We report the case of a patient presenting with severe postural instability in the context of a central vestibulo-cerebellar syndrome, who was initially not corrected for distance vision. Stabilometric assessment performed in 2023 revealed a markedly increased sway area, particularly under eyes-closed conditions. The introduction of a finely adjusted distance optical correction resulted in an immediate, marked, and reproducible reduction in the sway area of the center of pressure. Recordings were obtained using two different stabilometric platforms and repeated on several occasions at different time points, each time demonstrating a comparable ratio of improvement in stabilometric parameters following implementation of the optical correction. In 2025, a renewed increase in postural instability prompted a refractive reassessment, once again leading to an immediate improvement in stabilometric parameters.
