{"title":"Effect of mechanical ventilation mode on intra-and postoperative blood loss in patients undergoing posterior lumbar interbody fusion surgery","authors":"Xiaoshun Wu","doi":"10.3760/CMA.J.ISSN.1673-8799.2016.06.003","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1673-8799.2016.06.003","url":null,"abstract":"Objective \u0000To investigate the effect mechanical ventilation mode on intra-and postoperative blood loss in patients undergoing posterior lumbar interbody fusion (PLIF) surgery. \u0000 \u0000 \u0000Methods \u0000According to the random number table method, a total of 88 patients scheduled to undergo lumbar spine surgery were randomly divided into two groups. Patients were received pressure controlled ventilation and volume controlled ventilation in pressure controlled ventilation (PCV group, n=44) and volume controlled ventilation (VCV group, n=44). Mean arterial pressure (MAP), heart rate (HR) and central venous pressure (CVP) were monitored at the onset of anesthesia induction (T0), 10 min after supine position to prone position (T1) and skin closure immediately (T2), 10 min after prone position to supine position (T3) and when tracheal extubation (T4) respectively, and were determined at the above time points of two groups of hemoglobin (Hb) and hematocrit (Hct). The respiration parameters were recorded from T0 to T3. The indicators were recorded in the two groups as following: the volume of intranperative blood loss, 72 h postoperative blood loss, the volume of allogeneic blood transfusion, the volume of fluid input and the rate of re-operation for stopping the bleeding. \u0000 \u0000 \u0000Results \u0000Compared with VCV group, peak inspiratory pressure (PIP) were all lower increasingly from T1 to T3 in PCV group (P 0.05). The differences of Hb and Hct at different time points between the two groups have no statistical significance (P>0.05). According to the effect of different methods to HUVECs, HUVECs were divided into 3 groups: control group (containing 10% health serum medium), uremia group (containing 10% uremic patients serum medium), rhEPO group (medium containing 5, 10, 15 U/ml three different concentrations of rhEPO and 10% uremic patients serum medium medium) and 10% of patients with uremia. After Each medium of group culture effecting on endothelial cells 24 h, reactive oxygen species were detected by colorimetry and cell apoptosis was evaluated by TUNEL assay. \u0000 \u0000 \u0000Results \u0000rhEPO could reduce oxidative stress levels in cells, and had significant inhibition against apoptosis of endothelial cells induced by uremia serum (P<0.05), in addition, when the concentration of rhEPO increases, the oxidative stress levels and endothelial cell apoptosis index were reduced gradually (P<0.05). Intraoperative use of bilateral percutaneous puncture, according to the the puncture location during operation in different ways, which can be divided into PKP operation method using the new fast puncture positioning system of the group (35 cases), conventional PKP surgery group (35 cases), compared to the average operation time, two groups of patients in the X-ray exposure times, before and after the operation. The visual analogue scale, the average height of vertebral body, local improvement of Cobb angle. \u0000 \u0000 \u0000Results \u0000Two groups of patients were operated by the same surgeon successfully, the operation time of new fast puncture positioning system PKP, intraoperative X-ray exposure times respectively are (43±16) min, (32±6) times, the operation time of the traditional PKP, intraoperative X-ray exposure times respectively are (62±13) min, (53±11) times, between the two groups have significant difference (P 0.05). {"title":"Screening differentially expressed genes of gastric stromal tumors originated form the muscularis propria by gene chip technology","authors":"Huan-kai Hu, Liu-ye Huang, Bo Zhang","doi":"10.3760/CMA.J.ISSN.1673-8799.2016.06.004","DOIUrl":"https://doi.org/10.3760/CMA.J.ISSN.1673-8799.2016.06.004","url":null,"abstract":"Objective \u0000To screen the related gene expression in gastric muscularis propria tumor by gene chip assay, and to guide the accurate diagnosis and targeted therapygastric of gastric stromal tumors originated form the muscularis propria. \u0000 \u0000 \u0000Methods \u00006 cases of samples with gastric muscularis propria mesenchymal tumor and peritumoral tissue were treated by endoscopic treatment and laparoscopic surgery. cDNA was synthesized by reverse transcription with total RNA, then by in vitro transcription synthesised of biotin labeling cRNA that was hybridizationed with the Affymetrix mRNA expression profile chip, screened differentially expressed genes with bioinformatics analysis using PANTHER/KEGG database and Gene Ontology software. \u0000 \u0000 \u0000Results \u00003 293 differentially expressed genes were detected in Gastric stromal tumor tissue, including 2 588 up-regulated, and 705 down-regulated genes, especially in DPP10, ETV1, DKK4, CXCL14, MT1M gene expression significantly. These genes were involved in multiple signaling pathways related to tumor, such as Wnt pathway, Rap1 pathway, ECM receptor interaction, Ras signaling pathway, P13K-AKT signaling pathway, and cell adhesion. \u0000 \u0000 \u0000Conclusion \u0000There are significant differences of gene expression between gastric stromal tumor and peritumoral tissue, the aitered expression of DPP10, ETV1, DKK4, CXCL14 and MT1M might be involved in the regulation of gastric stromal tumor, rarely reported before in the literature, and might be new molecular markers or treatment targets. The correlation of integrinβ1 and p73 exprssion with clinicopathologic characteristics and between the two proteins were analyzed. \u0000 \u0000 \u0000Results \u0000The expression of integrinβ1 in the triple negative breast cancers was significantly higher than those of none triple negative breast cancer(83.87% vs 67.24%), with a significant difference(χ2=5.66,P=0.017). The expression of p73 in the triple negative breast cancers was significantly higher than those of none triple negative breast cancer (65.59% vs 46.55%), with a significant difference(χ2=5.33,P=0.021). Montelukast Sodium Tablets to A group,B group received Budesonide Nasal Spray,C group received Montelukast Sodium Tablets combined with Budesonide Nasal Spray.All were treated for 28 days. Respectively,before treatment and after,the quality of life scores and nasal symptoms scores were compared, and serum IgE were detected. \u0000 \u0000 \u0000Results \u0000The total effective rate of group C was 93.3%, better than the A group (83.3%) and B group (80%). The difference was statistically significant(P<0.05). The scores of clinical symptoms and quality of life of the 3 groups after treatment were significantly improved(P<0.05). But the C group compared with A group, B group improved more significantly(P<0.05). The 3 groups after the treatment of serum IgE content compared with before treatment were significantly decreased compared with before treatment,the differences were statistically significant(P<0.05), and C group than in A group,B group decreased significantly(P<0.05). The bone defect was filled by antibiotic loaded cement spacers after fracture was firmly fixed,and the wound was coverred by the vacuum sealing drainage on the end.In different stages of treatment,we would summarize inflammatory indicators.At last time we would do skin flap transplantation to the wound which was clean or we would continue to do the debridement and change antibiotic loaded cement spacers with the vacuum sealing drainage.After the first debridement,we defected the blood sedimentation, creactive protein, neutrophil percentage, etc after debridement and recorded the drain amount which was done bacterial culture on time from vacuum sealing drainage.In the second debridement,we evaluated whether antibiotic loaded cement spacers filled befect sufficiently,and the wound was clean. After finished skin flap transplantation,we assessed the time of infection control and the number of debridement. \u0000 \u0000 \u0000Results \u0000These patients with GustiloⅢB open tibial fractures went thought about one or two times and the infection was controlled effectively in an average of (7.5±2.3)days. All inflammatory markers of these patients are significantly increased within 24 hours after surgery and leveled off from 49 to 75 hours after sugery. What's more, the inflammatory index declined obviously, and returns to normal after 98 hours of the first debridement.We done cultures to testfluid drainage from vacuum sealing drainage and remaindered no bacterial growed.We saw that antibiotic loaded cement spacers fully filled in bone defect,and the wound was clean and there was a lot of fresh granulation tissue and no rheum. \u0000 \u0000 \u0000Conclusions \u0000To treat adult GustiloⅢB open tibial fractures,the vacuum sealing drainage with antibiotic loaded cement spacers can more effectively control the infection、short the average day in hospital and fill in bone defect adequately.This technique can lay the foundation of fixation treatment.
