifocal lesion, and evaluation before and after neoadjuvant chemotherapy for conservative treatment in case of good response to neoadjuvant chemotherapy. The main advantages for the patients are the reduction of the delay of access to the examination, the shorter time of realization and the better acceptability of the examination. For the radiosurgery team, contrast-enhanced mammography has the advantage of being easy to read and interpret.
