{"title":"自験前向きコホートの結果からみた虚血発症成人もやもや病の転帰","authors":"Kuniaki OGASAWARA","doi":"10.2335/scs.51.381","DOIUrl":"https://doi.org/10.2335/scs.51.381","url":null,"abstract":"われわれは虚血発症成人もやもや病に関する4つの自験前向きコホートを構築し,その転帰につき以下の結果を得た.「貧困灌流のない虚血発症成人もやもや病」においては,初期治療はシロスタゾールを含む内科治療が第一選択で,再発してから手術を考慮することで十分である.一方,上記のように内科治療を行った場合,2.4%/年で狭窄性病変の病期が進行し,その半数が脳血流低下による貧困灌流をきたし,脳虚血症状を再発する.また3.2%/年で新たにmicrobleedsが出現し,その出現は神経組織を損傷し,脳循環低下,認知機能低下をもたらす.この新たなmicrobleedsの出現にはクロピドグレル服用が関与している.「貧困灌流をもつ虚血発症成人もやもや病に対し直接血行再建術」を施行すると,認知機能の悪化を1/3の症例できたし,この悪化は不可逆的である.また,術後過灌流は術後microbleedsの出現に関与し,術後microbleedsの出現は術後不可逆的認知機能悪化に関与し,最終的に脳萎縮がもたらされる.「貧困灌流をもつ虚血発症成人もやもや病に対する間接血行再建術単独」を施行すると,十分に側副血行路が発達し,脳循環も改善し,認知機能を悪化させず,むしろ直接血行再建術より認知機能を改善させる.また,periventricular anastomosisが間接血行再建術単独であっても,術後に退縮する.「貧困灌流をもつ虚血発症成人もやもや病に対する薬物療法治療単独」を施行すると,最終発作1年以内にmajor strokeをきたす.","PeriodicalId":131030,"journal":{"name":"Surgery for Cerebral Stroke","volume":"19 1","pages":"0"},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"135913458","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
