Significant breakthroughs have been made in the development of related devices such as transcutaneous electrical acupoint stimulation (TEAS) devices, transcutaneous electrical nerve stimulation (TENS) devices, and transcutaneous auricular vagus nerve stimulation (taVNS) devices in recent years. Although the market for these devices is vast, there are still limitations that need to be optimized in terms of electrode materials and power supply methods, bulky instrument size, cumbersome wiring, restricted applications, and inadequate intelligent functionality. The paper reports one case of child with stiff skin syndrome and treated with combined therapy of acupuncture and cupping. Acupuncture was used at the lateral line 1 of vertex (MS8) on the right side, Jiaji (EX-B2) of L<sub>2</sub> to L<sub>4</sub>, Huantiao (GB30), <i>ashi</i> point, Juliao (GB29), Fengshi (GB31), Weizhong (BL40), and etc. on the left side. After <i>deqi</i>, the electrodes of KWD-808Ⅰimpulse electronic therapeutic device were attached to Jiaji (EX-B2) of L<sub>4</sub> and Huantiao (GB30), Fengshi (GB31) and Yanglingquan (GB34) on the left side respectively, at disperse-dense wave, a frequency of 2 Hz/100 Hz, and a current of 2 mA. The needles were retained for 20 min. Acupuncture was operated once every 2 days, 3 interventions a week. When acupuncture was completed in each intervention, moving cupping was followed till the skin turned to be red, along the distribution of the governor vessel, foot-<i>shaoyang</i> gallbladder meridian and foot-<i>taiyang</i> bladder meridian on the left side, of the lumbar region and leg. Moving cupping was delivered once every 2 days, 3 times a week. Once a week, after moving cupping, the cups were retained on the areas with skin stiffness for 8 min to 10 min. One course of the combined therapy of acupuncture and cupping was composed of 6 treatments. After 2 courses of treatment, the skin stiffness on the left buttock region and the lateral side of the lower limb was ameliorated, the swelling on the left lower limb relieved and the walking improved; and the patient could walk continuously for 2 000 m. It integrates the cyclic schema of the heavenly cycling, the meridian growth and decline schema of the <i>guaqi</i> theory, the twelve zodiac demarcation schema from astrological and astronomical studies, and the nine-palace and eight-trigrams schema of temporal and spatial locations. It presents the theoretical origins of the harmony between nature and humanity in acupuncture and moxibustion along with medical thoughts under the background of social concepts. The numerical arts of acupoint number and acupuncture-moxibustion quantity, those of acupuncture-moxibustion time and taboos, as well as the time-based acupuncture-moxibustion patterns, all of these thoroughly reflect the application of the body schema of acupuncture and moxibustion by early medical practitioners in ancient time. {"title":"[Research and development of an intelligent moxibustion instrument based on electromyography].","authors":"Xin Peng, Tianyi Zhang, Dongying Wang, Xuelian Gu, Zihao Yu","doi":"10.13703/j.0255-2930.20241128-0006","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20241128-0006","url":null,"abstract":"<p><strong>Objective: </strong>An intelligent moxibustion instrument based on electromyography was designed to evaluate the real-time therapeutic effect of moxibustion.</p><p><strong>Methods: </strong>Taking Shenshu (BL23) as the subject, surface electromyography (sEMG) at the center and equidistant points of Shenshu (BL23) were collected. The characteristic parameters, integrated electromyography (iEMG) and root mean square (RMS) were calculated before and after moxibustion. After analyzing the effect of moxibustion, a function algorithm for the end-of-moxibustion was obtained. Using this algorithm and combined with STM32 technology, the control system of moxibustion instrument and the upper computer software were designed to achieve the precise control during moxibustion delivery. Finally, the function, stability and safety of the moxibustion instrument were verified through clinical trials to ensure its effectiveness in practical application.</p><p><strong>Results: </strong>During one cycle of moxibustion at the center of Shenshu (BL23), the iEMG of sEMG decreased over time, meaning the decrease in muscle fatigue degree, and after one cycle of moxibustion, it elevated over time, showing the increase in muscle fatigue degree. RMS increased by 1.90% before and after moxibustion at the equidistant points of Shenshu (BL23), and the system indicated the end of moxibustion when RMS increased by 0.15%, and decreased by 0.13% at the center of Shenshu (BL23). The intelligent moxibustion instrument designed based on this algorithm can realize the function of mild moxibustion, and the effect of moxibustion can be evaluated by the real-time monitoring of RMS changes through the upper computer. During the operation of moxibustion instrument, moxa stick was fixed stably, remained a safe distance of 3 cm to 4 cm away from the skin surface. {"title":"[Research status and frontier trends of acupuncture and moxibustion for gastroesophageal reflux disease: a CiteSpace visual analysis].","authors":"Jing He, Rensong Ye, Mengdie Wu, Zhihai Hu, Guizhi Ma, Huangan Wu, Yeqing Dong, Aijia Zhang, Jing Li","doi":"10.13703/j.0255-2930.20240505-k0002","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20240505-k0002","url":null,"abstract":"<p><strong>Objective: </strong>To explore the research history, hotspots and development trends of acupuncture and moxibustion in the treatment of gastroesophageal reflux disease(GERD)based on knowledge graph technology, and to provide references for clinical and basic research in this field.</p><p><strong>Methods: </strong>The literature of acupuncture and moxibustion for gastroesophageal reflux disease was searched from the CNKI, Wanfang, VIP and SinoMed, from the establishment of the databases to December 31th, 2023. CiteSpace 6.2.R6 Advance was used to draw the knowledge graph of authors, institutions, keywords and other elements, and then perform the visual analysis.</p><p><strong>Results: </strong>A total of 341 articles were included, with the number of publications showing an upward trend and the research types continually diversifying. A total of 832 authors and 308 institutions were analyzed, with <i>XIE Sheng</i> from the First Affiliated Hospital of Guangxi University of CM and <i>BAI Xinghua</i> from the Beijing University of CM as representative figures, forming core research teams. However, there was a lack of close collaboration between institutions, and no significant cross-regional research networks had been formed. A total of 192 keywords were included, forming 8 cluster labels, which mainly included 4 categories:treatment methods, disease types, TCM syndrome types, and literature types. The burst analysis showed that the methods of acupuncture and moxibustion in the treatment of gastroesophageal reflux disease had gradually become more integrated, the treatment methods had transitioned from simple acupuncture therapy to combined therapies with proton pump inhibitors or TCM decoctions, the disease types had become more refined, the focus of mechanism research had shifted from lower esophageal sphincter pressure and esophageal motility to changes in gastrointestinal hormone levels, and the research hotspots had gradually shifted from improving clinical symptoms to considering both mental and psychological states. {"title":"[Effect of electroacupuncture on intestinal function after gastric cancer surgery].","authors":"Junjie Guan, Miaomiao Ge, Yuling Cai, Ting Wang, Zhiwei Jiang, Jianhua Sun, Gang Wang","doi":"10.13703/j.0255-2930.20240618-k0003","DOIUrl":"10.13703/j.0255-2930.20240618-k0003","url":null,"abstract":"<p><strong>Objective: </strong>To observe the effect of electroacupuncture combined with enhanced recovery after surgery (ERAS) protocol on promoting intestinal function in patients after gastric cancer surgery.</p><p><strong>Methods: </strong>Forty-four patients who underwent radical gastrectomy for gastric cancer were randomly divided into an experimental group (22 cases, 3 cases were excluded) and a control group (22 cases, 4 cases were excluded). Both groups received treatment under ERAS protocol, the experimental group was given electroacupuncture at bilateral Neiguan (PC6), Hegu (LI4), Zusanli (ST36) and Quchi (LI11), disperse-dense wave was selected, with frequency of 2 Hz/100 Hz. The control group received placebo electroacupuncture intervention, with the same acupoints as the experimental group, electrode pads were placed on the acupoints without electrical stimulation. Each session lasted 30 min, starting from 1 h after surgery, once every 24 h, until the patient resumed anal flatus. The intestinal sound rate of both groups was observed 24 h before surgery and 24, 48 h after surgery. The bowel sound recovery time (BSRT), time to first anal flatus, time to first defecation, and tolerance to oral enteral nutrition suspension were compared between the two groups. The levels of serum C-reactive protein (CRP), interleukin (IL)-2, IL-4, IL-6, IL-10, IL-12, IL-17, tumor necrosis factor-α (TNF-α) and interferon-γ (IFN-γ) were measured 24 h before surgery and 24 h after surgery in both groups.</p><p><strong>Results: </strong>The intestinal sound rate 24 h after surgery was decreased compared with that 24 h before surgery in the two groups (<i>P</i><0.05), the intestinal sound rate 24, 48 h after surgery in the experimental group was higher than that in the control group (<i>P</i><0.05). {"title":"[Acupuncture at <i>yinsanzhen</i> combined with auricular point sticking in the treatment of primary dysmenorrhea: a randomized controlled trial].","authors":"Chunxia Lu, Lin Feng, Hong Luo, Jin Cui, Bo Chen","doi":"10.13703/j.0255-2930.20241205-k0001","DOIUrl":"10.13703/j.0255-2930.20241205-k0001","url":null,"abstract":"<p><strong>Objective: </strong>To observe the clinical effect of acupuncture at <i>yinsanzhen</i> combined with auricular point sticking on primary dysmenorrhea (PDM).</p><p><strong>Methods: </strong>Sixty patients with PDM were randomly divided into an observation group and a control group, with 30 cases in each group. Patients in the observation group were treated with acupuncture at <i>yinsanzhen</i> combined with auricular point sticking. The acupuncture was given at <i>yinsanzhen</i> (Guanyuan [CV4] and bilateral Guilai [ST29], Sanyinjiao [SP6]) once daily for 5 consecutive days. Auricular point sticking was applied to gan (CO<sub>12</sub>), shen (CO<sub>10</sub>), neifenmi (CO<sub>18</sub>), etc. every other day, alternated between ears, totaling 3 sessions. All treatments were started 5 days before menstruation. Patients in the control group were treated with ibuprofen sustained-release capsules on the first day of menstruation for 3 consecutive days. Both groups were treated for 3 menstrual cycles. The scores of Cox menstrual symptom scale (CMSS) and visual analogue scale (VAS) were compared between the two groups before and after treatment and at the second menstrual cycle after treatment completion (follow-up). The serum contents of prostaglandin (PG) F2α and PGE2 were detected before and after treatment, and the clinical effect and safety of the two groups were evaluated.</p><p><strong>Results: </strong>After treatment and during follow-up, the CMSS severity and duration scores and VAS scores of the two groups were lower than those before treatment (<i>P</i><0.05 ), and the scores in the observation group were lower than those in the control group (<i>P</i><0.05). After treatment, the serum contents of PGF2α were decreased, and the contents of PGE2 were increased (<i>P</i><0.05) in the two groups. The total effective rate of the observation group was 93.3% (28/30), which was higher than 80.0% (24/30) of the control group (<i>P</i><0.05). {"title":"[Clinical observation on acupoint thread-embedding for overweight and obesity in young adults with phlegm-dampness constitution].","authors":"Yuxia Ma, Lihua Zhao, Xin'ge Huang, Yu Huang, Dingjian Huang","doi":"10.13703/j.0255-2930.20240610-k0004","DOIUrl":"10.13703/j.0255-2930.20240610-k0004","url":null,"abstract":"<p><strong>Objective: </strong>To observe the clinical efficacy of acupoint thread-embedding for overweight and obese young adults with phlegm-dampness constitution.</p><p><strong>Methods: </strong>A total of 66 young adults with overweight and obesity of phlegm-dampness constitution were randomly divided into an observation group (33 cases, 2 cases dropped out, 1 case was discontinued) and a control group (33 cases, 3 cases dropped out). Health education guidance was adopted in the control group. On the basis of the intervention in the control group, acupoint thread-embedding was applied in the observation group, two groups of acupoints were used alternately, i.e. bilateral Tianshu (ST25), Daheng (SP15), Daimai (GB26), Shuidao (ST28), Huaroumen (ST24), Fenglong (ST40) and bilateral Feishu (BL13), Geshu (BL17), Weiwanxiashu (EX-B3), Pishu (BL20), Shenshu (BL23), Dachangshu (BL25), once a week. Treatment of 8 weeks was required in the two groups. Before treatment, after 4 weeks and 8 weeks of treatment, the body weight, body fat rate, waist circumference and hip circumference were measured, and the body mass index (BMI) was calculated in the two groups; before and after treatment, the phlegm-dampness constitution score and the heart rate variability (HRV) related indexes (total power [TP], low frequency [LF], high frequency [HF], LF/HF, standard deviation of the normal to normal inter beat intervals [SDNN], root mean square of the successive differences [RMSSD], percentage of normal RR intervals with duration [PNN50]) were observed, and the clinical efficacy and constitution improvement rate were compared in the two groups. Six months after the treatment completion, the body weight of patients with effective clinical efficacy was observed, and the body weight rebound rate was compared in the two groups.</p><p><strong>Results: </strong>After 4 weeks of treatment, the body weight, BMI, body fat rate, waist circumference and hip circumference were decreased compared with those before treatment in the two groups (<i>P</i><0.05); after 8 weeks of treatment, the body weight, BMI, body fat rate, waist circumference and hip circumference were decreased compared with those before treatment and after 4 weeks of treatment in the observation group (<i>P</i><0.05), and the body weight was decreased compared with that before treatment in the control group (<i>P</i><0.05). After 8 weeks of treatment, the body weight, BMI, waist circumference, hip circumference and phlegm-dampness constitution score in the observation group were lower than those in the control group (<i>P</i><0.05). {"title":"[CT layered localization and clinical effect of acupuncture on lumbar disc herniation].","authors":"Yong Yang, Li Zhang, Shoufang Liu, Youlong Zhou, Quanliang Wang, Jian Liu","doi":"10.13703/j.0255-2930.20240605-0002","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20240605-0002","url":null,"abstract":"<p><strong>Objective: </strong>To explore the relationship between the effect of acupuncture and layered localization of computed tomography (CT) in treatment of lumbar disc herniation.</p><p><strong>Methods: </strong>Based on the CT layered localization, the herniated lumbar discs were positioned in 5 layers, A, B, C, D and E among 300 patients with lumbar disc herniation. Combined with the horizontal and the frontal planes, the three-dimensional location was formed. Acupuncture was delivered at acupoints including bilateral Shenshu (BL23), Dachangshu (BL25), and Huantiao (GB30), Weizhong (BL40) on the affected side. One intervention of acupuncture was 30 min, once daily; 1 course of treatment was composed of 10 interventions and 2 courses were required. Before and after treatment, Japanese orthopaedic association (JOA) score was recorded, and the effect was evaluated. The curative effect was classified and compared with the CT layered localization.</p><p><strong>Results: </strong>Of 300 patients, 226 cases were effective and the effective rate was 75.33%. The JOA scores of all patients, and in the effective group and the non-effective group were higher compared with the scores before treatment (<i>P</i><0.05). With the layered localization considered, acupuncture was more effective on the cases positioned in C layer. Regarding the horizontal plane, the effect was better on the cases with zone 1 and zone 1-2 involved. The CHF model was established in the modeling group by ligating the left anterior descending coronary artery. After successful modeling, the rats were randomly divided into a model group (<i>n</i>=9), a moxibustion group (<i>n</i>=8), a rapamycin (RAPA) group (<i>n</i>=9), and a moxibustion+RAPA group (<i>n</i>=9). In the moxibustion group, moxibustion was delivered at bilateral \"Feishu\"(BL13) and \"Xinshu\" (BL15), 15 min at each point in each intervention, once daily, for 4 consecutive weeks. In the RAPA group, RAPA solution was administered intraperitoneally at a dose of 1 mg/kg, once daily for 4 consecutive weeks. In the moxibustion+RAPA group, RAPA solution was administered intraperitoneally after moxibustion. Ejection fraction (EF) and left ventricular fractional shortening (FS) were measured after modeling and intervention. After intervention, morphology of cardiac muscle was observed using HE staining and Masson's trichrome staining. Total iron content in myocardial tissue was detected using a colorimetric method. Western blot and qPCR were adopted to detect the protein and mRNA expression of TfR1, FSP1, ANP, and CollagenⅠ in myocardial tissue.</p><p><strong>Results: </strong>Compared with the normal group, the EF and FS values decreased (<i>P</i><0.01); necrosis, edema, degeneration, and arrangement disorder were presented in cardiomyocytes; inflammatory cells were obviously infiltrated, the structure of myocardial fibers was disarranged, the collagen fibers were obviously deposited and fibrosis increased (<i>P</i><0.01); the total iron content and the protein and mRNA expression of TfR1, ANP, and CollagenⅠ in myocardial tissue were elevated (<i>P</i><0.01), while the protein and mRNA expression of FSP1 were reduced (<i>P</i><0.01) in the model group. Compared with the model group, the moxibustion group showed that EF and FS increased (<i>P</i><0.01); myocardial cell morphology was improved, and myocardial fibrosis was alleviated (<i>P</i><0.01); the total iron content and the protein and mRNA expression of TfR1, ANP, and CollagenⅠ in myocardial tissue decreased (<i>P</i><0.01), while the protein and mRNA expression of FSP1 increased (<i>P</i><0.01, <i>P</i><0.05).
