He current healthcare strategy of Russia, among other significant priorities, highlights the achievement of high indicators of accessibility, quality and effectiveness of medical care. One of the tools for assessing the level of achievement of these indicators, including in dynamics, is the study of patients' opinions. Many authors note the importance of taking into account the opinion of patients about the care provided for the development and adoption of management decisions. However, there is a lack of scientific data on patients' opinions on the availability, quality and effectiveness of medical care, taking into account the different profile of services and the type of health care facilities.\u0000Objective. To study the opinion of patients about the accessibility, quality and effectiveness of medical care. Materials and methods. A multicenter sociological study was carried out in medical organizations of the Ivanovo, Yaroslavl and Moscow regions. The research method is a sociological survey. Respondents were offered the questionnaire \"Patients' Opinion on the Availability, Quality and Effectiveness of Medical Care\", which included 15 closed-ended questions. A sample population formed by random selection. Statistical processing of the material provided for the formation of a database in spreadsheets, the calculation of relative values and the reliability of their difference according to the student’s criterion. An analytical method was used to compare the data and formulate conclusions.\u0000Results. The sociological survey showed a \"depression\" of patients' opinions about health care, which is manifested in the high frequency of negative assessments of accessibility (23, 5 per 100 respondents), quality (19,5 per 100 respondents) and effectiveness of medical care (14,5 per 100 respondents) in general. There are differences in the assessment of patients by the profile of services and types of institutions, which makes it necessary to form a set of measures to correct deviations and conduct regular patient surveys on the principle of monitoring in order to assess their effectiveness. The frequency of patients' positive opinions on the availability of medical care was higher in the pediatric profile of services (60,2 per 100 respondents), preventive services (63,8 per 100 respondents), inpatient services (67,0 per 100 respondents), in the Moscow region (49,7 per 100 respondents).\u0000Conclusion. The theoretical significance of the data for assessing patients' opinions about the accessibility, quality and effectiveness of medical care is associated with the possibility of identifying and structuring existing problems in health care. Laser interferometers are an important tool in prosthetic dentistry, used to measure microscopic structures and surfaces of teeth and oral tissues. These devices allow dentists to obtain accurate data on the shape and condition of teeth, which significantly improves the quality of treatment for patients in need of dental prosthetics. The relevance of the use of laser interferometers in orthopedic dentistry at present is due to several important factors, such as high measurement accuracy. Laser interferometers allow measurements to be taken with incredible precision, avoiding even the small errors that can occur with traditional dental measurement methods. Non-contact - one of the main advantages of laser interferometers is their ability to carry out measurements without contact with patient tissue. This minimizes discomfort for the patient and avoids the risk of infection or tissue damage. The speed and efficiency of laser interferometers can significantly speed up the diagnostic and prosthetic fabrication processes in prosthetic dentistry, making procedures more efficient and cost-effective. These devices allow for detailed analysis of the micro- and macrostructure of teeth, which is important for the precise manufacture of dentures and bite correction. To evaluate the quantitative composition of the microflora of the periodontal pocket in patients with chronic generalized periodontitis of moderate severity. Materials and methods. The study group included 39 patients, of which: 41% (16/39) were men, 59% (23/39) were women, with chronic generalized periodontitis of moderate severity at the age of 48.9±2.3 years. To determine the dental status, clinical research methods were used (questioning, examination), determination of the Green-Vermillion Oral Hygiene Index (OHI-S), (PMA) modified by Parma, the Mulleman-Cowell bleeding index, the degree of pathological tooth mobility was determined using the Miller scale (Miller) modified by Flesar, the complex periodontal index (CPI), identified furcation defects according to the classification of Lindhe J. (1983).Quantitative assessment of the periodontal pockets microflora was performed. Reagent kits “RIBO-prep” (Next-Bio LLC, Russia), DENTOSKRIN® (NPF Litech LLC, Russia) were used to isolate RNA/DNA from clinical material and subsequent analysis by polymerase chain reaction. Spectrum of identifiable bacteria: Porphyromonas endodontalis, Porphyromonas gingivalis, Aggregatibacter actinomycetemcomitans, Treponema denticola, Fusobacterium nucleatum, Prevotella intermedia, Tannerella forsythia.The results of the study and their discussion. Fusobacterium nucleatum was found in 84.6% (33) of patients in periodontal pockets, 30.8% (12) – Treponema denticola, 28.2% (11) – Prevotella intermedia, 17.9% (7) – Porphyromonas gingivalis and Porhyromonas endodontalis. Aggregatibacter actinomycetemcomitans was isolated only in 5.1% (2) of individuals, Tannerella forsythia – in 2.6% (1). Combinations of 4 microorganisms were detected in 20.5% (8), from 3 - in 12.8% (5), from 2 - 15.4% (6). In 12.8% (5) of the examined patients were not isolated from any of the studied periodontal pathogens. When checking correlations between the level of hygiene and the number of pathogenic microorganisms detected, a moderate direct relationship was established between the value of the OHI-S index and the amount of Fusobacterium nucleatum r=0.36 (p<0.05) and Prevotella intermedia r=0.30 (p<0.05), as well as a moderate direct relationship between the value of the PMA index and the amount of Porphyromonas gingivalis r=0.33 (p<0.05), Porphyromonas endodontalis r=0.31 (p<0.05), Prevotella intermedia r=0.30.Conclusion. In patients with chronic periodontitis of moderate severity, a microbial biofilm is formed on the surface of the periodontium, represented by anaerobic bacteria. In periodontal pockets, the amount of Fusobacterium nucleatum and Prevotella intermedia increases with the deterioration of the hygienic condition of the oral cavty. The increasing number of patients with diseases associated with neuropsychiatric disorders makes the search and introduction of highly effective anxiolytics and antidepressants into medical practice increasingly important. In parallel with the search for medicinal substances with such pharmacological properties, it is necessary to resolve issues of standardization and quality control. One of these drugs may be a new synthetic derivative of ortho-benzoylaminobenzoic acid, which has antidepressant and anxiolytic effects. This compound was synthesized at the department of organic chemistry of the Pyatigorsk Medical and Pharmaceutical Institute under the leadership of professor Kodonidi I.P. Several biologically active compounds were synthesized - derivatives of o-benzoylaminobenzoic acid, one of which is 2-benzoylamino-N-[4-(4,6-dimethylpyrimidin-2-ylsulfamoyl)-phenyl]-benzamide (laboratory code - PMFI-195) . Preliminary pharmacological studies of PMFI-195, carried out on the basis of the Federal State Budgetary Educational Institution of Higher Education of the Stavropol State Medical University of the Ministry of Health of Russia and the Federal State Budgetary Educational Institution of Higher Education of the North Caucasus Federal University, under the leadership of professor Manvelyan E.A., showed the promise of developing a drug based on the substance PMFI-195 for the treatment of such diseases such as depression, neuroses, psychopathy, neurosis-like and psycho-like states. To compare the results of hernia repair in reproductive men using transabdominal pre-peritoneal repair (TAPP) and the Lichtenstein method.\u0000Material and methods. The study involved 56 male patients aged 18 to 45 years who were operated on for a primary inguinal hernia. All patients were divided into two groups. The first group included 29 patients operated on using the Lichtenstein method. The second group included 27 patients operated on using the TAPP technique. Depending on the method of hernia repair, the following were studied: the dynamics of Doppler studies of testicular blood flow, the level of sex hormones in the blood plasma and spermogram before and after surgery.\u0000Results. The average percentage of sperm motility in the first group was 41.51±6.62 (median = 40%), and in the second group – 41.76±1.16 (median = 41%). The mean sperm count and motility were significantly higher in the second group (P < 0.001). The duration of the operation was significantly shorter in the first group, operated on using the Lichtenstein method, than in the second group, operated on using the TAPP technique (45.8 ± 6.28 vs. 68.5 ± 16.31 min; p < 0.001). According to the results of testicular ultrasound, the average volume of the right testicle in the first group was 14.19±3.16 (median = 14 ml), and in the second group – 14.68±3.07 ml (median = 14.8 ml). =12.15 ml), and in the second group – 14.25±3.19 (median = 14.40 ml), which turned out to be significantly lower in the observation group (p = 0.029). There was a significant difference between the two groups for postoperative dysesthesia and chronic pain: 5% in the TAPP group versus 14% in the Lichtenstein group, and 1.5% in the TAPP group versus 6% in the open group for chronic pain.\u0000 Conclusion. There was no significant difference between the two groups in terms of intraoperative complications (p=0.56), short-term postoperative complications (p=0.75), wound infection (p=1.0), and urinary retention (p=0.68). Premature loss of primary teeth in a patient aged 6 years led to underdevelopment and asymmetry of the dentoalveolar arch and disruption of occlusal contacts, which leads to the inclusion of additional elements in removable orthodontic appliances, an increase in the area of orthodontic equipment and the number of additional elements in them contributes to an increase in the contamination of the oral cavity with opportunistic pathogens and fungal microorganisms. Considering that the Republic of Tajikistan is a member of the World Health Organization and an eventual participant of the Eurasian Economic Union, it is advisable to consider international approaches to the organization of storage of medicines recommended by WHO and the EAEU in order to develop national storage rules in the Republic of Tajikistan.\u0000Objective: To study international requirements for the organization of storage of medicines and to form a theoretical basis for further development of Rules for the storage of medicines in the Republic of Tajikistan.\u0000Materials and methods: Through content analysis, WHO recommendations, regulatory legal documents of the EAEU and national regulations regarding the organization of drug storage were studied. The concept of forming a theoretical basis for the development of rules for the storage of medicines in the Republic of Tajikistan is illustrated by the method of grouping and graphical analysis. With the use of comparative, structural and logical analyses, the key principles of the formation of existing international and national requirements for the organization of drug storage are summarized and theoretical provisions for the development of Rules for the storage of drugs in the Republic of Tajikistan are substantiated.\u0000Results: The WHO Guidelines on Good Storage Practices for Pharmaceutical Products (GSP) were studied. The analysis showed that the GSP contains a description of the special measures necessary for the proper storage and transportation of pharmaceutical products. It is established that the basic principle of GSP formation can be defined as \"act by the rules\", i.e. the semantic emphasis of the document is shifted towards pedantic adherence to the requirements established in it, excludes the initiative of the participants in the process and indicates the need to comply with national and regional requirements regarding the storage of medicines.\u0000The requirements for the organization of storage of medicines in the EAEU countries have been studied. It is noted that the main principle of ensuring the quality of medicines in the EAEU countries is a risk-based approach, which can be defined as \"acting ahead of the curve\". To date, a large number of implantation systems are known, which are quite well studied, but so far, a number of issues remain unresolved, and the most important of them is the choice of the method of fixation of implantation prostheses. The main task in orthopedic treatment based on dental implants is to maintain a stable level of bone tissue, and, as a result, the long-term prospects for the functioning of the implant and the entire structure. There are two main types of fixations of such prostheses: screw and cement, but there is still no specificity in the choice of indications for them. In this study, based on mathematical modeling data, an analysis of the stress-strain state of bone tissue in the area of implants installed at different angles to each other was carried out. Mathematical models of screw-fixed bridges supported by two implants located strictly parallel to each other and at angles from 2о to 10о have been developed (conical and planar types of connection of implant platforms with abutments have been studied). The analysis of the maximum equivalent Mises stresses occurring inside the structure itself is also carried out. Stress localization zones under vertical load have been identified. The purpose of the study is to identify morphofunctional changes in the organs of the immune system under experimental conditions when using various methods of treating purulent wounds when modeling alloxan diabetes. Experimental studies were carried out on 150 animals (white outbred rats), in which purulent wounds were simulated against the background of alloxan diabetes. In control I group, the nature of the course of the purulent-inflammatory process was studied without correction of treatment, in control II group, purulent wounds were treated with miliacil, in control III group, EHF therapy was used. For local treatment of wounds of the main group of experimental animals, EHF therapy and miliacyl were used. In addition to taking into account clinical data, mesenteric lymph nodes, thymus, and pancreas were examined in animals of all groups after euthanasia. The most favorable treatment results were observed in experimental animals of the main group. As a result of a histological study of the thymus, atrophy of the thymic parenchyma (cortex and medulla) was discovered, combined with hyperplasia of the interlobar perivascular spaces with a simultaneous increase in the proportion of adipose tissue. The phenomenon of accidental involution of the thymus was less pronounced in animals of the main group than in rats of control groups. Morphofunctional changes in the organs of the immune system were established with various methods of treating purulent wounds against the background of alloxan diabetes. Poly-D,L-lactide-co-glycolide is one of the most frequently studied biodegradable polymeric drug carriers for drug delivery to target organs. Biodegradable polymers must have good biocompatibility. Targeted delivery of drugs obtained using modern technologies can provide a new therapeutic strategy for more localized treatment of diseases, potentially provide systemic delivery of drugs, reduce the dose of the administered drug and its possible side effects on the patient. In the experiment, we obtained microparticles based on cinnarizine with poly-D,L-lactide-co-glycolide using the co-precipitation method. The physicochemical characteristics of microparticles are of key importance for understanding their properties for further use in pharmaceuticals, since they can have different effects on the target organ due to their unique properties. The study of the dispersed composition of the obtained samples of cinnarizine microparticles based on poly-D,L-lactide-co-glycolide was carried out using photon correlation spectroscopy on a Photocor-Complex installation (Antek-97, Russia). The size and shape of microparticles explain their ability to penetrate blood vessels, tissues and target cells of target organs. The purpose of this work was to determine the effect of various cryoprotectants on the size and shape of cinnarizine microparticles at different ratios to poly-D,L-lactide-co-glycolide. Under experimental conditions, a cryoprotector was selected.
