The surgeon’s experience and modern technology, which makes it possible to identify hidden symptoms of certain pathologies at the diagnostic stage, allows for a favorable completion of surgery with minimal intraoperative complications. This article shows the approach and observations of patients at risk of increasing eye pressure in the postoperative period due to the anatomical structures of the eye (in these cases, patients with open-angle glaucoma) to antihypertensive drugs. 131 patients (134 eyes) with various signs of pseudoexfoliation syndrome participated in the observation. Before planned surgery for complicated cataracts, all patients underwent ophthalmological diagnostics, medical history, and laboratory tests according to generally accepted methods. All operations were performed by the same surgeon. Cataract surgery was performed according to the standard technique (Phaco + intraocular lens) through a 2.2 mm incision, two 1.2 mm paracentesis, but for some patients other components of surgical interventions were added. In the observation groups, for better fixation and a lower percentage of complications in long-term follow-up periods, RPR, T-19, Myol were implanted in 27.7%. A large percentage of 34% were implanted with the HO Acrylic IOL model lens, which requires a 2.2 mm corneal incision. In observation groups 2 and 3, 32.5 and 31.8% preferred lenses to the Acrysoft IQ model. And in 32.5% and 13.6%, respectively, the AFY model. The choice of the method of surgical intervention, as well as the model of intraocular lenses, made it possible to predict the long-term postoperative period with less toxicity for the patient’s condition. According to all available scientific data, umbilical cord abnormalities play an important and often decisive role in the development of complications during pregnancy and childbirth, including perinatal mortality. Thus, umbilical cord abnormalities include skinny umbilical cord, umbilical cord presentation, absolutely short umbilical cord, sheath attachment of the umbilical cord, presentation of umbilical cord vessels, hypoisvite and hyperisvite umbilical cord, single umbilical artery, persistence of the right umbilical vein, vein aneurysm and umbilical cord hematoma. There are several variants of localization of this vascular anomaly: in the area of attachment to the placenta, in the free part, in the area of the umbilical ring and in the intraabdominal (intrahepatic) part. There are no clinical recommendations for the management of pregnancy with an umbilical vein aneurysm. When detecting umbilical cord abnormalities, it is necessary to monitor the dynamics of changes in the size of the aneurysm with an assessment of the fetal condition. The article describes a clinical case of intraabdominal fetal umbilical vein aneurysm, which is extremely rare and accounts for less than 1% of all umbilical cord abnormalities. The patient's reproductive and genetic history is not burdened. The somatic history was aggravated by neurodegenerative diseases of the spine. One of the directions of the individual educational trajectory is the creation of additional educational programs that expand the opportunities for students to obtain additional competencies. Purpose. On the basis of a sociological survey, to study students' satisfaction with the additional educational program aimed at the formation of professional competencies for training doctors to perform new labor functions in the formation of preventive programs, maintaining a healthy lifestyle and giving up bad habits. Materials and methods. The study includes the development of an additional professional program \"Strengthening Public Health\" using digital technologies in the field of preserving and strengthening public health, for the training of health professionals. Based on the results of the course of the additional professional program, a sociological survey was conducted in an online format through the Google Forms resource. A link to the survey was sent via email and WhatsApp messenger. Results. The data obtained in the course of the study allow us to see the problem of motivation of students who adhere or do not adhere to a healthy lifestyle and to develop measures aimed at attracting medical university students to physical education, sports and the formation of a healthy lifestyle. Conclusion. The need for validation at the stage of drug development is due to the fact that it is possible to promptly identify the shortcomings of the method and significantly improve it. The purpose of this study is to validate methods for testing «The authenticity» and «Quantitative determination» of acyzole in dental gel. Materials and methods. To achieve this goal, an active pharmaceutical substance of pharmacopoeial quality was used. When developing methods for testing for “Authenticity,” a reaction with oxalic acid to the zinc cation determined in the composition of acyzole was used. For the quantitative determination of acyzole, the complexometric method was used. The objects of the study were three series of gel samples. Results and discussion. As a result of the research, a method for testing «Authenticity» was developed; it was found that the proposed method is characterized by a negative analytical signal on model mixtures free of the analyte component and placebo, and a positive analytical signal on model mixtures of various compositions containing the analyte component. The validation characteristics of methods for the quantitative determination of acyzole in the gel were studied. positive results were obtained. The proposed methods can be used for inclusion in the regulatory documentation for the developed gel. Conclusion. The article is based on the biographical data carefully collected in the Archive Service of the Kabardino-Balkar Republic and other sources of the first well - known regional doctors from the indigenous inhabitants of Kabardino-Balkaria, as well as their contribution to the development of medical services in the region, which makes the work unique. The selfless work of the first doctors shows the history of the formation and development of traditional medicine and all health care in the Nalchik district of the Terek region in the pre-revolutionary period. At the same time there are cases of non-estimation or extra-estimation of perinatal risk in obstetrical practice. The usage of famous prognostic systems gives the opportunity to select pregnant women into groups of high risk and provide the necessary organizational and tactic measures. But to define the character of possible complications, to establish the degree of their severity and time of appearance is really impossible. \u0000Objective of the study: to develop a new approach to estimation of perinatal risk based on the determination of prognostic criteria for antepartum and intrapartum fetal death, early neonatal death of a newborn in order to increase the efficacy of predicting of unfavourable perinatal outcomes.\u0000Materials and research methods. The retrospective case-control study in patients with antepartum (n=134) and intrapartum (n=41) fetal death, early neonatal death of the newborn (n=61) and favourable perinatal outcome (n=50) was conducted. There was used the Bayes algorhythm with Wald's consecutive analysis in the modification of Genkin-Gubler to determine prognostic criteria. The comparative estimation of the efficacy of the proposed prognostic method and traditional method of determining perinatal risk was performed retrospectively in patients with perinatal losses (n=102) and favourable perinatal outcome (n=100). ROC analysis was performed.\u0000Results. There were identified 42 risk factors divided into three groups: universal for all types of perinatal losses, common for two of them and specific for each of them (antepartum and intrapartum fetal death, early neonatal death of a newborn). There was determined the prognostic influence of factors in their presence and absence. It was found that universal and common risk factors had different prognostic influence for each type of perinatal losses. There was presented the method for differential predicting of perinatal losses. The sensitivity of the new and traditional prognostic methods was 95.1% and 69.6%, specificity was 80% and 53%, and the accuracy of predicting of unfavourable outcome was 87.6% and 61.4% respectively. \u0000Conclusion. Risk-reducing salpingo-oophrectomy is usually offered to women with mutations in BRCA1, BRCA2, or both of them, because, according to research, it improves overall survival and reduces the number of deaths from serous cancer and breast cancer. In this case, a comprehensive examination was carried out in a timely manner in a patient with breast cancer, including a medical and genetic one, which made it possible to identify the presence of a highly oncogenic type of mutation in the BRCA 2 gene. According to large–scale studies, women carrying this type of mutation represent a risk group for developing bilateral breast cancer, as well as ovarian cancer. In addition, carriers of a mutation in the BRCA2 gene are threatened by the occurrence of pancreatic and biliary tract cancers, stomach, fallopian tubes and skin melanoma. Carrying out preventive mastectomy and bilateral ovariectomy in such patients reduces their risk of malignant neoplasms. Therefore, the patient's complex therapy included these stages of surgical treatment. In addition, the detection of such a form of breast cancer associated with mutations in the BRCA genes makes it possible to expand the range of diagnostic search for malignant neoplasms, including in-depth examination of both the patient herself and her closest relatives. Of particular importance is the consumer behavior of modern pharmacy customers, which is changing due to an imbalance between the state of the pharmacy system's customer base and existing methods of customer counseling. There is a need to focus on different segments of consumers, each of which requires specific client technologies that contribute to the transformation of consumer behavior from \"standard\" to individual, corresponding to the status, mentality, financial capabilities, level of information literacy, typology of the buyer. Studying the assessments and opinions of respondents helps to identify patterns in consumer behavior and makes it possible to develop a more thoughtful strategy for information and advisory work with employees of the coal industry, paying special attention to issues of pharmaceutical consulting, information, attentive service, the use of new forms and methods of work that would increase consumer interest in purchasing effective and safe medicines. The consumer's decision-making process is considered, as a rule, as part of the following stages: consumer awareness of the problem, information search, evaluation and selection of purchase alternatives, purchase, use of purchase and evaluation of the solution. The factors of consumer behavior or purchasing decisions in relation to the buyer are conditionally divided into internal and external. Pharmaceutical organizations that sell medicines seek to influence consumer behavior by building their activities based on the concept of marketing. In these conditions, an important problem that the management has to solve in the process of circulation of medicines is the problem of quality of source information accompanying the business processes of pharmaceutical activities. The source information is not intended for the purposes of analysis, is presented, as a rule, rather raw material that requires preliminary processing by special methods, including their sampling, purification and transformation. The article discusses the advantages of practical application of the capabilities of Loginom analytical platform, which implements the above mentioned methods of pre-processing of raw material and allows solving the tasks of modeling the characteristics of drug circulation.\u0000Based on the concept of low-code architecture that provides visual design of complex data analysis scenarios, without involving IT-specialists and realize programming tasks by the business user.\u0000The object of the research was the current data obtained from the accounting system of material assets of a wholesale pharmaceutical organization. When organizing a request to the accounting system, based on the considerations of commercial confidentiality, it was decided to limit the 12-month time period and the assortment group, including only antibiotics.\u0000During the research, a data processing scenario was developed in the Loginom environment, representing the sequence of actions that need to be carried out to analyze the data. A processing script is a combination of data processing nodes configured by the developer to solve a specific problem. An objective criterion for evaluating the periodontal reaction is the hydrogen index of the secreted gingival fluid as the physiological environment of the body, the transudate of blood serum filling the gingival sulcus. The aim of the study was to determine the effect of non–removable prostheses made by traditional and digital methods on the condition of periodontal tissues by measuring the pH of gingival fluid. Materials and methods. To determine the pH of the gingival fluid, special strips of universal indicator paper were used, which allow measuring the pH of the biological fluid in the range from 3.0 to 7.5 units with a minimum step of 0,3. Results. Digital and traditional methods of manufacturing non-removable prostheses are statistically significantly distinguishable with a significance level of p < 0,025 by the pH value of the secreted gingival fluid. Thus, the hydrogen index of gingival fluid in patients with prosthetics using traditional technologies was shifted to a more acidic environment (the median samples differ by 15,4%). Our study made it possible to establish that the hydrogen index of gingival fluid in supporting teeth covered with non-removable dentures made by the digital method is closer to the boundary values of the norm, which indicates their weakly pronounced effect on the condition of the marginal periodontium, due primarily to the smaller thickness of the edge of the artificial crown, which does not have a repelling effect on the gingival margin, unlike from traditional designs with a thicker edge, secondly, the anatomical shape of the prosthesis, which differs little from a natural tooth, and thirdly, significantly less influence of the microbial landscape with a more precise fit of the edge of the artificial crowns to the posterior ledge.","PeriodicalId":263577,"journal":{"name":"\"Medical & pharmaceutical journal \"Pulse\"","volume":" 19","pages":""},"PeriodicalIF":0.0,"publicationDate":"2024-05-10","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"140991955","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
