Les plus étudiées, encore aujourd’hui, sont les saccades et la fixation. Elles permettent à la fois une exploration et une discrimination visuelle fine du monde environnant. Ce sont les mouvements oculaires les plus utilisés au quotidien que ce soit sur des tâches visuelles simples ou plus complexes. La poursuite est bien plus singulière. Sa fonction est de permettre aux yeux de suivre une cible mobile dans un contexte visuel global. La poursuite a longtemps été considérée comme difficile à étudier du fait de sa particularité puisqu’il s’agit d’un mouvement lent, fluide et continu. La poursuite est bien mieux connue aujourd’hui grâce aux neurosciences et aux études menées avec des systèmes d’oculométrie de plus en plus perfectionnés. La connaissance des différents systèmes et sous-systèmes corticaux nécessaires à la mise en œuvre de la poursuite permet une approche plus précise dans le bilan neurovisuel ainsi que dans le diagnostic orthoptique. Sa rééducation optomotrice est alors mieux ciblée et adaptée au patient même si elle reste complexe dans sa réalisation.</div></div><div><div>Oculomotor function includes several oculomotors movements. The most studied, even today, are saccades and fixation. They allow for both exploration and fine visual discrimination of the surrounding world. These eye movements are the most frequently used in daily life, whether for simple or more complex visual tasks. Smooth pursuit eye movements are much more singular. This function allows the eyes to track a moving target within a global visual context. Pursuit was long considered difficult to study due to its particularity, as it is a slow, smooth, and continuous movement. Smooth pursuit is way more understood today thanks to neuroscience and studies conducted with increasingly sophisticated eye-tracking systems. Knowledge of the various cortical systems and subsystems necessary for pursuit allows a more precise approach in neurovisual assessment as well as in orthoptic diagnosis. Si de nombreuses atteintes peuvent entraver sa mise en place et son efficience, il apparaît alors fondamental de mieux comprendre ses mécanismes puisqu’elle est, à l’image du modèle de Humphrey et Riddoch, la porte d’entrée du traitement visuel local et global de l’objet d’intérêt permettant d’aboutir à une représentation mentale et à sa dénomination. As a component of oculomotor function, it is nevertheless still frequently associated with a static gaze and a control system that is open to debate. While many factors can hinder its establishment and efficiency, it is therefore essential to better understand its mechanisms, since—as in the model proposed by Humphrey and Riddoch—it serves as the gateway to the local and global visual processing of the object of interest, leading to a mental representation and its naming. By reviewing the scientific literature, this article examines the existence and significance of eye movements during fixation. It also highlights the higher regulatory processes involved, which extend beyond the brainstem.</div></div>","PeriodicalId":101131,"journal":{"name":"Revue Francophone d'Orthoptie","volume":"19 3","pages":"Pages 148-151"},"PeriodicalIF":0.0,"publicationDate":"2026-07-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"148806700","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

This retrospective study (2021–2024) analyzes 190 patients aged 6 to 76, compared to themselves before and after refractive optimization, using standardized stabilometric recordings. The Romberg quotient (RQ) is used to assess the weighting of visual input. After optimization, the proportion of RQ<!--> <!--><<!--> <!-->1 decreases significantly, reflecting a better visual contribution to postural control, regardless of age. A clinical case illustrates a major increase in RQ (0.62 to 6.76) after appropriate correction, associated with a lasting clinical improvement. The results suggest that a finely tuned optical correction objectively changes postural strategy. The use of a force platform provides a quantifiable tool for assessment and monitoring.
