Li","doi":"10.3760/CMA.J.ISSN.1673-8799.2016.05.003","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1673-8799.2016.05.003","url":null,"abstract":"Objective \u0000To summarize clinical effect and experience for surgical treatment in off-pump coronary artery bypass in 128 patients. \u0000 \u0000 \u0000Methods \u0000128 cases of age older than 60 years with Coronary heart disease (CHD) undergoing surgical repair in off-pump coronary artery bypass from October 2013 to June 2016 were analyzed retrospectively in our department. All the patients received surgical repair in off-pump coronary artery bypass under general anesthesia by the sternum midline incision line. Summary the complications following surgery. \u0000 \u0000 \u0000Results \u0000Surgical operations were performed in all patients and no death occurred during surgery.The perioperative complications after surgery: low cardiac syndrome (3 cases), arrhythmia(7 cases),including supraventricular tachicardia(2cases),complete atrioventricular block (1 cases), atrial fibrillation(4 cases); pulmonary infection with hypoxemia(5 cases).Secondary endotracheal intubation (2 cases),pleural effusion(3 case),atelectasis (2 cases), kidney injury(1 case),brain injury(1 case). The average drainage of chest was 350 ml, the time for making use of respirometer was (10.12±3.46) h, the length of hospital stay was (10±6) d. Respectively, in patients with the third day, the 14th day, the 90th day, collecting blood of elbow vein, measuring the serum levels of LOX-1 by enzyme-linked immunosorbent assay, marking NIHSS score simultaneously. We analysed the relationship between LOX-1 level and NIHSS score , the size of this relationship. \u0000 \u0000 \u0000Results \u0000Serum LOX-1 concentrations of third day, 14th day and 90th day in the case group were compared with the control group. The study protocol was approved by the Ethical Review Board of Investigation in Human Being of Clinic Medical College of Yangzhou University. Informed consent was obtained from each parents.The serum level of total bilirubin(TB), the score of NBNA, the abnormality rate for brainstem auditory evoked potential (BAEP) and aEEG were compared among three groups,meanwhile the correlation for these parameters were assayed. \u0000 \u0000 \u0000Results \u0000(1)There were significant differences in the level of UCB ,the score of NBNA, the abnormality rate of BAEP and aEEG among different groups (F=20.16, 7.12,χ2=15.18,12.23,P 0.05). (2)Of the 7 mildly abnormal aEEG cases in the subclinical bilirubin encephalopathy group, 6 mildly abnormal and 1 moderately abnormal were found in BAEP test. All of the 9 severely abnormal aEEG cases in the subclinical bilirubin encephalopathy group were found 1 moderately abnormal and 8 severely/very severely abnormal of BAEP. By linear correlation analysis,the abnormality degree of aEEG was positively correlation with BAEP (γ=0.806,P=0.032). (3)22 cases of subclinical bilirubin encephalopathy group inclued 16 abnormal aEEG cases (72.73%) and 21abnormal aEEG with NBNA cases (95.45%) respectively,there were statistically significant differences (χ2=4.08,P<0.05). \u0000 \u0000 \u0000Conclusion \u0000aEEG combining with NBNA may be an reference indicator for early evaluation on brain damage in full-term neonates with hyperbilirubinemia. All patients were divided into two groups:observation group (APE with DM2,43 cases) and control group(APE without DM2,83 cases).The differences of clinical manifestations, complications,laboratory results,thrombus localizations,risk stratification and the recurrence rates of acute pulmonary embolism were analysed between the two groups. \u0000 \u0000 \u0000Results \u0000There were no significant differences in age,sex and clinical manifestations between the two groups. The combination of respiratory failure,coronary heart disease and pneumonia were more frequently seen in observation group(P<0.05). The level of creatine kinase, lactate dehydrogenase, fasting blood glucose,cTnI,NT-pro-BNP, CREA were obviously higher than control group(P<0.05). Wilcoxon test was performed to determine the difference of the two and the Spearman test for the correlation. Kappa test was used to evaluate the consistency of the staging of the patients, according to the gGFR and eGFR respectively. \u0000 \u0000 \u0000Results \u0000(1) The global eGFR (median 51.8)was significantly higher than gGFR(median 44.6),Z=-12.53,P<0.001; (2) there was a strong correlation between gGFR and eGFR (r=0.719,P<0.001); (3) the patients were divided into 3 stages (compensatory stage, moderately decreased stage and severely decreased stage ) according to eGFR and gGFR respectively, the global coincidence rate was 63.3% (598/945) with moderate consistency (Kappa=0.405,P<0.001); (4) when the patients were divided into 5 stages according to the gGFR, the serum creatinine value showed significant difference between every two stages (P<0.001) except stage1 and 2 (Z=-1.844,P=0.065), and the creatinine value increased as the stage going up. \u0000 \u0000 \u0000Conclusion \u0000There was a strong correlation between the GFR value estimated by99mTc-DTPA renal dynamic imaging and C-G formula, and a moderate consistency in the staging of the chronic renal disease patients. Expression of I-TAC、CXCR3 in surgical specimens were determined by reverse transcription-polymerase chain reaction and immunohistochemistry, respectively. The expression of I-TAC and CXCR3 was detected by immunohistochemical staining in surgical specimens of gastric carcinoma with and without hepatic metastasis. Associations between the above two indicators and clinicopathological characteristics were finally analyzed. \u0000 \u0000 \u0000Results \u0000Compared with those without liver metastasis of gastric cancer tissue, the gene expression of I-TAC and CXCR3 in the gastric carcinoma tissues with hepatic metastasis were significantly higher. Positive expression rate of I-TAC in the gastric tissues with hepatic metastasis were 93.33%%, while positive expression rate of I-TAC in the gastric tissues without hepatic metastasis were 31.43%%, which were significantly lower than that of the group with hepatic metastasis. Positive expression rate of CXCR3 in the research group were 83.33%, while the positive expression rate of CXCR3 in the control group were 45.71%, which were significantly lower than that of the group with hepatic metastasis. The patients in two groups were all given anti-TB medication, and the research group was also given chest injection of dexamethasone treatment on the basis of anti-TB medication treatment. The clinical efficacy and IL-6 and IL-8 level change was observed. \u0000 \u0000 \u0000Results \u000089 cases showed good effect, and 9 cases was invalid in research group and there were 74 and 21 cases in control group respectively, and there was significant difference between research group and control group (P 0.05). The levels of inflammatory cytokines IL-6 level after treatment of 3, 6 and 9 d in research group was lower than that in control group [(276.49±27.85) pg/ml vs (350.38±20.01) pg/ml,(195.32±14.35) pg/ml vs (312.51±14.82) pg/ml,(86.74±10.28) pg/ml vs (251.74±16.62) pg/ml, and there was significant difference(P<0.05).and IL-8 level[(576.51±19.79) pg/ml vs (850.64±27.70) pg/ml, (225.30±14.29) pg/ml vs (742.49±15.28) pg/ml, (127.68±11.23) pg/ml vs (443.47±12.63) pg/ml] \u0000 \u0000 \u0000Conclusions \u0000Intrapleural injection of glucocorticoid in the treatment of tuberculous pleurisy, can make the drug directly acts on the chest, better realize the control of inflammatory cytokines, encourage pleural effusion absorption, improve the clinical symptoms early. This was a retrospective study that characterized predictive values of baseline RDW to assess severity of CHF, repeated admission. \u0000 \u0000 \u0000Methods \u0000Hematological and biochemical parameters were obtained from 179 patients with New York Heart Association (NYHA) CHF class Ⅰ(n=44), Ⅱ(n=39), Ⅲ (n=41), and Ⅳ(n=55). Receiver operator characteristic (ROC) curves were used for assessing predictive values. \u0000 \u0000 \u0000Results \u0000RDW increased significantly in class Ⅲ and Ⅳcompared with class Ⅰ(14.3±2.3% and 14.3±1.7% vs. 12.9±0.8%, P<0.01).
