This opens the way for other important non-invasive processes of diagnosis including the role and utilisation of Shear Wave Elastography in coming years for liver diseases. This post-hoc study will firstly aim to establish the correlation existing between the currently preferred gold standard liver biopsy and 2D-Shear Wave elastography's diagnostic accuracy in paediatric patients presenting with suspected or confirmed liver disease.</p><p><strong>Methods: </strong>This retrospective study was conducted in Pakistan Kidney and Liver Institute and Research Center in Lahore, Pakistan from August 2017 to January 2024. In this analysis, 37 Paediatric patients with various liver diseases who underwent liver biopsy for autoimmune hepatitis, Budd-Chiari syndrome, Wilson disease and other liver-related pathologies were included.</p><p><strong>Results: </strong>Thirty-seven patients with a mean age of 10 years (ranging from 4-14 years) were enrolled. According to this study, sixteen out of thirty seven (43%) cases of chronic liver disease were caused by autoimmune hepatitis. Patients' average liver stiffness as determined by SWE was 12.14 ± 0.75 kPa. According to elastography, the average liver stiffness in individuals with F0-F1 fibrosis was 6 ± 0.01 kPa, 7.67 ± 0.29 in stage F2, 8.62 ± 0.20 in stage F3, and 14.05 ± 3.69 kPa in stage F4. We discovered that the mean level of hepatic stiffness varied significantly depending on the degree of fibrosis (p = 0.0001).</p><p><strong>Conclusion: </strong>SWE gauges the liver tissue's stiffness, which rises with fibrosis severity. To determine the significance of association between developmental dysplasia of hip with ethnicity in newborns using Graf's method ultrasound.</p><p><strong>Method: </strong>This analytical cross-sectional observational study was conducted in Karachi from January through June 2022 .A total of 115 healthy newborns (both male and female) aged under 28 days were included in the study after parental informed consent. Hip dislocation was assessed clinically using Barlow and Ortolani maneuvers. The gender, ethnicity and hip angles were recorded in the proforma. Graf's method ultrasound was used to measure the hip angles. Acetabular roof, head of femur, triradiate cartilage and acetabular labrum were used as landmarks for angle measurements. The hip joint was classified based on Graf method criteria.</p><p><strong>Results: </strong>The incidence of hip dysplasia was higher in male (16.08%) as compared to female infants (14.78%). The difference between gender distributions was not significant. The predominant ethnicities with pathological hip were Sindhi (9.13%), Punjabi (7.39%) and Pathan (6.08%). The association between ethnicity and pathological hips was statistically significant (p=0.021).</p><p><strong>Conclusion: </strong>There is no significant difference in incidence of DDH with gender. However, there is a significant association between ethnicity and incidence of developmental dysplasia of hip in neonates of 28 days age. Another objective was to analyze and compare the resistance rates of <i>E. coli</i> to specific antibiotics, conducting a year-by-year evaluation of these rates to identify trends and changes over the past seven years.</p><p><strong>Methods: </strong>Retrospective analysis of susceptibility data of <i>E. coli</i> isolated from midstream urine culture samples of patients presenting in outpatient department with uncomplicated cystitis, from January 2016 to December 2022 in the section of Microbiology, Liaquat University of Medical and Health Sciences was done. All the demographic data, clinical information and susceptibility results were obtained from laboratory data base. All the cultures were performed on CLED agar and <i>E. coli</i> was identified using biochemical tests, susceptibility test was performed by disk diffusion method and clavulanate inhibition test for analysis of extended spectrum β-lactamase (ESBL) was performed.</p><p><strong>Results: </strong>A total of 5169 patients with uncomplicated cystitis with no history of renal disease, pregnancy, hospitalization, catheterization and no elderly or pediatric population were included, 76% of which were of females. Mean age was 37 ±11 years Rising trends of resistance were observed in ampicillin (86.3%-95.3%), cotrimoxazole (12.8-48.6%), ciprofloxacin (57.3-81.6%) and low rates of resistance were seen in fosfomycin (0.57-2.96%), nitrofurantoin (0.72-2.96%) and amikacin (2.09-3.15%). Presence of ESBL and emerging resistance to ceftriaxone (15.7-48.7%), piperacillin/tazobactam (0-4.84%) and carbapenems (0-0.39%) was observed.</p><p><strong>Conclusion: </strong>The national guidelines offer an empirical treatment regimen for patients with uncomplicated cystitis. However, regional variations in antimicrobial resistance (AMR) complicate adherence to these guidelines. Challenges in implementing Emergency Obstetric Care (EmOC) in developing nations hinder effective reduction of these deaths. Our objective was to identify key challenges in EmOC practices among frontline healthcare providers, assess the severity and frequency of these barriers, and evaluate gaps in resources, training, and institutional support needed for effective resolution.</p><p><strong>Methods: </strong>This cross-sectional study assessed EmOC knowledge, practical skills, and challenges among Nurses, Lady Health Visitors (LHVs), and Lady Health Workers (LHWs) in Khyber Pakhtunkhwa, Pakistan from July 2017-June 2020. Participants were conveniently sampled from hospitals across eight districts, with data collected through a questionnaire on EmOC knowledge, practical interventions, and challenges faced.</p><p><strong>Results: </strong>Although most participants (91% of nurses, 90.2% of LHVs, 92.3% of LHWs, and 83.3% of midwives) expressed satisfaction with course content, important gaps emerged in the practical application of specific EmOC interventions. Nurses displayed 79% knowledge of the Partogram with only 60% utilization, while LHVs showed 80.5% knowledge but just 50.2% utilization. Assisted delivery knowledge and practice were particularly low among nurses (32% & 7%) & LHVs (17.1% & 2.4%) respectively. Significant gaps were found in retained placenta removal and manual vacuum aspiration, with only 12% of nurses and 3% of LHVs practicing the latter, with no utilization among LHWs or midwives. {"title":"Comparing the efficacy of probiotic plus antibiotic with the Antibiotic Therapy alone on the recurrence of Bacterial Vaginosis.","authors":"Saima Ashraf, Ammara Haroon, Asiya Fayyaz","doi":"10.12669/pjms.41.1.9922","DOIUrl":"10.12669/pjms.41.1.9922","url":null,"abstract":"<p><strong>Objective: </strong>To compare the efficacy of a probiotic plus antibiotic with antibiotic therapy alone for the prevention of the recurrence of bacterial vaginosis.</p><p><strong>Method: </strong>A Randomized control trial was conducted in the Gynecological Department of Nishtar Hospital, Multan, between July 2022 and June 2023. One hundred and twenty (N=120) women with bacterial vaginosis, all cured with metronidazole, were enrolled and randomized. In Group-A, Probiotics were administered once a day for 14 days following antibiotic treatment (Tab Metronidazole 400mg, three times a day for seven days). In Group-B, only oral antibiotics were given. The patients were followed up in four visits for five months for bacterial vaginosis recurrence (responders, partial responders, non-responders, and drop-outs) using Amsel criteria by vaginal swabs. All the data was collected on a structured Performa.</p><p><strong>Results: </strong>The mean age of enrolled 120 women was 32.28±2.58, and 111(92.5%) were married. Of 120 enrolled women, 8 (6.7%) were dropped out. Evaluating the recurrence rate, the probiotic group (n=57) had fewer recurrences than the placebo (n=55) (8.9% vs 21.4%, p-value <0.05). Group-A had a better responder rate at five months of follow-up than the placebo group (39.2% vs. 25.8%) (RR 0.31, 95%CI: 0.1450-0.7998, <i>p</i>-Value <0.05). {"title":"Influence of parity on weight gain during pregnancy in women with Gestational Diabetes: A retrospective cohort study.","authors":"Lin Lin, Ruiyun Chen, Haifeng Lin, Ting Yao, Zihua Chen, Lianghui Zheng","doi":"10.12669/pjms.41.1.11412","DOIUrl":"10.12669/pjms.41.1.11412","url":null,"abstract":"<p><strong>Objective: </strong>This retrospective cohort study aimed to investigate the effects of parity on gestational weight gain (GWG) and its association with maternal and neonatal outcomes in women with gestational diabetes mellitus (GDM).</p><p><strong>Methods: </strong>This retrospective cohort study data from 2,909 pregnant women with GDM who delivered between 2021 and 2023 at Fujian Maternity and Child Health hospital, were analyzed. Participants were categorized into nulliparous (no previous births), primiparous (one previous birth), and multiparous (two or more previous births) groups. Women aged 18-45 years with singleton pregnancies were included, while those with multiple pregnancies, pre-existing diabetes, hypertension, thyroid disorders, or incomplete BMI data were excluded. Data extracted from hospital records included demographic information, BMI, GWG at various gestational stages, and pregnancy outcomes. Statistical analysis involved logistic regression models, adjusting for confounders such as maternal age, BMI, and gestational age at delivery. P-values < 0.05 were considered significant.</p><p><strong>Results: </strong>Primiparous and multiparous women exhibited significantly higher GWG than nulliparous women (p < 0.01). Primiparous women had reduced risks of pregnancy-induced hypertension (adjusted OR, 0.64; 95% CI, 0.45-0.92) and premature rupture of membranes (adjusted OR, 0.58; 95% CI, 0.48-0.70). {"title":"Risk factors analysis and nomogram model construction for postoperative atrial fibrillation after off-pump coronary artery bypass grafting.","authors":"Long Qian, Chuanxian Hu, Mengtao Fan, Zhuqing Ji","doi":"10.12669/pjms.41.1.11087","DOIUrl":"10.12669/pjms.41.1.11087","url":null,"abstract":"<p><strong>Objective: </strong>To explore the risk factors associated with postoperative atrial fibrillation (POAF) after off-pump coronary artery bypass grafting (OPCABG) and to construct a nomogram predictive model.</p><p><strong>Methods: </strong>In this retrospective cohort study, clinical data of 193 patients who received OPCABG in Huai'an First People's Hospital Affiliated to Nanjing Medical University from June 2021 to November 2023 were retrospectively analyzed. Based on the established diagnosis of POAF, patients were divided into the POAF group (n=75) and the non-POAF group (n=118). Logistic regression analysis was used to screen for risk factors for POAF after OPCABG. A nomogram prediction model for POAF after OPCABG was constructed based on the independent risk factors, and the model was validated by calibration curve and the area under the receiver operating characteristic curve (AUC).</p><p><strong>Results: </strong>The incidence of POAF after OPCABG in the cohort was 38.86% (75/193). Age, diabetes, history of percutaneous coronary intervention (PCI), duration of operation, length of hospital stay and <i>left ventricular ejection fraction</i> (<i>LVEF</i>) were identified as independent risk factors for POAF after OPCABG (<i>P</i><0.05). The concordance index of the nomogram model for predicting the risk of after POAF after OPCABG based on the six independent risk factors was 0.820. The correction curve tended towards the ideal curve, and the area under the receiver operating characteristic curve was 0.820 (95% CI:0.758~0 882).</p><p><strong>Conclusions: </strong>Age, diabetes, history of PCI, duration of operation, length of hospital stay and LVEF are independent risk factors for POAF after OPCABG. One hundred and twenty patients with depression were treated in Hebei Provincial Mental Health Center between December 2020 and May 2022 and randomly divided into the experimental group and the control group, with 60 patients in each group. Patients in the control group were treated with selective serotonin reuptake inhibitor (SSRI) antidepressants and those in the experimental group were treated with modified SYD in addition to SSRIs for eight weeks. Explored the efficacy and adverse drug reactions of the two treatment options.</p><p><strong>Results: </strong>After eight weeks of treatment, there was no statistically significant difference in the total efficacy between the two groups(p>0.05); the total score of HAMD-17 and scores of factors except body weight were decreased in the experimental group compared with those in the control group, and the differences were statistically significant(p<0.05). The SSI score of the experimental group was decreased compared with that of the control group, and the difference was statistically significant(p<0.05).</p><p><strong>Conclusion: </strong>SYD combined with SSRI antidepressants in the treatment of depression can reduce the total score and the scores of factors of HAMD-17 without increasing the incidence of adverse drug reactions. {"title":"Efficacy of supra-inguinal fascia iliaca compartment block in proximal femoral nail antirotation internal fixation for patients with intertrochanteric fractures.","authors":"Genfeng Ding, Jinling Shang, Qing Wang","doi":"10.12669/pjms.41.1.10715","DOIUrl":"10.12669/pjms.41.1.10715","url":null,"abstract":"<p><strong>Objective: </strong>To evaluate the efficacy of supra-inguinal fascia iliaca compartment block (S-FICB) in patients undergoing proximal femoral nail antirotation (PFNA) internal fixation surgery for intertrochanteric fracture (ITF).</p><p><strong>Methods: </strong>Retrospective analysis of 95 patients with ITF undergoing PFNA internal fixation surgery in the First People's Hospital of Yong Kang from March 2021 to August 2023 was performed. Among them, 49 patients received general anesthesia (GA; GA group) and 46 patients received S-FICB combined with general anesthesia (S-FICB group). Mean arterial pressure (MAP), heart rate (HR), anesthesia effect, pain, stress index levels, functional recovery, and adverse reactions were compared between two groups.</p><p><strong>Results: </strong>Perioperative MAP and HR, extubation time, anesthesia recovery time, and dosage of remifentanil and propofol were significantly lower compared to the GA group (<i>P</i><0.05). Perioperative pain level in the S-FICB group was significantly lower than in the GA group (<i>P</i><0.05). On the first day after the surgery, stress response of the S-FICB group was significantly lower than that of the GA group (<i>P</i><0.05). Among them, 58 patients underwent conventional laparoscopic surgery (laparoscopic group), and 62 patients underwent ICG-F assisted laparoscopic surgery (ICG-F group). The perioperative conditions, levels of inflammatory factors before and after the operation, levels of liver function indexes, and incidence of complications in the two groups were statistically analyzed.</p><p><strong>Results: </strong>The perioperative indexes of patients in the ICG-F group improved compared to the laparoscopic group (<i>P</i><0.05). On the second day after the operation, the inflammatory response in the two groups was significantly higher than before the operation and significantly lower in the ICG-F group compared to the laparoscopic group (<i>P</i><0.05). Liver function of the two groups decreased two days after the surgery and was markedly lower in the ICG-F group than the laparoscopic group (<i>P</i><0.05).
