Researches carried stage-by-stage character: on I the stage 150women were inspected with the different degree of perinatal risk; on the II stage – 50 women withcomplications at previous pregnancy. In the complex of the conducted researches were includedclinical, echographic, doppler, cariotocographic, laboratory.\u0000\u0000Results. The ball measuring system of estimation of risk of gestational complications is offeredallows to ground expedience of conducting of medical measures in the group of risk. At the sametime the presented approach opens the prospect of selection to the contingent of pregnant,treatment-and-prophylactic measures, which do not need leadthrough which will allow to avoidgroundless interferences with a gestational process.\u0000\u0000Conclusion. High percent ofchanges of menstrual cycle to the operation (46,0% – with mature teratoma and 54,0% withoutage of serosal cystadenoma), in obedience to our information, direct proportion correlates withthe level of change of ovarian reserve.In a postoperative period the increase of number of patients is marked with violation of menstrualfunction in 1,3–1,4 time in a greater degree after the resection of ovary. An inspection and treatment was conducted 96 patients of genesial agewith infertility on a background passionately dystrophic and hyperplastic processes of endocervix.Basic group from 70 patients were made by women, which treatment of pathology of cervix whichwas studied was executed, by the methods of cryolysis or radio wave surgery (in particular by themethod of cryolysis – 30 patients, by the method of radio wave surgery – 40 patients); to the groupcomparisons entered 26 women which surgical operations – curettage and bougienage concerningpathology of cervix were executed.\u0000\u0000Results. Pathology of cervix is one of principal reasons of femaly infertility, thus in 45,8% – primaryand in 54,2% – secondary. Concomitant violations of menstrual function for these patients meet in89,6%, and chronic inflammatory processes of uterus and appendages – in 33,4% respectively. Forwomen with the secondary infertility after the different methods of operative treatment of pathologyof cervix the burdened genesial anamnesis takes place due to high-purity of artificial (73,7%)and involuntary abortions (44,7%), thus a level of inflammatory complications after differentendometrial interferences is 86,4%. The clinical results of auxiliary genesial technologies dependon the presence of concomitant dishormonal violations for women after the different methods ofoperative treatment of pathology of cervix: frequency of implantation – higher at 1,7 time, offensiveof pregnancy – in 2,0 times and births by a living fetus – at 2,1 time.\u0000\u0000Conclusions. The got results testify to efficiency of the offered algorithm of the differentiated goingnear the introduction of auxiliary genesial technologies for women with pathology of cervix takinginto account the presence of concomitant dishormonal violations. In research which was conducted in 2 stages, it was included 140pregnant in terms from 6 weeks of гестації and to delivery. On a 1 stage by us 3 groupsof women were selected, on the basis of results of clinical inspection and information ofultrasonic research : 60 pregnant of woman with a recurrent retrochorial haematoma (1group); 50 pregnant with a retrochorial haematoma, which appeared only on the early termsof pregnancy (6–12 weeks inclusive) (2 groups) and 30 the prospectively inspected patientswith uncomplicated pregnancy, which do not have meaningful extragenital pathology andburdened factors obstetric-gynaecological to anamnesis (control group). On 2 stages from60 pregnant with a recurrent retrochorial haematoma by us two sub-groups were selected:1.1 and 1.2.To the sub-group 1.1 30 women were included in the terms of gestation 20-40 weeks,pregnancy in which was conducted on the basis of findings complex clinical-and-laboratoryand echographic inspection, with application of approach which personalize, depending onthat which prevails and etiopathogenetic co-factors with obligatory registration of the writteninformed consent of patients. Sub-group 1.2 were 30 pregnant with the relapse of retrochorialhaematoma, also inspected in obedience to a mine-out design with application of all methods, but pregnancy in which was conducted in obedience to recommendations of MOZ of Ukraine.\u0000\u0000Results. Frequency of placenta disfunction at a recurrent retrochorial haematoma is 70,0%; atprimary – 30,0%; a level of early forms (to 32 weeks) of delay of development of fetus is 65,0% with the considerable level of II-III of degree – 23,3%. The premature ripening of placenta ata recurrent retrochorial haematoma is 33,3%; at primary – 16,0%. Echo-sign of mesenchymaldysplasia and heterogeneity placenta according to 11,7% and 4,0%.At a recurrent retrochorial haematoma frequency of shortage of water is 16,7% against 4,0%– at primary; violations of blood stream are diagnosed in 23,3% and 6,0%; in the structureof all violations of blood stream in 63,3% – make in an uterine artery and in 30,0% – it isconnected with violation of umbilical cord blood stream. In 5,0% only at a recurrent retrochorialhaematoma there is decompensated placenta disfunction with development of acute fetaldistress.\u0000\u0000Conclusions. The perspective inspection of 94 patients which appealed for a help ina medical clinic «Alternative» c. Lviv. All girls were up-diffused on five groups – 1 - 15 girls by age14 years; 2 – 19 girls by age 15 years and 3 – 24 girls by age 16 years: 4 – 18 girls by age 17years; 5 – 18 girls by age 18 years.\u0000\u0000Results. From 94 inspected girls-teenagers 10 teenagers had a weak type of sexual constitution,that was 10,9%, a middle type is 64 teenagers, that was 68%, and 20 girls-teenagers hada strong type of sexual constitution, that was 21,2%. The results of this research lead tocircumstance that sexual activity of girls-teenagers depends not only on the rates of the sexualripening (hormonal background) but also from other, individual features of development of everyteenager, on forming of which such factors can influence as: congenital psychotype, sociumand family. Findings will help us in the subsequent study of questions which touch the sexualconduct of girls-teenagers and influence of sexual activity on development of such seriouspathology as violation of menstrual function.\u0000\u0000Conclusion. It is conducted clinical-and-laboratory and functional inspection 200 women with the underweight, from what 100 got the generally accepted diagnostic and treatmentand-prophylactic measures and 100 – an algorithm is improved by us. 30 expectant mothers madea control group. With the purpose of decline of frequency of unmaturing and other complications ofpregnancy it is necessary to use the followings moments: a hormonal and metabolic correction isindividually directed. Clinical, echographic, cariotocographic, doppler, biochemical, morphologicaland statistical.\u0000\u0000Results. The results of the conducted researches testify that in the structure of underweightfor women to pregnancy of decline of feed (IMT – 19,1±0,4) is 50%; oligotrophy I degree (IMT– 18,0±0,5) – 41% and oligotrophy of II-III of degree (IMT - 16,3±0,9) – 9% respectively. Totalfrequency of genital pathology to pregnancy for women with the underweight is 51,5%; violationsof menstrual function (16,5%) prevail among different nosology forms, in other cases of salpingooophoritis(9,5%); violation of genesial function (8,5%) and gynaecological operations are carried(9,5%). Set, that for pregnant with the underweight of growth of mass of body takes place, mainly(70–80%), due to thin mass. Fatty fabric makes small part (to 20%) of raise of weight, thatindemnification of underweight is not at pregnancy and the insufficient raise of weight takes placeat pregnancy for patients with low mass of body.\u0000\u0000Conclusion. The use of the improved algorithm of diagnostic and treatment-and-prophylacticmeasures allows to reduce frequency of threat of terminating pregnancy in І trimester (from 20% to10%; р<0,001), vomit of pregnant (from 14% to 6%; р<0,001), iron-deficiency anaemia in І trimester(from 36% to 16%; р<0,001), arterial hypertension (from 20% to 12%; р<0,001), bacterial vaginosis(from 10% to 4%; р<0,001). candidiasis (from 8% to 4%; р<0,001), threatened miscarriage in aІІ trimester (from 44% to 24%; р<0,05), iron-deficiency anaemia (from 74% to 50%; р<0,001),placenta disfunction (from 32% to 20%; р<0,05), intrauterine hypoxia (from 30% to 18%; р<0,001),delays of growth of fetus (from 12% to 6%; р<0,05), preeclampsia of middle and heavy degree (from22% to 6%; р<0,001). Henyk","doi":"10.52705/2788-6190-2022-01-3","DOIUrl":"https://doi.org/10.52705/2788-6190-2022-01-3","url":null,"abstract":"The connection between chronic pelvic pain and the presence in the body of a pathological source of structural and morphological changes in tissue, e.g. in connection with uterine fibroids, endometriosis, chronic inflammatory processes of the pelvic organs remains a traditional opinion. Endometriosis, which is characterized by a chronic pelvic pain, affects about 10% of women of\u0000reproductive age, and the problem of studying of endometrioid cysts demonstrates its relevance by dominating in the structure of genital endometriosis up to 59%. Chronic pelvic pain causes inappropriate courses of antibacterial therapy and, as a consequence,\u0000dysbiosis, the development of ovarian insufficiency syndrome, and the surgical approach does not always show the desired results, often worsening the clinical picture and psycho-emotional status of the patient.\u0000The objective: of this study was to assess the severity of the pain syndrome and psychological status in patients with pelvic pain associated with ovarian endometrioma and to seek effective therapeutic options.\u0000Materials and methods. The study included 40 patients with ovarian endometriosis combined with pelvic inflammatory disease (main group) and 40 patients with isolated endometrioma (comparison group). The control group consisted of 30 patients without manifestations of genital endometriosis. Analysis of somatic and gynecological anamnesis was used, as well as the objective and gynecological examination, data from adapted Beck’s Depression Inventory and State-Trait Anxiety Inventory, pain assessment according to the visual-analog scale (VAS), sonographic examination of the pelvic organs, evaluation of magnetic resonance imaging, endoscopic and histological methods of examination.\u0000Results. Dyspareunia was noted in one third of patients of the main group; dysuric manifestations, intestinal disorders in one in four women, a combination with manifestations of asthenic-neurotic syndrome in a third of women, the intensity of pain was presented in a third as severe and unbearable. It should be noted that the percentage of secondary infertility increased by 2.6 times,\u0000in two thirds of cases there were menstrual disorders, which were dominated by manifestations of algodysmenorrhea and abnormal uterine bleeding. The analysis of psychological questionnaires noted equal percentage of moderate and mild\u0000depressive symptoms in the main group, as well as, along with a decrease in psychological and emotional adaptation, high rates of situational and personal anxiety. Proposed therapeutic options included the use of dienogest primarily in women with mild to moderate pain syndrome, the possibility of long-term use at risk of recurrence and taking into account the smaller percentage\u0000of probable side effects.\u0000Conclusions. We analyzed 10684 histories of pregnancy and childbirth for 3 years (2019–2021). The main group of pregnant women included 363 cases with fibroid (3.39% of the total). The comparison group consisted of pregnant women without fibroid (10,321 births (96.61% of total)).\u0000Results. We found that the following factors as the age of the pregnant woman, high BMI, smoking and alcohol consumption were of significant risk (p<0.05) regarding the fibroid progression and its effect on the course of pregnancy, which affected the development of gestational diabetes and hypertensive disorders in pregnant women with fibroid. The rate of delivery by caesarean section in women with fibroid reached 59.0% compared with 23.4% in the group without fibroid (p<0.001). An association between fibroid and an increased risk of caesarean section was confirmed (OR 4.7; 95% CI: 3.8–5.8). The size of the fibroid node significantly increased the likelihood of placentation pathology as placenta previa in the subgroup with the size of the node >5 cm, the likelihood of preterm birth, as well as the volume of postpartum hemorrhage (p<0.05). The fibroid node type proved a statistically significant effect on the incidence of placental abruption, especially in the case of placental attachment close to the submucosal node compared with another type of nodes (intramural or subserous) – 6.9% vs. 0.8% respectively in the case of intramural fibroid and 0% for subserous fibroid p<0.01.\u0000Conclusions. According to the results of a retrospective study, we proved that pregnant women with uterine fibroid are at greater risk of complications of pregnancy, childbirth and the postpartum period. There is a direct relationship between pregnancy in the background of fibroid and the likelihood of placenta previa, increased frequency of caesarean section and the development of\u0000postpartum hemorrhage.The relationship between different characteristics of the uterine fibroid (its type, size, localization,\u0000number of nodes) and the likelihood of massive obstetric bleeding has been proven.
