Recurrent or spontaneous epistaxis is the most prevalent symptom of hereditary hemorrhagic telangiectasia, whose severity varies greatly, ranging from moderate self-limiting epistaxis to severe, life-threatening epistaxis, which often requires multiple treatments and is therefore a challenge for otorhinolaryngologists. This case report retrospectively analyzed the clinical data and family history of a patient who presented to the Department of Otorhinolaryngology, Hangzhou First People's Hospital, with recurrent epistaxis for 60 years. The patient was treated with Little's area septomucotomy combined with electrocoagulation for epistaxis, and her clinical symptoms and specialty findings showed significant improvement after four postoperative follow-up visits. JenaValve, a second-generation device with unique positioning and anchoring mechanisms, was deployed successfully, resulting in immediate improvements in cardiac function without significant regurgitation or paravalvular leakage. This case highlights the potential of transcatheter aortic valve implantation as a viable alternative to surgical aortic valve replacement in patients with severe aortic regurgitation, emphasizing the importance of technological advancements and a multidisciplinary approach in managing complex cardiovascular cases. Health outcomes are influenced by both physiological factors and social determinants of health; however, the interplay between these remains underexplored.ObjectiveThis study aimed to identify risk factors for muscle weakness and physical decline in older adults, integrating physiological and social determinants of health variables, and develop a predictive model for early risk assessment.MethodsUsing prospective China Health and Retirement Longitudinal Study data with 9-year follow-up data (baseline predictors from 2011, outcomes from 2020), logistic regression, recursive feature elimination, and XGBoost algorithms were applied to construct predictive models.ResultsKey risk factors included age, ethnicity, cognitive function, and physical activity. Social determinants of health variables such as marital status, life satisfaction, and educational level were significant predictors. SHapley Additive exPlanations analysis revealed that social determinants of health variables significantly enhanced model performance and interpretability.ConclusionIntegrating social determinants of health with clinical indicators improves the prediction of muscle weakness and physical decline in older adults. We quantified NOD-like receptor protein 3 and interleukin-18 expression at the mRNA and protein levels using quantitative real-time polymerase chain reaction and western blotting, respectively. The diagnostic performance of these molecules was assessed using receiver operating characteristic curve analysis.ResultsOur results revealed that both NOD-like receptor protein 3 and interleukin-18 were significantly upregulated in basal cell carcinoma tissues compared with adjacent noncancerous tissue samples at the mRNA and protein levels (p < 0.05). Receiver operating characteristic curve analysis demonstrated strong diagnostic performance, with an area under the curve of 0.931 for NOD-like receptor protein 3 (95% confidence interval: 0.845-1.00) and 0.860 for interleukin-18 (95% confidence interval: 0.739-0.980).ConclusionsThe marked upregulation of NOD-like receptor protein 3 and interleukin-18 suggests their involvement in the inflammatory tumor microenvironment and positions them as promising candidate biomarkers for basal cell carcinoma detection. Eighty-seven patients diagnosed with lumbar spinal stenosis at L3-S1 levels underwent bilateral decompression and transforaminal lumbar interbody fusion between 2017 and 2022. The procedures were performed through a posterior midline incision, followed by insertion of a lordotic cage to restore spinal alignment. Fluoroscopy and microscopy confirmed the precise placement of the cage. Clinical outcomes were assessed using visual analog scale and Oswestry disability index scores, with radiological evaluations through computed tomography and magnetic resonance imaging.ResultsPostoperative imaging demonstrated the centralization of the lordotic cage in both anteroposterior and lateral planes, ensuring equal foraminal elevation bilaterally. The visual analog scale and Oswestry disability index scores significantly improved at all follow-up intervals. The incidence of contralateral radiculopathy was minimized (1%) due to prophylactic decompression, and the foraminal area increased by more than 20%.ConclusionsBilateral decompression combined with a unilateral transforaminal lumbar interbody fusion approach enables effective stabilization and alignment of spinal segments. Imaging and laboratory tests confirmed the presence of chylous effusions. Treatment included thoracentesis, paracentesis, antibiotics, somatostatin analogs, and supportive care. The patient showed significant improvement and was discharged with no recurrence at follow-up. The CHG index was calculated using total cholesterol, high-density lipoprotein, and fasting blood glucose levels. Weighted logistic regression was used to assess the association between the CHG index and type 2 diabetes mellitus prevalence, and restricted cubic spline analysis was applied to examine potential nonlinear relationships. Receiver operating characteristic curves were used to evaluate the diagnostic performance of the CHG index, and the results were compared with those of the triglyceride-glucose index.ResultsA higher CHG index was significantly associated with increased type 2 diabetes mellitus prevalence (odds ratio: 4.30, 95% confidence interval: 3.21-5.77, p < 0.001) after adjusting for multiple confounders. Restricted cubic spline analysis showed a nonlinear relationship, demonstrating an elevated risk of type 2 diabetes mellitus in patients with a CHG index of >5.24. The association remained consistent across all subgroups. The CHG index demonstrated good predictive value (area under the curve = 0.721; optimal cutoff = 5.47; sensitivity = 0.53; specificity = 0.80), comparable to that of the triglyceride-glucose index (area under the curve = 0.730; optimal cutoff = 8.84; sensitivity = 0.62; specificity = 0.73).ConclusionThe CHG index may be associated with type 2 diabetes mellitus prevalence and could serve as a simple, accessible biomarker for early risk identification. This study sought to ascertain whether there is a correlation between amygdala volume and suicidal behavior.MethodsThis case-control retrospective study included 193 participants-108 healthy controls and 85 individuals with a history of suicide attempts. Sagittal magnetic resonance imaging was conducted for each participant, and cross-sectional areas of the left and right amygdala were determined independently using freehand tracing. The total volume of the amygdala was determined by multiplying the thickness of the slices by the sum of the regions.ResultsPatients with a history of suicide attempts had significantly decreased amygdala volumes on both the right (<i>p </i>=<i> </i>0.018) and left (<i>p </i>=<i> </i>0.036) sides compared with controls. Receiver operating characteristic analysis determined a cutoff value of 1825 mm³ for both hemispheres. The cutoff sensitivity and specificity for the right amygdala were 30.6% and 88%, whereas the values for the left amygdala were 31.8% and 84.3%, respectively.ConclusionsThis is the first study to show that individuals who had attempted suicide had considerably decreased left and right amygdala volumes, with receiver operating characteristic analysis indicating good specificity (84.3%-88%). Patients with postoperative recurrence typically have limited treatment options. This study evaluated the safety and feasibility of computed tomography-guided radiofrequency ablation for treating high-risk pulmonary nodules in patients who have undergone lung cancer surgery and are ineligible for reoperation.MethodsWe retrospectively analyzed 40 patients (16 men and 24 women, aged 41-78 years) with radiologically suspicious recurrent nodules following lung cancer resection who were treated with radiofrequency ablation at West China Hospital (July 2023-August 2024). Outcomes included procedural success, occurrence of complications, hospitalization duration, 30-day mortality, and imaging response.ResultsAll procedures were technically successful. Five minor complications occurred in 12.5% of the patients, including fever (n = 2) and pneumothorax (n = 3). No deaths occurred within 30 days, and no major adverse events (e.g. hemothorax) were observed. The median postoperative length of hospital stay after ablation was 2.0 days. Furthermore, 82.5% of the patients were followed up and had achieved complete ablation at the 3-month mark.ConclusionComputed tomography-guided radiofrequency ablation is a minimally invasive, safe, and effective option for managing high-risk pulmonary nodules in postoperative lung cancer patients who are deemed ineligible for reoperation. Its low complication rate, rapid recovery, and promising local control support its integration into multimodal treatment algorithms. After the outbreak of coronavirus disease 2019, many studies have reported the potential effectiveness of ivermectin against coronavirus disease 2019; however, it is not recommended by the World Health Organization because of insufficient evidence and significant adverse effects. Owing to the abovementioned findings, the number of reports of poisoning and other serious reactions caused by ivermectin abuse have increased in recent years. Notably, no previous cases of transdermal ivermectin poisoning with documented blood concentrations has been reported to date. We report a rare fatal case of ivermectin misuse with a review of recent literature. The patient suffered from ivermectin poisoning due to transdermal overdose. The patient's plasma concentration was 27 ng/mL. The main clinical manifestations were gastrointestinal symptoms in the early stage and diffuse cerebral edema and intracranial hypertension in the later stage. Despite active treatment, including hemoperfusion and cardiorespiratory support, the patient died. Many recent in vitro studies have shown that ivermectin has the potential to become a new anticancer drug. If clinical research proves its effectiveness against cancer, it may also lead to ivermectin overuse.
