In cases where the defect is anterior in the cheek, reliance solely on buccal fat pad is insufficient for complete coverage of the affected area. The currently used reconstructive approach for cheek mucosal defects, biological membrane coverage, is associated with a risk of severe mouth opening restriction due to scar contracture, adversely affecting the patient's </span></span>quality of life. This study introduces a novel technique combining a cheek mucosal flap with the buccal fat pad to address anterior cheek defects. The aim of this study is to evaluate the efficacy of a combined cheek mucosal flap and buccal fat pad technique for reconstructing anterior cheek mucosal defects.</div></div><div><h3>Methods</h3><div>A retrospective analysis of five patients who underwent this surgical procedure at Department of Oral and Maxillofacial Surgery, School and Hospital of Stomatology, Wuhan University was conducted. The technique involved creating a cheek mucosal flap and advancing the buccal fat pad to cover the defect. Postoperative outcomes, including mouth opening and scar formation, were assessed.</div></div><div><h3>Results</h3><div>All patients achieved satisfactory healing with mouth openings exceeding 3.0 cm. No significant complications were observed, and the surgical sites were fully mucosalized within three months.</div></div><div><h3>Conclusion</h3><div>The combined cheek mucosal flap and buccal fat pad technique is a viable solution for reconstructing anterior cheek defects, offering improved functional and aesthetic outcomes with minimal complications. Certain post-traumatic deviated noses with a harmonious dorsum and no prominent hump can be managed through slide preservative rhinoplasty. This technic enables nasal straightening with minimal dissection of nasal and septal tissues. An intraoral approach is employed to perform osteotomies and stabilize the result using maxillary bone plates for osteosynthesis. Serum immunoglobulins G and A (IgG and IgA) were measured. Then, animals were sacrificed. Histological and immunohistochemical examinations were performed on the dissected SMGs and associated lymph nodes.</div></div><div><h3>Results</h3><div>The Pfizer group had the highest mean IgG and IgA readings, followed by the Sinovac, Influenza and control groups, with a significant variation between every 2 groups. Histopathological examination of the SMGs of all groups showed nearly normal gland architecture resembling the control group. However, mild changes were observed in vaccinated groups. Lymph nodes’ histopathological examination revealed inactive primary lymphoid follicles<span> in the control group. However, all vaccinated groups revealed active lymph nodes with secondary lymphoid follicles<span>. Regarding the area percentage of CD3 immunoexpression, the greatest value was reported in Pfizer group, followed by Sinovac, Influenza and control groups, with a significant difference between Pfizer and both Sinovac and influenza groups. However, no substantial variation was observed between Sinovac and Influenza groups.</span></span></div></div><div><h3>Conclusion</h3><div><span>All vaccines administered were effective. The Pfizer vaccination had the most </span>T cells<span> and the highest serum concentrations<span> of IgG and IgA. Studies that evaluated AI for OSCC diagnosis with sufficient data to calculate diagnostic accuracy were included. The methodological quality was assessed using QUADAS-2. The primary outcomes were pooled sensitivity, specificity, positive likelihood ratio (PLR), negative likelihood ratio (NLR), and diagnostic odds ratio (DOR). A bivariate random-effects model was used for analysis.</div></div><div><h3>Results</h3><div>Twenty-four studies comprising 18,574 specimens were included. The pooled sensitivity was 0.95 (95 % CI: 0.90–0.98), and the pooled specificity was 0.95 (95 % CI: 0.91–0.98). The pooled PLR was 2.60 (95 % CI: 1.91–3.28), and the NLR was 0.10 (95 % CI: 0.07–0.17), with a DOR of 26.0 (95 % CI: 12.1–55.9). Significant heterogeneity was observed across studies (I² = 97.5 % for sensitivity and I² = 97.8 % for specificity). Deep learning algorithms demonstrated superior performance compared to conventional machine learning methods.</div></div><div><h3>Conclusion</h3><div>AI demonstrates high diagnostic accuracy for OSCC detection, suggesting its potential value as an adjunctive diagnostic tool in clinical practice. The use of bone grafting seems widely used whereas its interest has not been shown to date. The aim of this study was to describe the current state of practice of bone graft in BSSO in France.</div></div><div><h3>Materials and methods</h3><div>An online survey was used to collect data about the use of bone grafting in BSSO procedures among French maxillofacial surgeons.</div></div><div><h3>Results</h3><div>One hundred and forty-eight surgeons responded to the survey, mostly maxillo-facial surgeons (96 %). Only 31.8 % of participants had already performed grafts in BSSO and the majority did it in less than 10 % of cases. The main grafting indication was the degree of advancement especially for advances over 10 mm. Different types of grafts were used, mainly: autologous bone grafting (mandibular or hip) or other bone grafting (xenograft, allograft).</div></div><div><h3>Conclusion</h3><div>This survey shows the diversity of practices in France regarding the use of grafting in BSSO. It's crucial to conduct an overview and in - depth analysis of the prevalence of edentulism and Disability - Adjusted Life Years (DALYs) at global, regional, and national levels from 1990 to 2021.</span></div></div><div><h3>Methods</h3><div><span>This study sourced edentulism data from the 2021 Global Burden of Disease (GBD) database, spanning 1990 - 2021. A Joinpoint regression model, with annual percentage change (APC) and average annual percentage change (AAPC) as indicators, identified </span>inflection points in the time - series. The slope index of inequality (SII) and concentration index (CI) were calculated to evaluate global health inequalities. Das Gupta’s decomposition method and frontier analysis evaluated the link between edentulism burden and sociodemographic development. Finally, a Bayesian age-period-cohort (BAPC) model predicted future prevalence and incidence trends over the next decades.</div></div><div><h3>Results</h3><div>In 2021, the global prevalence of edentulism was reported at 353,000,621 cases (95% uncertainty interval [UI]: 300,591,593–416,167,869), with 9,590,987 DALY (95% UI: 6,202,444–13,470,473), nearly double the figures from 1990. Decomposition analysis highlighted population aging and growth made significant contributions. Health inequality analysis revealed that the SII for DALYs was 93.497 (95% CI: 71.486–115.508) in 1990 and -18.412 (95% CI: -92.144–55.320) in 2021. From 1990 to 2021, the CI for DALYs showed an upward trend, changing from -0.202 in 1990 to 0.353 in 2021. The case involved a 6-year-old boy with a sublingual HGIC, with detailed documentation of the clinical presentation, imaging findings, surgical management, and histopathological features. Searches were conducted in PubMed, Scopus, Web of Science, Embase, and LILACS, supplemented by manual searches/gray literature. The patient exhibited a well-defined sublingual tumor, which was successfully treated by surgical excision. Histopathological analysis revealed both intestinal and gastric epithelium lining the cyst. The searches identified 111 cases of oral HGIC. The mean patient age was 5.4 years, with a slight male predominance. The tongue was the most frequently affected site, followed by the floor of the mouth. Larger cysts were associated with airway obstruction, feeding difficulties, or speech impairment. Microscopically, the predominant epithelial components were gastric, intestinal, and squamous. Surgical excision was the primary treatment and demonstrated low recurrence rates. Although rare, oral HGIC requires a high index of clinical suspicion due to its potential to mimic common </span></span>oral lesions. {"title":"Bilateral inferiorly based nasolabial flaps for full-thickness reconstruction of large lower lip defects following tumour resection","authors":"Mansour Elmoatasembellah ,&nbsp;Omar Hamdy ,&nbsp;Mohammad Attia ,&nbsp;Ibrahim Nazif ,&nbsp;Shadi Awny","doi":"10.1016/j.jormas.2025.102488","DOIUrl":"10.1016/j.jormas.2025.102488","url":null,"abstract":"<div><h3>Objective</h3><div>To report on the feasibility and outcomes of using inferiorly based nasolabial flaps for the reconstruction of large lower lip defects following tumour resection.</div></div><div><h3>Methods</h3><div><span><span>Using a retrospective cohort study design, we enrolled a cohort of patients with large lower lip squamous cell carcinoma who underwent ablative tumour resection and full-thickness reconstruction with bilateral nasolabial flaps at Mansoura University </span>Oncology Centre and Mansoura Health Insurance Hospital between June 2023 and August 2024. The predictor variables were categorized as preoperative, operative, or postoperative variables. The outcomes of interest were oral competence, </span>wound dehiscence<span><span>, flap necrosis, </span>reoperation<span>, and tumour recurrence. Appropriate statistics were computed using a cut-off value of 0.05.</span></span></div></div><div><h3>Results</h3><div><span>The study included six patients with a mean age of 62.5 ± 10.3 years and a mean body mass index (BMI) of 29.93 ± 1.7 kg/m</span><sup>2</sup><span><span>. The average tumour size was 4.7 ± 0.8 cm. All patients were male smokers who smoked more than twenty cigarettes per day. Two operations were complicated with partial flap necrosis (33.3 %), and three patients (50 %) experienced minimal drooling, which improved over time. Only BMI and tumor grade showed a significant relationship with the incidence of </span>postoperative complications (</span><em>P</em> = 0.009, 0.006, respectively). A good aesthetic outcome was observed in all cases.</div></div><div><h3>Conclusion</h3><div>Based on this small cohort, the bilateral inferiorly based nasolabial flap technique is a safe and easily harvested option that provides acceptable functional and cosmetic outcomes, with minimal donor site morbidity, for the reconstruction of large lower lip defects following tumour resection. While early functional orthopaedic intervention holds promise, evidence from long-term studies evaluating outcomes and predictors of treatment response remains sparse.</div></div><div><h3>Materials and Methods</h3><div>This prospective longitudinal study included 84 children (mean age: 12.3 ± 2.1 years) diagnosed with JIA and clinical TMJ involvement. All participants were treated with a standardized modified twin-block appliance over a 2-year period. Clinical, skeletal, inflammatory and quality-of-life assessments were performed at six time points using Maximum Interincisal Opening (MIO), Visual Analog Scale (VAS) for pain, cephalometry (SNB angle), C-reactive protein (CRP), erythrocyte sedimentation rate (ESR), autoantibodies (RF, ANA, anti-CCP), cone-beam computed tomography (CBCT) and the OHIP-14 questionnaire. Prognostic factors and relapse were evaluated using multivariate regression and Kaplan–Meier survival analysis.</div></div><div><h3>Results</h3><div>Over 24 months, patients demonstrated statistically significant improvement in MIO (+7.6 mm), VAS pain score (–3.9), SNB angle (+3.6°), ESR (–14.7 mm/hr) and CRP (–6.9 mg/L) (p &lt; 0.001). Poorer outcomes were associated with RF/anti-CCP positivity, elevated baseline ESR/CRP, severe TMJ changes on CBCT and extra-articular involvement. In contrast, children aged ≤12 years with overjet ≤5 mm, SNB angle ≥76° and mild radiographic findings showed better therapeutic response. The 2-year relapse-free survival rate was 63.1 %.</div></div><div><h3>Conclusion</h3><div>Modified twin-block therapy appears to be a promising adjunct for managing TMJ involvement in JIA.
