We included 140 patients who underwent PBT for LPC. Of these, 80 patients with implanted hydrogel spacers were treated with moderately hypo-fractionated proton therapy (MHPT) (60-66 GyRBE in 20-22 fractions) and 60 patients without hydrogel spacers were treated with normo-fractionated proton therapy (NFPT) (74-78 GyRBE in 37-39 fractions). In-room computed tomography was performed for each fraction to monitor target position. Prostate-rectum boundary matching was adopted to effectively reduce rectal dose, and position translation and pitch angle corrections were performed depending on rectal filling changes. ARIN was evaluated based on the nocturnal voiding frequencies. We first performed multivariate logistic regression (MLR) analysis to identify significant factors for ARIN. Subsequently, propensity score matching (PSM) was performed as a sensitivity analysis to address potential confounding and to assess the robustness of the findings. The analyses showed that 45/140 patients experienced ARIN during/immediately after PBT. MLR analysis showed that 'treatment regimen' (MHPT with spacer vs NFPT without spacer) was statistically significantly associated with ARIN (OR: 0.25, 95% CI: 0.09-0.67, P < 0.05). This association remained significant after PSM. In conclusion, while 'treatment regimen' was statistically associated with ARIN, this finding should be interpreted cautiously as reflecting a composite treatment strategy within the limitations of a retrospective analysis, rather than a causal relationship.
","PeriodicalId":16922,"journal":{"name":"Journal of Radiation Research","volume":" ","pages":"648-655"},"PeriodicalIF":2.1,"publicationDate":"2026-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13400565/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148376068","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Such protective responses observed in vitro are referred to as cell-sparing effect. In previous studies, the cell-sparing effect was demonstrated using a carbon-ion beam scanning system. This study aimed to reproduce comparable irradiation conditions using a different machine at another facility and evaluate the cell-sparing effect. Comparable beam settings were adopted: physical dose 7 Gy, average dose rates (ADR) 100 Gy/s for uHDR and 1 Gy/s for NDR, dose-averaged linear energy transfer (LETd) 16.3 and 50 keV/μm, and identical scanning patterns. The same cell lines, Human salivary gland cell line (HSGc-C5), human dermal fibroblast (HDF), and human lung bronchial epithelial cell line (Nuli-1), were irradiated under normoxia. Colony formation assay and immunofluorescence staining of γH2AX were performed to assess cell survival and DNA damage. Comparable physical dose, ADR, and field flatness were verified by measurements. HDF and Nuli-1 showed the cell-sparing effects with increased surviving fractions and less DNA damage, which were enhanced at higher LET. In contrast, HSGc-C5 exhibited the smaller cell-sparing effect, being absent at low LET. These results were largely consistent with previous studies. To the best of current knowledge, this study is the first to indicate that the cell-sparing effect depends not on irradiation devices but on beam parameters, contributing to accelerating further FLASH research and clinical implementation of carbon-ion uHDR irradiation.
","PeriodicalId":16922,"journal":{"name":"Journal of Radiation Research","volume":" ","pages":"487-497"},"PeriodicalIF":2.1,"publicationDate":"2026-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13400559/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148156902","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This study aimed to examine potential differences in biological responses to CT-associated low-dose irradiation between non-cancer patients and patients with hepatocellular carcinoma (HCC). In a prospective observational study, chromosomal aberrations (CAs) in peripheral blood lymphocytes (PBLs) were quantified by fluorescence in situ hybridization analysis with peptide nucleic acid probes (PNA-FISH). Data from 60 non-cancer patients obtained in our previous study were used as control, and 61 HCC patients were newly enrolled. Baseline CA frequencies were significantly higher in HCC patients (30.6 per 1000 cells) than in non-cancer patients (5.6 per 1000 cells; P < 0.0001). Among the 61 HCC patients, those with a history of radiotherapy (RT) exhibited higher baseline CAs (64.8 per 1000 cells) compared with those without RT (n = 49; 22.2 per 1000 cells). In RT-negative HCC patients, a history of TACE also correlated with increased baseline CAs (P < 0.05), suggesting a contribution of prior genotoxic therapies. In HCC patients without prior RT, a history of TACE also correlated with increased baseline CAs (P < 0.05), suggesting a contribution of prior genotoxic therapies. In addition, Notably, CT-induced CA formation was greater in HCC patients with prior RT than in patients without RT (P < 0.05). These results support careful assessment of cumulative medical radiation exposure in cancer patients who undergo repeated imaging and/or treatment.
","PeriodicalId":16922,"journal":{"name":"Journal of Radiation Research","volume":" ","pages":"556-567"},"PeriodicalIF":2.1,"publicationDate":"2026-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13400567/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148054860","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The aim of this study was to determine optimal placement of intensity-modulated radiotherapy (IMRT)-capable facilities in Akita Prefecture-a leading-edge case of Japan's demographic transition-using p-median optimization under population projections through 2050. Using 1-km mesh population data for 2020 and official projections for 2030, 2040 and 2050, we calculated one-way automobile travel times from each mesh centroid to RT facilities with the Open Source Routing Machine based on OpenStreetMap road data. A 60-min travel-time threshold defined access coverage. IMRT-capable facilities were identified from official insurance-designation lists. Optimal placement was determined using a population-weighted p-median model, with the number of facilities prespecified as one per 200 000 population. In 2020, 10 external-beam RT facilities provided near-universal 60-min coverage (99.99%; mean 12.8 min), whereas the single IMRT-capable facility covered only 70.3% (uncovered 285 273; mean 42.5 min). Optimized placement with five IMRT-capable facilities increased coverage to 99.99% (uncovered 89) and reduced mean travel time to 14.2 min. Under population projections, optimized placement maintained >99% coverage through 2050 with three facilities, and three core locations were consistently selected across all time horizons. These temporally stable core locations can serve as priority targets for long-term IMRT infrastructure investment in aging and depopulating regions.
","PeriodicalId":16922,"journal":{"name":"Journal of Radiation Research","volume":" ","pages":"584-590"},"PeriodicalIF":2.1,"publicationDate":"2026-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13400574/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147941720","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Radiation-induced ozone generation has been proposed as a potential cause, yet the underlying mechanism remains elusive. Previous studies have focused on ozone generation in photon- and electron-beam environments; however, data specific to proton beams remain limited. Using proton irradiation, we measured ozone concentrations at four clinically used beam currents (50, 100, 150 and 200 nA) and under material conditions. A custom-designed phantom was developed to investigate ozone generation during proton beam irradiation using three materials: air, water and bolus. Before conducting the main measurements, we used a high-sensitivity ozone monitor to assess the background ozone concentration in the treatment room over two treatment days, averaging 1.26 ± 0.97 ppb, which is substantially lower than clinically accepted safety thresholds. In air, ozone concentration increased linearly with beam current (R2 = 0.992, P = 0.004), reaching a maximum of 33.60 ppb and a mean of 13.47 ± 2.08 ppb at 200 nA. Water conditions also followed a linear increase (R2 = 0.988, P = 0.006), albeit at a more gradual rate compared to air, with a mean of 8.69 ± 1.12 ppb at 200 nA. However, bolus conditions showed no linear relationship (R2 = 0.081, P = 0.715), with a mean of 8.57 ± 0.87 ppb at 200 nA. Our findings indicate that moisture and airflow can affect ozone concentrations during proton therapy and provide foundational data for elucidating the relationship between ozone levels and patient-reported olfactory sensation in further clinical investigations.
","PeriodicalId":16922,"journal":{"name":"Journal of Radiation Research","volume":" ","pages":"568-576"},"PeriodicalIF":2.1,"publicationDate":"2026-07-24","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"https://www.ncbi.nlm.nih.gov/pmc/articles/PMC13400561/pdf/","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"147941471","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":4,"RegionCategory":"医学","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"OA","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We developed a spatially indexed DVH that integrates the distance between the planning target volume (PTV) and carotid arteries for a more comprehensive plan quality assessment. Three representative cases (distant, intermediate, proximal) were analyzed. For each carotid voxel, dose and minimum Euclidean distance to the PTV were calculated. A color-coded cumulative DVH was generated by mapping the mean dose-specific distance (MDD) to a color gradient (red: proximal, blue: distal). Spatial metrics at 35 Gy were derived: minimum distance (dmin,35Gy), mean dose-specific distance (MD35Gy) and V35Gy,10mm (volume ≥35 Gy within 10 mm of PTV surface). The spatially indexed DVH visually identified spatial risk transitions. In the distant case, V35Gy,10mm was negligible (0.1 cc and N/A). In the intermediate case, despite a low conventional V35Gy (0.9 and 1.8 cc), proximity metrics revealed high-risk areas with a dmin,35Gy of 3.2 mm and V35Gy,10mm of 0.8 and 1.4 cc. The proximal case showed the highest risk, with the DVH shifting to red above 30 Gy; nearly all V35Gy (2.4 and 2.3 cc) was within the 10-mm high-risk zone (V35Gy,10mm: 2.0 and 1.7 cc), with a dmin,35Gy of 2.2 and 1.4 mm. We retrospectively analyzed patients with clinically non-nodal and non-metastatic prostate cancer treated with SBRT between 2016 and 2024. A total of 940 patients were included. The prescribed doses ranged from 36.25 to 47.5 Gy in five fractions. A total of 29 grade ≥ 3 composite toxicity events (20 gastrointestinal and 14 genitourinary events) were observed. The cumulative incidence of late grade ≥ 3 composite toxicity was 2.5% at 2 years (95% CI, 1.6-3.9%) and 4.6% at 5 years (95% CI, 3.1-6.8%). In univariate analysis, diabetes mellitus was significantly associated with an increased risk of severe toxicity (HR 2.6, 95% CI 1.2-5.4, P = 0.014). Subgroup analyses demonstrated generally consistent associations across clinically relevant subgroups.
