Un traitement recent, le facteur VII, a montre indirectement l'importance de l'hemostase dans cette affection.\u0000\u0000Objectif \u0000Explorer certains aspects de l'hemostase dans une serie d'hematomes intra-cerebraux spontanes. Methode : Des tests d'hemostase ont ete realises chez des patients porteurs d'un hematome intra-cerebral spontane. Les taux de fibrinogene et des produits de degradation de fibrinogene (PDF) ont ete doses a l'entree, tandis que la numeration des thrombocytes, l'International Normalized Ratio (INR), le temps de cephaline activee (TCA) ont ete par ailleurs determines.\u0000\u0000Resultats \u0000Treize patients successifs ont ete etudies. Chez 3 patients, une elevation des facteurs de degradation du fibrinogene a ete observee (3/13 soit 23%). Un cas de thrombopenie associee a l'augmentation des PDF a ete note.\u0000\u0000Conclusion \u0000Dans un sous-groupe non negligeable d'hematomes, il existe une fibrinolyse concomitante dans les 24 premieres heures. Ce phenomene est soit primitif, soit secondaire a la rupture vasculaire.\u0000\u0000 Background \u0000Intracerebral haematoma is considered as an essentially vascular disease related to the structure of vessels. A recent treatment, VII factor has indirectly shown the importance of haemostasis in this ailment.\u0000\u0000Objective \u0000explored certain aspects of haemostasis in a series of intracerebral spontaneous haematomas.\u0000\u0000Method \u0000Haemostasis tests have been achieved among patients with a spontaneous intracerebral haematoma. The rate of fibrinogen and fibrinogen degradation products (FDP) have been measured at the admission, while the count of thrombocyts, and INR and ACT have also been determined.\u0000\u0000Results \u0000Thirteen successive patients have been studied. In three patients, an elevation of fibrinogen degradation factors has been observed (3/13 or 23%). A case of thrombopenia associated with the increase of FDP has been recorded.\u0000\u0000Conclusion \u0000In a considerable subgroup of haematomas, one could observe a concomitant fibrinolysis during the first 24 hours. We report data on the frequency of neurological manifestations, associations with CD4 counts, and outcome of its various presentations in a 3-year prospective hospital-based study.\u0000\u0000Methods \u0000All HIV-infected patients attending the Infectious Disease clinic for various neurological manifestations between Jan 2001 and December 2003 were included in the study. Their clinical details, treatment received, and response to treatment were accessed and analyzed.\u0000\u0000Results \u0000During this period, 57 had some neurological manifestations and made 145 clinic visits, with 158 distinct episodes of neurological related symptoms, giving a crude rate of 307 neurological episodes per 1000 person year of observation\u0000\u0000Conclusion \u0000Opportunistic infections were the leading cause of neurological disorders in our study population. Identification and management of risk factors remains the key to reducing morbidity and mortality from stroke. Eighty patients with clinical presentation of stroke were recruited consecutively from the Emergency Departments of the University Teaching Hospital and Specialist Hospital - both situated in Benin City, Nigeria. The patients were followed up for a two year period (June 2000 - June 2002) and risk factors analysis was done on all patients. The patients were compared with eighty age and sex matched subjects without stroke (controls). Hypertension remained the dominant risk factor with an odds ratio of 2.68 (95% CI 1.29 - 5.59). Diabetes mellitus independently conferred a risk of 3.23 (95% CI 1.09 - 5.71) and in combination with hypertension enhanced stroke risk (odds ratio 7.21; 95% CI 5.79 - 13.27; p Introduction En etant la deuxieme cause de mortalite et l'une des causes majeures de handicap dans le monde entier, les accidents vasculaires cerebraux (AVC) representent un defit medical majeur. L‘identification et la maitrise des facteurs de risque restent la cle de la reduction de la mortalite et de la morbidite.\u0000\u0000Materiel et Methode 80 patients presentant des signes vasculaires cerebraux ont ete recrutes consecutivement au niveau du service des urgences et du service de neurologie du centre hospitalier universitaire ; Benin (Nigeria). Les patients ont ete suivis pendant une periode de deux ans (juin 2000 - juin 2002) et les facteurs de risque ont ete analyses chez tous les patients . Les patients ont ete compares avec un groupe temoin de 80 personnes sains, sans notion d'AVC.\u0000\u0000Resultat L'hypertension arterielle reste le facteur dominant avec un odds ratio de 2,68 (95 % CI 1,29 - 5,59) le diabete sucre confere independamment un risque de 3,23 (95 % CI 1,09 - 5.71) et en combination avec l'hypertension arterielle le risque de survenu d'un AVC est plus eleve (odds ratio 7,21 ; 95% CI 5,79 - 13,27 ; P inferieur a 0,05, la consommation de cigarette, l'obesite, la fibrillation auriculaire et l'inactivite physique augmentent de maniere significative le risque d'AVC (P inferieur a 0,05) Par ailleurs, les habitudes dietetiques, la consommation d'alcool et l'hypercholesterolemie n'etait pas un facteur de risque important.\u0000\u0000Conclusion Cette etude insiste sur la necessite de controler la pression arterielle et la glycemie dans le cadre de la prevention des AVC. Il importe dans cette optique de veiller egalement au poids, a eviter la consommation de tabac avec pratique d'un exercice physique. L'IRM est une technique d'imagerie cerebrale pouvant preciser les types de lesions a long terme.\u0000\u0000Materiel et Methode \u0000Dans une etude retrospective, en 2003 et 2004, nous avons analyse 75 dossiers d'expertise post - traumatique a l'Universite Libre de Bruxelles (ULB). Les sequelles cerebrales ont ete explorees a l'IRM . Ont ete exclus les patients qui ont subi une intervention chirurgicale cerebrale.\u0000\u0000Resultats \u000075 cas de traumatises cerebraux ont ete repertories ; 64,4% sont de sexe masculin, l'âge moyen etait de 35 ans (7 a 80 ans) ; 40 cas avait une lesion sequellaire cerebrale. Les lesions axonales diffuses ont ete retrouvees dans 11 cas dont 5 cas en region pericalleuse, 2 cas avaient une atteinte des pedoncules cerebelleux. La plupart de ces lesions etaient situees dans l'interface substance blanche/substance grise. Des lesions focales de la base du cerveau etaient observees dans 10 cas. Les sequelles cortico-sous corticales etaient les plus frequentes, 26 cas . L'hemosiderine est un indicateur a long terme de l'atteinte cerebrale.\u0000\u0000Conclusion \u0000Nous concluons que l'IRM est une excellente technique pour etudier les sequelles cerebrales traumatiques et permet d'etablir leur pronostic.\u0000\u0000 Background \u0000Disabilities are frequent in western countries and may be due to cerebral traumatic sequella. MRI with high resolution is an excellent technique to visualise cerebral sequella.\u0000\u0000Materials and Method \u0000In a retrospective study, we reviewed 75 cases of traumatic sequella at the Universite Libre de Bruxelles (ULB), in 2003 and 2004 explorated with MRI. We excluded patients who underwent cerebral surgery.\u0000\u0000Results \u0000Focal sequellea cortical and cortico-sub cortical are most frequent, 26 patients. Hemosiderin and encephalomalacia atrophy are common signs. Depuis quelques annees, dans sa forme cerebrale, elle est au premier plan de l'actualite medicale avec l'ampleur du SIDA dont elle est une affection opportuniste. Objectif Le but de ce travail est d'etudier les caracteristiques generales de la toxoplasmose cerebrale en milieu hospitalier a Cotonou.\u0000\u0000Methode Il s'agit d'une etude prospective transversale a visee descriptive menee a la Clinique Universitaire de Neurologie du CNHU-HKM de Cotonou de Janvier 2001 a Avril 2003. Elle a porte sur dix patients dont les donnees ont ete traitees et analysees a l'aide du logiciel EPI-INFO version 6.04d fr.\u0000\u0000Resultats La frequence de la toxoplasmose est de 2,8%. Les 10 sujets recrutes etaient âges de 39,7  14,39 ans et domines par les hommes (6 cas). La semeiologie clinique etait dominee par 4 signes : l'hyperthermie, le deficit moteur, les crises convulsives et les cephalees. Le scanner cerebral realise dans 8 cas a montre des lesions evocatrices dans 6 cas. La serologie toxoplasmique etait positive a 100% ; neuf patients etaient VIH positifs avec une lymphopenie a 777,28 ± 301,18. Le cotrimoxazole et l'association Sulfadiazine - Pyrimethamine ont ete utilises en premiere intention. La surveillance a ete seulement clinique et a objective une amelioration au bout de 16,5 jours en moyenne de traitement.\u0000\u0000Conclusion Ces resultats suggerent l'existence de la toxoplasmose cerebrale a une frequence non negligeable en milieu hospitalier a Cotonou.\u0000\u0000 Introduction The toxoplasmosis is an ubiquitary parasitose and cosmopolit who was retoutable and fearsome because of its congenital form. But, since many years, in its cerebral form, its is part of the affections of first rank of medical actuality with width of VIH SIDA whom its is an opportunist affection.\u0000\u0000Objective The main objective of this work is to study general caracteristics of the toxoplasmose cerebral in hospital environment in Cotonou (Benin).\u0000\u0000Method The transversal study cross-examined and conducted in clinical universitary of neurology of CNHU-HKM of cotonou with sighting descriptive, from January 2001 to April 2003. This stand on ten (10) patients whom data have been treated and analysed by aid of logiciel EPI-INFO version 6.04 d fr.\u0000\u0000Results The frequency of the toxoplasmose in hospital environment was 2,8%. The 10 subjects recruted were aged of 39,7 ± 14,39 years and dominated by men (6 cases). The clinical semeilogy is dominated by 4 signs: hyperthermy , motor deficit, convulsive crisis and pains. The scanner cerebral realized in 8 cases showned suggestive lesions in 6 cases.The serology was positive in 100% of cases ; nine patients were VIH + with lymphocytes at 777,28  301,18. Cotrimoxazol and sulfadiazim - Pyrimethamin have been utilized in first intention. Andrews","doi":"10.4314/AJNS.V24I2.7570","DOIUrl":"https://doi.org/10.4314/AJNS.V24I2.7570","url":null,"abstract":"Introduction Neuroendoscopic surgery is commonly utilized for the management of intracranial cystic lesions, hydrocephalus, tumor resections and biopsies and for all types of microsurgical procedures that can involve endoscopic assistance.\u0000\u0000Patients and Methods This study presents a retrospective evaluation of the clinical results of the first twenty consecutive patients who underwent neuroendoscopic procedures. The following parameters were examined; demographics, clinical, radiological, operative and outcome data. Patient follow up averaged 17months (R 3-38months).\u0000\u0000Results Twenty (15M, 5F) patients with a mean age of 34 years (R 10 months-74years) underwent a total of 23 neuroendoscopic procedures. Eighty five percent of the patients had a preop diagnosis of supratentorial tumor; 53% of these were extraventricular tumors (EVT). Forty eight percent of the neuroendoscopic procedures were at an extraventricular site. Eighty eight percent of the patients with IVT presented with non-communicating hydrocephalus (NCHC), [chi sq, p Introduction La chirurgie neuro-endoscopique est utilisee habituellement pour le traitement des kystes intracrâniens, les hydrocephalies, l'exerese des tumeurs et les biopsies ainsi que pour tout type de gestes microchirurgicaux demandant une assistance endoscopique\u0000\u0000Patients and Methodes Cette etude retrospective evalue les resultats cliniques des 20 premiers patients consecutifs qui ont beneficie d'une technique neuroendoscopique. Les parametres suivants ont ete examines : demographiques, cliniques, radiologiques donnees operatoires et evolutions. La moyenne du suivi est de 17 mois (R3-38 mois).\u0000\u0000Resultats Vingt patients (15 hommes et 5 femmes) avec une moyenne d'âge de 35 ans (R 10 mois-74 ans) ont subi un total de 23 interventions neuroendoscopiques. Quatre-vingt pour cent des patients avaient un diagnostic preoperatoire de tumeur supra-tentorielle, dont 53% etaient extra-ventriculaires. Quarante-huit pour cent avaient une localisation extra-ventriculaire.\u0000\u0000Quatre-vingt huit pour cent des tumeurs intra-ventriculaires se presentaient sous forme d'une hydrocephalie non communicante ; 57% presentaient une cecite. Deux de ces patients ont vu leur hydrocephalie disparaitre apres la procedure neuroendoscopique. Deux autres ont necessite une verification externe suivi par une derivation ventriculo-peritoneale. Trois patients ont eu une ventriculostomie. Deux patients - une stenose de l'acqueduc congenitale et en rapport avec un tumeur de la fosse posterieure - ont beneficie d'une acqueducoplastie et d'une ventriculostomie. Un volumineux hematome putaminal d'origine hypertensive a ete evacue. Les complications suivantes ont ete observees : un deces, la morbidite etait de 8,7%. This article discusses recent developments in stroke management worldwide. We reviewed and highlighted published clinical guidelines from several countries. The current thoughts on stroke care are discussed and summarized in concise and unambiguous terms. Limitation to optimal management in developing countries as well as areas requiring development and research are highlighted. It should be possible to utilize this in stimulating the development of management strategies for stroke, customized to the unique health structure in Nigeria.\u0000\u0000The management of stroke in Nigeria is suboptimal as there are significant deficiencies in the provision of diagnostic, treatment, rehabilitation and support services. The limited resources, manpower shortage, lack of organized stroke unit, neuro-imaging facilities, ambulance services, education of patients and general practitioners as well as impracticable use of thrombolytics are contributory. Training of stroke experts in collaboration with experts in the developed world with provision of neuro-imaging facilities would improve the outlook of stroke management in Nigeria.\u0000\u0000The focus in Nigeria must be on preventive strategies and ways to harness local resources in the acute treatment of stroke patients. Health education of the community with emphasis on control of the predisposing factors would reduce the burden of stroke in the country. Avant l'avenement de la tomodensitometrie a Dakar, une incidence de 0,8 % etait trouvee.\u0000\u0000Objectif L'objectif de notre travail est de reevaluer cette incidence a la lumiere de cet examen et de determiner les divers facteurs etiologiques.\u0000\u0000Patients et Methode Nos patients ont ete operes par derivation ventriculo-peritoneale et sont âges de 8 a 30 ans au moment du diagnostic. Quatre sont de sexe feminin. Le signe le plus frequent est l'hypertension intra-cranienne. La tomodensitometrie a etabli le diagnostic dans tous les cas. Elle montrait dans 4 cas, un hematome unilateral et dans deux cas, une forme bilaterale. D'importantes calcifications sont retrouvees dans deux cas. Cinq patients ont ete operes et nous notons deux deces. Les valves utilisees sont a debit de drainage fixe.\u0000\u0000Resultat Il existe une augmentation de l'incidence par rapport a notre premiere serie datant de 12 ans. Par rapport aux autres systemes de drainage, nous ne notons pas de difference significative. Aucun facteur etiologique determinant n'est retrouve.\u0000\u0000Conclusion Cette complication pouvant pauci-symptomatique, la realisation d'examens tomodensitometriques systematiques permettrait de constater une plus grande frequence de cette complication\u0000 Introduction Subdural hematoma is a complication which can occur seen after setting a shunt for hydrocephalus. Prior to CT scan installation in Dakar, the incidence was 0,8%.\u0000\u0000Objective Our aim is to consider twelve years later this complication and others causal factors. Six patients were observed, between 8 and 30 years. Four of them were female. The main sign is increased intra cranial pressure. Elles sont benignes mais peuvent evoluer vers l'endocrane.\u0000\u0000Objectif L'objectif de notre etude est de decrire les aspects cliniques,paracliniques et therapeutiques d'un telle affection.\u0000\u0000Patients et methodes Il s'agit d'une serie retrospective de six patients colligee de Janvier 1994 a Decembre 2003. Tous les patients ont beneficie d'un examen clinique et d'explorations radiologiques : radiographie standard et tomodensitometrie. Une voie d'abord neurochirurgicale a ete effectuee dans tous les cas. Le diagnostic est confirme par un examen histologique. Le suivi est assuree pour une duree minimale de 6 mois.\u0000\u0000Resultat Il s'agit de trois hommes et trois femmes. L'âge moyen est de 33 ans. Le signe clinique le plus frequent est la tumefaction frontale progressive. Le delai de consultation varie de 1 a 6 ans. Aucun deficit neurologique n'est note. Le signe radiologique standard le plus frequent est la lyse de la paroi sinusale. La tomodensitometrie montre une extension intra cranienne dans 2 cas. Une voie d'abord frontale a permis une exerese complete dans tous les cas.\u0000\u0000Conclusion Malgre leur benignite,les mucoceles du sinus frontal peuvent presenter d'importantes extensions intracrâniennes sources de souffrance cerebrale. Le diagnostic tardif explique l'etendue des lesions.\u0000\u0000 Introduction Frontal sinus mucoceles are benign mass lesions developing slowly. They can involve intra cranial areas et make brain tissues compression.\u0000\u0000Objective The aim of our work is to describe clinical, radiological and therapeutical aspects of such affection.\u0000\u0000Patients and methods Six patients are received from 1993 to 2003. Three Xrays examination and three CT scans are performed. All patients are operated by neurosurgical approach. Diagnosis is confirmed by histological examination.\u0000\u0000Results The main clinical sign is frontal tumefaction growing during one till six years. None neurological deficit is found. Radiological examinations show lytic frontal sinus wall destruction with intracranial involvement in two cases.
