{"title":"Total hip arthroplasty: 30 days readmission at a tertiary care hospital.","authors":"Muhammad Omer Farooq, Mahnoor Tariq, Ahsan Sulaiman, Shahryar Noordin","doi":"10.12669/pjms.41.4.11041","DOIUrl":"https://doi.org/10.12669/pjms.41.4.11041","url":null,"abstract":"<p><strong>Objectives: </strong>To assess 30-day readmission at a tertiary care hospital following Total Hip Arthroplasty (THA) and to assess complications for which the patients were readmitted along with associated factors.</p><p><strong>Methods: </strong>This prospective observational study examined patients undergoing THA at The Aga Khan University Hospital from February 2023 to December 2023. Patients were followed after THA up to 30 days. They were followed using an electronic medical record number system. Data on patient demographics and procedure was collected, along with reason for re-admission within 30-days. Statistical analysis was done using STATA version 15.1, where in descriptive statistics median and interquartile range, and frequencies and percentages were determined followed by Fisher's exact test.</p><p><strong>Results: </strong>A total of 67 patients were included in the study. Four male patients above 55 years were readmitted within 30-days of THA. Underlying diagnosis was a significant factor for re-admission (p=0.05). Although, OA and AVN were commonly reported in patients >55 years, two of the readmitted patients had an underlying diagnosis of femoral neck fracture.</p><p><strong>Conclusion: </strong>Diagnosis at the time of admission for THA is a substantial risk factor for readmission that warrants further investigation. The presentation is non-specific i.e loose motions, non-specific abdominal pain, nausea and vomiting. Presenting as a sole initial clinical finding in the absence of other common findings of systemic lupus erythematosus (SLE) makes the diagnosis challenging. After a detailed workup, our patient was diagnosed with LE based on history, physical and laboratory investigations and responded well with initial management. Our report emphasizes the rarity of LE and the significance of its prevalence in the childhood population due to increased morbidity, mortality and complications such as intestinal bleeding, perforation and infarction. {"title":"Development and validation of a Nomogram for the prediction of patients with sepsis-induced multiple organ dysfunction syndrome.","authors":"Jinling Ji, Qiong Wang, Kai Wang, Ting Shi, Chang Li","doi":"10.12669/pjms.41.4.10421","DOIUrl":"https://doi.org/10.12669/pjms.41.4.10421","url":null,"abstract":"<p><strong>Objective: </strong>To develop and validate a model capable of predicting the risk of Sepsis-induced multiple organ dysfunction syndrome (SI-MODS) in hospitalized sepsis patients.</p><p><strong>Methods: </strong>A retrospective cohort study was performed to analyze the clinical data of 415 patients admitted to Department of Medical Laboratory, The Affiliated Huai'an No.1 People's Hospital of Nanjing between January 2019 and January 2022. The least absolute shrinkage and selection operator (LASSO) regression analysis was employed to pinpoint potential variables. A nomogram was developed through multivariate logistic regression. For internal validation, the bootstrapping method was utilized. The nomogram's performance was assessed through calibration, discrimination, and clinical utility analyses.</p><p><strong>Results: </strong>Among the 415 patients, SI-MODS was identified in 46 individuals (11.1%). This model identified seven key variables. The model's internal validation yielded an area under the curve of 0.903 (95% CI: 0.863-0.943). {"title":"Effect of integrated care model on urinary retention after radical hysterectomy for cervical cancer.","authors":"Qiang Zhao","doi":"10.12669/pjms.41.4.9917","DOIUrl":"https://doi.org/10.12669/pjms.41.4.9917","url":null,"abstract":"<p><strong>Objective: </strong>To explore the impact of integrated care model on urinary retention after extensive hysterectomy for cervical cancer.</p><p><strong>Methods: </strong>This was a retrospective study. Sixty eight patients with cervical cancer who underwent extensive hysterectomy in Tangshan People's Hospital from March 2021 to March 2023 were divided into experimental group and control group, 34 cases each. The experimental group received an integrated care model, including periurethral irrigation, pelvic floor muscle and abdominal muscle exercise, induction of urination, bladder function training, and psychological care; the control group adopted a routine care model.</p><p><strong>Results: </strong>The incidence of complications in the experimental group was lower than that in the control group, and the difference was statistically significant (χ<sup>2</sup>=4.221, P=0.040). After nursing, the SAS, SDS and SF-36 scores of the two groups of patients were significantly improved compared with those before nursing, and the degree of laboratory improvement was higher than that of the control group. The differences were statistically significant (P<0.05). {"title":"Value of serum NT proBNP, HMGB1, and SIRT1 in the diagnosis and prognosis of neonatal respiratory distress syndrome.","authors":"Qingqing Yang, Hongbin Zhu","doi":"10.12669/pjms.41.4.10095","DOIUrl":"https://doi.org/10.12669/pjms.41.4.10095","url":null,"abstract":"<p><strong>Objective: </strong>To analyze the value of serum aminoterminal pro-brain natriuretic peptide (NT-proBNP), High mobility group protein B1(HMGB1), and silent information regulator factor 2 related enzyme 1 (SIRT1) in the diagnosis and prognosis of neonatal respiratory distress syndrome (NRDS).</p><p><strong>Methods: </strong>This was a retrospective study. Eighty cases of NRDS infants were selected in Maternity & Child Care Center of Qinhuangdao from January 2022 to January 2024 as the study group, with 80 cases of normal newborns during the same period selected as the control group. The NRDS infants were divided into the excellent prognosis group (n=49) and the poor prognosis group (n=31). The levels of serum NT-proBNP, HMGB1, and SIRT1 were compared among the groups, along with an investigation of the value of these biomarkers for the diagnosis and prognosis of NRDS infants.</p><p><strong>Results: </strong>The study group exhibited higher levels of serum NT-proBNP and HMGB1(P< 0.05) and lower levels of serum SIRT1(P< 0.05) than the control group. The ROC curve analysis suggested that AUC values for serum NT-proBNP, HMGB1, SIRT1, and their combination in the diagnosis of NRDS were 0.903, 0.829, 0.794, and 0.958, respectively. The study was conducted in a private medical institute in Karachi, Pakistan, from September 2022 to April 2023. To select participants, a modified menstrual symptom questionnaire was used, and to measure the pain, the Visual Analogue Scale (VAS) was used during the four months of the study. A convenient sample of 50 healthy medical students were included who suffered from PD and were not using analgesics. After taking informed written consent, in the first month, VAS scoring was done without intervention on the first three days of the menstrual cycle (MC). In the subsequent three MCs, acupressure was self-applied (after being taught by a qualified acupressurist) three times a day at acupoint SP6 for two to five minutes on both legs one by one during the first three days of each intervention MC and VAS pain scoring was done.</p><p><strong>Results: </strong>A statistically significant reduction in pain intensity (p<0.01) was observed in post-acupressure VAS scores during the first three days of each intervention menstrual cycle.</p><p><strong>Conclusion: </strong>Acupressure at the SP6 point has been shown to effectively reduce pain intensity in medical students with primary dysmenorrhea. This study aimed to explore the impact of staged rehabilitation guidance based on symptom management theory (SMT) on functional rehabilitation and QOL in elderly patients undergoing HA.</p><p><strong>Methods: </strong>This retrospective case-control analysis included a cohort of 60 patients who received SMT-based staged rehabilitation guidance between January 2023 to April 2024 at The Affiliated Yangming Hospital of Ningbo University. Patients of the SMT group were matched 1:1 with patients who received routine care (the control group). Bronchiectasis not associated with cystic fibrosis is gaining an increasing place among chronic respiratory diseases worldwide. The purpose of this study was to identify the levels of MMP-9, known to cause bronchial damage in chronic pulmonary illness, and SIRT-1, an anti-aging and anti-infective regulatory protein, in patients with bronchiectasis and to evaluate the importance of these biomarkers in diagnosis.</p><p><strong>Methods: </strong>This cross-sectional study was conducted in the Chest Diseases Clinic of Sivas Cumhuriyet University Medical Faculty Hospital between November 2020 and September 2022. We recruited 30 patients with bronchiectasis whose diagnosis was verified by high-resolution chest CT scan and 30 healthy controls. SIRT-1 and MMP-9 levels in the serum of the study group were determined by the ELISA method.</p><p><strong>Results: </strong>SIRT-1 and MMP-9 concentrations were found to be statistically significant. In comparison to the control group, it was observed that the bronchiectasis group had a lower serum SIRT-1 levels (p<0.001). The bronchiectasis group had higher serum MMP-9 values than the control group (p<0.001). Age-related differences in SIRT-1 and MMP-9 concentrations were not observed. No correlation was found between MMP-9 and SIRT-1. The results of Receiver Operating Characteristic (ROC) analysis indicated that MMP-9 has relatively high sensitivities.</p><p><strong>Conclusions: </strong>We concluded that, higher inflammation elevates MMP-9 levels while decreasing SIRT-1 levels. {"title":"Effectiveness of animation as a learning tool in applied medical sciences education: A comparative cross-sectional study among university students.","authors":"Faisal Mousa Alzahrani","doi":"10.12669/pjms.41.4.11204","DOIUrl":"https://doi.org/10.12669/pjms.41.4.11204","url":null,"abstract":"<p><strong>Background & objectives: </strong>This study investigates the effectiveness of animation compared to traditional lecture methods on student performance, cognitive outcomes, and knowledge retention among Applied Medical Sciences University students.</p><p><strong>Methods: </strong>In this cross-sectional comparative study, a total of 110 students from the College of Applied Medical Sciences (CAMS), Imam Abdulrahman bin Faisal University participated in the study conducted between 1<sup>st</sup> April to 30<sup>th</sup> June 2024. Participants were divided into two groups: one receiving traditional lecture-based instruction and the other receiving animated instructional content. Quizzes were structured into two components to evaluate knowledge retention and cognitive skills. The study further analyzed performance, gender differences, and academic year to assess the efficacy of animation.</p><p><strong>Results: </strong>The animation group outperformed the traditional group in overall scores of 72.48±10.13 vs. 61.16±12.30, (p=0.001), and the passing rate scaled were 60% and 80%. Largely significant differences were seen in male students (p=0.002) and fourth-year students (p=0.001) in the animation group. Knowledge and cognitive assessments showed statistically significant gains in the animation group compared to traditional methods (p< 0.05).</p><p><strong>Conclusion: </strong>Animation-based learning significantly enhances student performance, particularly in cognitive tasks, and is especially beneficial for fourth-year students. This highlights the effectiveness of animation as a teaching tool. The study conducted at Physical Therapy gym Khyber Medical University Peshawar from January 2019 to June 2019. Over four weeks, participants underwent a submaximal interval training program, comprising treadmill walking or running at 70-80% of their maximum heart rate for one minute, followed by one minute of active recovery conducted three time weekly. Assessments comprised of endurance VO2 max using the Bruce Protocol, flexibility of the lower back extensors and hamstring muscles through the Young Man Christian Association (YMCA) sit and reach test, body mass index (BMI) and waist-hip ratio (WHR).</p><p><strong>Results: </strong>The submaximal interval training intervention yielded significant improvements in endurance VO2 max, (p=0.001), flexibility (p=0.001) and BMI (p=0.016). However, the waist-hip ratio did not show any significant changes, mean pre-score (0.83±0.95) remaining the same in the post-score (0.83±0.93) (p=0.64).</p><p><strong>Conclusion: </strong>The submaximal interval training intervention demonstrated a high level of tolerance, as evidenced by a remarkable adherence rate of 91% among participants.
