{"title":"[Construction of interpretable predictive model of acupuncture for methadone reduction in patients undergoing methadone maintenance treatment based on machine learning and SHAP].","authors":"Baochao Fan, Qiao Zhang, Chen Chen, Yiming Chen, Peiming Zhang, Liming Lu","doi":"10.13703/j.0255-2930.20250110-0002","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20250110-0002","url":null,"abstract":"<p><strong>Objective: </strong>To construct a predictive model for the reduction in methadone maintenance treatment (MMT) and evaluate the effects of different interventions and other clinical factors on methadone reduction using Shapley additive explanations (SHAP).</p><p><strong>Methods: </strong>Two clinical trials of acupuncture for methadone reduction in MMT patients were analyzed, and the baseline data, MMT related information, intervention measures, the data related to dose-reduction outcomes were collected. The predictive model was constructed by means of 6 machine learning algorithms including support vector machine (SVM), K-nearest neighbors (KNN), logistic regression (LR), Naive Bayes (NB), random forest (RF) and categorical-boosting (CatBoost), and 2 integration methods, blending-ensemble method (Blending) and Stacking-ensemble method (Stacking). SHAP was employed for the interpretability analysis of the optimal model.</p><p><strong>Results: </strong>A total of 251 MMT patients were included, 128 cases in the acupuncture group and 123 cases in the non-acupuncture group. CatBoost and Stacking performed optimally in the test set. CatBoost obtained an accuracy of 0.780 0±0.060 8, a precision of 0.500 0±0.120 0, a recall of 0.818 2±0.140 2, F1 score of 0.620 7±0.114 0, and receiver operating characteristic-area under curve (ROC-AUC) of 0.857 8±0.140 2 for the subjects. In MMT patients with acupuncture as an adjunctive therapy, the top 5 important features for methadone reduction, included intervention measures, body mass index (BMI), the duration of MMT, the history of opioid use and occupation; and SHAP values were 1.25, 0.36, 0.21, 0.19 and 0.12, respectively. Their concepts and theoretical properties had not been clarified in later generations. The unclear profiles of their connotation and meanings have affected the understanding of <i>sijie</i> (four pathways of <i>qi</i> located on the heat, chest, abdomen and leg) and <i>sihai</i> (four seas, including sea of marrow, sea of <i>qi</i>, sea of water and food, and sea of blood). Hence, it becomes a crucial point where the meridian theory and acupoint theory are not clearly identified. Based on <i>wuzang tongtian</i> (interrelationship between five-<i>zang</i> organs and nature) in <i>tianhui yijian</i> (<i>tianhui</i> medical bamboo slips), focusing on the association with meridian <i>biaoben</i>, the paper analyzes the formation and connotation of these concepts. Simultaneously, the ultrasound imaging was employed to record the muscle tissue responses of the participants. Amplitude and angular features were extracted from the movement data of operators, and muscle fascicle slope features were derived from the data of ultrasound images. The dynamic time warping barycenter averaging algorithm was adopted to align the dual-source data. Various machine learning techniques were applied to build quantitative models, and the performance of each model was compared. The most optimal model was further analyzed for its interpretability.</p><p><strong>Results: </strong>Among the quantitative models built for the two types of MAMs, the random forest model demonstrated the best performance. For the quantitative model of the \"lifting and thrusting\" technique, the coefficient of determination (<i>R</i><sup>2</sup>) was 0.825. For the \"twisting and rotating\" technique, <i>R</i><sup>2</sup> reached 0.872.</p><p><strong>Conclusion: </strong>Machine learning can be used to effectively develop the models and quantify the effects of MAMs on subcutaneous muscle tissue. In the acupuncture group, &lt;i&gt;Tiaowei Jiannao&lt;/i&gt; acupuncture was applied at bilateral Shenmai (BL62), Zhaohai (KI6), Hegu (LI4), Taichong (LR3), and Baihui (GV20), Sishencong (EX-HN1), Yintang (GV24&lt;sup&gt;+&lt;/sup&gt;), Shenting (GV24), once a day, 1-day interval was taken after 6-day treatment, for 3 weeks totally. In the medication group, eszopiclone tablet was given orally, 1-3 mg a time, once a day for 3 weeks. In the sham-acupuncture group, non-invasive sham acupuncture was applied, the acupoint selection, frequency and course of treatment were the same as the acupuncture group. Before treatment, after 2,3 weeks of treatment, the scores of Pittsburgh sleep quality index (PSQI), self-rating sleep scale (SRSS), National Institutes of Health Stroke scale (NIHSS), Hamilton depression scale-17 (HAMD-17) were observed; before and after treatment, the sleep parameters were recorded using polysomnography (PSG); and the efficacy and safety were evaluated after treatment in the 3 groups.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;After 2,3 weeks of treatment, the scores of PSQI, HAMD-17 and SRSS in the acupuncture group and the medication group, as well as the SRSS scores in the sham-acupuncture group were decreased compared with those before treatment (&lt;i&gt;P&lt;/i&gt;&lt;0.05); after 2 weeks of treatment, the NIHSS score in the acupuncture group was decreased compared with that before treatment (&lt;i&gt;P&lt;/i&gt;&lt;0.05); after 3 weeks of treatment, the NIHSS scores in the acupuncture group, the medication group and the sham-acupuncture group were decreased compared with those before treatment (&lt;i&gt;P&lt;/i&gt;&lt;0.05). This acupuncture device consists of a needle body and a handle. The diameter of the needle body ranges from 0.16 mm to 0.55 mm, and the length from 7 mm to 150 mm. The spiral spray technology is adopted to modify the micron-level insulating coat on stainless steel needle body. The needle holder is connected to the electroacupuncture device (conductive), the micro-film insulated needle body (non-conductive) and the membrane-free needle tip (conductive) can provide a precise electrical stimulation for different tissue layers of acupoints (such as deep nerves and fascia). The intradermal stimulation test, cytotoxicity test and hypersensitivity reaction test have showed a favorable biocompatibility, laying a solid and reliable safety for clinical application. Except the blank group, KOA model was prepared by the injection with papain. The suspension exercise therapy (10 min each time, three times daily), acupuncture (at \"Yanglingquan\" [GB34], \"Zusanli\" [ST36], and \"Dubi\" [ST35] on the right side, 30 min each intervention, once daily) and the combined therapy (the suspension exercise therapy combined with acupuncture) were delivered in the suspension group, the acupuncture group and the combined therapy group, respectively. The intervention of each group was performed continuously for 6 days, and 4 consecutive weeks, at the interval of 1 day. Before and after intervention, Lequesne MG score was assessed in the rats. After intervention, HE staining was adopted to observe the cartilaginous tissue morphology of the right knee joints, and Mankin score was evaluated; the serum levels of interleukin (IL)-1β, IL-6, and tumor neurosis factor-α (TNF-α) were measured using ELISA; the real-time PCR was provided to determine the mRNA expression of collagen protein type Ⅱ(COL2), collagen protein type Ⅹ (COL10), phosphatidylinositol 3-kinase (PI3K), protein kinase B (Akt), mammalian target of rapamycin (mTOR), and autophagy-regulated protein (Beclin-1) in the cartilaginous tissue of the right knee joint; Western blot was employed to detect the protein expression of PI3K, phosphorylated PI3K (p-PI3K), Akt, phosphorylated Akt (p-Akt), mTOR, phosphorylated mTOR (p-mTOR) and Beclin-1 in the cartilaginous tissue of the right knee joint.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Compared with the blank group, the rats in the model group showed the higher Lequesne MG score (&lt;i&gt;P&lt;/i&gt;&lt;0.01), thinner cartilage of the right knee, reduced chondrocytes and disordered arrangement, and higher Mankin score (&lt;i&gt;P&lt;/i&gt;&lt;0.01). This paper reviews the development status of intelligent acupuncture-moxibustion medical equipment and related new technologies, as well as the challenges faced. It is found that, with the advancement of technologies such as big data and artificial intelligence, acupuncture-moxibustion medical equipment has shown characteristics of greater precision, miniaturization, intelligence, and portability. However, deficiencies remain in areas such as standardization and regulation, including relatively low rates of effective transformation and a lack of innovation in research outcomes. Therefore, there is an urgent need to formulate corresponding strategies: improving the development of relevant standards for intelligent acupuncture-moxibustion medical equipment, encouraging the integration of medicine and engineering, cultivating interdisciplinary talents, and strengthening the protection of invention patents. {"title":"[<i>Bafa</i> in Tianhui medical bamboo slips].","authors":"Xiantong He, Changhua Liu, Feng Gao","doi":"10.13703/j.0255-2930.20241030-k0002","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20241030-k0002","url":null,"abstract":"<p><p>In 2012, a total of eight medical books were compiled from Tianhui medical bamboo slips unearthed in Chengdu, Sichuan, China, which for the first time, recorded <i>bafa</i> (<i>ba</i> technique) and acupuncture technique. <i>Bafa</i> is only found in Tianhui medical bamboo slips. Based on the records in medical bamboo slips, it is believed that the essential theory of <i>bafa</i> refers to an outlook of life for the harmony between form and spirit, meaning \"the heart mind is the monarch of form\". It is the initial view of physique centered on five-<i>zang</i> organs. In clinical application, <i>bafa</i> is used for prevention of diseases in four seasons, specially for <i>wubi</i> (five-<i>bi</i> disorders) and <i>wufeng</i> (five-wind disorders), covering a extensive parts of the body. Compared with the stone technique, \"<i>bafa</i> must work on meridian points, while stone on meridians\". <i>Bafa</i> is often used for deficiency syndrome, while stone technique is for excess syndrome. Hence, these two types of technique should be applied cautiously. An acquisition holder installed at the handle tail of filiform needle was designed to display the movement trajectory of the needle during acupuncture delivery by collecting the movement trajectory of holder. The 3-month old male Bama miniature pig was selected as the experimental subject, and 6 points, \"Bojian\" \"Qiangfeng\" \"Housanli\" \"Xiaokua\" \"Huiyang\" (BL35) and \"Baihui\" (GV20), were selected during acupuncture manipulation. The optical navigation system was used to collect the real-time data, and these data were per-processed and analyzed using mean filtering and Fourier transform. The acupuncture procedure was divided into 3 stages, inserting, lifting-thrusting, and twisting. The results showed that the accuracy was 96.3% at lifting-thrusting stage, and that was 100.0% at twisting stage. The decomposition effect of the entire procedure was satisfactory. This study provides a new approach to the quantitative analysis of acupuncture manipulation. The main symptom was repeated painful penile erections during nighttime sleep. The syndrome was differentiated as \"water failing to nourish wood\" and \"liver <i>qi</i> stagnation\". Acupuncture was applied at Baihui (GV20), Sishencong (EX-HN1), Guanyuan (CV4), and bilateral Neiguan (PC6), Hegu (LI4), Taichong (LR3), Taixi (KI3), Guilai (ST29), Yongquan (KI1), Yanglingquan (GB34), and Sanyinjiao (SP6). The treatment was combined with abdominal vibration therapy. Treatment was given once daily, five times per week. After the first treatment, the severity and duration of nocturnal erection pain were reduced, and sleep quality was improved.
