Cet article propose une lecture physiopathologique du PPPD. Des enregistrements EEG/EOG lors de stimulations devant le Panneau Panoramique Stéréoscopique (PPS) ont été réalisés et mettent en évidence l’apparition chez le sujet sain d’un rythme alpha centro-pariétal, marqueur de la bonne harmonie physiologique des mécanismes intégratifs. Chez le patient PPPD, ce rythme n’apparaît pas et suggère une dysplasticité des mécanismes intégratifs. La rééducation par le PPS, centrée sur le mécanisme clé d’un renforcement de la voie visuelle parvocellulaire en inhibant les voies visuelles magnocellulaire et vestibulaire permet la plupart du temps la réapparition de ce rythme alpha centro-pariétal.</div></div><div><div>Persistent postural perceptual dizziness (PPPD) is characterised by chronic instability and intolerance to visually complex environments, reflecting excessive visual dependence and multisensory integration dysfunction. This paper proposes a pathophysiological interpretation of PPPD. EEG/EOG recordings during stimulations in front of the Panoramic Stereoscopic Panel (PPS) were performed and emphasised the appearance of a centro-parietal alpha rhythm in healthy subject, interpreted as a marker of the harmonious physiology of the integrative mechanisms. For PPPD patients, this rhythm does not appear and suggests a dysplasticity of the integrative mechanisms. The PPS rehabilitation centered on the key-mechanism of a reinforcement of the visual parvocellular pathway by inhibiting the visual magnocellular and vestibular pathways allowed most often the reappearance of this centro-parietal alpha rhythm.</div></div>","PeriodicalId":101131,"journal":{"name":"Revue Francophone d'Orthoptie","volume":"19 2","pages":"Pages 83-87"},"PeriodicalIF":0.0,"publicationDate":"2026-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148247364","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

L’être humain, sur ses deux jambes, ajuste constamment sa posture pour ne pas tomber. Pour y parvenir, le cerveau intègre les informations provenant des yeux, des muscles, des articulations, de la peau, de l’oreille interne, et même des viscères, et bien sûr, des mémoires. La gravité est un référentiel essentiel de l’espace. La tête et le regard sont soit stabilisés, soit réorientés afin de guider les mouvements et éviter la chute. L’équilibration est assurée par le système nerveux central (SNC) grâce à des ajustements permanents, automatiques et réflexes, qui s’adaptent aux changements de l’environnement.</div></div><div><div>Balance allows humans to stand upright and move around despite gravity. L’instauration d’une correction optique de loin précisément ajustée a entraîné une diminution immédiate, majeure et reproductible des paramètres stabilométriques, confirmée sur deux plateformes distinctes. En 2025, la recrudescence de l’instabilité a conduit à une nouvelle adaptation réfractive. Celle-ci a de nouveau permis une amélioration instantanée et significative de la stabilité posturale.</div></div><div><div>We report the case of a patient presenting with severe postural instability in the context of a central vestibulo-cerebellar syndrome, who was initially not corrected for distance vision. Stabilometric assessment performed in 2023 revealed a markedly increased sway area, particularly under eyes-closed conditions. The introduction of a finely adjusted distance optical correction resulted in an immediate, marked, and reproducible reduction in the sway area of the center of pressure. Recordings were obtained using two different stabilometric platforms and repeated on several occasions at different time points, each time demonstrating a comparable ratio of improvement in stabilometric parameters following implementation of the optical correction. In 2025, a renewed increase in postural instability prompted a refractive reassessment, once again leading to an immediate improvement in stabilometric parameters.</div></div>","PeriodicalId":101131,"journal":{"name":"Revue Francophone d'Orthoptie","volume":"19 2","pages":"Pages 99-103"},"PeriodicalIF":0.0,"publicationDate":"2026-04-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148246771","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
