The clinical picture in the debut was polymorphic, the patient had arthritis, skin lesions, general inflammation, myopathy, Raynaud’s syndrome, dysphagia, angio-neurotic edema. The immunological panel reflected the presence of antibodies characteristic of autoimmune diseases (antinuclear antibodies, antibodies to ribonucleoprotein, antimitochondrial antibodies, antibodies to cytoplasmic antigen SS-B). Glucocorticosteroids (GCS), immunosuppressive drugs (metatrexate, sulfasalazine, hydroxychloroquine, azathioprine) were used as starting therapy, but the disease progressed. Pulse therapy of GCS and plasmapheresis did not give a positive result. We used Rituximab, because the pathogenesis of the disease was characterized by the activation of B-cells. Currently, two infusions have been performed, positive dynamics has been noted. The research objective was to study the comorbid pathologies in patients with episodic migraine, characteristic for central sensitization — anxiety, depression, cervicalgia, cervicogenic headache, and sleep disorders. We found the relationship between the intensity of migraine pain syndrome, migraine duration in anamnesis, and the central sensitization index — CSI. At the same time, the CS intensity also directly depended on the number of involved muscles and triggers. That is why clinical manifestations of APS are varied and patients turn to various physicians. The article describes a clinical case of a critical lower limb ischemia in AP. A 62 y. o. female patient was under outpatient observation for two years with the clinic of intermittent claudication and a diagnosis of atherosclerosis of lower limb arteries. At the same time, a history of thrombosis of the splenic artery — atypical for atherosclerosis, laboratory phenomena: a false-positive Wassermann reaction and an elongation of the phospholipid-dependent test (APTT) indicated APS. The patient was admitted into Department of Vascular Surgery of the Republic Clinical Hospital with critical lower limb ischemia and a threat of amputation. Antiphospholipid antibodies were detected. Due to the therapy pathogenetic for APS, amputation of the lower extremity was avoided. Abdrakhmanova was an honored scientist, the author of more than 100 scientific papers and an outstanding doctor. She was a leading freelance pulmonologist of the Republic of Tatarstan and an excellent teacher and mentor. Professor Abdrakhmanova devoted her entire life to medicine. Most part of her labor activity was connected with the Republic Clinical Hospital. Material and methods. 782 patients with diabetic foot (DF) were under observation. 4 groups were identified: group 1 — VAC therapy was used; group 2 — ultrasonic cavitation; group 3 — modern dressings were used for treatment; group 4 — with a standard treatment. Low doses of statins were used (atorvastatin 10–20 mg/day or rosuvastatin 5–10 mg/day) in 213 patients (27.2%) and 89 patients (11.4%) received moderate or high doses of statins (atorvastatin 40–80 mg/day or rosuvastatin 20 mg/day) in combination with ursodeoxycholic acid (UDCA) preparations (750–1750 mg/day). Results. In the low-dose statin subgroup, the rate of low-density lipoprotein cholesterol levels reduction averaged 17.6 (5.6) % of the initial values; triglycerides, respectively, 12.3 (3.5) % of the initial values; that was statistically significantly lower than in the moderate/high dose statin plus UDCA subgroup of 32.9 (5.4) % and 18.7 (3.2) %, respectively, all p<0.001. In group 1, the average duration of hospital stay and the percentage of postoperative complications were statistically significantly lower in patients who received lipid-lowering therapy (28.5 (3.6) days and 8.8 (2.9) %), compared with those who did not receive this treatment (34.0 (2.8) days and 13.1 (2.6) %); p < 0.001. Similar differences in the length of hospital stay and the percentage of postoperative complications, depending on the lipid-lowering therapy, also occurred in groups 2, 3, and 4 (p < 0.001). Conclusions.
