Traditional treatments have shown limited efficacy, highlighting the need for novel therapeutic approaches. Tofacitinib, a Janus kinase (JAK) inhibitor, has shown promise in treating various autoimmune diseases, including autoimmune dermatological disorders. This study aims to evaluate the efficacy of tofacitinib in treating patients with LPP.</p><p><strong>Methods: </strong>We conducted a retrospective, single-center observational study at Shohadaye Tajrish Hospital, reviewing records of 74 patients with biopsy-confirmed LPP who had extensive and treatment-resistant disease. Patients were treated with tofacitinib 5 mg twice daily for at least 16 weeks. Efficacy was assessed using the LPP Activity Index (LPPAI), and adverse events were monitored.</p><p><strong>Results: </strong>This study evaluated 74 patients with LPP, predominantly female (83.3%), with a mean age of 46.64±8.05 years. The mean LPPAI score significantly decreased from 4.61±1.26 before treatment to 1.73±1.68 after six months (<i>P</i><0.0001). Response rates varied: 21.62% within 1‒3 months, 24.32% within 3‒6 months, 33.78% within 6‒12 months, and 8.10% within 12‒24 months, with 6.75% showing no response. Adverse effects included headache (8.10%), hyperlipidemia (2.70%), elevated liver enzymes (5.40%), nausea (6.75%), and high blood pressure (4.15%).</p><p><strong>Conclusion: </strong>Tofacitinib represents a promising treatment for LPP, providing significant improvement in disease activity for most patients. However, the association of UA with intraventricular hemorrhage (IVH) has not been adequately evaluated. In this study, we determined the relationship between UA and IVH in patients with intracerebral hemorrhage (ICH). We included 721 patients with ICH who were admitted to a tertiary hospital in South Korea. The patients were stratified into quartiles based on their UA levels. IVH decreased continuously across all quartiles of UA. After adjusting for confounding factors, the odds ratio (OR) for IVH was significantly lower in the fourth quartile compared with that in the first quartile (OR: 0.713; 95% CI: 0.546-0.934; <i>P</i>=0.045). EBV encephalitis is a rare manifestation, and even rarer is its presentation as status epilepticus. To date, fewer than ten cases of EBV encephalitis presenting as status epilepticus have been reported globally. We report a case of an 18-year-old girl who presented with fever followed by seizures. She went into status epilepticus the next day. Cerebrospinal fluid (CSF) analysis showed lymphocytic predominance. Brain imaging showed bilateral symmetric hyperintense lesions in the caudate and the putamen. EBV IgG and IgM were found to be positive. The patient was treated with ganciclovir and anti-epileptics and she improved gradually. Our case emphasizes the importance of considering EBV encephalitis as a potential cause of status epilepticus, even in individuals without immunodeficiency. This study aimed to examine the role of psychiatric disorders in pedestrians ' traffic behavior.</p><p><strong>Methods: </strong>This cross-sectional study was conducted on 275 pedestrians of the Persian Traffic Cohort (PTC) in 2022. The Pedestrian Traffic Behavior Questionnaire, Kessler's Psychological Distress Scale, and the Structured Assessment of Personality Abbreviated Scale (SAPAS) screening questionnaires for people over 14 years were completed. Then, those who had a score above 3 on the SAPAS or a score above 20 on Kessler's scale took part in a psychiatric interview by a psychiatrist or psychiatric resident. The data were analyzed in SPSS v. 26 via independent samples <i>t</i>-test, analysis of variance, and multiple linear regression.</p><p><strong>Results: </strong>The score of positive behaviors was significantly higher in pedestrians without depressive disorder than in those with depressive disorder (95% CI: 42.49-51.17, <i>P</i> value<0.001). The score of aggressive behaviors was significantly higher in pedestrians with depressive disorder than in those without (95% CI: 24.98-28.87, <i>P</i> value=0.001). There was no statistically significant difference between the two groups of pedestrians with and without generalized anxiety disorder in the scores of traffic behavior dimensions (<i>P</i> value>0.05).</p><p><strong>Conclusion: </strong>There was a significant relationship between pedestrians with depressive disorder and the score of positive and aggressive traffic behaviors. Meanwhile, the pedestrians' anxiety disorder was not significantly related to any of their behavioral dimensions. This investigation aimed to determine the pooled prevalence of factors contributing to suicide in Iran, including socio-economic, demographic, and geographical factors.</p><p><strong>Methods: </strong>A systematic search was conducted across Web of Science, Scopus, PubMed, SID, Magiran, Elmnet, ISC, Irandoc, and Noormags databases up to July 2023. We included primary observational studies of acceptable quality that examined the prevalence of factors contributing to suicide in Iranian regions. The findings were screened for eligibility and quality, followed by a review of selected articles, from which data were extracted and analyzed.</p><p><strong>Results: </strong>Out of 1646 initial articles, 68 were selected for review and 54 for meta-analysis. The pooled prevalence rates of contributing factors were calculated as follows: male gender (64.3, 95% CI: 62.6‒66.0%), age over 25 (57.9%, 95% CI: 51.0‒64.5%), under diploma education (73.4%, 95% CI: 62.1‒82.3%), employment issues (66.4%, 95% CI: 59.7‒72.5%), urban living (61.7%, 95% CI: 53.8‒69.1%), past medical history (8.5%, 95% CI: 4.9‒14.2%), past psychiatric history (20.7%, 95% CI: 15.5‒27.1%), past suicidal attempt (12.2%, 95% CI: 8.5‒17.0%), substance abuse history (28.4%, 95% CI: 20.1‒38.3%), spring season (29.8%, 95% CI: 26.7‒33%), and hanging method (46.1%, 95% CI: 41.6‒50.6%). Significant regional differences were observed in the prevalence of gender, age, and suicide methods between western and non-western areas.</p><p><strong>Conclusion: </strong>This study describes key factors of suicides in Iran. Despite higher rates among those over 25, many young individuals are affected. Urban living and low educational attainment are significant factors. Moreover, notable regional differences were observed in gender, age, and suicide methods between western and non-western areas. Clinical Profile and Outcomes of Pulmonary Embolism in Central Iran: A Retrospective Cohort Study. 伊朗中部肺栓塞的临床概况和结局:一项回顾性队列研究。

This retrospective cohort study examines the clinical profiles and outcomes of patients diagnosed with PE at a medical center in central Iran, aiming to identify mortality predictors during hospitalization and follow-up.</p><p><strong>Methods: </strong>Data from 109 patients diagnosed with PE were analyzed, with a median follow-up of 23 months. The collected information included demographic and clinical characteristics, laboratory findings, treatment protocols, and outcomes. Logistic regression and Kaplan-Meier survival analysis were used to identify independent mortality predictors and assess survival impact.</p><p><strong>Results: </strong>The mean age was 59.2 years (±19.7), with 51.4% male. Common symptoms included dyspnea (86%) and chest pain (53%), with non-massive PE being the most prevalent (63%). Independent mortality predictors identified were age (odds ratio [OR] 1.065 per year, <i>P</i><0.001), female sex (OR 4.421, <i>P</i>=0.009), and PE severity (OR 0.262, <i>P</i>=0.023). Kaplan-Meier analysis showed reduced survival probabilities in females (<i>P</i>=0.009), those with provoked PE (<i>P</i>=0.002), patients over 65 (<i>P</i>=0.016), and individuals with comorbidities (<i>P</i>=0.018). She denied previous history of trauma or surgery. Computed tomography (CT) pulmonary angiography showed a paravertebral, soft tissue mass which raised suspicion of a pleural tumor.
