Il se définit par une réticence ou un refus fréquent d’aller à l’école accompagnée par de la détresse émotionnelle se traduisant par une crainte excessive, des crises de colère, de la tristesse, des symptômes physiques inexpliqués. Les absences scolaires ne sont pas dissimulées aux parents, il n’y a pas de comportements antisociaux significatifs et les parents font des efforts raisonnables pour assurer la présence de l’enfant à l’école. La prévalence est estimée entre 1,2 % et 4,8 % en population générale, jusqu’à 8 % en population pédiatrique consultant en pédopsychiatrie. Le RSA est un trouble complexe, dont les étiologies sont principalement anxieuses telles que l’anxiété de séparation chez les enfants et l’anxiété sociale chez les adolescents. Les conséquences sur la scolarité et la socialisation sont importantes et peuvent conduire à un décrochage scolaire. Les élèves sont rescolarisés à terme dans 50 à 70 % des cas. Il persiste souvent des troubles psychiatriques et une difficulté d’ajustement social. La prise en charge se doit d’être précoce et multidisciplinaire, incluant les professionnels de soins psychique et socio-éducatif. Elle repose sur le traitement des comorbidités et une psychothérapie basée sur un modèle cognitivo-comportemental.</div></div><div><div>Although it is currently a public health issue, school refusal anxiety (SRA) is an old problem. It is defined by frequent reluctance or refusal to attend school accompanied by emotional distress such as excessive fear, anger outbursts, sadness, and unexplained physical symptoms. School non-attendance is not hidden from parents, there are no significant antisocial behaviors, and parents make reasonable efforts to ensure the child's attendance at school. The prevalence of SRA is estimated between 1.2% and 4.8% in the general population, and up to 8% in clinical populations. SRA is a complex disorder, whose etiology is primarily rooted in anxiety, including separation anxiety in children and social anxiety disorder in adolescents. The consequences for schooling and socialization are significant and can lead to school dropout. Students are reintegrated into school in 50%–70% of cases. Psychiatric disorders and social adjustment difficulties often persist. Early management is essential and must be multidisciplinary, involving mental health and socio-educational professionals.
