Since at present there are no generally recognized algorithms for the management of such patients, there is a need to search for approaches to determine the risk of adverse outcomes of this disease phenotype.\u0000Aim of the study. To study the peculiarities of clinical course and to build a prognostic model of lethal outcomes risk in patients with ACS combined with SARS-COV 2 infection at the hospital stage\u0000Characteristics of patients and methods of research. The study included 225 patients admitted to a regional vascular center with a diagnosis of ACS who had a verified diagnosis of SARS-Cov 2 infection. Initially, 120 general clinical, biochemical and instrumental parameters were evaluated.\u0000Results and Conclusion. A predictive model was constructed with the identification of eight variables most influencing outcomes: age, the presence of atrial fibrillation (except for the first time detected in ACS), severe and extremely severe course of SARS-COV 2 infection, AKI, CKD, ferritin, albumin and glycemia in blood serum on admission.\u0000Conclusion. In the studied clinical case, the joint drawing up of a management plan for the patient by a cardiologist, pulmonologist, obstetrician-gynecologist, holding medical consultations, including specialists from leading medical institutions, in accordance with the nosology of the patient’s diseases, carrying out optimal surgical intervention using laparoscopic access, allowed for removal of the formation ovary, its histological examination and to prevent in a patient at risk the progression of the process and the possibility of malignancy of a benign ovarian tumor. This possibility is quite probable with the long-term existence of ovarian tumors. Verification of the histotype of an ovarian tumor is carried out first by morphological examination of the intraoperatively removed specimen, followed by final confirmation of the conclusion. In addition, even in the presence of benign ovarian tumors, complications such as possible infection, torsion or rupture of the capsule of the formation may develop. These complications can arise suddenly and require emergency surgical intervention, which is extremely difficult in a patient with severe combined extragenital pathology. In this case, there may also be no possibility of conducting an intraoperative histological examination of the removed specimen, which will lead to an inadequate amount of surgical intervention. After studying this clinical case, the following conclusions can be drawn. In quality control of pharmaceutical formulations, there are methodological approaches based on the preliminary separation of sample components and approaches without preliminary separation. The use of solid-phase extraction (SPE) allows to separate analytes during sample preparation, with the separation selectivity determining the analysis selectivity as a whole. Currently, sorbents that provide selectivity in interaction with the analyte are of interest. Various derivatives of cellulose are used to obtain selective sorbents: ethylcellulose, hydroxypropylmethylcellulose (hypromellose), carboxymethylcellulose, etc.\u0000On the pharmaceutical market of the Russian Federation, there are pharmaceutical formulations with the active pharmaceutical substance salicylic acid. The use of selective SPE for analysis is a promising direction of research in drug quality control.\u0000Aim. Evaluation of the metrological characteristics of benzocaine quantitation by UV-spectrophotometry in certain pharmaceutical formulations using hypromellose sorbent for solid-phase extraction.\u0000Material and methods. The sorbent using a method developed by us was obtained. Salicylic acid (FS.2.1.0634, Russia State Pharmacopoeia XV ed.) to form active binding sites in sorbent structure was applied. The sorbent structure is polycyanoacrylate matrix with hypromellose fragments, the surface area is 255.50 m2/g, pore volume is 0.1433 cm3, pore diameter is 5.32 nm. The sorption capacity of hypromellose sorbent for benzocaine was 12.2 ± 0.8 μg/g.\u0000The SPE technique is proposed. SPE includes the stages: conditioning, sample addition and step-by-step elution with purified water and sodium hydroxide solution 0.1 mol/L. The spectrophotometer SF-56 was used to measure the absorbance at an analytical wavelength of 286 nm.\u0000To evaluate the selectivity (specificity) of sample preparation during chromatographic (SPE) separation of pharmaceutical formulations, absorption spectra of eluates in the wavelength range of 200-400 nm were scanned. The salicylic acid identification in the eluate was carried out based on absorption peaks at 228 and 298 nm.The salicylic acid calibration curve in the range of 1-20 μg/ml. \u0000To determination the metrological characteristics of salicylic acid quantitation method, 16 parallel determinations of samples of each pharmaceutical formulation were made. Previously, the prognosis for their reproductive function was considered unfavorable. In modern conditions, by improving IVF techniques using oocyte donation, treatment of infertility in these patients becomes possible. Therefore, studying the features of the tactics of managing such patients at the stage of preconception preparation, the features of IVF protocols, as well as the features of managing pregnancy and labor and delivery is relevant. The purpose of the study is to substantiate the possibility of successful implementation of reproductive function in a patient with gonadal dysgenesis. A clinical case of pregnancy and childbirth in a patient with a typical form of gonadal dysgenesis (karyotype 45 X0) is presented. The diagnosis was made at the age of 15 during a comprehensive examination for primary amenorrhea, and hormone replacement therapy was prescribed. The patient's pregnancy occurred after preparation of the endometrium with estrogens and gestagens as a result of the fourth IVF attempt with the donation of an egg fertilized by her husband's sperm, and was complicated by the threat of termination at 8 and 19 weeks. At 36 weeks, the pregnancy was complicated by severe preeclampsia, which required emergency caesarean section. The course of the postoperative period was uneventful with continuation of therapy for complications of gestation. It is recommended after cessation of lactation - HRT using drugs with an estrogenic component similar in chemical structure to natural ones, gestagenic - metabolically neutral. The study of this clinical case is of undoubted interest, taking into account the peculiarities of management tactics. It is necessary to implement a multidisciplinary approach to the management of such complex patients. Taking into account the peculiarities of the body condition in patients with gonadal dysgenesis and the absence of hormonal ovarian function, it is necessary to ensure optimal preconception preparation, the use of an individual approach to the implementation of the IVF protocol, and the prevention of complications during pregnancy. The purpose of the study is to analyze the main modern trends in the treatment and prevention of postoperative pancreatitis. Material and methods: analysis of publications on the prevention and treatment of APP, found according to PRISMA recommendations, for 2015-2023. Result of the study: three main strategies in the prevention and treatment of APP were analyzed, based on assessing risk factors for the patient, improving technical methods for performing interventions in the abdominal cavity, as well as pharmacological methods. Conclusion. It is advisable to develop comprehensive measures for the prevention of postoperative pancreatitis should be based on an assessment of the risks of its development associated with the characteristics of the patient, the prophylactic use of various drugs, as well as in relation to improving the technical techniques used in abdominal surgery. The purpose of the study: to assess the organization of medical care for children living in rural areas of the Orenburg region. Materials and methods. An assessment of the organization of medical care for children in rural areas in the Orenburg region was carried out for the period 2016–2020. A retrospective study was conducted using the methods of statistical and epidemiological analysis. Data provided by the state budgetary healthcare institution of the Orenburg region \"Medical Information and Analytical Center\" (MIAC). Results. A decrease in the population of the Orenburg region in rural areas, a decrease in the number of outpatient clinics, medical outpatient clinics and FAPs have been established. There was an increase in the capacity of polyclinics by 5.8% (from 196.8 to 213.9); decrease in the indicator of provision with pediatric and infectious beds (44.0%; 9.5%); the provision of pediatricians (per 10,000 population) decreased by 11.6% (2016 - 18.1; 2020 - 16.0). The incidence of children (0-14 years old), adolescents (15-17 years old) decreased respectively by 17.6% (in cities - by 14.5%, in districts - by 15.9%) and 20.2% (in cities - by 10.7%, in districts - by 30.9%). Infant mortality (per 1000 live births) decreased by 50% (from 6.5‰ in 2016 to 3.7‰ in 2020). In rural areas, infant mortality was on average 40% higher than in urban areas. Conclusions. Structural changes in rural healthcare are associated with a reduction in the rural population and optimization of healthcare. For child and adolescent psychiatry, the issue of structuring psychosocial rehabilitation seems particularly relevant (due to the severity of the outcomes of child and adolescent mental illnesses) and poorly studied. The purpose of this work was to analyze the results of practical activities on social rehabilitation of minor patients, with the formation of proposals for comprehensive support for these patients in the outpatient network. The study material was a cohort of 1203 patients (m. - 914 (76%), female - 289 (24%)) who initially applied to a psychoneurological clinic in childhood and adolescence. They were followed clinically and follow-up until adulthood (catamnesis was carried out upon reaching the age of 23-25 years (1095 patients - 91%)). Research methods: clinical-psychopathological, psychometric, clinical-follow-up, statistical. Results: Based on the results of the clinical follow-up study, it could be concluded that medical and social assistance to minor patients should be reformatted in the direction of more active identification of initial mental disorders; During the initial contact, it is necessary to build trusting relationships with relatives and the patients themselves, and engage in their psychoeducation. The issues discussed with relatives are similar to those that are subsequently discussed with teenagers. The topic is dealt with in an individually adapted manner for each interlocutor. Identification and discussion with parents of the most problematic areas of the child’s life should culminate in the development of rehabilitation programs to develop lost or undeveloped daily functioning skills. Conclusion: In general, achieving highly functional clinical and social remission in patients suffering from early mental disorders is a complex problem, the final solution of which is associated with future achievements in a number of areas of clinical and social sciences. Despite a large number of studies, the issue of risk factors for this condition, as well as the associated metabolic syndrome, remains relevant to date. Also interesting is the question of analyzing risk factors in patients with obesity and metabolic syndrome, and without them. The purpose of the study was to study risk factors in adolescents with obesity and metabolic syndrome. The study examines the role of perinatal factors, heredity, peculiarities of eating behavior, the role of psychological status and microsocia. It was shown that perinatal factors (complicated course of pregnancy and childbirth), problems of early breast-feeding (previously artificial feeding, perinatal damage of the central nervous system, anemia, rickets), as well as burdened heredity for cardiovascular and endocrine pathology (obesity, diabetes mellitus 1 type) had all obese adolescents, regardless of the presence of metabolic syndrome; the peculiarities of the nutritional status (food with a lot of carbohydrates and refined fats, protein deficiency) in combination with physical inactivity and the formation of an unhealthy lifestyle were noted in all examined adolescents. Psychological problems, such as a high level of anxiety, low self-esteem, social phobia, violation of personal perception and motivation, as well as pathology of the microsocial environment, that is, family problems prevailed in adolescents with obesity and components of metabolic syndrome. At the moment, the problem of child injuries is being discussed all over the world. Childhood trauma is one of the main causes of hospitalization and mortality. In this regard, a detailed analysis of the causes of childhood injuries and their consequences is necessary for the development and implementation of preventive work. The aim is to estimate the situation related to child injuries in the Arkhangelsk region with the proposal of some organizational measures to reduce the frequency of injuries and their consequences. Materials and methods. The analysis was carried out on the basis of information statistical materials obtained from the Arkhangelsk Regional Children's Clinical Hospital named after P.G. Vyzhletsov. The annual changes in the number of injuries among children of the Arkhangelsk region from 2020 to 2022 were studied. The age characteristics, the nature/localization of injuries, as well as the type (place) where the injury was received were determined. The results of the study. Throughout the study period, the number of injuries among children of the Arkhangelsk region decreased by almost 20%. Annual changes in the number of injured children in different age ranges showed a decrease in the studied indicator in the infant and preschool, as well as high school groups. At the same time, in the categories of primary and secondary school age, this indicator has been constantly increasing for three years, eventually increasing by almost 10% . At the moment, there is an increase in the incidence of malignant neoplasms in the Russian Federation. The Arkhangelsk region is no exception in this case. A fairly significant share of all malignant diseases is occupied by neoplasms of the genitourinary system, which are sometimes difficult to diagnose and eliminate in time. The aim of the work is to identify of the features of the epidemiological situation of morbidity and mortality of the population of the Arkhangelsk region with malignant neoplasms of the genitourinary system, as well as the effectiveness of the organization of the processes of medical care, prevention and early detection of this pathology. Materials and methods. Statistical data (Federal State Statistics Service, Unified Interdepartmental Information and Statistical System, Cancer Register) linked to the incidence of cancer of the genitourinary system in residents of the Arkhangelsk region were used. A comparative analysis was carried out with those on average in the Russian Federation. The results of the study. Throughout the entire study period (from 2011 to 2019, with the exception of 2020), there was an increase in the detection of oncological pathology of the genitourinary system in the population of the Arkhangelsk region. The decline in 2020 is closely related to the spread of COVID-19 and limiting measures. The average mortality rates in the Arkhangelsk region from malignant neoplasms of the genitourinary system from 2011 to 2021 practically did not differ from the average Russian. Since 2016, there has been a slight increase in the indicator of early diagnosis of oncological diseases of the genitourinary system in the region. It reflects the effectiveness of the measures taken to fight cancer, in particular, the introduction of the Federal program \"Fight against oncological diseases\".
