The direct neurotropic impact of HIV creates the pathogenic basis for this clinical syndrome. The purpose — to evaluate the cognitive status and neuron-specific enolase (NSE) level in cerebrospinal fluid (CSF) in HIV-encephalopathy patients. Material and methods. 35 patients were included in the study. They were divided into two groups: 20 patients (9 females and 11 males) with cognitive dysfunction and confirmed diagnosis of HIV/AIDS and 15 patients (6 females and 9 males) were in comparison group. The cognitive status was investigated by mini-mental state examination (MMSE), international HIV-dementia scale (IHDS) and frontal assessment battery (FAB). CSF level of NSE were investigated by ELISA test. Results. Correlation between cognitive dysfunction severity and NSE level in CSF was determined in HIV-encephalopathy patients. Conclusion. NSE level changing in CSF is an objective indicator of clinical severity of HIV-encephalopathy and neurotropic impact of HIV.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"77862495","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The purpose — to investigate and evaluate the indicator significance of the functional potential of colonization defense in respiratory tract diseases.The paper presents the results of a study of colonization resistance indicators using modern research methods (colonization index, artificial colonization of the buccal epithelium and antiadhesive activity of saliva) in 184 patients aged 5 to 18 years with acute bronchitis, recurrent bronchitis and community-acquired pneumonia.Results. In a comparative aspect, the heterogeneity of the colonization level on buccal epitheliocytes of indigenous microflora (oral streptococci) was clearly traced, depending on the topic of the respiratory tract inflammatory lesion. Colonization resistance suffered the most in patients with community-acquired pneumonia. In patients of this group, the colonization index was within 0.89 (0.07) standard units (p < 0.01), which was combined with a pronounced adhesion of Candida albicans on buccal epitheliocytes (r = -0.72). In patients with acute bronchitis, the colonization index was 1.86 (0.21) standard units; with recurrent course of bronchitis – 1.27 (0.19) units (p < 0.01), while the indicators of the control group were 2.03 (0,18). The presence of less typical microorganisms in the oropharynx reflects the weakening of colonization resistance, signaling destabilization processes focused on the mucous membranes level.Conclusion. The deficiency of the mucosal protection functional reserve should be regarded as an unfavorable diagnostic marker of the course of a respiratory infection and the initiation of a bacterial inflammatory process in the respiratory system.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"78754214","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Results. Based on the study results, an algorithm for restorative operations on tendons after an injury with a profiled metal sheet was proposed. When the skin-tendon flap width-to-length ratio is 1:3 or more, one-stage reconstruction of the deep flexor tendon is possible. If the flap width-to-length ratio is less than 1:3, it is necessary to conduct a primary delayed (in 7–10 days) or staged reconstruction of the deep flexor tendon with skin restoration. Conclusion. The presented graphical model of «profiled metal sheet injury» allows giving a clinical assessment of the blood supply to the skin-tendon flap and differentiating treatment of patients with varying degrees of injury severity. In turn, the restoration of normal blood circulation in the injured segment accelerates the healing process, reduces the percentage of complications, and ultimately determines the success of the surgical intervention.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"79404491","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

The currently studied effects of ID therapy in patients with CAD and CHF are highlighted. Most of the published studies demonstrate the negative impact of iron deficiency on the structure and function of the heart, quality of life and prognosis of patients with myocardial infarction (MI) and CHF. The effectiveness of ID correction in patients has been studied extensively, nevertheless there are ongoing studies on the evaluation of efficacy of various parenteral iron forms, as well as on their influence on long-term prognosis in patients with CHF. The correction of iron deficiency reduces the damage and necrosis size in patients with MI, improves left ventricular systolic function and exercise capacity after MI. At the same time, the available data on the ID correction in patients with CHF after myocardial infarction are insufficient and require further study.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"86520906","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Often this is directly related to the effect of the virus on the epithelial cells of the gastrointestinal tract, as well as the prescription of drugs for the new coronavirus infection treatment. In some patients, gastrointestinal disturbances precede the onset of respiratory symptoms and remain for a long time after recovery, significantly reducing the quality of life. Approximately every second patient who had a COVID-19 infection suffers from stool disorders and dyspeptic symptoms. The most common variant of post-covid lesions of the gastrointestinal tract is functional dyspepsia and irritable bowel syndrome, which outpatient doctors have recently encountered more and more often. In most cases, the attending physician pays more attention to the pulmonary and cardiological manifestations of coronavirus infection, while the symptoms of gastrointestinal lesions are considered minor. The purpose of this article — is to consider possible methods for correcting the functional pathology of the gastrointestinal tract in patients after a COVID-19 infection.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"75809615","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}

Chronic instability of the acromioclavicular joint leads to changes in the orientation of the scapula, which provokes kinematic disorders leading to chronic pain. In case of acute dislocation of the acromial end of the clavicle, minimally invasive techniques are usually used, such as fixation with spokes or a hook-shaped plate, or arthroscopic fixation. Surgical techniques for the treatment of patients with acromioclavicular joint chronic symptomatic instability differ significantly from those used to treat patients with acute dislocation of the acromioclavicular joint. In this case, reconstruction of torn ligaments is necessary, because the biological potential of restoring these structures is exhausted after 3–4 weeks. The range of possibilities includes anatomical and non-anatomical techniques, open and minimally invasive procedures using arthroscopy, as well as biological and synthetic transplants. This article presents the authors’ approach to treating such patients and describes the modern reliable surgical technique.","PeriodicalId":9821,"journal":{"name":"中国实用医药","volume":null,"pages":null},"PeriodicalIF":0.0,"publicationDate":"2023-01-01","publicationTypes":"Journal Article","fieldsOfStudy":null,"isOpenAccess":false,"openAccessPdf":"","citationCount":null,"resultStr":null,"platform":"Semanticscholar","paperid":"83306783","PeriodicalName":null,"FirstCategoryId":null,"ListUrlMain":null,"RegionNum":0,"RegionCategory":"","ArticlePicture":[],"TitleCN":null,"AbstractTextCN":null,"PMCID":"","EPubDate":null,"PubModel":null,"JCR":null,"JCRName":null,"Score":null,"Total":0}
