Les études observationnelles à grande échelle sont utiles à cet égard. L’étude NutriNet-Santé est une cohorte prospective française en ligne, lancée en 2009, ayant permis la publication de 12 articles originaux portant sur les facteurs de risque du DT2. Parmi ces 12 études, 11 sont prospectives (suivi moyen : 4 à 9 ans) sur des échantillons de 33 256 à 107 377 participants. La consommation d’aliments ultra-transformés, l’exposition à certains mélanges de pesticides ou une première prise alimentaire tardive ont été associées à un risque plus élevé de DT2. Au contraire, une plus forte adéquation aux recommandations nutritionnelles françaises ou une consommation importante d’aliments issus de l’agriculture biologique étaient associées à un risque plus faible de DT2. D’autres études en relation avec le DT2 sont en cours dans la cohorte NutriNet-Santé, portant sur l’adhésion à des régimes durables, l’exposition à certains additifs et contaminants liés à la transformation des aliments, ou les troubles de santé mentale.</div></div><div><div>Given the increasing prevalence of type 2 diabetes (T2D) worldwide, it is important to better understand its risk and protective factors in order to guide future prevention and disease management policies. Observational studies on a large scale provide valuable information in this regard. The NutriNet-Santé study is a French prospective online cohort, launched in 2009. Since then, 12 original articles focused on T2D have been published. Of those studies, 11 used a prospective design (mean follow-up: 4 to 9 years), analyzing risk or protective factors for T2D related to dietary intake, with samples ranging from 33,256 to 107,377 participants. Consumption of ultra-processed foods, exposure to certain pesticide mixtures or the late timing of the first food intake were associated with a higher risk of T2D. On the other hand, greater adherence to French dietary guidelines and higher consumption of organic food were associated with a lower risk of T2D. The aim of the present study was to identify which minimum dietary changes would be necessary, in addition to halving meat consumption (including poultry and deli meat), to reduce the carbon impact of diet (CI-d) without worsening other environmental impacts, while satisfying a set of nutrient intake recommendations (nutritional adequacy). Based on the average consumption of adults from the INCA3 survey, 10 diets were modeled under different assumptions. The results show that it is possible to achieve nutritional adequacy with 2 times less meat than today (62<!--> <!-->g/d vs. 124<!--> <!-->g/d), without supplements or fortified foods, while significantly reducing CI-d (by up to 50%). The modeled diet that was the closest to current consumption, which had CI-d reduced by 35%, contained meat (excluding deli meat), fish or eggs around 1 time per day, 3 dairy products per day, an abundance of minimally processed and unrefined plant-based foods (70% of solid weight, including 65<!--> <!-->g/d of pulses and 30<!--> <!-->g/d of nuts), 2.5 times fewer fatty, sweet and/or salty products. Il consistait en des tests sensoriels supraliminaires de reconnaissance et d’intensité sur différentes localisations sur la langue, des questionnaires et des prélèvements salivaires avant, pendant et lors du suivi trimestriel jusqu’à un an après le traitement.</div></div><div><h3>Résultats</h3><div>Les altérations gustatives autodéclarées et psychophysiques étaient déjà présentes avant le traitement chez certains patients, se détérioraient significativement dans le temps et puis s’amélioraient tout en persistant au moment de l’arrêt du suivi. L’atteinte était significativement plus prononcée à la base de la langue qu’à la pointe. De plus, un lien entre l’atteinte gustative subjective et le type de traitement du CVADS (plus fréquente en cas de radiothérapie ou radiochimiothérapie) était mis en évidence (<em>p</em> <!-->=<!--> <!-->0,0360). Enfin, un stress oxydatif salivaire était également présent en même temps que les modifications sensorielles.</div></div><div><h3>Conclusion</h3><div>Cette étude soulignait la fréquence et l’importance des altérations gustatives chez les patients atteints de CVADS. Elle apporte des informations nécessaires pour mieux informer les patients avant leur traitement et servir de base à des solutions culinaires industrielles ou domestiques.</div></div><div><h3>Objectives</h3><div>To model the temporal and topographical evolution of gustatory and salivary changes before, during and after treatment for Head and Neck Cancer (HNC), in order to better inform patients about future sensory disorders during their life course, and to devise personalized culinary advice.</div></div><div><h3>Method</h3><div>A prospective, longitudinal follow-up was carried out on 44 patients diagnosed with HNC. It consisted of suprathreshold recognition and intensity taste tests on different locations on the tongue, questionnaires and saliva sampling before, during and at quarterly follow-up up to one year after treatment.</div></div><div><h3>Results</h3><div>Subjective and objective taste alterations were already present before treatment in some patients, deteriorated significantly over time and then improved but persisted when follow-up was discontinued. Impairment was significantly more pronounced at the base of the tongue than at the tip. Dans la lignée des travaux de C. Brocard, M. Saujot et N. Haschar-Noé pour qui les politiques alimentaires et nutritionnelles sont toujours « <em>fragmentées</em> », nous suggérons que le manque d’interconnaissance et d’acculturation dans le domaine de l’« alimentation santé » entre les agents des contrats locaux de santé et des projets alimentaires territoriaux expliquerait leur difficulté à coopérer. Cet article recense les freins à la transversalité des politiques publiques « alimentation santé », notamment entre les CLS et les PAT ; puis présente les journées « passerelle » comme piste de solution. Afin d’identifier les freins au partage d’une culture commune entre les services concernés ont été utilisées la recherche documentaire et sur le terrain : nos observations, des entretiens semi-directifs et un questionnaire. Les résultats confirment une faible coopération des agents, laquelle contraint la mise en œuvre d’une politique « alimentation santé » globale. Pour y remédier, l’interconnaissance des agents, de leurs élus et leur acculturation serait un prérequis nécessaire.</div></div><div><div>The National Food, Nutrition, and Climate Strategy (<em>Stratégie nationale alimentation nutrition climat</em>) must establish a comprehensive, cross-cutting food policy aimed at reducing health inequalities by engaging various stakeholders. Building on the work of C. Brocard, M. Saujot, and N. Haschar-Noé, who highlight the “fragmented” nature of food and nutrition policies, we suggest that the lack of mutual understanding and cultural alignment within the “food-health” domain between local health contract agents and territorial food projects contributes to their challenges in collaborating effectively. This article identifies the barriers to the cross-sectoral integration of “food-health” public policies, particularly between the CLS and PAT, and proposes “bridge days” (“journées passerelles”) as a potential solution. To examine the challenges of fostering a shared culture among the relevant services, we conducted research and fieldwork, including observations, semi-structured interviews, and a questionnaire. The findings confirm a low level of cooperation among agents, which impedes the effective implementation of a cohesive “food-health” policy. Cognitive-behavioural approaches must be implemented. This pilot study analyses the effects of an online cognitive behavioral therapy program incorporating mindful eating and therapeutic education over a treatment duration of 6 to 9 months. Seventy patients (BMI 32.7<!--> <!-->±<!--> <!-->3.1<!--> <!-->kg/m<sup>2</sup>) were included in this program. Fifty-four subjects completed the study and were subject to statistical analysis. Weight, waist circumference, and energy intake significantly decreased (<em>P</em> <!--><<!--> <!-->0.02 for all three elements). Improved listening to internal hunger (<em>P</em> <!--><<!--> <!-->0.0001) and satiety signals (<em>P</em> <!--><<!--> <!-->0.03), as well as a decrease in emotional eating (<em>P</em> <!--><<!--> <!-->0.003) and external eating (<em>P</em> <!--><<!--> <!-->0.0001), were observed. The level of program adherence is positively associated with better outcomes for these two aspects. However, there were no changes in cognitive restriction, food pleasure, mood, and anxiety. Overall, these data highlight that this type of program can be useful in the management of overweight and obesity.</div></div><div><div>L’accompagnement des personnes souffrant de surpoids ou d’obésité ne peut pas reposer sur des recommandations simplistes et autoritaires visant à obtenir une restriction énergétique. Des approches cognitivo-comportementales doivent être mises en œuvre. Cette étude pilote vise à observer les effets d’un programme de thérapie cognitivo-comportementale en ligne qui inclut également des outils d’éducation thérapeutique et de pleine conscience centrée sur l’alimentation. 70 patients (IMC 32,7<!--> <!-->±<!--> <!-->3,1<!--> <!-->kg/m<sup>2</sup>) ont été inclus dans ce programme de 6 à 9 mois. Cinquante-quatre sujets ont terminé l’étude et ont fait l’objet de l’analyse statistique. Les sciences de l’implémentation se développent dans le domaine de la santé pour vérifier les conditions de déploiement d’interventions fondées sur les preuves. Avant de la développer à large échelle, il convient d’évaluer « l’implémentabilité » afin de s’assurer que l’implémentation sera faisable. Nous avons étudié la mise en œuvre de l’intervention multimodale sur les DEterminants et Facteurs de l’ACtivité physique après les Traitements en Oncologie (DEFACTO2) où 55 participants dans l’après-cancer bénéficiaient, pendant 3 mois, d’une séance d’APA, d’ateliers éducatifs collectifs et d’entretiens motivationnels. Les facteurs d’implémentabilité – acceptabilité, fidélité et faisabilité – ont été évalués via des indicateurs quantitatifs et qualitatifs en tenant compte des lieux de mise en œuvre (9 comités départementaux de la Ligue contre le Cancer), des professionnels impliqués et des participants qui reçoivent l’intervention. L’intervention innovante DEFACTO2 témoigne d’une bonne implémentabilité bien que certains éléments devront être améliorés afin de passer à une autre échelle. Ces perspectives et stratégies de mise en œuvre sont décrites.</div></div><div><div>Post-cancer period is complex and individuals living beyond cancer require support to adopt and maintain active behaviors. Implementation science receive growing attention in healthcare, in order to verify the conditions under which an evidence-based intervention can be deployed. Before developing it on a large scale, it is important to assess the “implementability” to ensure that it will be feasible to implement. We studied the multimodal intervention on DEterminants and Factors of physical ACTivity after Oncology treatments (DEFACTO2) in which 55 post-cancer participants benefited from a 3-month program with one APA session, group educational sessions and motivational interviews. Implementability factors – acceptability, fidelity and feasibility – were assessed using quantitative and qualitative indicators, taking into account the places of implementation (9 departmental committees of the Ligue contre le Cancer), the professionals involved and the participants receiving the intervention. The innovative DEFACTO2 intervention demonstrates good implementability, although some elements will need to be improved in order to scale up.
