Frequent visitors to the ophthalmology emergency department: Clinical characteristics and management

Henry","doi":"10.1016/j.jfo.2025.104448","DOIUrl":"10.1016/j.jfo.2025.104448","url":null,"abstract":"<div><h3>Purpose</h3><div>To describe the profiles and management patterns of frequent visitors to the ophthalmology emergency department in comparison to patients who visit only occasionally.</div></div><div><h3>Methods</h3><div>This study conducted a retrospective analysis spanning a 2.5-year period encompassing all visits to the ophthalmology emergency department. Patients who visited four or more times during this period were identified as frequent users and compared with a control group randomly selected from patients who visited only once. Severity scores associated with diagnoses, ranging from 1 to 5 (Base Score), along with clinical characteristics and patient management strategies, were analyzed using both univariate and multivariate analyses.</div></div><div><h3>Results</h3><div>Of 18,802 visits, 331 patients (constituting 2.23% of the total) were identified as frequent users during the 2.5-year period, with numbers of visits ranging from 4 to 23. In comparison to the control group, frequent users tended to be approximately 10 years older and presented with more severe diagnoses (Base Score 2.43<!--> <!-->±<!--> <!-->1.15 versus 1.89<!--> <!-->±<!--> <!-->0.94, <em>P</em> <!-->=<!--> <!-->6.35<!--> <!-->×<!--> <!-->10<sup>−11</sup>). Their visits were predominantly associated with non-traumatic anterior segment pathologies (56% versus 27%) rather than traumatic pathologies (8.7% versus 27%). They were more likely to be referred to senior practitioners within the department (43% versus 33%), necessitated more frequent hospitalizations (33 cases versus 1), and required post-emergency follow-ups or referrals to hyper-specialized consultants.</div></div><div><h3>Conclusion</h3><div>Despite comprising a relatively small percentage of the total patient population, frequent users of ophthalmological emergency departments tend to present with more severe diagnoses. This underscores the importance of optimizing resources to alleviate some of the burden on specialized emergency services.</div></div><div><h3>Objectif</h3><div>Décrire les caractéristiques cliniques et la prise en charge des usagers fréquents des urgences ophtalmologiques et les comparer à celles des usagers occasionnels.</div></div><div><h3>Méthodes</h3><div>Étude rétrospective de l’ensemble des consultations aux urgences ophtalmologiques d’un CHU de France sur une période de 2,5 ans. We discuss the course of rehabilitation for a patient who underwent ISBCS and developed toxic anterior segment syndrome (TASS) in both eyes.</div></div><div><h3>Methods</h3><div>This study is a case report.</div></div><div><h3>Results</h3><div>The patient was a 42-year-old woman with no remarkable past medical history who underwent ISBCS at another hospital in 2023. Intraoperatively, mitomycin C (MMC) was inadvertently instilled into the anterior chamber of both eyes at the conclusion of surgery instead of carbachol (Miostat®). The next day, the patient noticed vision loss, and an ophthalmological examination revealed corneal edema in both eyes. She was referred to our hospital with the diagnosis of bilateral TASS. Although the left eye began to improve after a week of medical treatment, the right eye finally required Descemet's membrane endothelial keratoplasty. At the 2-month postoperative visit, uncorrected distance visual acuity was 0.7 bilaterally, and both corneas were clear on slit lamp biomicroscopy.</div></div><div><h3>Conclusion</h3><div>Although ISBCS provides convenience in many areas, we believe that it should be performed only for particular indications and conditions, because it may lead to severe problems such as bilateral blindness.</div></div><div><h3>Objectif</h3><div>La fréquence et la popularité de la chirurgie de cataracte bilatérale séquentielle immédiate (ISBCS) augmentent. Anti-CD63 immunogold labeling was also performed to confirm the nature of elements morphologically suggestive of extracellular vesicles.</div></div><div><h3>Results</h3><div>Human tears were rich in cup-shaped vesicular structures, identified as extracellular vesicles by immunogold labeling. These vesicles were highly heterogeneous in morphology and size. Filamentous structures 200 to 700<!--> <!-->nm in length, presumed to be structures composed of phospholipids or mucopolysaccharides, were also observed.</div></div><div><h3>Conclusions</h3><div>Two main structures were observed in human basal tears of healthy subjects, mostly extracellular vesicles. To improve applicability and translation from the laboratory bench to the hospital bedside, we provide a quick and feasible workflow for real-time analysis of human tears using transmission electron microscopy. Analysis of vesicle richness and morphology in tears could help for detecting biomarkers of ocular and systemic disease.</div></div><div><h3>Objectif</h3><div>Décrire les composants des larmes humaines par analyse extemporanée en microscopie électronique à transmission par coloration négative et identifier des vésicules extracellulaires par immunomarquage.</div></div><div><h3>Méthodes</h3><div>Les larmes de 13 sujets sains ont été recueillies à l’aide de tubes microcapillaires en verre, préparées en coloration négative et observées par microscopie électronique à transmission. Clinical manifestations and diagnostic tests are described.</div></div><div><h3>Results</h3><div>Methylmalonic acidemia with homocystinuria is a multisystem disease, severe and occasionally lethal, with a wide clinical spectrum ranging from fetal/neonatal disease to a benign disorder starting in adulthood. The patient in this report was referred for a routine evaluation. Retinal photography, fluorescein angiography, optical coherence tomography (OCT), en-face OCT angiography, visual fields, and electroretinography were performed. OCT, including en-face mode, revealed retinal dimples, a finding not previously described. Retinal dimples corresponded to OCT angiography flow deficits at the level of the deep vascular plexus.</div></div><div><h3>Conclusions</h3><div>Retinal dimples might be a consequence of retinal ischemia secondary to microangiopathic disease. Ocular findings remained stable during the 4-year follow-up.</div></div><div><h3>Objectif</h3><div>Décrire les manifestations cliniques ophtalmiques chez une patiente atteinte d’acidémie méthylmalonique avec homocystinurie à l’âge adulte, y compris les capitons rétiniens en tant que nouvelle découverte rétinienne.</div></div><div><h3>Méthodes</h3><div>Rapport de cas observationnel d’une femme de 25<!--> <!-->ans. Le traitement actuel repose sur l’usage curatif et préventif d’analogues nucléosidiques acycliques (ANA) anti-herpétiques, en particulier, l’aciclovir (ACV) ou d’autres molécules de même mode d’action. Les KH causées par un virus résistant à l’ACV sont de plus en plus rencontrées en pratique clinique. Leur prise en charge repose sur l’obtention du diagnostic virologique de résistance sur prélèvement oculaire acheminé en laboratoire spécialisé. La caractérisation génotypique par séquençage ADN des enzymes virales cibles des antiviraux permet l’adaptation thérapeutique en fonction de la mutation identifiée. Les mutations de la thymidine kinase (TK) virale sont les plus fréquentes et ne laissent que peu d’alternatives viables pour un traitement prophylactique au long cours parmi les molécules actuellement disponibles sur le marché. L’aménamévir (AMNV), nouvelle molécule ciblant le complexe d’enzymes virales hélicase-primase (HP) et ayant fait ses preuves dans le traitement d’autres localisations herpétiques, a récemment montré son intérêt dans la prise en charge des KH résistantes à l’ACV. Son mécanisme d’action indépendant de la TK lui permet de conserver une activité en cas de résistance avérée à l’ACV.</div></div><div><div>Herpetic keratitis (HK) caused by herpes simplex virus 1 (HSV-1) is the major cause of infection-related blindness in developed countries. Current treatment is based on curative and preventive use of antiherpetic acyclic nucleoside analogues (ANAs), namely acyclovir (ACV) or other molecules with the same mechanism of action. The frequency of HK caused by acyclovir-resistant viruses is steadily increasing. Virological proof of resistance is obtained from an ocular sample sent to a specialized laboratory. Genotypic characterization by DNA sequencing of the viral enzymes targeted by antivirals allows adaptation of treatment according to the mutation identified. Mutations of the viral thymidine kinase (TK) are the most frequent and leave few viable alternatives for long-term prophylactic treatment among currently available drugs. Amenamevir (AMNV), a new drug targeting the viral helicase-primase (HP) enzyme complex, with proven efficacy for treatment of herpetic infections of other sites, has recently shown value in the management of ACV-resistant HK.
