Comprende entre el 35 y 50 % de todos los casos de dificultad respiratoria no infecciosa que ingresan a los cuneros patológicos o unidades de cuidado intensivo neonatal.1 Mientras que las tasas de criptorquidia e hidrocele son más altas en los niños nacidos por cesárea (3.3 y 4.7 %, respectivamente), en comparación con los obtenidos por vía vaginal (1.7 y 1.6 %).2\u0000Descripción del caso: neonato de 39 semanas de gestación con taquipnea transitoria del recién nacido, criptorquidia e hidrocele atendido en el servicio de atención al recién nacido de un hospital de segundo nivel de atención.\u0000Objetivo: proporcionar cuidados especializados, utilizando el proceso de atención de enfermería basado en los conceptos teóricos del modelo de autocuidado de Dorothea E. Orem.\u0000Método: estudio de caso, dado que en este diseño se observan los fenómenos en su contexto natural, el cual se realizó en la tercera semana de mayo 2021. Consideraciones éticas: se tomaron en cuenta aspectos bioéticos para la investigación clínica basada en evidencia científica, como la ley de Helsinki y el código de Nuremberg. The Mini Mental State Examination (MMSE), the </span>Montreal Cognitive Assessment<span> (MoCA) and the Barthel index were used to assess, respectively, the patient’s cognitive status and ADL. The Zarit scale was applied to assess the burden on caregivers. A Spearman correlation was used assuming data abnormality on the Zarit scale and considering a significance of </span></span><em>P</em> &lt; .05.</p></div><div><h3>Results</h3><p>28 patients and 28 caregivers were evaluated. The Zarit scale showed correlations with the MMSE (Rho = −.48) and MoCA (Rho = −.43), as well as in the cognitive domains associated with temporo-spatial orientation (Rho = −.45) and reading (Rho = −.45) of the MMSE and language (Rho = −.41), memory (Rho = −.40) and temporo-spatial orientation (Rho = −.46) of the MoCA. Therefore, countless specialists have established relationships between t people who perform physical exercise and success in the treatment of diseases, such as Parkinson’s disease in older adults.</p></div><div><h3>Objective</h3><p>The aim of this research is to analyse the influence of resistance training in the correlation of balance and postural control in people with Parkinson’s disease.</p></div><div><h3>Method</h3><p>A systematic review<span> was conducted according to the recommendations of the Preferred Reporting Items for Systematic Reviews And Meta-Analyses (PRISMA), in consultation with the PubMed, Scielo, BVS (Lilacs), ScienceDirect and Cochrane databases<span>, based on the influence of resistance training on postural balance and control in older adults with Parkinson’s disease, highlighting as the main assessment tool the Movement Disorders Society — Unified Parkinson’s Disease Rating Scale.</span></span></p></div><div><h3>Results</h3><p>After searching the selected databases and considering the inclusion and exclusion criteria of this study, 10 studies were selected to compose this review and a total of 556 participants were pooled. The Mini Mental State Examination (MMSE), the </span>Montreal Cognitive Assessment<span> (MoCA) and the Barthel index were used to assess cognitive status and ADL, respectively. A Pearson and Spearman correlation was used considering the normality of the data in cognitive status.</span></p></div><div><h3>Results</h3><p>62 patients were evaluated. The most impaired domains were associated with writing and drawing in the MMSE (mean = 24.9 ± 4.1) and language, visuospatial/executive function, and memory in the MoCA (mean = 18.3 ± 5.6). The ADLs with most dependency on the Barthel index (mean = 78.1 ± 14.0) were eating, dressing/undressing, bathing, and going up and down stairs. On the other hand, the ADL dependency showed a correlation with the MMSE (Rho = 0.3) and the MoCA (r = 0.2). In addition, MMSE was correlated with activities including dressing/undressing (Rho = 0.3) and bathing (Rho = 0.3) and MoCA with transfer from chair to bed (r = 0.2).</p></div><div><h3>Conclusions</h3><p>The domains with the highest cognitive impairment were related to memory and visuospatial/executive function. Adherence to treatment is essential to modify the course of the disease. Cognitive impairment affects up to 70% of these patients and is a limitation for administration and adherence to treatment.</span></span></p></div><div><h3>Objective</h3><p>The main objective was to assess the degree of adherence to treatment in patients with MS and cognitive impairment. Secondary objectives were to describe the tools that can influence adherence to treatment and to evaluate the effectiveness of the technological devices used to alert the patient that he/she must self-inject.</p></div><div><h3>Method</h3><p>The method consisted of a descriptive, retrospective, and cross-sectional pilot study. It was carried out in the MS clinic of the Torrecárdenas University Hospital from December 2018 to September 2019. The sample included 20 patients diagnosed with MS currently treated with immunomodulatory injectable drugs for at least 6 months. The Symbol Digit Modalities Test (SDMT) was used to assess cognitive impairment and to determine adherence, patients answered the Morisky-Green test (MG).</p></div><div><h3>Results</h3><p>Twenty patients (8 men and 12 women) were included over 10 months with MS and cognitive impairment (SDMT score less than 50th percentile). Regarding gender-differentiated adherence, 75% of men were not adherent to treatment; by contrast, 66.6% of women were com-pliant with the dosage schedule. There were no statistically significant differences in terms of differentiated adherence by type of treatment. The main methods used to promote adherence were: 10% calendar, 20% family members, and 30% alarms as reminder.</p></div><div><h3>Discussion and conclusion</h3><p>Adherence results show a predominance of adherence in women, even with cognitive impairment. This reinforces our initial hypothesis that relates good therapeutic adherence to lesser progression of cognitive impairment in MS. {"title":"Frailty in people over 80 years after an ischemic stroke treated with mechanical thrombectomy","authors":"Alicia Murias Quintana ,&nbsp;Lorena Benavente Fernández ,&nbsp;Germán Morís de la Tassa","doi":"10.1016/j.sedeng.2021.07.001","DOIUrl":"10.1016/j.sedeng.2021.07.001","url":null,"abstract":"<div><h3>Introduction</h3><p>Although the advanced age is not an exclusion criterion for the stroke treatments, is characterized by a frailty state that affected the evolution and prognosis in such patients.</p></div><div><h3>Objectives</h3><p>To describe the possibility of a relation between frailty in elderly people (≥ 80 years old) after mechanical thrombectomy, and clinical severity, and to describe stroke clinical features.</p></div><div><h3>Methods</h3><p>Observational study in people over 80 year-old who suffered an ischemic stroke and underwent to mechanical thrombectomy, between February and October 2018. Data about cardiovascular risk factors, medical treatment, stroke episode and frailty were collected.</p></div><div><h3>Results</h3><p><span><span>The sample was constituted by 65 people. A 72% suffered hypertension and 43% atrial fibrillation<span>. A 75% did not take anticoagulants. A 82% did not suffer complications with the procedure, and a complete arterial </span></span>reperfusion was achieved in 83%. A 27.69% (n = 28) was not fragile. There was statistical association between age and frailty (</span><em>P</em> = .003).</p></div><div><h3>Discussion</h3><p><span>In our sample, the neurological and functional state caused by the stroke were more severe (described by the NIHS and Rankin modified scales score), but the recovery after </span>hospital discharge was more positive. Elderly survivor patients were characterized as no fragile. {"title":"Factores de riesgo del personal de enfermería del área quirúrgica de un hospital de tercer nivel","authors":"Antonio de Jesús Hernández-Silverio, Miriam Gómez-Ortega, Genoveva González-González, Janeth Delfina Salgado-Guadarrama","doi":"10.51422/ren.v20i3.340","DOIUrl":"https://doi.org/10.51422/ren.v20i3.340","url":null,"abstract":"Introducción: La bioseguridad es proteger que la calidad de que la vida sea libre de daño, riesgo o peligro; el personal de enfermería que labora en el quirófano está expuesto a múltiples factores de riesgo laborales,  como el uso de químicos para procesar material quirúrgico, riesgos biológicos por el estrecho contacto con el pacientes y fluidos corporales, psicosociales ya que se encuentra con cargas de trabajo y ambientes tensos, o riesgos ergonómicos por estar en posiciones de pie por largos periodos de tiempo en procedimientos quirúrgicos. La Organización Mundial de la Salud (OMS) fundamenta que para luchar contra las infecciones asociadas a la atención sanitaria es necesario reducir las normas de bioseguridad para el personal ocupacionalmente expuesto. \u0000Objetivo Analizar los factores de riesgo del personal de enfermería del área quirúrgica de un hospital de tercer nivel. \u0000Material y método: Estudio de tipo cuantitativo, descriptivo y transversal, Universo: integrado por el personal de enfermería del área quirúrgica de un hospital de tercer nivel. Muestra: no probabilística a criterio del investigador, integrado por 50 enfermeras y enfermeros  de quirófano de todos los turnos. Criterios de inclusión: todos los enfermeros y enfermeras que se encuentren laborando en el área quirúrgica de un hospital de tercer nivel, con más de 2 meses dentro del área quirúrgica.  Criterios de exclusión: Personal de enfermería que no firmó consentimiento informado y qué no deseó participar, personal de enfermería que estaban de permiso, vacaciones, incapacidad, ausentismo, o no se encuentren laborando por algún otro motivo. El instrumento fue un cuestionario de 88 preguntas, con respuestas dicotómicas, con una confiabilidad de .99 utilizando formula de Kuder-Richardson. \u0000Resultados: El personal de enfermería tiene un riesgo biológico del 100 %, en riesgos químicos 87%, riesgos físicos 94%, en cuanto a riesgos psicosocial el 94 % de los encuestados consideran tener este riesgo, para riesgos ergonómicos el 67% de los encuestados refieren presentar lesiones músculo esqueléticas. \u0000Conclusión: La bioseguridad se considera como una disciplina en el comportamiento del personal de enfermería dentro del área quirúrgica, que está dirigida al logro de actitudes y conductas con el objetivo de minimizar el riesgo de quienes trabajan en la prestación de servicios de salud, la cual se debe basar en principios fundamentales como son la universalidad, uso de barreras de protección y una adecuada eliminación de residuos sólidos. Los profesionales de enfermería dentro del área quirúrgica están expuestos a los diferentes riesgos. Estos peligros afectan individual y colectivamente, tienen la probabilidad de desencadenar una alteración a la salud, afectando inevitablemente el proceso de trabajo, conlleva al bajo rendimiento laboral e incumplimiento de los objetivos en las instituciones públicas y privadas.
