In urgent situations, there is always a danger of operating the patient when it was not necessary, or viewing the acute surgical process with prolonged observation, which contributes to the development of complications and even death of the patient. Orishchak","doi":"10.52705/2788-6190-2022-1-9","DOIUrl":"https://doi.org/10.52705/2788-6190-2022-1-9","url":null,"abstract":"In the structure of female infertility, the uterine factor occupies one of the leading places and ranges from 25.20% to 42.30%, and in women with repeated unsuccessful attempts at in vitro fertilization, intrauterine pathology is detected in 45% of cases. In the structure of gynecological diseases, hyperplastic processes of the endometrium are 15–40%, and in women of reproductive age – 24.0–62.0%. The objective: to examine the risk predictors of the developing benign hyperproliferative pathology of the endometrium in women of reproductive age to improve preventive approaches of management of this group of patients. Materials and methods. The results of analytical processing of medical documentation of 860 patients with uterine factor of subfertility (pregnant woman’s card (form 111 / y), ambulatory card (form 025 / y-04)), witch were selected by the method of continuous sampling, were presented. This allowed to establish the structure and frequency of endometrial hyperplastic processes in women of reproductive age, to assess the features of reproductive health parameters in this category of patients and to determine prognostic predictors of endometrial hyperplastic processes. For comparison, we analyzed 100 forms of medical records of patients who were routinely preventively examined in these medical institutions. Results. Dominating influences are such clinical nosologies: endometrial polyposis and hyperplastic processes (44.19%), adenomyosis (22.33%), multiple uterine fibroids (10.47%), including submucous myomas (7.79%), uterine synechia (14.19%), uterine retinas and anomalies in the development of the uterus were rarely inserted (2.09% and 1.28% respectively). The uterine factor was the main factor in terms of reproductive losses and subfertility (38.68%), the incidence of the primary form of infertility was 17.63%, the secondary form (43.68%), the incidence in IVF programs was 119 cases (31.32%), including one in five failed IVF attempts (21.85%). Patients ranged in age from 20 to 45 years, and endometrial hyperplasia and polyposis were more pronounced in late reproductive age (60.00%) versus (40.00%) in early reproductive age, respectively (p<0.05). The performed statistical calculations allowed to present the risks of developing hyperplastic processes of the endometrium in relation to such factors as chronic pelvic inflammatory disease (OR = 6.16; 95% CI: 3.52–10.78), adenomyosis and external genital endometriosis (OR=10,59; 95%: 4.21–26.67), uterine fibroids (OR=3.07; 95%: 1.19–7.92), benign tumors and retention of ovaries (OR=4.98; 95%: 1.96–1.67). It should be noted that the share of somatic disorders and their comorbidity increased with age by 1.7 times in cases of hypertension, metabolic disorders, thyroid pathology and diseases of the urinary system (p<0.05). Natural killers NK lymphocytes play a key role in these processes.\u0000The objective: to study the characteristics of the immunity of pregnant women after influenza in the early stages of gestation on the basis of determining NK-cytotoxicity.\u0000Materials and methods. We examined 120 women who contracted influenza in the first trimester of pregnancy: the main group – 68 patients with placental dysfunction, the comparison group – 52 pregnant women without signs of placental insufficiency. The cytotoxicity of natural killers was assessed by flow cytometry. Statistical processing was performed using the methods of variation statistics (Mann-Whitney test and Fisher’s test) odds ratio (OR).\u0000Results. Women with placental insufficiency after influenza in early pregnancy are characterized by both increased and decreased NK-cytotoxicity, which increases the risk of placental insufficiency several times (from 2.87 to 5.03 per OR, depending on one indicator or a combination thereof). The factor “deviation from the norm of both indicators” has the greatest prognostic value\u0000(OR=5.03; 95% CI: 1.38–18.32; p<.05). At the same time, the increase, which indicates a high pro-inflammatory potential and potentially damaging effects of NK cells, has a higher prognostic value than the decrease: among women with placental insufficiency, the proportion with significantly higher levels of NK cytotoxicity was highest: 18 (26.5%) 5 (9.6%) patients of the comparison group (p<0.05).\u0000Conclusions. Excessive inflammatory response in combination with increased NK cytotoxicity, has a damaging effect on the endothelium, and reduced immune responses, causes a longer course of influenza and insufficient immune control of trophoblast invasion. The study involved 120 women with genital endometriosis. The main group included 62 women with genital endometriosis after COVID-19 disease, the control group consisted of 58 patients with genital endometriosis, no symptoms of COVID-19 and negative PCR test at the time of examination. Impact of Covid-19 on lifestyle changes, symptoms and treatment of endometriosis was measured with a questionnaire. Pain assessment was performed by a visually analog scale (VAS). The psychoemotional state of women was analyzed by a Peritraumatic Distress Inventory. Statistical analyses based on Microsoft Excel statistical analysis package and “Statistica 7.0”(StatSoft Inc., USA).\u0000Results. The scientific work demonstrated high level of peritraumatic stress, lifestyle changes (decreased physical activity, increased sleepiness), worsening of clinical manifestations (increased fatigue, severe menstrual bleeding, dysmenorrhea, increased pelvic pain) and changes in treatment measures (increased analgesics and non-steroidal anti-inflammatory drugs using, discontinuation of\u0000hormonal therapy) in women with genital endometriosis after COVID-19 disease compared with group of women with genital endometriosis and without symptoms of coronavirus infection.\u0000Conclusions. Symptoms of the disease significantly reduce women life quality. Clinical sings of vaginal candidiasis are nonspecific. The most common method of diagnosis – microscopy of vaginal discharge, does not always provide enough information.\u0000Among the causative agents of candidiasis, Candida albicans and Candida non-albicans have different prevalence in different regions of the world. Resistance to antifungal drugs is increasing due to it widespread applying. Risk factors for RVVK are: antibiotic therapy, immunodeficiency conditions, diseases of the gastrointestinal tract, contraceptives, corticosteroids, hormonal disorders of\u0000women, allergic diseases. Recent studies indicate the multifactorial cause of candidiasis, especially the imbalanced composition of the vaginal microflora and genetic predisposition. Resistance to antifungal drugs is increasing, especially to Fluconazole, that is most often applied in clinical practice.\u0000The role of immunoregulatory mechanisms is reflected in the reduced activity of immunity in the vagina. Changes in Toll-like receptor (TLR) genes have been reported in women with RVVC. Also, interlexins can lead to a prolonged inflammatory process in the vagina. Currently, polymorphism of the mannose-binding lectin gene in women with RVVK relevant topic for research. Determination of mannose-binding lectin for diagnostic and therapeutic purposes is promising research area. There is\u0000increasing scientific interest in the study of this pathology. Information of prospective inspection appeared research material 171 patient in a postmenopausal period. All patients passed a complete clinical, laboratory and instrumental inspection. During a deep inspection two clinical groups were formed. In a basic group 89 women entered with the presence of metabolic syndrome; a control group was made by 82 women, not having a metabolic syndrome. Into a basic group two sub-groups of women are selected: 1 – with the isolated form of pathology of эндометрия (30) and 2 – with complex pathology of uterus (59); in a control group observed accordingly: a 1 sub-group is 48 patients with the isolated pathology of эндометрия and 2 a sub-group is 34 women with the complex form\u0000of proliferative processes of uterus. In the complex of inspection of women, being under a supervision, entered: careful collection of\u0000anamnesis of life and disease, objective inspection, additional methods of inspection: gisteroscopy on the generally accepted methods.\u0000Results. At hysteroscopic research the women of basic group (in both sub-groups) had hyperplasia of endometrium more frequent, while in the control group of women, having the isolated pathology of endometrium, more frequent the noted atrophy of endometrium. Coincidence of conclusions of morphological research of sraping endometrium and hysteroscopic researches exposed for 156 (91,22%) women. In the complex inspection of patients simple and extended colposcopy of neck of uterus\u0000entered also with a subsequent aiming biopsy. Pathology of exocervix was confirmed by given morphological research of biopsies.\u0000Conclusion. Results of the spent researches testifies about high degree information hysteriscopical estimation complex pathologies of a uterus at women postmenopausal period. For the decision of the put purpose there was the conducted analysis of birth 346 children – in the group of the induced pregnancy after auxiliary genesial technologies (from them 134 in the group of multifetation) and 415 living children in the group of spontaneous pregnancy (from them 142 in the group of multifetation).\u0000Results. The analysis of the state of new-born rotined that at the induced monocyesis the followings pathosiss meet more frequent, than at involuntary pregnancy: asphyxia at birth (4,7% against 1,1%, p<0,05), neonatal icterus (31,6% against 18,6%, p<0,05), intraventricular hemorrhage (3,3% against 0,73%, р<0,05). At multifetation after auxiliary genesial technologies at new-born\u0000also more frequent registered asphyxia at birth, neonatal icterus, respirator distress-syndrome, intraventricular hemorrhage, however statistically reliable differences it is not discovered. By disorders which meet more frequent than all, this group of children had a delay of intrauterine development (29,3%), asphyxia at birth (90,5%), neurological changes (53,6 %).\u0000Conclusions. Auxiliary genesial technologies are not instrumental in the increase of frequency of innate anomalies of development at a fruit, and also to growth of level of perinatal morbidity. Far more frequent a large value has a premonstratensian background of matrimonial pair, reasons of nfertility, efficiency of diagnostics and treatment, and also variant of the in-use programs of\u0000auxiliary genesial technologies. For the decision of the put purpose we conducted a complex clinical and laboratory, functional and immunohistochemical inspection 109 women with combined pathology of uterus and violation of genesial function. 1 (n=69) and 2 (n=19) groups were formed from patients with chronic endometritis and concomitant genital pathology. In 3 (n=21) groups there\u0000were the included women after different endometrial manipulations. For the study of new aspects of pathogeny of violation of genesial function for women with combined pathology of uterus immunohistochemical researches of endometrium conducted the method of double antibodies with immunoperoxidase (streptovidin-biotinic) by a mark, used serial paraffin cuts.\u0000Results. The retrospective study of the given medicare rotined that a desire to have children had served by reason of active treatment at the most inspected women. Anti-inflammatory (including antibacterial) therapy is before conducted at 93,3% patients, thus in the half of cases – repeatedly. Hormonal preparations were before appointed 66,2% to the patients. Endoscopic surgical interferences on the organs of the genesial system are executed for 46,7% patients. Used the more than half of women programs of auxiliary reproductive technologies which did not bring the desired effect. The results of laboratory-instrumental inspection confirmed a presence different structurally functional changes of endometrium, which could be an independent factor, impedimental to implantation and successful development of pregnancy. The increase of proangiogenic potential plays a considerable role in pathogeny of combined pathology of uterus and violations of genesial function.\u0000Conclusions. Combined pathology of uterus is reason of violations of genesial function of women. The establishment of their rational work regime depends on the results of monitoring women’s health, since there is a large list of contraindications that restrict\u0000women from performing certain jobs in harmful and dangerous working conditions.\u0000There are many known accidents at work that have been observed in workers as a result of exacerbation or complications of a general disease. These cases were more common in workers who were not allowed to perform hazardous work due to their health. There are two assumptions about the occurrence of these situations: 1) poor quality medical examinations and 2) admission\u0000to the performance to the performance hazardous work of workers, who for health reasons should be eliminated.\u0000Medical examinations and monitoring of women’s health at the Institute of Occupational Health and Occupational Diseases of KhNMU found that only 10–12% of women are practically healthy, 20–25% have hypertension of varying severity, 5–7% – varicose veins of the lower extremities, 10% – various gynecological diseases, 5% – anemia and other diseases.\u0000When monitoring women’s health on psychophysiological indicators, it was found that in Ukraine about 20% of women perform dangerous types of work.\u0000Medical examinations of the working population for many years revealed that there were significantly more healthy people among males than among females.
