Currently, there is a lack of research on VO in PD patients specifically focusing on the unique group of long duration PD patients. No studies have yet examined the impact of long dialysis vintage on VO from the perspective of research subjects. This study conducted in-depth interviews with long duration PD patients to explore their positive experiences in volume management.</p><p><strong>Methods: </strong>Using purposive sampling, 10 long duration PD patients from a nephrology department at a tertiary hospital in Baoding City, China, from September, 2024 to October 2024 were selected as interview subjects in this retrospective study. The phenomenological approach in qualitative research was employed, conducting semi-structured in-depth interviews with these patients using the \"knowledge-attitude-practice\" theoretical framework and incorporating a literature-designed interview guide.</p><p><strong>Results: </strong>Among the 10 interviewees, there were four males and six females aged 35 to 64 years. Data analysis extracted four main themes: illness perception, positive attitude, excellent compliance, and social support.</p><p><strong>Conclusion: </strong>The volume management in long duration PD patients was influenced by multiple factors. This study identified themes of illness perception, positive attitude, excellent adherence, and social support. In the study, the data of pregnant women who applied to the Family Health Center for pregnancy examinations and follow-ups within the five years between January 1, 2018, and December 31, 2022, were taken from the database and evaluated retrospectively. History of current and previous pregnancies, sociodemographic findings, current diseases, allergies, cancer, genetic disease, Rh incompatibility, medication use, pregnancy risk factors, pregnancy complications, and delivery method and outcomes were obtained. Blood total β-hCG measurement results were examined for pregnancy and differential diagnoses. P-values below 0.05 were considered statistically significant. Analyses were made in the SPSS 25.0 program.</p><p><strong>Results: </strong>The mean age of the 239 pregnant women who participated in the study was 28.74±4.02, and the mean total gestational age was 37.28±7.68 weeks. It was determined that 96.2% of the pregnant women had total β-hCG levels in the blood above 5 mIU/ml. Risk factors were present in 26.4% of the pregnant women, and 3.8% of them developed complications during pregnancy. Blood β-hCG levels were higher in those who had an additional disease during pregnancy, those who were on medication, those who had risk factors, and those who developed complications. While total β-hCG measurement levels were lower in those who had normal or cesarean live births, they were higher in those who had abortions (p=0.03).</p><p><strong>Conclusion: </strong>In our study, blood β-hCG levels were higher in pregnant women who had an additional disease, those who were on medication, those who had risk factors and those who developed complications. Total β-hCG measurement levels were lower in those who had normal or cesarean live births, while they were higher in those who had abortions. Different results in blood β-hCG levels require a holistic approach to evaluate pregnancy. Dipotassium glycyrrhizin, known for its anti-inflammatory properties, has shown potential in AD treatment. This study aims to evaluate the effectiveness of emollients containing dipotassium glycyrrhiza extract and the impact of varying application methods on improving the clinical severity of atopic dermatitis in adults during remission periods.</p><p><strong>Methods: </strong>systemic emollient, topical emollient, or no emollient, for an eight weeks duration. The study compared the incidence, disease assessment, and occurrence of adverse events across these groups.</p><p><strong>Results: </strong>After 12 weeks, the SCORAD score in the systemic emollient group decreased significantly by 6.56 points, more than the 4.03-point reduction in the topical emollient group (p < 0.05). The NRS itching score also showed notable decreases in both emollient groups compared to the no-emollient group (p < 0.05). Pharmacological treatment is largely symptomatic, relying on laxatives and neuromodulators. Previous studies have linked IBS to individual factors such as stress, poor sleep, lack of dietary discipline and physical activity. However, data on remission through a comprehensive lifestyle medicine approach remains limited. This intervention included 12 week period of structured sleep routine. It included compliance with a nocturnal sleep schedule from 10-11 pm (bedtime) to 7 am, reading before bedtime, limiting screen time and caffeine consumption especially in late evening. The intervention at the level of stress management involved counseling by a psychiatrist and a nutritionist and mindfulness-meditation. The intervention at the level of physical activity included tracking steps for at least 45 minutes per day using a diary. Personal weekly zoom meetings were conducted for each participant by research team members to ensure thorough understanding of compliance diary. This was in addition to consultation with a nutritionist and a psychiatrist. Compliance was monitored and tailored guidance was provided. To assess comprehension, participants were asked to summarize the instructions provided during the sessions. They were subsequently able to explain the contents and structure of their compliance diaries in a clearly and consistently manner. These efforts ensured a standardized lifestyle management over 12 weeks period. Dietary management included self-documented adherence to a low FODMAP diet after providing them with a list of high versus low FODMAP diet. Participants demonstrated notable improvements in sleep quality, a decrease in stress levels, physical activity, and adherence to a low FODMAP diet which was significantly associated with remission of symptoms. A limitation of the study was small sample size (n=6) as patients became increasingly difficult to follow up with due to multiple dropouts (attrition) during the study. These constraints reduced the power and generalizability of the results. Stroke is the second leading cause of death and a leading cause of long-term disability worldwide, with the highest prevalence in developing countries. Among them, ischemic stroke is the most common type, accounting for more than 80% of the total number of strokes and is the focus of current stroke research. More than one-third of ischemic stroke patients will be affected by post-stroke functional impairments, including but not limited to claudication, upper limb dysfunction, visual impairment, swallowing dysfunction, etc. Swallowing disorders refer to symptoms such as pain, weakness in chewing, prolonged swallowing time, choking when eating or drinking, and coughing due to abnormal function of the swallowing center and motor-sensory pathways and reduced control of the oral and throat muscles. Severe dysphagia will not only affect the patient's nutritional intake and cause malnutrition, but may also cause serious complications such as dehydration, aspiration pneumonia, and suffocation, leading to poor prognosis and even death. Therefore, the care and rehabilitation treatment of such patients have important clinical significance. The treatment strategies integrated in this review provide a variety of approaches for the rehabilitation of patients with post-stroke dysphagia, emphasizing the importance of personalized medicine and the necessity of multidisciplinary collaboration. A total of 120 patients with diabetic peripheral neuropathic pain treated at Affiliated Hospital of Hebei University from May 2023 to May 2024 were selected and randomly divided into two groups. The control group received standard treatment, including maintaining the cleanliness and dryness of the rash, wearing loose clothing, local ultraviolet or infrared irradiation. The study group received LIPUS plus hormone therapy in addition to the standard treatment. Clinical outcomes, pain intensity, sleep quality and anxiety status before and after treatment were compared between the two groups.</p><p><strong>Results: </strong>The overall response rate was 88.33% in the study group and 70.00% in the control group, with the study group showing significantly higher efficacy (p= 0.013). There were no significant differences in VAS and PSQI scores between the two groups before treatment(p>0.05, respectively). After treatment, the study group exhibited more pronounced reductions in the VAS and PSQI scores compared to the control group, with statistically significant differences between the two groups (p< 0.05, respectively). There was no significant difference in the SAS score between the two groups before treatment (p= 0.556).</p><p><strong>Conclusion: </strong>LIPUS plus hormone therapy may provide significant clinical benefits in treating PHN, with marked improvements in pain relief, without increasing the risk of adverse reactions. Of them, 58 patients received routine rehabilitation training (control group) and 62 patients received CRT (observation group). Range of motion of lower limb joints, activities of daily living (ADLs), pain intensity, and incidence of complications were compared between the two groups.</p><p><strong>Results: </strong>After surgery, curvature, straightness, and degree of flexion and extension of the lower limb joints in both groups significantly increased, and was significantly greater in the observation group (<i>P</i><0.05). Modified Barthel Index (MBI) scores of both groups significantly increased after surgery, and was significantly higher in the observation group (<i>P</i><0.05). Post-surgery visual analog scale (VAS) scores of both groups significantly decreased, and were significantly lower in the observation group (<i>P</i><0.05). (Peri-hyst + BIIAL).</p><p><strong>Methods: </strong>It was a cross-sectional comparative study conducted over two years from April 2021 to March 2023, conducted in OBGYN department of Lady Reading Hospital, Peshawar, KPK. The primary outcome was the failure of primary procedure in both group to control blood loss and required relaparotomy. Group-1 were patients undergoing pripartum hysterectomy alone and was compared with Group-2 of the patient in whom bilateral internal iliac artery ligation was also carried out at the time of Peripartum hysterectomy (Peri-hyst +BIAAL). Secondary outcomes analyzed were complications like damage to structure like ureters or sigmoid colon, internal iliac vein injury and haematoma formation in both procedures.</p><p><strong>Results: </strong>When we compare the mean outcomes of both groups by t test, it is seen that group-I had more ICU admission 15(57.7%) verses 11(42.3%), more maternal morbidity in ICU and more maternal deaths Five (55.6%) versus 4(44.4%) massive heamorrhage 25(65.8%)versus 13(34.2%), acute renal failure, acute respiratory failure p=0.818.</p><p><strong>Conclusion: </strong>Failed primary surgical management to control haemorrhage without BIIAL leads to grave outcomes as seen in most admission in ICU and maternal deaths. {"title":"Association between liver biomarkers and risk of cognitive impairment and dementia: A systematic review and meta-analysis.","authors":"Ying Zhong, Lei Li","doi":"10.12669/pjms.41.7.12321","DOIUrl":"10.12669/pjms.41.7.12321","url":null,"abstract":"<p><strong>Objective: </strong>We aimed to systematically examine the literature and conduct a meta-analysis to assess if liver biomarkers like alanine aminotransferase (ALT), aspartate aminotransferase (AST), gamma-glutamyl transpeptidase (GGT) could be predictors of cognitive impairment and dementia.</p><p><strong>Methods: </strong>Online databases of Scopus, Embase, PubMed, CENTRAL, and Web of Science were searched for all observational studies examining the research question. The last date of the search was August 20, 2024.</p><p><strong>Results: </strong>We selected 13 studies. Pooled analysis showed that there was no significant association between ALT levels or AST levels and risk of cognitive impairment. Meta-analysis showed the higher AST: ALT ratio and higher GGT levels were associated with a statistically significant increased risk of cognitive impairment. Lower ALT levels were associated with a significant increase in the risk of dementia but no significant association was noted with AST or GGT levels.</p><p><strong>Conclusions: </strong>Preliminary evidence indicates that liver biomarkers may have a role in predicting the risk of cognitive impairment/dementia. A high AST: ALT ratio may be linked with an increased risk of cognitive impairment and dementia. Ninety children admitted to Baoding Hospital, Beijing Children's Hospital Affiliated to Capital Medical University from July 2022 to December 2023 for laparoscopic appendectomy were randomly divided into the control group and the experimental group(n=45 in each group). The control cohort received routine perioperative nursing care, whereas the experimental cohort received fast-track surgery during the same perioperative period. Postoperative complications, including abdominal distension, nausea, vomiting, urinary tract infection, poor incision healing, were compared between the two cohorts. Evaluated the contrasts in stress biomarkers, including TNF α, IL-6, CRP, and serum cortisol (cort), as well as pain symptoms and satisfaction levels between the control and experimental cohorts.</p><p><strong>Results: </strong>The experimental cohort exhibited considerably shorter postoperative exhaust time, feeding time, getting out-of-bed time, and hospital stay compared to the control cohort, with statistically meaningful contrast(P=0.00). At 12 and 24 hours after surgery, the VAS scores in the experimental cohort were considerably lower than those in the control cohort, with a statistically meaningful contrast(P=0.00).
