Le test d’activation des basophiles (TAB) présente une spécificité élevée, utile lorsque les tests cutanés ou IgE spécifiques sont insuffisants et dans les cas discordants. Il réduit le recours au test de provocation orale dans les allergies alimentaires et médicamenteuses, et permet le suivi de l’immunothérapie spécifique. Le test d’activation des mastocytes (TAM), nécessitant une différenciation in vitro de mastocytes à partir de PBMC, montre des performances complémentaires, notamment en allergie médicamenteuse. Les hypersensibilités retardées sont explorées par des tests basés sur la prolifération ou l’activation lymphocytaire (TTL, TAL, ELISpot, cyto-LTT), en particulier dans les toxidermies sévères. L’absence de standardisation, le coût et la complexité technique de ces tests limitent leur diffusion. Cette revue de la littérature présente les performances diagnostiques, les indications validées ainsi que les avantages et limites de chaque approche.</div></div><div><h3>Abstract</h3><div>Cellular assays offer a functional assessment of immune mechanisms in patients suspected of type I (immediate) and type IV (delayed) hypersensitivity reactions. The Basophil Activation Test (BAT), based on flow cytometry, quantifies surface markers (CD63, CD203c) expression after allergen stimulation. With high specificity, BAT is useful when conventional tests (skin tests, specific IgE) are inconclusive, and it reduces the need for allergen challenge, particularly in food and drug allergies. BAT also supports monitoring of allergen-specific immunotherapy. The Mast Cell Activation Test (MAT), using patient serum-sensitized mast-cells, provides complementary data, especially in drug allergy. Delayed hypersensitivity is assessed by lymphocyte-based assays (LTT, LAT, ELISpot, cyto-LTT), particularly in severe cutaneous drug reactions. Limitations include lack of standardization, technical complexity, and limited accessibility. En 2050, les projections les plus citées considèrent que 50 % des enfants en seront atteints. Il s’agit d’un exemple typique de maladie multifactorielle dans laquelle un terrain génétique prédisposant, l’atopie, rencontre des modifications multiples de notre environnement liées au mode de vie occidental. Ceci induit un déséquilibre du microbiote intestinal, puis du compartiment des lymphocytes T régulateurs et in fine une réponse immunitaire de type 2 excessive, impliquant notamment les ILC2 et les lymphocytes Th2. Il en résultera la formation de plasmocytes sécrétant des IgE spécifiques de l’allergène, puis la sensibilisation des mastocytes et des polynucléaires basophiles. Dans un second temps, les polynucléaires éosinophiles seront recrutés à leur tour dans une réaction tardive complémentaire de type IVb. La compréhension des mécanismes physiopathologiques à l’origine de l’allergie a permis le développement de nouveaux biomarqueurs, ainsi que de nouvelles biothérapies, permettant désormais un phénotypage des patients et une prise en charge selon les principes de la médecine personnalisée.</div></div><div><h3>Abstract</h3><div>IgE-dependent allergy or type I hypersensitivity is a real societal problem, growing rapidly worldwide. By 2050, the most cited projections consider that 50 % of children will be affected. This is a typical example of a multifactorial disease in which a predisposing genetic background, atopy, encounters multiple changes in our environment linked to the Western lifestyle. This induces an imbalance of the intestinal microbiota, then of the regulatory T lymphocyte compartment and ultimately an excessive type 2 immune response, notably involving ILC2 and Th2 lymphocytes. This will result in the formation of plasma cells secreting allergen-specific IgE, then the sensitization of mast cells and basophils. In a second phase, eosinophils will be recruited in turn in a complementary late type IVb reaction.
