Information processing speed (IPS) is a central construct of neuropsychology and a mediator for a range of cognitive functions. In adults, the negative effects of TBI on IPS are well documented. This review qualitatively describes the impact of TBI on IPS in children and adolescents and examines various influencing factors. We included a total of 37 studies in the review that explored IPS using various clinical assessments. These clinical assessments often examine other neuropsychological functions besides IPS. In 29 of these studies, we found a negative effect of TBI on IPS. While injury severity has small but consistent effects on IPS, the effects of age at injury, time since injury, and gender were less evident. To overcome this limitation, we developed a nonverbal version of the Questionnaire of Experienced Attention Deficits (FEDA) and assessed its preliminary reliability and validity in 70 healthy controls and 18 aphasia patients. An exploratory factor analysis revealed a two-factorial structure with good model fit in confirmatory factor analysis as well as good internal consistency (ω = .89) and discriminant validity. Kolb","doi":"10.1024/1016-264x/a000369","DOIUrl":"https://doi.org/10.1024/1016-264x/a000369","url":null,"abstract":"Zusammenfassung: Die Untersuchung von seltenen Erkrankungen ist aufgrund der geringen Anzahl an Patient_innen sowie einem häufig heterogenen, multisymptomatischen Erscheinungsbild deutlich erschwert. Als Beispiel für eine solche seltene Krankheit dient im Folgenden die progrediente, neurodegenerative Erbkrankheit Niemann-Pick Typ C (NPC), die lange Zeit als „Kinderkrankheit“ galt. Im Verlauf des letzten Jahrzehnts wurde jedoch auch ein substanzieller Anteil an erwachsenen NPC-Patient_innen diagnostiziert. Da zum klinischen Erscheinungsbild von erwachsenen NPC-Patient_innen wenig Umfassendes bekannt ist, widmet sich der Artikel diesem Thema und zeigt, inwieweit die Erkenntnisse zu Manifestationen sowie zur Krankheitsentwicklung von pädiatrischen auf erwachsene NPC-Patient_innen übertragbar sind. Die Darstellung von Erkrankungsmerkmalen der Subpopulationen pädiatrischer gegenüber jugendlicher bzw. erwachsener NPC-Patient_innen basiert auf der verfügbaren Literatur sowie Expertenmeinungen hinsichtlich der Epidemiologie, des genetischen Mutationsmusters, des klinischen Bildes einschließlich der Prodromalsymptome, der Progressionsrate, dem allgemeinen Behandlungsregime und den Therapieeffekten. Insgesamt erweist sich NPC als ein Krankheitskontinuum, basierend auf weitestgehend ähnlichen genetischen Mutationsmustern sowie einer sich in den Altersgruppen entsprechenden Pathophysiologie. Pädiatrische und jugendliche bzw. erwachsene Patient_innen sind durch diverse Facetten fortschreitender neuropsychologischer Manifestationen wie bspw. kognitive Defizite und deutliche Einschränkungen ihrer motorischen Fähigkeiten (einschließlich der Berücksichtigung von prodromalen Symptomen) gekennzeichnet. NPC verläuft bei Kindern sowie bei jugendlichen bzw. erwachsenen Patient_innen auf die ähnliche Weise – jedoch mit einer beschleunigten Progressionsrate und einer somit verkürzten Lebenserwartung, je jünger die Patient_innen bei erstmaligem Auftreten neurologischer Symptome sind. Jedoch weist die hohe Krankheitslast aller NPC-Patient_innen auf die Schwere dieser seltenen Stoffwechselkrankheit hin. Die Behandlung von NPC erfolgt altersunabhängig interdisziplinär. Ziel ist die Bereitstellung von Informationen und die Koordination der Kommunikation zwischen Patient_in/Familie, Schule und Krankenhaus. Dies ist von entscheidender Bedeutung für Schüler_innen mit Tumoren des Zentralnervensystems (ZNS), insbesondere in der Nachsorge, da sie eine Risikogruppe für Spätfolgen darstellen. Um die Kooperation zu verbessern, wurden mehrere praktische Unterstützungsmöglichkeiten eingeführt. Im Rahmen des vorgestellten Pilotprojekts wurde darüber hinaus ein Webinar konzipiert und evaluiert, das speziell auf die Fragen von externen Lehrer_innen eingeht. To counteract social isolation and negative consequences (e.g. lack of a sense of belonging, reduced well-being and self-esteem, more frequent class repetitions or low educational attainment) telepresence systems are increasingly being discussed as a way to enable participation for children and adolescents with chronic illnesses. Method: This paper presents different telepresence systems and their advantages and disadvantages, effects of use, and potential difficulties. Nach einer kurzen Erläuterung der aktuellen universitätsrechtlichen Rahmenbedingungen in Deutschland, Österreich und der Schweiz werden (i) am Beispiel von Aufmerksamkeitsstörungen und spezifischen Lernstörungen charakteristische Schwierigkeiten von betroffenen Studierenden beim Studium anhand von Fallvignetten dargestellt und (ii) Ziele in Hinblick auf eine gelingende Partizipation und Chancengleichheit für Studierende mit Beeinträchtigungen skizziert. Since disease-centered assessments do not allow for differentiated biopsychosocial profiling, this study aims to evaluate the psychometric quality of “The School Participation Scales 24/7,” a novel ICF-CY-based assessment tool. Results show high fit indices for most subscales, signifying structural equality between the tool and the ICF-CY. Acceptable criterion validity is given for established neuropsychological tests. Internal consistency and retest analyses revealed that most subscales reliably and stably measure the intended domains. Finally, the tool was standardized using a representative healthy sample. However, frequent neuropsychological assessments are costly and not always feasible. A computerized assessment of basic cognitive functions could contribute to better patient care in this regard. The primary cognitive inventory of the Memory and Attention Test (MAT-COBI) constitutes such a tool. In this feasibility study, we tested N = 44 patients with MS with a standardized neuropsychological assessment consisting of established screening procedures (Brief International Cognitive Assessment in MS [BICAMS] and Brief Repeatable Battery [BRB] and three subtests of the Test Battery of Attentional Performance (TAP). The results of these procedures were considered a gold standard for assessing which patients were cognitively impaired or cognitively unaffected. We also administered the MAT-COBI and compared the results to the gold standard. While the results of an exploratory correlation analysis pointed toward the interpretation that the the MAT-COBI adequately operationalized the basic cognitive domains of attention, memory, and executive, sensitivity was insufficient (32.3 %), while specificity reached 92.3 %. A closer look at the single cognitive domains showed marginal differences across domains (sensitivity ranged from 0–24 %, specificity: 89.5 %-100 %). Based on these results, the MAT-COBI does not appear to be a suitable tool for differentiating between cognitively preserved and cognitively impaired patients with MS.
