Rares sont les recherches sur les lieux d’approvisionnement fréquentés et les raisons de ces choix. Cette étude explore les stratégies d’approvisionnement des habitants du quartier Porte de la Chapelle à Paris, marqué par une forte précarité. Elle vise à caractériser les espaces que les habitants fréquentent et parcourent pour s’approvisionner, et à mieux comprendre leurs choix de lieux de courses alimentaires à partir d’une cartographie de l’offre alimentaire et de 25 entretiens. Les résultats confirment que le prix est un déterminant majeur, qui amène les enquêtés à s’approvisionner à l’extérieur du quartier de résidence car les produits disponibles dans les commerces du quartier sont trop chers. Les autres dimensions de l’accessibilité sont également présentes dans les discours. Les participants regrettent le manque de commerces dans le quartier, pas assez diversifiés ou inadaptés à leurs pratiques alimentaires. Les pratiques et les représentations varient en fonction des individus et des secteurs du quartier. Nos résultats invitent donc à construire une politique de diversification des commerces alimentaires avec les habitants des différents secteurs du quartier.</div></div><div><div>Where do you shop for food when you live in a disadvantaged neighborhood? Few studies analyze where people purchase their foods and factors, which may influence food store choice. This study explores the food purchasing behaviors and strategies of residents living in Porte de la Chapelle (Paris, France), neighborhood with high proportion of deprived population. The aim is to characterize food purchase places of inhabitant to better understand food purchase location choices based on food stores mapping and 25 interviews. The results confirm that food price is a major determinant, leading all respondents to purchase outside their residential neighborhood as foods in food outlets of the neighborhood are too expensive for their budgets (price). But the other dimensions of accessibility are also present in interviews. Participants declare the lack of food outlets in the neighborhood (spatial dimension), not enough diversified (availability) or unsuited to their eating habits (cultural acceptability, for example). Behaviors and representations vary according to individual characteristics, and according to the area of live. Cependant, la CB peut entraîner une perte excessive de poids ou de masse musculaire, définissant une dénutrition. Certains facteurs, notamment les troubles du comportement alimentaire, les complications chirurgicales, les CB multiples et la grossesse, peuvent favoriser la dénutrition post-CB, nécessitant un dépistage précoce et une intervention appropriée. En cas de dénutrition sévère, une prise en charge par nutrition artificielle, de préférence par voie entérale, est nécessaire et efficace, sans effets indésirables majeurs. De plus, la CB entraîne des carences nutritionnelles importantes en vitamines et oligoéléments, justifiant une surveillance clinicobiologique régulière et une supplémentation à vie. Une approche pluridisciplinaire, impliquant des diététiciens, des psychologues, des médecins, et chirurgiens, est essentielle pour garantir une qualité de vie optimale aux patients ayant subi cette intervention.</div></div><div><div>Bariatric surgery (BS) is an effective treatment for obesity that enhances patients’ quality of life by inducing significant weight loss, reducing comorbidities such as type 2 diabetes, hypertension, dyslipidemia, sleep apnea, and the physical limitations associated with obesity. However, BS can result in excessive weight loss or significant muscle mass loss, resulting in malnutrition. Some factors, including eating disorders, surgical complications, multiple CB revisions and pregnancy, may predispose to post-BS malnutrition, necessitating early screening and appropriate intervention. In cases of severe malnutrition, management through artificial nutrition (AN), preferably via enteral route, is necessary and effective, without major adverse effects. Additionally, BS can lead to significant nutritional deficiencies in vitamins and trace elements, justifying regular clinical and biological monitoring and lifelong supplementation. Les processus qui sous-tendent ces origines développementales de la santé et des maladies ne sont pas entièrement compris. Les mécanismes épigénétiques, qui gouvernent la détermination de l’identité cellulaire au cours de la différenciation des organes, sont d’excellents candidats pour expliquer comment des évènements précoces peuvent modifier la physiologie de l’adulte. Les descendants mâles nés de mères souris obèses ont un risque plus élevé d’avoir un petit poids de naissance et de devenir obèses à l’âge adulte. L’expression des gènes codant les modificateurs épigénétiques est altérée dans le placenta et le foie fœtal. Les descendants de mères ayant perdu du poids avant la gestation, grâce à une intervention nutritionnelle, ne présentent pas d’anomalie phénotypique, mais l’expression des gènes n’est pas normalisée. Des phénotypes délétères pourraient alors apparaître avec le vieillissement. Ainsi, les mécanismes épigénétiques sont particulièrement sensibles, in utero, au régime et au métabolisme énergétique maternel, ce qui ouvre de nouvelles pistes de recherche.</div></div><div><div>Early exposures to biological, chemical, or social stress can have a long-term impact on adult health. However, the processes underlying these developmental origins of health and disease (DOHaD) are not fully understood. Epigenetic mechanisms govern the determination of cellular identity during organ differentiation. This developmental plasticity gives them a particular sensitivity to the environment. They are therefore good candidates to explain how early events can modify physiology until adulthood. Male offspring of obese mouse mothers have a higher risk of having low birth weight and developing obesity. In addition, the expression of genes encoding epigenetic modifiers is altered in the placenta and fetal liver. The offspring of mothers who lost weight before gestation, due to a nutritional intervention, do not present phenotypic abnormalities, but gene expression is not normalized. This suggests that deleterious phenotypes could appear with aging. After 15, 25, and 50 years of follow-up, a strong negative relation was observed between adherence to Mediterranean Diet (MedDiet) and coronary heart disease (CHD) mortality. Since that study, very numerous prospective observational studies and 3 randomized clinical trials, as well as meta-analyses of cohorts and RCTs have demonstrated the primary and secondary preventive effect of MedDiet towards cardiovascular diseases and more specifically CHD. This review, which is an update of a review published by the same author in 2022, aims to present the more recent results from prospective cohorts, RCTs and meta-analyses. A protective effect of MedDiet towards CHD is clearly confirmed.</div></div><div><div>Entre 1958 et 1964, Keys et ses collaborateurs ont mené l’étude la « Seven Countries Study », qui a recruté des hommes âgés de 40 à 59 ans dans l’une des 16 cohortes de sept pays (Finlande, Grèce, Italie, Yougoslavie, Japon, États-Unis, et Italie). Après un suivi de 15, 25, et 50 ans, une forte relation négative a été observée entre l’adhésion à l’alimentation méditerranéenne (MedDiet) et la mortalité coronarienne. Depuis lors, de très nombreuses études prospectives et 3 essais cliniques randomisés (ECRs), ainsi que des méta-analyses de cohortes et d’ECRs ont démontré l’effet préventif primaire et secondaire de la MedDiet sur les maladies cardiovasculaires et plus particulièrement vis-à-vis de la maladie coronarienne (MC). Cette revue, qui est une mise à jour d’une revue publiée par le même auteur en 2022, a pour but de présenter les résultats les plus récents de cohortes prospectives, d’ECRs et de méta-analyses.
