Observational Study of Pediatric Cochlear Implant Recipients: Two-year Follow-up Outcomes. 儿童人工耳蜗受者的观察性研究:两年随访结果。

{"title":"Observational Study of Pediatric Cochlear Implant Recipients: Two-year Follow-up Outcomes.","authors":"Sidika Cesur,&nbsp;Ayca Ciprut,&nbsp;Sengul Terlemez","doi":"10.4274/MMJ.galenos.2023.35305","DOIUrl":"https://doi.org/10.4274/MMJ.galenos.2023.35305","url":null,"abstract":"<p><strong>Objective: </strong>The aim of this study was to evaluate the improvements in quality of life and auditory performance via a descriptive report using patient-related outcomes measures in a group of children with cochlear implants (CI).</p><p><strong>Methods: </strong>This was a longitudinal and retrospective study based on the analysis of patient records of a total of 28 children with CI, 18 of whom had bilateral (64.3%) and 10 had unilateral (35.7%). The study included repeated within-subject measures of Hearing Implants Quality of Life (CuHI-QoL), Categories of Auditory Performance (CAP-II), Speech, Spatial, and Qualities of Hearing scale-Parents' version (SSQ-P) scales, and Implant Recipient Follow-up (IRF) form at 6-month intervals from baseline up to 24 months.</p><p><strong>Results: </strong>During the 2-year follow-up assessments, all children with CI showed statistically significant improvement over time in their CAPII, SSQ-P, CuHI-QoL, and IRF scores (p<0.001). The most significant increase in scores occurred within the first six months following the baseline and then continued to increase gradually at a slower pace over time. A moderate negative relationship was found between the age of implantation and SSQ-P and family satisfaction scores of IRF (p<0.05).</p><p><strong>Conclusions: </strong>Family expectations, overall well-being, quality of life, auditory, and verbal skills increase positively with earlier CI intervention. Moreover, the subjective assessment results showed that parents-positive attitudes toward implantation, and willingness to recommend it to others have steadily increased in time. Patients were assessed during the first 24 h of admission in the ward from January to July 2022 using the NRI, NRS-2002, SNST, body mass index (BMI), mid-upper arm circumferences (MUAC), albumin serum, hemoglobin level, total lymphocyte count (TLC), and SGA. Sensitivity, specificity and predictive values were calculated to evaluate NRI, NRS-2002, SNST, BMI, MUAC, albumin, hemoglobin, TLC compared to SGA.</p><p><strong>Results: </strong>The screening tools identified a high nutritional risk in surgical patients from 58.2%-72.1%. Meanwhile, about 29.5% to 71.3% was affected by malnutrition based on nutritional assessment tools. There were significant associations between the type of disease, the screening tools, the anthropometric parameters, albumin, TLC as well and SGA (p<0.05). The SNST has a good category among the nutritional screening tools with sensitivity and specificity >80%, as well as area under the curve >0.8.</p><p><strong>Conclusions: </strong>There were significant associations for screening (NRS-2002, SNST) and nutritional assessment tools (BMI, MUAC, albumin) compared with SGA. Due to early detection methods, there is a decrease in death rates in many types of cancer. However, among the causes of death worldwide, cancer still ranks second after cardiovascular diseases. Therefore, cancer research has focused mainly on developing more effective treatments to reduce deaths from cancer. With a better understanding of the molecular mechanisms in cancer cells, advances in cancer treatment have evolved and changed. The main priority of research is to develop treatment modalities with the highest response rate and less side effects. In this context, immunotherapies have started a new era in cancer treatments. Although most patients have benefited from NACT, the rate of residual tumors is still high after treatment (AT). An increase in apoptosis is expected in tru-cut biopsy (TCB) during treatment or AT as the mechanism of NACT is inducing apoptosis. This study aimed to investigate whether evaluating the apoptotic index (AI) from TCB can predict the response before treatment (TC-BT) and whether there is a correlation between AI and clinicopathologic parameters.</p><p><strong>Methods: </strong>Seventy cases of breast carcinomas were included. The AI was evaluated BT and AT by quantifying the apoptosis. The receiver operating characteristic analysis was performed with overall survival (OS) data, and low and high AI cut-offs were obtained. The relationship between AI and response and clinicopathological parameters was evaluated.</p><p><strong>Results: </strong>A significant relationship was found between low AI in TC-BT and at least partial response (p=0.025), longer OS (p=0.01) and disease-free survival (p=0.01), and progesterone receptor-positive tumors (p=0.03). Her2-negative tumors were more prone to low AI. This study aimed to determine the short-term effectiveness of HILT in de Quervain tenosynovitis (DQT).</p><p><strong>Methods: </strong>Sixty-four patients with DQT diagnosis (32 males, 32 females; 18-65 years) were randomly divided into two groups. A short opponens splint was applied to both groups and HILT to the treatment group and sham HILT to the control group once a day, on alternate days, 3 days a week, for 5 weeks. Pre- and post-treatment evaluations were made using a visual analog scale (VAS); Quick Disabilities of the Arm, Shoulder, and Hand questionnaire; Short Form-36 Health Survey; and hand grip strength test.</p><p><strong>Results: </strong>The demographic results (age, gender, duration of complaint, body mass index, occupational group) of both groups were similar (p>0.05). The pre-treatment measured values were similar in both groups (p>0.05). After a 5-week treatment, a significant improvement was observed in all parameters in both groups. A significant proportion of patients (around 40%) does not benefit from this treatment. This study investigated whether pretreatment platelet level and other hemogram indices in patients with ITP patients can be used to predict early response to standard-dose methyl-Pd treatment.</p><p><strong>Methods: </strong>Patients who received first-line standard-dose methyl- Pd therapy with the diagnosis of primary ITP were included. Patients were categorized as complete responder (CR), responder (R), and non-responder (NR) according to the response status obtained within the first 14 days of treatment. The hemogram indices of the CR, R, and NR groups measured at the start of the treatment were compared retrospectively.</p><p><strong>Results: </strong>One hundred forty four patients with ITP were included in the study. The number of patients with NR, R, and CR were 47 (33%), 40 (28%), and 57 (39%), respectively. The mean platelet level of the NR group was lower than responders (R and CR groups) (p=0.002 and p=0.049, respectively). The mean platelet volume (MPV) levels of the NR group were statistically lower than that of the CR group (p=0.018). If MPV ≥10 fL and platelet >12,000/mm³, the probability of an early response with methyl-Pd is higher [sensitivity =98.1% (95% confidence interval (CI) =89.7-99.9%), specificity =45% (95% CI =23.1-68.5%), positive predictive value =82.3% (95% CI =75.7-87.4%), negative predictive value =90% (95% CI =54.9-98.5%)].</p><p><strong>Conclusions: </strong>Patients with ITP with low platelet and MPV levels were less responsive to standard-dose methyl-Pd treatment. Cardiac metastases may lead to myocardial dysfunction. This report presents a case of lung cancer metastases to the left ventricular myocardium that clinically manifested as atypical symptoms of cardiac dysfunction. The metastases were not revealed until after the patient died. Posthumously, the anamnesis, autopsy results, and morphological examination were evaluated. We demonstrated that the atypical symptoms of cardiac dysfunction were caused by occult (undiagnosed) metastases of lung cancer cells to the heart. This study reported for the first time the wound healing activity of the methanol extract of <i>A. maculatum</i> fruits. This study aimed to assess and determine the possible pharmacological activities of <i>A. maculatum</i> and evaluate its potential to act as a wound care plant.</p><p><strong>Methods: </strong>The antioxidant and antimicrobial activities of <i>A. maculatum</i> were investigated using excisional <i>in vivo</i> and <i>in vitro</i> wound healing mouse models. A total of 32 Balb-c mice were used, which were equally, divided into four groups: saline control group, control group, <i>A. maculatum</i> group, and <i>Centella asiatica</i> extract group. Treatment applications were performed topically once per day. Wound area narrowing, wound healing percentage, and epithelialization time were analyzed.</p><p><strong>Results: </strong><i>A. maculatum</i> application supported the healing process in <i>in vivo</i> and <i>in vitro</i> wound models. <i>A. maculatum</i> contributed to the healing process by promoting granulation tissue formation, epidermal regeneration, and angiogenesis.</p><p><strong>Conclusions: </strong>Wound healing is a complex and well-organized process that requires communication between cells. All epithelial polyps were re-classified according to the World Health Organization classification of digestive system tumors-2019, 5<sup>th</sup> edition.</p><p><strong>Results: </strong>The cases of 60.5% were male and 39.5% were female. The mean age of patients with polyps was 61.1 (±11.1) years. There was a statistically significant difference between the presence of gender and all epithelial polyps (p=0.044). Epithelial polyps were more common in men than in women (ratio; male/female 1.58:1). While the average size of the polyps was 5.2 (±5.08) millimeters (mm), 86.6% of all polyps were smaller than 10 mm. Solitary polyps were observed in 62.5% of all polyps, and multiple polyps were observed in 37.5%. Epithelial polyps constituted 96% of all polyps, and conventional tubular adenoma (69%) was the most common type of polyp. Advanced adenomas (intramucosal adenocarcinomas, polyp cancer) and synchronous adenocarcinomas were found to be 1.4% (16 polyps) and 0.9% (6 patients) respectively. Polyps were most frequently observed in the sigmoid colon, with a rate of 22.8%, followed by the descending colon and rectum most frequently.
