Objective: To study, to systematize and to evaluate the prognostic significance of clinical and laboratory markers of nervous system damage associated with pregnancy with and without arterial hypertension (AH) in order to determine the main risk factors for the development of stroke. Materials and methods. Assessment of autonomic dysfunction, neuro-ophthalmological examination, additional research methods: laboratory and instrumental methods, including the study of gene polymorphism, CT, MRI Results. Based on the results of constructing a set of logistic regression models that generalize their results of a high probability detection algorithm, it is shown that in order to identify the final group of high risk of developing ischemic stroke in pregnant women, it is necessary to exclude patients without autonomic dysfunction before pregnancy, and also take into account the risks if they are detected in at least two stages. The study shows the values of heart rate variability in patients with paroxysmal atrial fibrillation treated with allapinin monotherapy and a combination of allapinin with metoprolol, allapinin with sotahexal and allapinin with diltiazem with preventive antiarrhythmic therapy. Analysis of heart rate variability allows you to determine the state of the autonomic nervous system. A high probability of an unfavorable outcome of diseases of the cardiovascular system is observed with the predominance of the tone of the sympathetic nervous system and a decrease in the tone of the parasympathetic nervous system. A decrease in heart rate variability and a predominance of sympathetic nervous system tone are predictors of the occurrence of another paroxysm in patients with paroxysmal atrial fibrillation. Indicators of heart rate variability are of great importance in the selection of antiarrhythmic therapy. The initial predominance of the tone of the parasympathetic nervous system in comparison with healthy ones and the absence of negative dynamics after 1 year was recorded in patients with paroxysmal atrial fibrillation with allapinin monotherapy. Patients treated with a combination of allapinin and metoprolol had a predominance of the tone of the parasympathetic nervous system initially compared with the healthy group and a transition to sympathicotonia after 1 year compared with healthy, control and their own baseline values. Patients receiving allapinin and sotahexal had no significant differences either at the beginning of the study or after 1 year. The article presents the results of the effectiveness of preventive antiarrhythmic monotherapy with sotahexal and cordarone based on heart rate variability and examination of patients at intervals of every 3 months for one year. The relevance of studying heart rate variability indicators is beyond doubt. The simplicity and non-invasiveness of this procedure dictates its increasing use. As a result of the study, the mechanism of occurrence of paroxysms of atrial fibrillation was shown both in patients with the existing paroxysmal form of atrial fibrillation, and in comparison, groups, where the debuts of this disease were recorded as a result of the study. It is known that with age there is an increase in diseases of the cardiovascular system, including atrial fibrillation. Elderly patients make up the bulk. Therefore, properly selected antiarrhythmic therapy based on heart rate variability indicators can help reduce morbidity in this group of patients. When studying the effectiveness of antiarrhythmic therapy on the example of representatives of the III class of antiarrhythmic drugs sotahexal and cordarone, it was found that the effectiveness of monotherapy with sotahexal and cordarone by the end of 1 year of the study was 74 and 71%, respectively. Indicators of heart rate variability in patients receiving sotahexal after 1 year did not significantly differ from each other, and at the initial stage of treatment, indicators characterizing the tone of the parasympathetic nervous system prevailed over the same indicators of patients receiving cordarone, over the tone differed from the initial ones in the downward direction. In the work, a pre-experimental screening of pyridinecarboxylic acid derivatives was performed by the PASS program, The work identified potential molecular targets for the implementation of the antitumor effect of a new domestic compound, an analogue of pyridine LHT-13-19. Using the \"Autodock 4.2\" software environment, flexible receptor-directed molecular biological docking was carried out in virtual reality, which makes it possible to most accurately predict the formation of a complex between a molecular structure and a biological target in real conditions of a specific biological system. For molecular docking, three-dimensional structures of the epidermal growth factor receptor (EGFR, PDB ID: 1M17, 4KN2) from the open electronic library Protein Data Bank (USA) were used. Molecules of compounds - pyridine derivatives LHT-13-19, LHT-16-19 and LHT-17-19 were synthesized in the Department of Chemistry, All-Union Research Center for Biological Active Compounds Safety (Russia). As a result of the experiments, it was found that all the studied molecules have inhibitory activity against proto-oncogenic kinases, however, in terms of the totality of predictive characteristics, as well as the likelihood of forming an antitumor effect, LHT-17-19, which was studied in docking studies, turned out to be the most promising compound. It was shown that LHT-17-19 has a high affinity for the epidermal growth factor kinase receptor EGFR-K, exhibits an affinity for the CSF1 receptor system, superior to that of all comparators - imatinib, erlotinib and pemetrexed. Background: Penetration of plasmid DNA into the nucleus by a passive process is ineffective due to the barrier effect of lipid membranes and diffusion barriers inside the cell. An effective gene delivery system should ensure binding to the target cell, penetration by endocytosis, protection from degradation and, ultimately, delivery of the expression plasmid to the nucleus. The distribution routes of transfected agents play a crucial role in achieving the targeted effect of drugs. In our experiment, plasmid DNA of the reporter protein GFP was used to study the distribution pathways. Aim: to evaluate the distribution of GFP reporter protein in B16 melanoma cells and distant organs of C57Bl6 mice as a method for determining the effectiveness of transfection of genetically engineered structures for the diagnosis and therapy of tumors in vivo. Materials and methods: An in vitro study was conducted on skin melanoma cell lines B16. Cells were transfected with a plasmid of the reporter protein GFP. In vivo, the study was conducted on mature female mice of the C57Bl/6 line, to which tumor cells were transplanted and GFP plasmid was intraperitoneally injected using commercial transfectants Lipofectamine 3.0 and Invivofectamine. The efficiency of transfection in organ and tumor cells was evaluated using the cell imaging station Floid. Results: A high survival rate of B16 melanoma cells was established and GFP+ B16 melanoma cells were obtained within 48 hours. We found that with intraperitoneal administration of a lipocomplex containing GFP plasmid, both with the use of Lipofectamine 3.0 transfectant and Invivofectamine, effective incorporation into liver and kidney parenchyma cells occurs. The penetration and retention of GFP reporter protein in liver, kidney and tumor node cells was revealed. Conclusion: The effectiveness of transfection of GFP plasmid into liver, kidney, and melanoma B16 tumor node cells of C57Bl6 mice has been proven. The molecular level of physiological processes in the context of studying the pathogenesis of diseases is the basis for their diagnosis and treatment. Thus, the features of the pathogenesis of periodontitis are the direct participation of pro-inflammatory cytokines in the development of alterative inflammation. The study of the cytokine profile is relevant for both diagnostic and therapeutic purposes, since the dynamics of indicators of inflammatory mediators reflects the degree of activity and specificity of pathological reactions present in periodontitis. The purpose of the study Study of cytokine profile indicators in experimental periodontitis. Materials and methods. In an experiment on 50 white non-linear rats weighing 190-280 g, the model of experimental periodontitis proposed by Shkolnaya K.D., Atrushkevich V.G. was reproduced. (Patent RU No. 2625295 dated 07/12/2017). Assessment of the dynamics of the cytokine status of blood serum was carried out by the level of pro-inflammatory (IL-1β, TNF, IL-6, IL-17), anti-inflammatory (IL-4, IL-10) and regulatory (IL-2) cytokines using enzyme-linked immunosorbent assay using reagent kit \"Bender MedSystems\". Results. The study revealed the presence of an imbalance in the cytokine system, characterized by a violation of the physiological balance between pro-inflammatory and anti-inflammatory cytokines with a predominance of the former. Hyperproduction of cytokines persisted throughout the experiment with an increase in the above uncoupling by day 25, which indicates the formation of an immunoinflammatory process in the periodontium. Conclusion. We studied the neurotoxic/neurotropic effects of a new derivative of dimethylacetamide and benzoic acid LHT-5-01 in a mixed neuroglial culture to determine both the effect of various concentrations of the compound on cell survival and their interaction with the glutamate noncainate receptor. To do this, a mixed neuroglial culture of the hippocampus of linear newborn (P 1-3) Sprague Dawley rats was resuspended in a neurobasal medium containing 0.5 mmol glutamine, 2% Supplement B27 and 15 μg/ml gentamicin. The cytoplasmic concentration of calcium in the ionized form [Ca2+]i was determined using a Fura-2 fluorescent probe (USA). The viability of culture cells—neurons and glial cells—was determined by the immunocytochemical method. Experimental studies were performed on cultures aged from 7 to 9 days, to which the test compound of dimethylacetamide LHT-5-01 was added at various concentrations in the laboratory of intracellular signaling of the Institute of Cell Biophysics of the Russian Academy of Sciences - a separate division of the Federal State Budgetary Scientific Institution Federal Research Center \"Pushchino Scientific Center for Biological Research of the Russian Academy of Sciences\" (Pushchino) in strict accordance with the literary descriptions. Incubation of the culture with 10 μM of the LHT-5-01 compound completely suppresses the first phase of receptor activity, but does not prevent the formation of the second phase of NMDA receptor excitation. Relevance: Squat cloudberry (Rubus chamaemorus) has a sufficiently wide range of therapeutic effects for various diseases. At the same time, it does not refer to pharmacopoeia medicinal plants, but is used as folk medicine. Tannins (plant tannins) detected in Rubus chamaemorus hold a special place among the other biologically active substances of this plant. They have anti-inflammatory, antimicrobial, antioxidant and anti-cancer activity. Also, their properties are applicable in the prevention of cardiovascular and metabolic diseases. The aim of the study is to establish the presence of tannins and their quantitative determination in the leafs of Cloudberry squat, growing in the Vologda region for the possible determination of the raw materials of this plant as medicinal. Materials and methods: the presence and quantitative content of tannins in the raw materials of the squat cloudberry (leaves) growing on the territory of the Vologda region was determined using various methods of chemical analysis. Results of the study: qualitative samples confirmed the presence of tannins in the leaves of the studied plant (gelatin sample), as well as the sufficient presence of their hydrolyzable forms (iron-ammonium alum sample); quantitative (by permanganatometry) determined the content of tannins relative to the total dry weight of raw materials, it was 15.03 ± 1.14% (95% confidence interval = 14.12-15.95). The results obtained are consistent with the data of other researchers, the average relative proportion of tannins in relation to the total mass of the dry plant correlates to the average scientific results. Currently, the development and implementation of digital medicine is closely related to the use of IT technologies. They, in turn, provide Internet navigation of population in the healthcare system, disease prevention and healthy lifestyle organization and clinical telemedicine, as well as support for scientific clinical solutions. Over the last 5 years, the Arkhangelsk region has made a significant advance in the implementation of telemedicine technologies. The number of medical organizations that can carry out telemedicine consultations has increased many times. At the same time, more than 80 % of telemedicine studios are located on central district hospitals and outpatient clinics. The most frequently employed specialists for telemedicine consultations were physicians in the following areas: x-ray (14%), traumatology (9.5%), surgery (9%), neurosurgery (8.5%), hematology (8%), neurology (7.5%), cardiology (7%). A particular place in the telemedicine system in the Arkhangelsk region is taken by the Perinatal Center of the Arkhangelsk Regional Clinical Hospital, which was started in 2018. It is equipped with all the necessary medical and technological equipment. It organizes the work of auxiliary offices with the use of telemedicine technologies: obstetric external consulting room; neonatal remote consulting room.
