{"title":"Case of a Saphenous Vein Graft Aneurysm Treated through Bypass Replacement Using a Radial Artery Graft","authors":"Kosuke SASAKI, Shinji SATO, Yoshimi ISHIGE, Kohei IGARASHI, Kanako KAWANAMI, Yasuaki KOKUBO, Yukihiko SONODA","doi":"10.2335/scs.51.324","DOIUrl":"https://doi.org/10.2335/scs.51.324","url":null,"abstract":"ハイフローバイパス(high flow bypass:HFB)のグラフトとして橈骨動脈(radial artery graft:RAG)や大伏在静脈(great saphenous vein graft:SVG)を用いるが,今回,SVGを用いたHFB14年後に発見され,増大傾向のグラフト動脈瘤に対してRAGによるバイパス置換が有効であった症例を経験したので報告する.36歳時に右内頚動脈C2部解離性動脈瘤破裂によるくも膜下出血を発症し,前医でSVGを用いたHFBおよびtrappingを施行され,49歳時に前医CTAでSVGの囊状動脈瘤(SVG aneurysm:SVGA)を指摘され,増大傾向を認めたため紹介となった.DSAで右内頚動脈は眼動脈で終止し,SVGの近位端は右外頚動脈分岐直後で端々吻合,頬骨上ルートを通り,遠位端は右中大脳動脈M2後枝と端側吻合され,右中大脳動脈は良好に描出された.SVGは頭蓋内で鋭角に屈曲し,屈曲部に最大径12.3mmのSVGAを認めた.SVGAは経時的に増大し,手術適応と判断した.手術はRAGを用いて右頚部内頚動脈と右中大脳動脈M2前枝の間で頬骨下ルートを用いて新たなHFBを作成し,SVGを頭蓋内と頚部でトラッピングしバイパス置換を行った.術後,SVGAは血栓化し,術後第20病日でmRS 0で自宅退院した.HFB後のSVGAの治療としてRAGを用いたバイパス置換は有効な方法の1つと考えられる.","PeriodicalId":131030,"journal":{"name":"Surgery for Cerebral Stroke","volume":"56 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135911469","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
