The total duration of clinical data collection was six months (April and September 2022) from 183 diabetic patients across different centers in Pakistan. Patients who had received IDegLira in combination with basal/premixed insulin, OHAs, or both regimes (basal/premixed insulin and OHAs) for the treatment of Type-II diabetes (T2D) in the Pakistani population were analyzed.</p><p><strong>Results: </strong>After receiving IDegLira therapy for six months, mean HbA1c was reduced by -1.1%, mean body weight reduced by 1.2 kg, fasting blood glucose levels (mg/dl): 164.7 (±36.2) to 134.4 (±32.9)] and random blood sugar (mg/dl): 240.1 (±71.4) to 207.6 (±77.8)]. The mean IDegLira dose steps were 19.9 (±6.7) at initiation and 23.5 (±7.9) at follow-up. Overall, adverse events reported were generally related to gastrointestinal, and few were related to headache, fatigue, and hypoglycemia events.</p><p><strong>Conclusion: </strong>The values of HbA1c and body weight decreased for the patients after using iDeglira for six months. However, nation-wide training standards linked to general residency has certain limitations. It's important to have dedicated neuro-anaesthesia fellowship program, basis of which should be the three pillars of neuro-anaesthesiology. The objective of this survey was to assess the need of neuro-anaesthesia fellowship based on opinions of practicing anaesthesiologists and neurosurgeons of Pakistan.</p><p><strong>Methods: </strong>After getting ethical approval and consent, data related to this cross sectional survey was collected using online questionnaire at the Aga Khan University, Pakistan. The form was circulated through social media to individuals and relevant groups. The survey link remained active for a duration of one month, from November 3rd, 2023, to December 3rd, 2023.</p><p><strong>Results: </strong>Total 282 forms were received, out of which 63 were incomplete while 219 were analyzed. 80.2% respondents were anaesthesiologists and 19.8% neurosurgeons. A significant finding was utilization of non-neuro-anaesthesiologist based clinical care for majority of surgical cases. Gaps in training during residency years were also identified. Survey participants also agreed that neurosurgical case setup is complex, which mandates the need of specialized knowledge and skills. Overall, 87% of participants favored the need of neuro-anaesthesia fellowship training. In general, 45.2% anaesthesiologist wished to pursue a further fellowship in neuro-anaesthesia, and 65% believed that one-year duration to be adequate. Only 7.0% of physicians proposed that there is no need of additional fellowship training.</p><p><strong>Conclusion: </strong>The need of neuro-anaesthesia fellowship program is well established based on findings of this survey. This was linked to training gaps as a part of general residency curriculum. They were included in this study with non-probability purposive sampling technique from Radiology Department of Aziz Fatimah Teaching Hospital and Aziz Fatimah Medical and Dental College, Pakistan. The duration of study was December 5, 2021 to April 4, 2024. Blood samples (10mL) were drawn for liver enzymes, gene analysis and ultimately PCR products were sent for Sanger sequencing. Data was analyzed using SPSS version 26 with p-Values ≤0.05 considered significant.</p><p><strong>Results: </strong>From 158 studied participants, 43.7% were male, and 56.3% were female. Of total population 51.3% were MAFLD positive while 48.7% were control subjects. Present results showed that CT and TT genotypes of rs738408 were frequently found in MAFLD group (p=0.039 and 0.060, respectively) and significant predictor for developing MAFLD (OR=2.484, p=0.019 and OR=5.167, p<0.001, respectively). Concerning progression of the disease CT genotype showed that subject with this genotype progress to moderate grade of fatty liver disease (OR=2.832, p=0.013). Whereas Odd ratio for TT genotype was significant for severe grade fatty liver diseases (OR=15.50, p=0.007). GG genotype of rs738409 was frequently found in MAFLD group (p=0.032). {"title":"Comparison of the effects of drug-coated balloon and bare metal stent in the treatment of lower extremity arteriosclerosis obliterans.","authors":"Yu Zhang, Kefan Wang, Lijing Zhou, Nan Guo, Qingyou Chen, Yuanyuan Zhai","doi":"10.12669/pjms.41.5.11520","DOIUrl":"10.12669/pjms.41.5.11520","url":null,"abstract":"<p><strong>Objective: </strong>To compare the efficacy of drug-coated balloons (DCB) and bare metal stents (BMS) in treating lower extremity arteriosclerosis obliterans (LEASO).</p><p><strong>Methods: </strong>Clinical data of 83 patients who underwent LEASO angioplasty with DCB or BMS at the Third Affiliated Hospital of Qiqihar Medical College from July 2021 to January 2024 were retrospectively analyzed. Patients were divided into the DCB (n=43) and BMS (n=40) groups based on the treatment method. Changes in ankle-brachial index (ABI), pulse volume recording (PVR), vascular endothelial function indicators, incidence of complications, and patency rate were compared between the two groups.</p><p><strong>Results: </strong>Three months after the surgery, the ABI and nitric oxide (NO) levels in the DCB group were higher, while the upstroke time (UT), percentage of mean arterial pressure (%MAP), brachial-ankle pulse wave velocity (baPWV), and endothelin-1 (ET-1) levels were lower than those in the BMS group (P<0.05). There was no significant difference in the incidence of complications between the two groups (P>0.05). We aimed to explore the concept and domains of well-being from PhD scholars' perspectives, focusing on the factors that affect their well-being during doctoral studies.</p><p><strong>Methods: </strong>This qualitative study was conducted over a period of six months, involving eleven in-depth interviews with medical and allied health sciences PhD scholars from nine universities across Pakistan using a qualitative phenomenology approach. Purposive sampling was employed to recruit participants, and an interview guide was developed after critically appraising the literature, feedback from medical education experts, and a pilot test. Thematic analysis was employed to analyse the data.</p><p><strong>Results: </strong>Five main themes, accompanied by ten sub-themes, have been developed based on the socioecological theoretical framework, aligning with the key findings from eleven in-depth interviews. Five themes were identified from this study: (i)Well-being at the Individual Level, with the subthemes of mental, physical and emotional well-being and time management and work-life balance, (ii)Well-being at the Interpersonal Level, with the subthemes of supervisor and peer support and family and social support, (iii)Well-being at the Institutional Level, with the subthemes of administrative and financial support and academic stress and resources, (iv)Well-being at the Community Level, with the subthemes of professional growth and networking and cultural and gender influences and (v)Well-being at the Policy Level with the subthemes of HEC and institutional policy gaps and financial and research policy. This multifaceted approach offers a comprehensive understanding of the well-being of PhD scholars by addressing factors at multiple levels.</p><p><strong>Conclusion: </strong>This study highlighted the multifaceted dimensions of well-being of PhD scholars at individual, interpersonal, institutional, community, and policy levels. PhD scholars' mental, physical, and emotional health is influenced by time management and academic pressure. PhD scholars' interpersonal well-being depends on supervisors, peers, and family support. Effective administrative systems, financial support, and academic resources reduce scholar stress and improve institutional well-being. Opportunities for professional growth, networking, and cultural and gender recognition in the community are crucial for improving PhD scholars' well-being. While some cases may have unknown causes, the majority are attributed to specific underlying conditions such as kidney tumors, inflammation, stones, vascular diseases, and coagulation disorders. It is important to note that IRR is not commonly observed in patients undergoing hemodialysis. Herein, we present a unique case of a middle-aged male patient with chronic nephritis syndrome and COVID-19. The individual experienced severe headache and abdominal pain during the initial stages of hemodialysis, leading to the timely diagnosis of subarachnoid hemorrhage (SH) and IRR. Following the diagnosis, the patient underwent symptomatic conservative treatment for SH and super-selective renal artery embolization for IRR, resulting in gradual relief of his symptoms within three days. After a week, the patient's overall condition showed significant improvement. {"title":"Metabolically healthy obesity (MHO) and factors influencing its prevalence in a population in North-West Lahore.","authors":"Mohammad Perwaiz Iqbal, Tehreem Sajjad, Saleem Perwaiz Iqbal, Muhammad Khurram","doi":"10.12669/pjms.41.5.11694","DOIUrl":"10.12669/pjms.41.5.11694","url":null,"abstract":"<p><strong>Objective: </strong>To investigate the prevalence of metabolically healthy obesity (MHO) in a population residing in Northwest of Lahore and study the association of some lifestyle factors influencing it.</p><p><strong>Methods: </strong>In a cross-sectional design, a study was conducted at the Department of Life Sciences, University of Management and Technology, Lahore, in which 496 adults of either gender having abdominal obesity were included with informed consent. Those having none or one of the following metabolic abnormalities - hypertension, hypertriglyceridemia, hyperglycemia and low HDL-cholesterol along with abdominal obesity were classified as having metabolically healthy obesity (MHO). These metabolic abnormalities were measured in all recruited subjects. A questionnaire comprising demographic and lifestyle habits was used for information about factors influencing MHO. Chi-square test and logistic regression were used for studying the association of these factors with MHO. The duration of the study was from August 2022 to October 2023.</p><p><strong>Results: </strong>Prevalence of MHO was 11% of the population studied. It was inversely related to age. Regarding association of MHO, compared to males, the adjusted odds ratio (AOR) among females was 74% lower [AOR = 0.26; 95% CI (0.11-0.59)]. Moreover, married individuals had 0.36 times odds of MHO compared to single ones [AOR = 0.36; 95% CI (0.16-0.8)]. Arm/chest circumference was measured. BMI was calculated. Standard protocols were followed to measure BP and HR. FBS was assessed through biochemical testing. A t-test and ANOVA were used to compare variables. For associations, the Pearson correlation was used.</p><p><strong>Results: </strong>Significant differences were noted among all categories of BMI, HR, systolic-BP, and FBS. Females had a BMI 14.9% lower than males in overweight/obese (p=0.021). In the normal-HR category, females showed rates 5.82% lower than males (p=0.0002), while in the normal-BP category, females comprised 83.83% compared to 16.16% for males (p=0.027). Males had an FBS 3.79% higher than females in elevated-FBS (p=0.039) despite 70% of those affected being female. A positive correlation between FBS and HR was found in borderline and normal FBS. Being overweight/obese was positively associated with systolic-BP (p=0.006), diastolic-BP (p=0.011), arm-circumference (p=0.040), and chest circumference (p=0.018).</p><p><strong>Conclusion: </strong>Overweight/obese females had a lower BMI but higher FBS than males, who had higher HR and BP. FBS is positively associated with HR in borderline and normal FBS categories. BMI significantly influenced arm/chest size, BP, and FBS. In this study we tried to determine the association of CAD with polymorphisms in renin-angiotensin-aldosterone system (RAAS) genes AGT(M235T) rs699 and AGTRI(A1166C) rs5186.</p><p><strong>Method: </strong>This case-control study was conducted at Ziauddin University and National Institute of Cardiovascular Diseases Karachi from January, 2019 to June, 2020. It included 239 participants between 30-70years from both genders via convenient sampling. The participants were divided into two groups of 160 controls and 79 angiographically diagnosed CAD patients. Genotyping of AGT(M235T) and AGTRI(A1166C) was investigated by the allele-specific polymerase chain reaction (AS-PCR). Statistical analysis was done using SPSS Version-22. Independent sample t-test was applied for comparison of quantitative variables. The AGT(M235T) and AGRT1(A1166) genes were compared by Chi- square test.</p><p><strong>Results: </strong>There was no significant association found between CAD and AGT(M235T) gene variants CC, CT and TT (p=0.3; p=0.1; p=0.6 respectively). AGTRI(A1166) of AA and CC variety showed significant association with CAD(p<0.001), while its AC variant showed no significant association with the disease. The odds of CC of AGRT1(A1166C) having CAD were 14 times more, whereas having CAD with AA of AGRT1(A1166C) were 70% less.</p><p><strong>Conclusion: </strong>Individuals with CC polymorphisms of AGTRI(A1166) gene are 14 times more likely to develop CAD, whereas those with AA variation are less likely to develop the disease. The study focused on patients over 18 years old with known cases of non-alcoholic fatty liver disease (NAFLD) visiting outpatient departments. Demographic details, co-morbidities, examination findings, and pertinent laboratory tests were systematically documented utilizing a standardized proforma. Additionally, ultrasound reports of liver scans were scrutinized to categorize NAFLD severity into mild, moderate, and severe cases.</p><p><strong>Results: </strong>In this study of 246 NAFLD patients, severity distribution was 35% mild, 24.4% moderate, and 40.2% severe, with a mean age of 53.1 years and a female majority (52%). Significant associations were found between NAFLD severity and age, gender, blood pressure, tobacco use, diet, and obesity. Biomarker analysis revealed elevated levels in severe NAFLD cases, particularly uric acid (7.63 vs. 6.6 vs. 5.96; P<0.001) and HbA1c (6.77 vs. 6.15 vs. 5.8; P<0.001). Hyperuricemia was significantly associated with NAFLD severity (P: 0.001), with 56.6% of severe cases exhibiting hyperuricemia.
