The classification of hypertriglyceridemia according to the Endocrinology Society include mild and moderate hypertriglyceridemia, severe hypertriglyceridemia and very severe hypertriglyceridemia. Mild and moderate hypertriglyceridemia increase the risk for cardiovascular events while severe and very severe hypertriglyceridemia is a risk factor for acute pancreatitis. Conventional pharmacological therapy of hypertriglyceridemia includes: fibrates, niacin, statins, ezetimibe, omega-3-fatty acid. Other triglyceridelowering therapies are represented by plasmapheresis and lipoprotein lipase gene therapy. The present work refers to a 59-year old man without history of family diabetes, dyslipidemia, premature coronary artery disease, diagnosed with T2DM in 2012, from 2014 on insulin treatment; he was hospitalized for endocrine evaluation. History: high blood pressure for approximately 25 years, chronic kidney disease, very severe hypertriglyceridemia, thyroid papillary carcinoma. The patient followed treatment with hypoglycemic, hypolipemic, low salt content diet, fibrates, statins, omega-3-fatty acid. Due to the fact that the patient did not respond to conventional therapy plasmapheresis was considered.","PeriodicalId":38700,"journal":{"name":"Romanian Journal of Diabetes, Nutrition and Metabolic Diseases","volume":"25 1","pages":"425 - 430"},"PeriodicalIF":0.0,"publicationDate":"2018-12-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46616570","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

We assess if type 1 diabetes mellitus promote modification in the autonomic nervous system using the main two types of algorithms to estimate a Fractal Dimension: Higuchi and Katz. Material and methods: 46 adults were divided into two equal groups. The autonomic evaluation consisted of recording heart rate variability (HRV) for 30 minutes in supine position in absence of any other stimuli. Fractal dimensions ought then able to determine which series of interbeat intervals are derived from diabetics’ or not. We then equated results to observe which assessment gave the greatest significance by One-way analysis of variance (ANOVA1), Kruskal-Wallis technique and Cohen’s d effect sizes. Results: Katz’s fractal dimension is the most robust algorithm when assisted by a cubic spline interpolation (6 Hz) to increase the number of samples in the dataset. This was categorical after two tests for normality; then, ANOVA1, Kruskal-Wallis and Cohen’s d effect sizes (p≈0.01 and Cohen’s d=0.814143 –medium effect size). Conclusion: Diabetes significantly reduced the chaotic response as measured by Katz’s fractal dimension. Katz’s fractal dimension is a viable statistical marker for subjects with type 1 diabetes mellitus.","PeriodicalId":38700,"journal":{"name":"Romanian Journal of Diabetes, Nutrition and Metabolic Diseases","volume":"25 1","pages":"289 - 295"},"PeriodicalIF":0.0,"publicationDate":"2018-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44724499","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Chronic hyperglycemia is accompanied by significant physiological, biochemical, and histological changes, e.g. development of oxidative stress that affects the motor activity of the intestine. This study aimed to evaluate the indices of nitric oxide (NO) system in blood serum and colon tissue supernatant of rats with carrageenan-induced enterocolitis combined with streptozotocin-induced diabetes. Material and methods: The total NOS activity was determined by monitoring the rate of conversion of L-arginine into citrulline. The total quantity of NO metabolites was assessed by evaluating their amount, which included nitrite ions that were initially present in the sample (NO2−) and nitrate ions reducted to nitrites (NO3−). Results: We found a significant increase in total NOS activity in colon tissue of all experimental groups vs. control animals (54.8, 30.6 and 79.2 % respectively). The total content of NO metabolites in colon tissue of all experimental groups also significantly increased (2.8, 1.9 and 3.4 times respectively) compared to the control animals. Conclusions: We observed activation of nitroxydergic process in blood serum and colon tissue of rats with carrageenan-induced enterocolitis. Nitroxydergic processes markedly intensified in rats with carrageenan-induced enterocolitis combined with diabetes mellitus.","PeriodicalId":38700,"journal":{"name":"Romanian Journal of Diabetes, Nutrition and Metabolic Diseases","volume":"25 1","pages":"283 - 288"},"PeriodicalIF":0.0,"publicationDate":"2018-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"44466825","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Chronic periodontal diseases and dental caries are the primary reasons for tooth loss in adults, which is further increased in people with diabetes. In most cases of bone loss, the treatment of partial edentulous patients with implant supported restorations impose additional surgical procedures, like sinus lift elevation and bone augmentation, which can complicate the healing process. Case report. This case report presents a type 2 diabetes female patient with several oral health problems, like periodontal disease, poor decay control, bad oral hygiene, a severe maxillary atrophy and the presence of a large maxillary periapical cyst. After a careful examination, based on clinical and radiographic findings, a comprehensive treatment plan was established. The sequential treatment plan consists in extraction, surgical removal of periapical cyst, bilateral external sinus lift procedures and bone augmentation. The surgical protocol was adapted to the particular health conditions of this type 2 diabetes patient. Conclusion. Sinus elevation and bone augmentation are predictable procedures often required when restoring the posterior maxilla with dental implants. In case of diabetes patients with bone resorption and defects due to periapical cyst, if the correct protocol is followed, no post-surgical complications and good result in bone augmentation can be attaint.","PeriodicalId":38700,"journal":{"name":"Romanian Journal of Diabetes, Nutrition and Metabolic Diseases","volume":"25 1","pages":"313 - 319"},"PeriodicalIF":0.0,"publicationDate":"2018-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"43424165","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The principal aim of the present work was to review the basic factors related to the possible advantages of providing early psychological attention to T2DM patients as a coadjuvant for achieving adequate metabolic control. Material and methods: A literature review was conducted to explore the interaction between stress and depression and the relation of both to the ability of T2DM patients to effectively manage their disease. Results: It was found that stress is one of the factors linked to the etiology of depression, which is a disorder with high prevalence in diabetic patients. Consequently, an inter- and multidisciplinary approach to treating diabetic patients was developed. One of the main focuses of this approach is early psychological attention, starting shortly after the initial diagnosis. Conclusions: The ability to create consciousness among health care professionals about the importance of early psychological attention for T2DM patients under an inter- and multidisciplinary strategy could possibly improve pharmacological adherence, metabolic control, the quality of life and the life expectancy of patients, as well as save economic resources for patient families and health institutions.","PeriodicalId":38700,"journal":{"name":"Romanian Journal of Diabetes, Nutrition and Metabolic Diseases","volume":"25 1","pages":"329 - 334"},"PeriodicalIF":0.0,"publicationDate":"2018-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"49436199","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Low Carb Diet – To Love or to Hate? 低碳水化合物饮食——爱还是恨?

{"title":"Low Carb Diet – To Love or to Hate?","authors":"A. Sima","doi":"10.2478/rjdnmd-2018-0026","DOIUrl":"https://doi.org/10.2478/rjdnmd-2018-0026","url":null,"abstract":"Nutrition is a field of major interest in the last decades, not only for the medical community, but also for the general population. We know today that what one eats can influence one’s health and well-being. This simple truth is unfortunately not so simple at all, because there is no consensus about what exactly one should eat in order to stay healthy and there is a lot of information in the medical (and not only) literature about nutrition and a debates are going on about the best diet composition. More than that, things are changing, information that was true and proven yesterday is not valid anymore today. This is why we need to be informed, to think and to choose in order to be able to pick the best data. A trend that is, on one hand, gaining more and more credit and, on the other hand, having a lot of opponents, is the low carbohydrate, known as low carb, diet. There are quite impressive theories stating the beneficial effects of a low carb diet, but also, in a traditional way, it is (or was?) a habit to take a high percentage of total energy intake from carbs. Where is this tradition coming from? First of all, from the landmark studies published in the ‘60s and ‘70s that showed the negative effects of fats on weight, cardiovascular disease, etc. One of these publications is The Seven Countries Study that showed an association between saturated fat intake and cardiovascular mortality [1]. Since then, the guidelines recommended a low fat diet that provided subsequently a higher percentage of carbohydrates (around 50-60% of total energy intake). Newer study showed that a diet high in carbohydrates elevates postprandial glycaemia and serum insulin level, leading to an accelerating in the progression of type 2 diabetes and atherosclerotic cardiovascular disease [2]. So maybe it is time for a paradigm change. Before debating about the effects of a low carb diet, we need to answer the question: what is a low carb diet? There are several definitions in the medical literature; the easiest way is to consider that restricted carbohydrate intake means an intake below the currently recommended amount of 45-65% of total daily energy intake [3]. But for a more detailed definition the classification in to three different types of low carb diet is useful [4]: moderate carbohydrate diet (26-45% of total daily energy intake), low carbohydrate diet (<26% of total energy intake or <130 g carbohydrates/day) and very low carbohydrate (ketogenic) diet (20-50 g carbohydrates/day or <10% of total daily energy intake in a 2000 kilocalories/day diet). There are many studies published in the last decades that show evidence for the beneficial effect of a low carb diet on various parameters of the cardio-metabolic syndrome. © 2018 ILEX PUBLISHING HOUSE, Bucharest, Romania http://rjdnmd.org Rom J Diabetes Nutr Metab Dis. 25(3):233-236 doi: 10.2478/rjdnmd-2018-0026","PeriodicalId":38700,"journal":{"name":"Romanian Journal of Diabetes, Nutrition and Metabolic Diseases","volume":"25 1","pages":"233 - 236"},"PeriodicalIF":0.0,"publicationDate":"2018-09-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"46148182","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

They can be involved in the causality patterns or can act as useful markers of the cerebrovascular lesions (or both) and for which there are strong proofs, including the poor glycemic control, hypoglycemia, microvascular diseases, inflammation or depression. For the macrovascular affections, the association with the cognitive disorders seems to devolve on the examined vascular system.
