{"title":"[Key questions of translational research on international standards of acupuncture-moxibustion techniques: an example from the WFAS <i>Technical Benchmark of Acupuncture and Moxibustion</i>: <i>General Rules for Drafting</i>].","authors":"Shuo Cui, Jingjing Wang, Zhongjie Chen, Jin Huo, Jing Hu, Ziwei Song, Yaping Liu, Wenqian Ma, Qi Gao, Zhongchao Wu","doi":"10.13703/j.0255-2930.20240723-k0003","DOIUrl":"10.13703/j.0255-2930.20240723-k0003","url":null,"abstract":"<p><strong>Objective: </strong>To provide the experience and demonstration for the transformation of acupuncture-moxibustion techniques standards from Chinese national standards to international standards.</p><p><strong>Methods: </strong>Questionnaire research, literature research, semi-structured interviews and expert consultation were used.</p><p><strong>Results: </strong>The safety of acupuncture-moxibustion techniques was evaluated through literature research, and based on the results of the questionnaire survey, expert interviews, and expert consultation, 11 main bodies and structure of the former Chinese national standard, <i>Technical Benchmark of Acupuncture and Moxibustion</i>: <i>General Rules for Drafting</i>, were adjusted and optimized in accordance with the requirements of international standard (including the language, normative references, purpose, scope, applicable environment, target population, work team, terms and definitions, general principles and basic requirements, structural elements and text structure, and compilation process); and the first international standard, World Federation of Acupuncture-Moxibustion Societis (WFAS) <i>Technical Benchmark of Acupuncture and Moxibustion</i>: <i>General Rules for Drafting</i> was formulated to specify the general rules for drafting.</p><p><strong>Conclusion: </strong>The 3 key questions, \"international compatibility\", \"technical operability\" and \"safety\" should be solved technically on the basis of explicit international requirements. {"title":"[Therapeutic effect of <i>Rendu Tongtiao</i> acupuncture on hyperandrogenism in polycystic ovary syndrome of kidney-<i>yin</i> deficiency induced fire hyperactivity].","authors":"Yuane Liu, Baidan Liao, Xian Zhang, Chang Zhou, Chen Chen","doi":"10.13703/j.0255-2930.20240612-0003","DOIUrl":"10.13703/j.0255-2930.20240612-0003","url":null,"abstract":"<p><strong>Objective: </strong>To investigate the clinical therapeutic effect of <i>Rendu Tongtiao</i> acupuncture (acupuncture for regulating and improving the circulation of the conception and governor vessels) on hyperandrogenism (HA) in polycystic ovary syndrome (PCOS) with kidney-<i>yin</i> deficiency induced fire hyperactivity.</p><p><strong>Methods: </strong>A total of 80 PCOS-HA patients were selected and randomly divided into an observation group and a control group, 40 cases in each group. In the control group, ethinylestradiol and cyproterone acetate tablets were administered orally,2 mg each time, once daily and for 21 consecutive days as one menstrual cycle. In the observation group, <i>Rendu Tongtiao</i> acupuncture was delivered at Qihai (CV6), Zhongwan (CV12), Guanyuan (CV4), Zhongji (CV3), Mingmen (GV4), Yaoyangguan (GV3), etc. once daily till ovulation, which was taken as the treatment session of one menstrual cycle. The treatment was completed after 3 menstrual cycles in each group. Before and after treatment, the serum levels of testosterone (T), dihydrotestosterone (DHT), luteinizing hormone (LH), follicle-stimulating hormone (FSH), prolactin (PRL), sex hormone-binding globulin (SHBG), and the scores of acne and hirsutism were compared in the two groups; besides, menstrual recovery rate, ovulation recovery rate, basic body temperature (BBT) biphasic rate and clinical effect were compared between the two groups.</p><p><strong>Results: </strong>Compared with those before treatment, the levels of T, DHT, LH and PRL, as well as the scores of acne and hirsutism were reduced in the two groups after treatment (<i>P</i><0.05), and the levels of FSH and SHBG were increased (<i>P</i><0.05). After treatment, the levels of T, DHT, LH and PRL, as well as the scores of acne and hirsutism in the observation group were lower than those in the control group (<i>P</i><0.05); and FSH and SHBG were higher (<i>P</i><0.05). After treatment, the menstrual recovery rate and ovulation recovery rate, as well as BBT biphasic rate in the observation group increased in comparison with the control group (<i>P</i><0.05). It is discovered that the optimization and substitution in diagnosis and treatment of acupuncture are involved in the development of acupuncture medical devices. The technology application of these devices are composed of traditional and emerging engineering technologies; and the theoretical guidance for their development requires the integration of traditional acupuncture principles with modern medical theories. The development of acupuncture medical devices highlights the characteristics of multidimensional integration, treatment for specific ailments, portability and wearability, painlessness and non-invasion, precision and personalization, as well as intelligent automation. Upon analysis, it is shown that in the development and product transformation of acupuncture medical devices in recent years, the theoretical principles of acupuncture of TCM have not been fully utilized yet, the transformation of patented product is low, the clinical evidence of product is insufficient, and the market competitiveness needs improvement. IBS-D model was prepared by the combined measures for the rest rats, including neonatal maternal separation, acetic acid enema and chronic restraint stress. Twenty-four successfully-modeled rats were randomized into a model group and a moxibustion group, 12 rats in each one. In the moxibustion group, suspending moxibustion was delivered at bilateral \"Tianshu\" (ST25) and \"Shangjuxu\" (ST37), 20 min each time, once daily and for 7 consecutive days. Separately, before acetic acid enema (aged 35 days), after modeling (aged 45 days) and after intervention (aged 53 days), the body mass, loose stool rate (LSR) and and the minimum volume threshold when abdominal withdrawal reflex (AWR) scored 3 were observed in the rats of each group. After intervention (aged 53 days), using HE and PAS staining, the morphology of duodenum was observed, the length of villus and the depth of crypt were measured, the ratio of the length of villus to the depth of crypt was calculated; and the numbers of mucosal intraepithelial lymphocytes (IELs) and goblet cells were counted. With ELISA adopted, the contents of γ-interferon (IFN-γ), interleukin-4 (IL-4) and secretory immunoglobulin A (sIgA) in duodenal mucosa of rats were detected. The proportion of T cell subsets in duodenal mucosa was detected using flow cytometry. The microvilli and tight junctions of duodenal mucosal epithelial cells were observed by transmission electron microscopy, and the integrity of duodenal mucosa observed by scanning electron microscopy.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Compared with the normal group, for the rats in the model group, the body mass, the minimum volume threshold when AWR scored 3, the length of duodenal villus and the the ratio of the length of villus to the depth of crypt, as well as the proportion of CD&lt;sub&gt;8&lt;/sub&gt;&lt;sup&gt;+&lt;/sup&gt; T subset were all reduced (&lt;i&gt;P&lt;/i&gt;&lt;0.01, &lt;i&gt;P&lt;/i&gt;&lt;0.05), the counts of goblet cells in duodenal mucosa decreased (&lt;i&gt;P&lt;/i&gt;&lt;0.01); LRS, the proportion of CD&lt;sub&gt;4&lt;/sub&gt;&lt;sup&gt;+&lt;/sup&gt; T subset and CD&lt;sub&gt;4&lt;/sub&gt;&lt;sup&gt;+&lt;/sup&gt;/CD&lt;sub&gt;8&lt;/sub&gt;&lt;sup&gt;+&lt;/sup&gt;, as well as the contents of IFN-γ, IL-4 and sIgA in duodenal mucosa and IFN-γ/IL-4 were all elevated (&lt;i&gt;P&lt;/i&gt;&lt;0.01); and the numbers of IELs rose (&lt;i&gt;P&lt;/i&gt;&lt;0.01). The morphology of duodenal mucosa was irregular, the villi got shorter, sparse and scattered, with uneven density. The morphology of epithelial cells was destroyed and the tight junctions damaged, with larger spaces. In the Daoqi acupuncture group, <i>Daoqi</i> acupuncture technique in <i>Huangdi Neijing</i> was applied at Shanglianquan (Extra), Fengfu (GV16), and bilateral Lieque (LU7), Zhaohai (KI6); in the conventional acupuncture group, conventional acupuncture was applied at Shanglianquan (Extra), Fengfu (GV16), Yamen (GV15), and bilateral Lieque (LU7), Zhaohai (KI6), Zusanli (ST36), Fenglong (ST40). The treatment was adopted once every other day, 3 times a week, 4 weeks as one course and 3 courses were required totally in both groups. Before and after treatment, the polysomnography (PSG) indexes [apnea-hypopnea index (AHI), hypopnea index (HI), apnea index (AI), longest apnea duration, lowest nocturnal SaO<sub>2</sub> (LSaO<sub>2</sub>)], and scores of Epworth sleepiness scale (ESS), Pittsburgh sleep quality index (PSQI), World Health Organization quality of life-BREF (WHOQOL-BREF) were observed, and the clinical efficacy was evaluated after treatment in the two groups.</p><p><strong>Results: </strong>After treatment, the AHI, HI, AI and longest apnea duration were reduced compared with those before treatment in the two groups (<i>P</i><0.01), the LSaO<sub>2</sub> was increased in the Daoqi acupuncture group (<i>P</i><0.01); in the Daoqi acupuncture group, the AHI, HI, AI and longest apnea duration were lower than those in the conventional acupuncture group (<i>P</i><0.05), and the LSaO<sub>2</sub> was higher than that in the conventional acupuncture group (<i>P</i><0.05). After treatment, the ESS and PSQI scores were decreased compared with those before treatment (<i>P</i><0.01), while the WHOQOL-BREF scores were increased compared with those before treatment (<i>P</i><0.01) in the two groups; in the Daoqi acupuncture group, the ESS and PSQI scores were lower than those in the conventional acupuncture group (<i>P</i><0.05, <i>P</i><0.01), and the WHOQOL-BREF score was higher than that in the conventional acupuncture group (<i>P</i><0.05). Except in the normal group, the intraperitoneally injection with streptozotocin was used to induce diabetes mellitus model in the rest groups. In the EA group, acupuncture was delivered at bilateral \"Zusanli\" (ST36), \"Sanyinjiao\" (SP6), \"Pishu\" (BL20) and \"Shenshu\" (BL23), and electric stimulation was attached to \"Zusanli\" (ST36)-\"Sanyinjiao\" (SP6) and \"Pishu\" (BL20)-\"Shenshu\" (BL23), on the same side, with continuous wave and a frequency of 2 Hz, for 10 min in each intervention. The intervention measure of each group was delivered once every 2 days, 3 times a week, for 8 consecutive weeks. Body weight, random blood glucose (RBG), thermal withdrawal latency (TWL), and mechanical withdrawal threshold (MWT) before intervention, and in 4 and 8 weeks of intervention, separately, as well as sensory nerve conduction velocity (SCV) and motor nerve conduction velocity (MCV) of the sciatic nerve after intervention were measured. Metagenomic sequencing (MS) was used to analyze gut microbiota and screen for differential species. Liquid chromatography-mass spectrometry (LC-MS) was employed to detect the differential metabolites in plasma, and the metabolic pathway enrichment analysis was performed on the differential metabolites. Spearman correlation analysis was adopted to assess the relationship between gut microbiota and metabolomics.</p><p><strong>Results: </strong>After 4 and 8 weeks of intervention, when compared with the model group, the EA group showed the increase in body weight, TWL, MWT (<i>P</i><0.01), and the decrease in RBG (<i>P</i><0.01). Compared with the normal group, SCV and MCV, as well as Chao1 index were dropped in the model group (<i>P</i><0.01), and those were elevated in the EA group when compared with those in the model group (<i>P</i><0.01). The dominant bacterial phyla of each group were <i>Firmicutes</i> (F) and <i>Bacteroidota</i> (B), the ratio of them (F/B) in the model group was lower than that of the normal group (<i>P</i><0.05), and F/B in the EA group was higher when compared with that in the model group (<i>P</i><0.05). In comparison with the normal group, the relative abundance increased in <i>Prevotella</i>, <i>Segatella</i>, <i>Prevotella-hominis</i> and <i>Segatella-copri</i> (<i>P</i><0.05); and it decreased in <i>Ligilactobacillus</i>, <i>Eubacterium</i>, <i>Pseudoflavonifractor</i>, <i>Ligilactobacillus-murinus</i> (<i>P</i><0.05) in the model group. Compared with the model group, the relevant abundance of the above mentioned gut bacteria was all ameliorated in the EA group (<i>P</i><0.05, <i>P</i><0.01). Professor <i>SUN</i> believes that the etiology of refractory facial paralysis is complex. Acupuncture and moxibustion treatment should be based on cortical localization, Baihui (GV20), lower 1/5 of motor area and brainstem area are selected, and repetitive transcranial acupuncture is applied. Under the ultrasonic positioning, acupuncture is performed on the starting and ending points of the mimetic muscles in different lesion sites. Combined with the TCM pathogenesis of refractory facial paralysis with deficiency of healthy <i>qi</i> and retention of pathogenic factors, acupuncture and moxibustion treatment takes strengthening the healthy <i>qi</i> and eliminating pathogenic factors as the core, and reuses the acupoints of <i>yangming</i> meridians (Yingxiang [LI20], Sibai [ST2], Dicang [ST4], Hegu [LI4], Zusanli [ST36], etc.) as the main acupoints to dredge the meridians. The main facial mimetic muscles and related collateral points are selected for cluster needling to dredge the collaterals. Acupuncture at Yangbai (GB14)-toward-Tongziliao (GB1), Sibai (ST2)-toward-Dicang (ST4), Dicang (ST4)-toward-Jiache (ST6) is applied and combined with the needle-sticking and lifting technique to nourishing tendons. Qihai (CV6) and Guanyuan (CV4) are selected for acupuncture before moxibustion. In addition, Professor <i>SUN</i> emphasizes that the three methods of kneading, acupuncture and moxibustion should be used in Yifeng (TE17), Qianzheng (Extra) and Xiaguan (ST7). In the model group and the EA group, COPD model was established by intratracheal instillation of lipopolysaccharide combined with cigarette fumigation. In the EA group, EA was applied at bilateral \"Feishu\" (BL13) and \"Zusanli\" (ST36), with disperse-dense waves, in frequency of 4 Hz/20 Hz, current of 1-3 mA, 20 min a time, once a day for 14 days continuously. Before and after modeling, as well as after intervention, body weight was observed; after intervention, the lung function indexes (forced expiratory volume in 0.1 second [FEV&lt;sub&gt;0.1&lt;/sub&gt;], FEV&lt;sub&gt;0.1&lt;/sub&gt;/forced vital capacity [FVC]%, forced expiratory volume in 0.3 second [FEV&lt;sub&gt;0.3&lt;/sub&gt;] and FEV&lt;sub&gt;0.3&lt;/sub&gt;/FVC%) were measured, serum levels of inflammatory factors (tumor necrosis factor-α[TNF-α], interleukin-6[IL-6], interleukin-1β[IL-1β] and interleukin-10[IL-10]) were detected by ELISA, histopathology of lung and colon tissues was observed by HE staining, the intestinal flora were analyzed by 16S rRNA, and the correlations between lung function and intestinal flora were analyzed.&lt;/p&gt;&lt;p&gt;&lt;strong&gt;Results: &lt;/strong&gt;Compared with the NC group, in the COPD group, the body weight and lung function indexes were reduced (&lt;i&gt;P&lt;/i&gt;&lt;0.01); the lung and colon tissues were damaged, the mean linear intercept (MLI) of alveolus and inflammatory cell numbers of 100 μm&lt;sup&gt;2&lt;/sup&gt; in lung tissue were increased (&lt;i&gt;P&lt;/i&gt;&lt;0.01); the serum levels of TNF-α, IL-6 and IL-1β were increased (&lt;i&gt;P&lt;/i&gt;&lt;0.01, &lt;i&gt;P&lt;/i&gt;&lt;0.05), and the serum level of IL-10 was decreased (&lt;i&gt;P&lt;/i&gt;&lt;0.01); α-diversity indexes of intestinal flora were increased (&lt;i&gt;P&lt;/i&gt;&lt;0.01); the relative abundance of &lt;i&gt;Bacteroidetes&lt;/i&gt;, &lt;i&gt;Proteobacteria&lt;/i&gt; and &lt;i&gt;Oscillospira&lt;/i&gt;, &lt;i&gt;Bacteroides&lt;/i&gt;, &lt;i&gt;Coprococcus&lt;/i&gt; was increased (&lt;i&gt;P&lt;/i&gt;&lt;0.01), the relative abundance of &lt;i&gt;Firmicutes&lt;/i&gt;, &lt;i&gt;Actinobacteria&lt;/i&gt;, &lt;i&gt;Tenericutes&lt;/i&gt;, TM7 and &lt;i&gt;Lactobacillus&lt;/i&gt;, &lt;i&gt;Allobaculum&lt;/i&gt;, &lt;i&gt;Bifidobacterium&lt;/i&gt;, YRC22 was decreased (&lt;i&gt;P&lt;/i&gt;&lt;0.01, &lt;i&gt;P&lt;/i&gt;&lt;0.05); 31 different expressed metabolic pathways were identified between the two groups.
