The conventional obstetrical nursing was given in the control group. On the basis of the intervention in the control group, acupuncture was applied at bilateral Hegu (LI4), Taichong (LR3) and Sanyinjiao (SP6) in the acupuncture group. The delivery mode and labor stage, the scores of visual analogue scale (VAS) for uterine contraction pain and Hamilton anxiety scale (HAMA) before and after acupuncture, the intrapartum/postpartum blood loss and massive hemorrhage, as well as the neonatal Apgar score after 1, 5, and 10 min of birth, were compared in the two groups.</p><p><strong>Results: </strong>The cesarean section rate was 4.1% (2/49) in the acupuncture group, which was superior to 10.0% (5/50) in the control group (<i>P</i><0.05). In the acupuncture group, the time of latent phase of 2-cm cervical dilation, active phase, first and second stages of labor, and total labor stage was shorter than that in the control group (<i>P</i><0.001), the intrapartum blood loss and massive hemorrhage rate were lower than those in the control group (<i>P</i><0.001, <i>P</i><0.05). After acupuncture, the VAS and HAMA scores were decreased compared with those before acupuncture in the acupuncture group (<i>P</i><0.001), the VAS and HAMA scores were increased compared with those before acupuncture in the control group (<i>P</i><0.001). In the acupuncture group, the VAS and HAMA scores after acupuncture were lower than those in the control group (<i>P</i><0.001), the changes of the VAS and HAMA scores before and after acupuncture were larger than those in the control group (<i>P</i><0.001). {"title":"[Development and research of an AI-assisted decision-making platform in treatment of insomnia with acupuncture of <i>Tongdu Yangxin</i> acupoint prescription].","authors":"Chi Wang, Chengyong Liu, Xiaoqiu Wang, Enqi Liu, Juguang Sun, Jin Lu, Min Ding, Wenzhong Wu","doi":"10.13703/j.0255-2930.20241122-k0003","DOIUrl":"https://doi.org/10.13703/j.0255-2930.20241122-k0003","url":null,"abstract":"<p><strong>Objective: </strong>To construct and validate a predictive model for the therapeutic effect of acupuncture at <i>Tongdu Yangxin</i> prescription (acupoint prescription for promoting the circulation of the governor vessel and nourishing the heart) on insomnia, so as to develop an open-access interactive artificial intelligence (AI)-assisted decision-making platform.</p><p><strong>Methods: </strong>Clinical data of 139 insomnia patients treated with <i>Tongdu Yangxin</i> acupuncture therapy were included. All the patients had received acupuncture at Baihui (GV20), Yintang (GV24<sup>+</sup>), bilateral Shenmen (HT7), and bilateral Sanyinjiao (SP6); and electric stimulation was attached to Baihui (GV20) and Yintang (GV24<sup>+</sup>), using a continuous wave and a frequency of 2 Hz. The treatment was delivered once every other day, 3 treatments a week, and for 2 consecutive weeks. Patients with Pittsburgh sleep quality index (PSQI) score reduction rate <50% were classified as the \"no response group\", and those with ≥50% were as the \"response group\". Outliers were addressed using the 1.5×IQR rule, and missing values were imputed via predictive mean matching. Key features were selected by intersecting the feature importance results from eXtreme Gradient Boosting (XGBoost) and random forest algorithms. After balancing class distribution using the Synthetic Minority Over-sampling Technique (SMOTE), 20% of the data was reserved as a validation set. The remained data underwent the stratified sampling iterations to generate 200 pairs of 3∶1 training-test sets, which was employed for training and internal validation of 8 machine learning algorithms. The optimal algorithm and data partitioning strategy were selected to construct the final model, followed by external validation. The best-performing model was deployed online via Streamlit to create an interactive AI platform.</p><p><strong>Results: </strong>Key predictive features for model construction included insomnia duration, the total PSQI score, PSQI sleep efficiency subscore, the proportion of N1 and N2 sleep stages in total sleep duration, and the maximum pulse rate during sleep. The CatBoost-based model achieved an AUC of 0.92, the average precision of 0.77, and accuracy, average recall, and average F1-score of 0.75 on the test set. On the validation set, it attained an AUC of 0.84, with accuracy, average precision, average recall, and average F1-score all at 0.72, demonstrating robust predictive performance. An interactive AI platform was subsequently developed (https://tdyx-catboost.streamlit.app/).</p><p><strong>Conclusion: </strong>This study successfully establishes and validates a CatBoost-based efficacy prediction model for <i>Tongdu Yangxin</i> acupuncture therapy in treatment of insomnia. Through the studies on philology and the annotation by the doctors in the past dynasties, a part of point prescriptions for the contralateral bloodletting is investigated, and those not belonged to the category of this needling technique in <i>Suwen: Miuci Lun</i> (<i>On Contralateral Bloodletting</i>) are distinguished. It is concluded that the failure of consensus-making in classifying point prescriptions is due to the lack of understanding on the characteristics of contralateral bloodletting and the misunderstanding on the errors of sentences and words. It is suggested that the study on theoretic concepts should lay on the problems relevant with the generalization and alienation of concepts. The mode of \"combination of the detail with the whole\" should be adopted. Based on the classical theories of <i>Huangdi Neijing</i> (<i>the Yellow Emperor's Inner Classic</i>) such as the \"twelve sinew meridians\" \"short needling\" and \"<i>shu</i> needling\", Professor <i>WANG</i> applies a self-designed blade needle with crescent needling method. This approach forms a distinctive treatment philosophy characterized by \"sinew meridians as the foundation, selecting points at the pain site; using chicken-claw needling technique deliverd at both internal and external; integration of heaven, earth, and man, using both short and <i>shu</i> needling; treating tendon injuries and inflammation with the crescent technique of blade needle\". After releasing, using the moxibustion sensation localization method in the abdominal regions with high heat-sensitivity frequency (including acupoints such as Zhongwan [CV12], Tianshu [ST25], Qihai [CV6], Guanyuan [CV4]), 2 heat-sensitive acupoints were selected for moxibustion and moxibustion was applied at each acupoint for 40 min, or until the heat-sensitive moxibustion sensation disappeared. The patients were treated once every other day for a total of 20 times. The weight, body mass index (BMI), body fat rate, waist circumference and hip circumference of the patients before and after treatment were observed, and the clinical efficacy was evaluated after treatment.</p><p><strong>Results: </strong>After treatment, the weight, BMI, body fat rate, waist circumference and hip circumference of the patients were decreased compared with those before treatment (<i>P</i><0.05). Acupuncture was applied at Baihui (GV20), Yintang (GV24<sup>+</sup>), Sanyinjiao (SP6), Shenmen (HT7), Hegu (LI4), Taichong (LR3), once a day, continuous treatment for 4 weeks. Ginger moxibustion was applied at the bladder meridian of foot <i>taiyang</i> and governor vessel, once a day, continuous treatment for 4 weeks. In treatment of umbilical therapy, Chaihu (<i>Radix Bupleuri</i>), Yujin (<i>Radix Curcumae</i>), Rougui (<i>Cortex Cinnamomi</i>) were ground and mixed with the same amount of honey, put into the application, and the application was placed on the navel after filling the navel with fine salt, once a day for 4 weeks. Hamilton anxiety scale (HAMA) score, Hamilton depression scale (HAMD) score, urodynamic indexes (maximum urinary flow rate [Qmax], maximum detrusor pressure [Pdet-max], residual urine volume [RUV]), neurogenic bladder symptom score (NBSS), urinary symptom distress scale (USDS) score were compared before and after treatment, and the clinical efficacy was evaluated.</p><p><strong>Results: </strong>After treatment, the scores of HAMA, HAMD, NBSS, USDS and RUVwere lower than those before treatment (<i>P</i><0.05), and Qmax and Pdet-max were higher than those before treatment (<i>P</i><0.05). The patient presented with neck stiffness accompanied with limb tremor as the primary symptoms. The pattern was attributed to <i>yang qi</i> deficiency, and the treatment principle focused on unblocking the governor vessel, warming <i>yang</i>, and regulating <i>qi</i>. Acupuncture was applied to Dazhui (GV14), below the spinous processes of C<sub>4</sub>-C<sub>6</sub> and bilateral C<sub>4</sub>-C<sub>6</sub> Jiaji (EX-B2) points, Feishu (BL13), Xinshu (BL15), Pishu (BL20), Ganshu (BL18), Shenshu (BL23), Dachangshu (BL25). After achieving <i>deqi</i> sensation, bilateral C<sub>4</sub> Jiaji (EX-B2) and C<sub>6</sub> Jiaji (EX-B2) points were separately connected to an SDZ-Ⅴ electroacupuncture device, with continuous wave, 2 Hz of frequency, intensity should be within the tolerance of the patient, the needles were retained for 20 min. For the first 5 months, the treatment was 1-2 times a week, then reduced to 1-2 times a month thereafter. After 30 times of treatment, the patient's bilateral lower limb tremor resolved, the clinical rating scale for tremor (CRST) score was 13 points, the scoliosis showed improvement with about 10° reduction in Cobb angle. In the modeling group, AR model was prepared using systemic and local attack sensitization method with ovalbumin. The successfully-modeled rats were randomized into a model group (6 rats), a medication group (6 rats) and a moxibustion group (14 rats). In the moxibustion group, the suspending moxibustion was operated at bilateral \"Feishu\" (BL13), 40 min each time, once daily, for 21 consecutive days; during which, the temperature of the body and tail was recorded. During intervention, if the temperature of the body and tail increased by >1 ℃, the heat-sensitive reaction at the point was determined in the rats of the moxibustion group, and these rats were collected in a heat-sensitive moxibustion group (8 rats involved and 6 rats of them were randomly collected to ensure the sample-size consistency); and those without heat-sensitive moxibustion reaction were assigned to a traditional moxibustion group (6 rats). In the medication group, fluticasone propionate nasal spray was applied, 8 μL on each side, once daily and for 21 days. The behavioral score for AR symptoms after modeling and intervention, and the content of serum immunoglobulin E (IgE) after modeling were observed. After intervention, the histological morphology of the nasal mucosa was observed using HE staining, the positive expression of thymic stromal lymphopoietin (TSLP) in the nasal mucosa was detected using immunohistochemistry, the levels of IgE, interleukin (IL)-4, IL-5, IL-13 and interferon-γ (IFN-γ) were detected by ELISA, and the protein expression of the member 4 of tumor necrosis factor receptor superfamily (OX40), phosphorylated protein kinase B (p-AKT), phosphorylated phosphatidylinositol 3-kinase (p-PI3K) in nasal mucosa was detected by Western blotting.</p><p><strong>Results: </strong>After modeling, the behavioral score of AR symptoms and serum IgE level in the modeling group were higher than those of the blank group (<i>P</i><0.01), suggesting the success of AR modeling. After intervention, compared with the blank group, the behavioral score of AR symptoms was increased (<i>P</i><0.01);the nasal mucosa structure was disordered, the inflammatory infiltration was severe; the positive expression of TSLP in the nasal mucosa increased (<i>P</i><0.01), the levels of serum IgE, IL-4, IL-5, and IL-13 elevated (<i>P</i><0.01), and the level of IFN-γ decreased (<i>P</i><0.01); and the protein expression of OX40, p-AKT, and p-PI3K in the nasal mucosa increased (<i>P</i><0.05) in the model group. Inspired by this theory, the authors analyze factors affecting moxibustion sensation-including moxibustion temperature, location, dose, and technique-and summarize its guiding role in clinical practice. It is proposed that in clinical application, the generation pattern of the initial sensation phase can be used to judge the immediate efficacy of moxibustion and adjust treatment locations accordingly; the diffusion pattern of the diffusion phase can help assess individual moxibustion dose to ensure optimal therapeutic effects; and the conduction phase not only indicates the endpoint of moxibustion therapy but can also reveal hidden lesions and deeper pathological causes in patients. Both groups received exercise and cognitive training. In addition, the experimental group underwent acupuncture treatment at acupoints including Baihui (GV20), Sishencong (EX-HN1), and bilateral Fengchi (GB20), Xuanzhong (GB39), Zusanli (ST36), Yanglao (SI6), Xinshu (BL15), and etc., once every other day, three times per week, for a total of 12 weeks. Before and after treatment, the Montreal cognitive assessment (MoCA) and mini-mental state examination (MMSE) scores were evaluated in the two groups, changes in gut microbiota were detected using 16S rDNA sequencing technology. The clinical efficacy was assessed after treatment.</p><p><strong>Results: </strong>Compared before treatment, MoCA and MMSE scores were increased in both groups after treatment (<i>P</i><0.001), with higher scores in the experimental group than those in the control group (<i>P</i><0.001, <i>P</i><0.05). After treatment, the relative abundance of <i>Faecalibacterium</i>, <i>Clostridia</i>, and <i>Ruminococcaceae</i> was increased in the experimental group compared with that before treatment (<i>P</i><0.05). Moreover, the relative abundance of <i>Faecalibacterium</i> in the experimental group was higher than that in the control group (<i>P</i><0.05).
