{"title":"[<i>Fu</i>'s subcutaneous needling based on anatomy train theory for nonspecific low back pain: a randomized controlled trial].","authors":"Shuang Liang, Kaiyu Huang, Xinxin Feng, Yongyi Xu, Xu Chen","doi":"10.13703/j.0255-2930.20240830-k0006","DOIUrl":"10.13703/j.0255-2930.20240830-k0006","url":null,"abstract":"<p><strong>Objective: </strong>To observe the clinical effect of <i>Fu</i>'s subcutaneous needling based on anatomy train theory for nonspecific low back pain (NLBP).</p><p><strong>Methods: </strong>A total of 120 patients with NLBP were randomized into an anatomy train Fu's subcutaneous needling group (40 cases, 3 cases dropped out), a conventional acupuncture group (40 cases, 2 cases dropped out) and a conventional Fu's subcutaneous needling group (40 cases, 2 cases dropped out). Acupuncture was applied at <i>ashi</i> points and bilateral Shenshu (BL23) and Dachangshu (BL25) in the conventional acupuncture group, once every other day, 3 times a week. <i>Fu</i>'s subcutaneous needling was applied at lumbodorsal myofascial trigger points (MTrPs) in the Fu's subcutaneous needling group, once every 3 days, twice a week. On the basis of the treatment in the Fu's subcutaneous needling group, <i>Fu</i>'s subcutaneous needling was applied at MTrPs along the posterior superficial line and lateral line in the anatomy train Fu's subcutaneous needling group, once every 3 days, twice a week. All groups were treated for 2 weeks. Before and after treatment, the scores of numeric rating scale (NRS) and Oswestry disability index (ODI) were observed, the distance of Schober test was measured and the endurance of trunk extensors was assessed in the 3 groups.</p><p><strong>Results: </strong>After treatment, in the 3 groups, the NRS and ODI scores were decreased compared with those before treatment (<i>P</i><0.05), the Schober test distance was increased compared with that before treatment (<i>P</i><0.05), the static and dynamic muscle endurance was increased compared with that before treatment (<i>P</i><0.05). Designated by the international organizations such as World Federation of Chinese Medicine Societies, World Health Organization, the phonetic translation method has been adopted for the standardization of acupuncture nomenclature. But this method neglects the cultural attributes of acupoint names. The liberal translation should be considered appropriately. English translation of acupoint name should be composed of two steps, intralingual translation (exegesis) and interlingual translation. During exegesis, the methods for discriminating phonetic loan character, selecting meanings and identifying character patterns should be sufficiently used. It refers to \"the diseases caused by the invasion of pathogenic factors into meridians\", and \"diseases of the affected meridians\", recorded in <i>jingmai</i> (<i>Meridian</i>) of <i>Lingshu</i> (<i>Miraculous Pivot</i>). The proposal of this term is related to the academic tendency of textbook writers and the influence of TCM policy in China. Through collating and analyzing the records of meridian disorders in ancient works, it is found that besides the classic \"meridian symptoms\", many <i>zangfu</i> disorders, the disorders along the running course of meridian based on meridian differentiation, collateral disorders and the disorders of the exterior-interior relationship of meridians should be classified as meridian disorder. {"title":"[Efficacy of heat-sensitive moxibustion combined with western medication for preconception intervention in prethrombotic state of recurrent spontaneous abortion with kidney deficiency and blood stasis].","authors":"Lili Sun, Zixue Sun, Pengchao Li","doi":"10.13703/j.0255-2930.20240617-0001","DOIUrl":"10.13703/j.0255-2930.20240617-0001","url":null,"abstract":"<p><strong>Objective: </strong>To observe the clinical efficacy of heat-sensitive moxibustion combined with western medication for preconception intervention in prethrombotic state of recurrent spontaneous abortion (RSA) with kidney deficiency and blood stasis.</p><p><strong>Methods: </strong>A total of 100 RSA patients of prethrombotic state with kidney deficiency and blood stasis were randomized into a combination group (50 cases, 5 cases were eliminated) and a medication group (50 cases, 5 cases were eliminated). In the medication group, the aspirin enteric-coated tablet was given orally at a dose of 75 mg a time, once daily. On the basis of the treatment in the medication group, in the combination group, heat-sensitive moxibustion was applied at the heat-sensitive points selected among the areas of Guanyuan (CV4), Shenque (CV8), and bilateral Sanyinjiao (SP6), Zusanli (ST36), Qihai (CV6), Taixi (KI3), Zigong (EX-CA1), Luanchao (Extra), Xuehai (SP10), and Yinlingquan (SP9), about 40 min a time, once every two days. Both groups were treated for 3 menstrual cycles continuously. Pregnancy success rate of 12 weeks was recorded in the two groups in follow-up of 3 months after treatment completion, during which conception was tried under the guidance of doctor. The TCM symptom score was observed and the coagulation-fibrinolysis indexes (activated partial thromboplastin time [APTT], prothrombin time [PT], platelet count [PLT], D-dimer [D-D], fibrinogen [FIB], protein S [PS], protein C [PC] and antithrombin Ⅲ [AT-Ⅲ]) were detected before and after treatment in the two groups.</p><p><strong>Results: </strong>The pregnancy success rate of 12 weeks was 80.0% (32/40) in the combination group, which was higher than 54.3% (19/35) in the medication group (<i>P</i><0.05). After treatment, the TCM symptom scores were decreased compared with those before treatment in the two groups (<i>P</i><0.05), and the TCM symptom score in the combination group was lower than that in the medication group (<i>P</i><0.05). Compared before treatment, the APTT and PT was prolonged (<i>P</i><0.05), the levels of PLT, FIB and D-D were reduced (<i>P</i><0.05), the activity of AT-Ⅲ, PS and PC was increased (<i>P</i><0.05) after treatment in the two groups. The main clinical manifestation was right eye deviation, which was diagnosed as liver and kidney deficiency and spleen and stomach <i>qi</i> deficiency. The treatment principles focused on harmonizing <i>qi</i> and blood, dispersing wind and unblocking orifices, and tonifying the liver and kidney. <i>Tuina</i> was applied at the head, face, and neck regions, including \"opening the heavenly gate\", \"pushing kan palace\", \"rubbing the forehead\" and point stimulation of Jingming (BL1), Cuanzhu (BL2), Yuyao (EX-HN4), Sizhukong (TE23), Tongziliao (GB1), as well as massaging Taiyang (EX-HN5), Qiuhou (EX-HN7), Quanliao (SI18), Chengqi (ST1), Sibai (ST2), Muchuang (GB16), Chengguang (BL6), Yifeng (TE17), and Yiming (EX-HN14). <i>Tuina</i> was also applied at upper limbs, including massaging Binao (LI14), Quchi (LI11), and the lower limbs, including pressing and plucking along the liver meridian. The treatment was combined with eye exercises with a \"\" character pattern. The <i>tuina</i> was administered daily during the first week; every other day from the second to fourth weeks; every four days in the second month; and once weekly in the third month. After 3 months of treatment, the patient's eye position returned to normal. In the modeling group, CUMS was performed to establish depression model. The successfully-modeled rats were randomized into a model group, a Tongdu Tiaoshen acupuncture group (referred to as the acupuncture group), and a fluoxetine group, with 12 rats in each group. In the acupuncture group, \"Baihui\" (GV20), \"Shenting\" (GV24), \"Shuigou\" (GV26) and \"Dazhui\" (GV14) were stimulated with acupuncture. This intervention measure was delivered once a day, continuously for 6 days; it was discontinued on day 7 and was completed in 28 days. In the fluoxetine group, intragastric administration was done with fluoxetine solution (2.1 mg/kg), once a day, and for 28 consecutive days. Before and after modeling, and after intervention completion, the body mass, sucrose preference rate and the total distance of movement and the boxes of horizontal crossing in the open field experiment were observed in each group. After intervention, using HE staining, the hippocampal neuron morphology was observed; using Nissl staining, the hippocampal Nissl body number was counted. The hippocampal mitochondria was observed under transmission electron microscopy. The average fluorescence intensity of ROS in hippocampal was determined using flow cytometry. With Western blot method, the protein expression of Endophilin A1, growth associated protein 43 (GAP-43), and brain-derived neurotrophic factor (BDNF) in hippocampal was detected; and with RT-qPCR method, the mRNA expression of Endophilin A1, GAP-43, and BDNF was recorded. {"title":"[Rules of acupoint selection and compatibility of acupuncture and moxibustion in treatment of chronic cough based on ancient and modern literature mining].","authors":"Xinyu Deng, Yilin Liu, Guixing Xu, Qi Li, Junqi Li, Si Huang, Ziwen Wang, Hangyu Li, Xi Chen, Fanrong Liang","doi":"10.13703/j.0255-2930.20240822-0006","DOIUrl":"10.13703/j.0255-2930.20240822-0006","url":null,"abstract":"<p><strong>Objective: </strong>To explore the rules of acupoint selection and compatibility of acupuncture and moxibustion in treatment of chronic cough using data mining.</p><p><strong>Methods: </strong>The ancient and modern medical record cloud platform, and the databases, i.e. CNKI, Wanfang, VIP, EMbase, Web of Science and PubMed, were searched to screen the ancient and modern literature on acupuncture and moxibustion treatment of chronic cough. The prescription database was established for acupuncture and moxibustion treatment of chronic cough, and the analysis conducted on the frequency and use percentage in the aspects of intervention measures, acupoint selection, acupoint distribution, meridian tropism, special points and acupoint combination, as well as the association rules and clustering rules of acupoint selection. The subgroup analysis was performed in accordance with the etiology of chronic cough and intervention measures.</p><p><strong>Results: </strong>A total of 106 articles were included and 158 prescriptions were extracted. The intervention measures were acupuncture, moxibustion, herbal medication and the combination of several measures. The high-frequency acupoints included Feishu (BL13), Zusanli (ST36), Dazhui (GV14), Pishu (BL20), Danzhong (CV17), Shenshu (BL23), Lieque (LU7), Dingchuan (EX-B1), Tiantu (CV22), and Fenglong (ST40). These acupoints are mainly distributed on the back, lumbar region, chest and abdomen. The involved meridians were bladder meridian of foot-<i>taiyang</i>, conception vessel, and lung meridian of hand-<i>taiyin</i>. The special points covered back-<i>shu</i> points, crossing points and five-<i>shu</i> point. Regarding the compatibility of acupoints, the combination of upper and lower points, and the combination of front and back points were predominant in treatment. The analysis of association rules found that the support of Feishu (BL13)→Zusanli (ST36) was the highest; the cluster analysis obtained 8 clusters of acupoints. The acupoint compatibility and overall rules were similar when cough variant asthma (CVA) or the mixed reasons were involved, and the local treatment approach was adopted if the etiology of disease was related to upper airway cough syndrome (UACS) and gastroesophageal reflux cough (GERC). The acupoint selection was similar among different intervention measures. When two kinds of measures were combined in treatment, Feishu (BL13), Pishu (BL20) and Zusanli (ST36) were the most common.</p><p><strong>Conclusion: </strong>In treatment with acupuncture and moxibustion for chronic cough, the acupoints are selected on the affected local area, depending on syndrome differentiation, and focusing on back-<i>shu</i> points. The main acupoints are Feishu (BL13), Zusanli (ST36), Dazhui (GV14), Pishu (BL20), Danzhong (CV17) and Shenshu (BL23). This device consists of five components: a moxa cylinder, an adjustable arm, a supporting tube, a temperature alarm, and a fixing strap. It allows infrared heat radiation from the moxa to pass through while blocking falling ash, thereby ensuring therapeutic efficacy and preventing burns. The device accommodates both perpendicular and oblique needling angles and adapts to various body positions, effectively avoiding deformation of the needle handle. This article introduces the academic features of this method in treating post-stroke dysphagia from four aspects: theoretical framework, treatment principles and point selection, mechanistic research, and clinical case studies. In clinical application, the method emphasizes syndrome differentiation based on meridians, harmonization between the conception and governor vessels; precise acupoint selection to treat both form and spirit; stage-specific differentiation with targeted needling.
