Material and methods. 241 females with exogenous-constitutional obesity and 45 healthy females were examined. The patients of the main (123 persons) and control (118 persons) groups were under observation during 8 months. The body mass index, the leptin hormone level, and the duration of blink reflex were assessed on monitoring. Reflexotherapy, diet therapy and physiotherapy were applied in the main group. Statistical processing was performed by nonparametric methods. Results. Variants of developing obesity recurrence were determined: 1) with primary change of psychophysiological reactivity; 2) with primary change of neurohumoral regulation. In the first variant, reflexotherapy was prescribed, and in the second — hypocaloric diet and physiotherapy. The obesity recurrence in the control group was registered in 85.6% of all observations, and in the main group — in 14.6% of observations (p < 0.01). Conclusions. The proposed method of preventive differentiated therapy based on monitoring of psychophysiological reactivity and neurohumoral regulation allowed decreasing the recurrence rate in females with exogenous-constitutional obesity.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":"68 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"74012309","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

An MRI morphometric study was carried out in 59 patients with clinical symptoms of СМ1 (29 with syringomyelia) with ectopia of the cerebellar tonsils of 3 mm or more, a tight cisterna magna, the length of Clivus and Supraocciput of 40 mm or more. The comparison group consisted of 60 patients with classical СМ1 (30 with syringomyelia), and the control group consisted of 30 healthy subjects. As a result of the study, the features of bone phenotype of the pathology in question were revealed, and their pathogenetic role was discussed. The main features of the bone phenotype of clinically symptomatic CM1 without short bones and without signs of secondary cerebellar ectopia, along with the large size of the clivus, supraocciput and the volume of the posterior cranial fossa, are a decrease in the extra-bone part of the posterior cranial fossa, a sharpening of the angle of the bottom, or «funnel» of the posterior cranial fossa, narrowing of the anterior cerebrospinal spaces of the foramen magnum and a decrease in the anteroposterior diameter of the spinal canal at C2 and C6 levels.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":"102 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"76891298","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Material and methods. The study included 96 patients with new onset myocardial infarction who underwent coronary revascularization procedures. Serum angiotensin II levels were measured on days 30–40. Patients were divided into groups according to the value of this biomarker. Echocardiography was performed on all patients included in the study on days 30–40 after myocardial infarction; the risk of repeat cardiovascular events was assessed in a 12-month follow-up. Results. Against the background of routine treatment (including angiotensin-converting enzyme inhibitors) early after infarction, according to the recommendations of the Ministry of Health of the Russian Federation, 10 patients had the elevated angiotensin II level. Significant differences in LV wall thickness and myocardial mass were observed in patients with elevated angiotensin II levels; elevated angiotensin II at 30 to 40 days was not associated with the risk of repeat cardiovascular events in a 12-month follow-up. Conclusions. Elevated angiotensin II levels are associated with increased left ventricular wall thickness but do not affect the risk of repeat cardiovascular events.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":"117 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78197426","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

They are formed as a result of systemic inflammation, which is characterized by hyperproduction of adipokines (resistin) and indicators of systemic inflammation (TNF-α, IL-4, IL-6, IL-18). Among such patients, a special category consists of individuals with polymorphisms of candidate genes that determine bone mineral density. Accordingly, the combination of the above-mentioned modifiable and unmodifiable risk factors leads to an increase in the incidence of fractures of both vertebrogenic and non-vertebrogenic localization, which enhances the medical and social significance of this pathology. 110 people were examined: 20 of them made up the control group, 45 were included in the COPD group and 45 in the group of COPD combined with metabolic syndrome (COPD + MS). In patients of the studied groups, a genetic study was conducted to identify polymorphism of the calcitonin receptor gene CALCR, to determine indicators of systemic inflammation of tumor necrosis factor alpha (TNF-α), interleukin 4 (IL-4), interleukin-6 (IL-4), interleukin-18 (IL-18), markers of bone metabolism: beta- terminal C-telopeptide of collagen-1 (β-CTx), osteocalcin (OC), alkaline phosphatase (ALP), osteoprotegerin (OP), parathyroid hormone (PTH). In patients with COPD combined with metabolic syndrome, a high frequency of occurrence of pathological alleles Leu/Leu- calcitonin receptor gene (CALCR) was revealed. The levels of markers of systemic inflammation and bone metabolism were significantly higher in the group of patients with COPD+MS. Significant relationships were revealed between the indicators of FEV1, T-criterion, TNF-α, IL-6, VAS, β- STx, PTH and functional defective alleles of Leu/Leu in patients with COPD+MS.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":"55 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"76170826","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose to study the clinical, epidemiological and laboratory features of a food outbreak of salmonellosis. Material and methods. A retrospective analysis of 29 medical records of patients from a food outbreak of salmonellosis hospitalized in the Republican Clinical Infectious Diseases Hospital of Kazan in September 2021. Results. The likely transmission factors for Salmonellosis were poached egg and/or lightly salted salmon. Young females predominated (83%) among patients. The disease proceeded according to the gastroenteric variant (69%) of moderate severity (96.6%) with a favorable outcome (100%). The incidence of colitis syndrome was 31%. Among the biochemical parameters, high levels of CRP (161.97 (108.54) mg/l) attracted attention, reaching values characteristic of a severe bacterial infection. In bacteriological examination of faeces and/or vomit, Salmonella enteritidis was isolated from all patients. The isolated strains of S. enteritidis were sensitive to ampicillin, cefepime, ceftriaxone, chloramphenicol, salmonella bacteriophage, 80% of the isolates were resistant to pefloxacin and 40% to co-trimoxazole. Conclusions. A persistent increase in CRP can be used at the initial stage of differential diagnosis of etiologically different types of diarrheas to decide on the tactics of treating patients with infectious diarrhea. In order to maximize the effectiveness of antimicrobial treatment of salmonellosis, it is necessary to focus on regional antibiotic susceptibility profiles.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":"33 1","pages":""},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"73320801","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Material and methods. The article presents an overview of publications based on search results in the electronic resources PubMed, Elibrary, EMBASE and Google Scholar. The study included 247 women living in the Republic of Tatarstan: 86 with endometrial polyps (PE), 60 with endometrial cancer (RE), 101 patients without pathological changes in the endometrium. Clinical and laboratory examination, genotyping of the polymorphism rs11841589 of the KLF12 gene, pathomorphological examination of the endometrium were carried out. The biomaterial for genotyping the polymorphism rs11841589 of the KLF12 gene was DNA from whole blood (TestGen, Ulyanovsk). A set of reagents «Rapid-Genetics Test» by DNA-Technology LLC (Moscow, Russia) was used. The study was carried out within the framework of KSMU research No. 122082300042-2 in compliance with the principles and conditions of the Helsinki Declaration. Results. Endometrial polyps, due to their prevalence, high frequency of abnormal uterine bleeding and the risk of malignancy, represent one of the most serious gynecological problems worldwide. The mutant allele with the rs11841589 polymorphism of the KLF12 gene and the CC genotype, according to the study, do not significantly increase the risk of developing endometrial polyps and endometrial cancer in women living in the Republic of Tatarstan. It is proved that the prevalence of alleles and genotypes is comparable to the European population. Conclusion.
