L’étiologie de ces lésions est avant tout traumatique. En dehors de la période néonatale et des classiques fractures de clavicules en lien avec la naissance et en l’absence de traumatisme plausible évident, un traumatisme infligé doit être évoqué jusqu’à preuve du contraire. Une hospitalisation en urgence s’impose dans le but de diagnostiquer des lésions profondes associées et d’éliminer les diagnostics différentiels dont les maladies osseuses constitutionnelles. Au cours de ce temps d’hospitalisation, la réponse aux besoins des nourrissons sera évaluée par les unités d’accueil pédiatriques enfants en danger (UAPED). En cas de danger suspecté chez un nourrisson non déambulant, une information préoccupante ou un signalement judiciaire sera adressé(e) aux autorités compétentes.</p></div><div><p>Bone fractures in non-ambulant children are a very challenging situation for healthcare professionals. The most common cause of these lesions is trauma. After the neonatal period and the common clavicle fractures associated with birth, in the absence of obvious plausible trauma, inflicted trauma must be considered. Emergency hospitalization is necessary in order to diagnose associated lesions and rule out differential diagnoses such as constitutional bone disorders. During hospitalization, the response to the infant's needs will be assessed by the Child Advocacy Center. Le recueil continu de données depuis le domicile des enfants est de plus en plus simple : les inhalateurs connectés permettent de suivre l’utilisation du traitement d’urgence en cas de crise d’asthme et l’observance du traitement de fond ; les bracelets et montres connectées collectent la fréquence cardiaque, la saturation, l’activité physique ; les bases de données environnementales et les capteurs de qualité de l’air intérieur renseignent sur les facteurs déclenchants des crises. Des algorithmes de décision autonomes pourraient utiliser ces données pour adapter la prise en charge des enfants en temps réel, sans intervention médicale systématique. Enfin, les enfants et leurs familles pourraient être accompagnés par des robots compagnons ou des assistants virtuels dans la mise en œuvre de la prise en charge proposée par les algorithmes. La combinaison de ces trois entités, capteurs, algorithmes de décision, et actionneurs, correspond au concept de système cyber-physique. L’objectif de cet article est de montrer que les développements technologiques actuels convergent vers la création de systèmes cyber-physiques dans l’asthme de l’enfant.</p></div><div><p>The digital revolution could lead to profound changes in asthma management. Continuous passive data collection from children's homes is becoming increasingly simple: Connected inhalers can be used to monitor the use of emergency treatment in the event of an asthma attack and adherence to controller treatment; connected bracelets and smart watches collect heart rate, oxygen saturation, and physical activity; environmental databases and indoor air quality sensors provide information on the factors triggering asthma attacks. Autonomous decision-making algorithms could use these data to adapt children care in real time, without the need for systematic medical intervention. Finally, children and their families could be accompanied by companion robots or virtual assistants as they implement the treatment proposed by the algorithms. The combination of these three entities -- sensors, algorithms, and actuators -- corresponds to the concept of a cyber-physical system.
